RESUMEN Introducción . La hipertensión pulmonar (HP) se asocia con elevada morbimortalidad y su pronóstico está determinado por la función del ventrículo derecho y el acople ventrículo-arterial pulmonar. Diversos parámetros hemodinámicos han sido propuestos para caracterizar el riesgo de evolución adversa. Objetivos . Determinar la capacidad de predicción de eventos graves, de diversos parámetros hemodinámicos medidos por cateterismo derecho. Material y métodos . Estudio multicéntrico, descriptivo, de cohorte retrospectiva de pacientes con HP. Se dividió a los datos hemodinámicos en alto o bajo riesgo según lo definido por las recomendaciones europeas de 2022, o según media o mediana de nuestra población. Resultados . Fueron incluidos 324 pacientes con HP, con edad media 61,5 años y 69% de sexo femenino; 62,1 % de los pacientes del Grupo 1 y 19,2 % del Grupo 3. La tasa de eventos (muerte o internaciones) en un seguimiento mediano de 23 (RIC 14-44) meses fue 60 % y la mortalidad global 24,5 %. Los valores de corte para definir riesgo de las diferentes variables fueron. índice cardíaco (IC) 2,72 L/min/m2, índice de volumen sistólico (IVS) 33,1 mL/lat./m2, resistencia vascular pulmonar (RVP) 6 unidades Wood (UW), índice de pulsatilidad de la arteria pulmonar (IPAP) 3,76, índice de trabajo sistólico del ventrículo derecho (ITSVD) 11,6 g.m/m2, compliance vascular pulmonar 1,84 mL/mmHg. La RVP elevada se asoció a mayor frecuencia de eventos combinados, mientras que valores de alto riesgo de IC, IVS, RVP y compliance presentaron asociación con mayor mortalidad en el seguimiento. Conclusiones . Si bien el IC y el IVS presentaron una adecuada predicción de riesgo, variables como la RVP y la compliance mostraron similar, e incluso mejor predicción de eventos graves en HP. Se necesitan nuevos estudios que validen estos parámetros en el seguimiento.
ABSTRACT Background. Pulmonary hypertension (PH) is associated with high morbidity and mortality and its prognosis is determined by the right ventricular function and right ventricular-pulmonary artery coupling. Several hemodynamic parameters have been proposed to better characterize the risk for adverse disease progression. Objective. The aim of this study was to determine the predictive ability of various hemodynamic parameters that can be calculated during RHC to predict serious events. Methods. In this retrospective multicenter and descriptive cohort study of patients with PH, hemodynamic variables values were divided as high or low risk based on the 2022 European guidelines, or mean or median values in our population. Results. A total of 324 patients with PH were included; mean age was 61.5 years and 69% were women; 62.1 % of the patients were in Group 1 and 19.2 % in Group 3. In a median (IQR) follow-up of 23(14-44) months, the event rate (death or hospitalization for heart failure) was 60.6% and all-cause mortality was 24.5%. The cut-off values associated with risk for the different hemodynamic variables were: cardiac index (CI) 2.72 L/min/m2, systolic volume index (SVI) 33.1 ml/m2, pulmonary vascular resistance (PVR) 6 Wood units (WU), pulmonary artery pulsatility index (PAPI) 3.76, right ventricle systolic work index (RVSWI) 11.6 g.m/m2, and pulmonary arterial compliance 1.84 mL/mm Hg. Elevated PVR was associated with a higher rate of composite events, while high-risk CI, SVI, PVR, and compliance were associated with higher mortality at follow-up. Conclusions. While CI and SVI are adequate predictors of risk, PVR and compliance demonstrate similar or even better risk prediction in patients with PH. Additional research is necessary to validate these parameters during follow-up.

Abstract Introduction: Low-dose ketamine infusions have shown analgesic effectiveness for the management of postoperative pain. The impact of low-dose ketamine infusions on cardiovascular response is dose-dependent and requires a better knowledge about its effects on this population. Objective: To conduct a systematic review to describe changes in systolic, diastolic and mean arterial pressure, and heart rate 24, 48 and 72 hours after surgery. Methods: Randomized, controlled trials were reviewed in the Cochrane Library, PubMed, EMBASE, SciELO, Lilacs and grey literature on low-dose ketamine infusions for the study variables. The quality of the studies was assessed using the Cochrane's risk of bias tool. Results: Six randomized, controlled trials with 641 patients were included. Low-quality evidence was found suggestive of a lack of certainty of any significant differences in the systolic blood pressure variables at 24 hours (mean standard deviation -1.00, 95 % CI: -7.27 to 5.27). A statistically significant higher mean heart rate at 24 hours was identified in the low-dose ketamine infusion group, (mean standard deviation 1.64 95 % CI: 0.38 to 2.90) which did not reach clinical significance. A lower pain level and less use of opioids was identified in the low-dose ketamine infusion group. Conclusions: Low quality evidence was found, suggesting that low-dose ketamine infusions are not associated with significant changes in blood pressure or heart rate 24 - 48 hours after surgery. It is important to individualize cardiovascular risk for each case, before initiating treatment.
Resumen Introducción: Las infusiones en dosis bajas de ketamina han mostrado eficacia analgésica en el manejo del dolor posoperatorio. El impacto de las infusiones en dosis bajas de ketamina en la respuesta cardiovascular es dosisdependiente y requiere un mejor conocimiento de sus efectos en esta población. Objetivo: Realizar una revisión sistemática para describir los cambios en la presión arterial sistólica, presión arterial diastólica, presión arterial media, frecuencia cardiaca a las 24, 48 y 72 horas del posoperatorio. Métodos: Se revisaron ensayos controlados aleatorizados en Cochrane Library, PubMed, EM-BASE, SciELO, Lilacs y literatura gris de infusiones en dosis bajas de ketamina para las variables del estudio. La calidad de los estudios se evaluó usando la herramienta de riesgo de sesgos de Cochrane. Resultados: Se incluyeron seis ensayos controlados aleatorizados con 641 pacientes. Se encontró evidencia de baja calidad sugestiva de ausencia de certeza de diferencias significativas en las variables presión arterial sistólica a las 24 horas (diferencia de medias estandarizada -1,00, IC95 %: -7,27 a 5,27). Para las 24 horas se halló una media de frecuencia cardiaca mayor en el grupo de infusiones en dosis bajas de ketamina, estadísticamente significativa (diferencia de medias estandarizada 1,64 IC95 %: 0,38 a 2,90) sin alcanzar significancia clínica. Se encontró menor nivel de dolor y consumo de opioides en el grupo de infusiones en dosis bajas de ketamina. Conclusiones: Se encontró evidencia de baja calidad, sugestiva de que las infusiones en dosis bajas de ketamina no se asocian a cambios significativos en la presión arterial o frecuencia cardiaca a las 24-48 horas en el posoperatorio. Es importante individualizar el riesgo cardiovascular para cada caso previo al inicio del tratamiento.

ResumoObjetivo: Avaliar a dificuldade na visibilidade e palpação da rede venosa periférica em adultos e idosos admitidos em uma unidade de intervenção hemodinâmica. Método:Trata-se de um estudo observacional e prospectivo com abordagem quantitativa. Realizado com 105 participantes, dentre adultos e idosos, admitidos em unidade de intervenção hemodinâmica, em um hospital geral público e de ensino. Os dados foram coletados por meio de instrumento estruturado contendo variáveis demográficas e clínicas, com observação da punção intravenosa periférica. Realizou-se a análise descritiva, calculando as proporções. Resultados: A dificuldade durante a palpação e a visualização da veia foram de 11,4% e 16,2%, respectivamente. Entre os participantes que apresentaram dificuldade para visualização da veia, 60% tiveram insucesso na primeira tentativa de punção. Conclusão: A punção periférica em veias que possuem maior dificuldade para visibilidade e palpação contribuem para o insucesso na primeira tentativa.
AbstractObjective: To evaluate the difficulty in visibility and palpation of the peripheral venous network in adults and older adults admitted to a hemodynamic intervention unit. Method: An observational prospective study was conducted with 105 adults and older adults admitted to a hemodynamic intervention unit, in a public general teaching hospital. Data were collected by means of a structured instrument consisting of demographic and clinical variables, with observation of peripheral intravenous puncture from October to December 2022. Descriptive analysis was performed, calculating proportions. Results: Results show a difficulty during palpation and visualization of the vein of 11.4% and 16.2%, respectively. Among those who had difficulty visualizing the vein, 60% were unsuccessful in the first puncture attempt. Conclusion: Peripheral puncture in veins with greater visibility and palpation difficulty contribute to failure in the first attempt
ResumenObjetivo: Evaluar la dificultad de visibilidad y de palpación de la red venosa periférica en adultos y ancianos ingresados en una unidad de intervención hemodinámica. Método: Se trata de un estudio observacional y prospectivo con enfoque cuantitativo. Participaron 105 adultos y ancianos ingresados en una unidad de intervención hemodinámica, en un hospital general público y universitario. Los datos se recolectaron mediante un instrumento estructurado, que contiene variables demográficas y clínicas, con observación de punción intravenosa periférica, durante los meses de octubre a diciembre de 2022. Se realizó un análisis descriptivo en el cual se calculó las proporciones. Resultados: La dificultad reportada durante la palpación y la visualización de la vena fue del 11,4% y el 16,2%, respectivamente. Entre los participantes que tuvieron dificultades en la visualización de la vena, el 60% falló en el primer intento de punción. Conclusión:La punción de venas periféricas que presentan mayores dificultades en la visibilidad y la palpación tiene tendencia a fallar en el primer intento

OBJECTIVE To investigate the effects of dexmedetomidine (DEX)-assisted general anesthesia on hemodynamics and cognitive function in elderly patients undergoing painless enteroscopy. METHODS From July 2020 to February 2022, 180 elderly patients undergoing painless enteroscopy in the outpatient operating room of the Second People’s Hospital of Jiaozuo were selected and divided into a control group (n=90) and an observation group (n=90) according to the random number table method. The control group was given routine general anesthesia (induced with sufentanil and propofol anesthesia, maintained with propofol anesthesia), while the observation group was given DEX-assisted general anesthesia after preoperative preparation. The hemodynamic indexes [mean arterial pressure (MAP), heart rate (HR)], dosage of general anesthesia, awakening time, cognitive function [minimized mental status examination (MMSE)], and the incidence of adverse drug reactions were compared between the two groups. RESULTS There was no significant difference in various indicators before anesthesia (T0) between 2 groups (P> 0.05). Compared with T0, MAP and HR of the two groups were reduced significantly 10 minutes after anesthesia (T1), at the time of enteroscope reaching the ileum and cecum (T2), enteroscope withdrawal after the examination (T3), and 10 minutes after surgery (T4); but MAP and HR of the observation group at T1, T2, T3, and T4 were all higher than those of the control group (P<0.05). Compared with the control group, the dosage of general anesthesia and the recovery time in the observation group were significantly reduced or shortened, the MMSE scores at 1, 2 and 3 days after the operation were significantly increased, while the incidence of cognitive dysfunction and adverse reactions were significantly reduced (P<0.05). CONCLUSIONS DEX can effectively improve the hemodynamics and cognitive function of elderly patients undergoing painless enteroscopy, which is beneficial to reduce the dosage of general anesthesia, shorten recovery time, and has better safety.

ObjectiveTo investigate the changes in cerebral blood perfusion in patients with acute cerebral infarction after taking Tongnaoyin, a traditional Chinese medicine, based on head and neck computed tomography (CT) angiography (CTA) combined with brain CT perfusion imaging (CTP). MethodA total of 240 patients with cerebral infarction of phlegm and blood stasis syndrome treated in Jiangsu Province Hospital of Traditional Chinese Medicine from March 2018 to September 2023 were randomly divided into a control group (99 cases) and a Tongnaoyin group (141 cases). Based on the guidelines, the control group was treated with conventional treatment such as anti-aggregation, anticoagulation, lipid-lowering and plaque stabilization, brain protection, and supportive treatment. The Tongnaoyin group was treated with Tongnaoyin of 200 mL in warm conditions in the morning and evening on the basis of the control group. Both groups underwent CTA combined with CTP within 24 hours after admission, and they were reexamined by CTA and CTP in the sixth month after admission. The degree of intracranial artery stenosis was determined according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) method. The relative cerebral blood volume (rCBV), relative cerebral blood flow (rCBF), mean transit time (MTT), and time to peak (TTP) of the lesion area before and after treatment were compared. The adverse outcomes of the two groups within six months after discharge were compared. ResultCompared with the group before treatment, the degree of vascular stenosis in the Tongnaoyin group was significantly reduced, and the difference was statistically significant (Z=105.369,P<0.05). Compared with the control group after treatment, the improvement rate of vascular stenosis in the Tongnaoyin group was higher (χ2=84.179,P<0.01), and the curative effect was better.After treatment, the rCBV and rCBF of patients in the Tongnaoyin group were significantly increased, and the difference was statistically significant (P<0.01). MTT and TTP showed a trend of shortening, but the difference was not statistically significant. There was no statistically significant difference in rCBV, rCBF, MTT, and TTP in the control group. Compared with those in the control group after treatment, the rCBV and rCBF in the Tongnaoyin group were significantly increased, while MTT and TTP were significantly reduced (P<0.01). After six months of discharge, the risk of poor prognosis in the Tongnaoyin group was significantly reduced compared with the control group (P<0.05). ConclusionTongnaoyin has a good effect on improving cerebral blood perfusion in patients with acute cerebral infarction. It can be used as an effective supplement for the conventional treatment of ischemic stroke to improve clinical efficacy.

Objective To explore the application of diagnostic system with artificial intelligence(AI)in the evaluation of patients with coronary heart disease(CHD)at high altitude.Methods A total of 318 pa-tients underwent coronary CT angiography(CTA)at the hospital from January to December 2022 were pro-spectively collected.According to the altitude gradient,the patients were divided into the 2 000-3 000 m group and>3 000 m group.Coronary angiography(CAG)was used as the gold standard to verify the diag-nostic performance of AI diagnostic system.Coronary artery diagnosis system with AI technology and CT de-rived fractional flow reserve(CT-FFR)measurement system were used to evaluate the plaque structure char-acteristics and hemodynamic changes in the two groups of patients.Results Calcified plaques and vulnerable plaques in the>3 000 m group were more than those in the 2 000-3 000 m group(χ2=3.976,6.482,P= 0.046,0.011).The incidence of multi-vessel coronary artery disease,moderate stenosis,severe stenosis and complete occlusion in the>3 000 m group was higher than that in the 2 000-3 000 m group,and the inci-dence of single-vessel coronary artery disease and mild stenosis in the 2 000-3 000 m group was higher than that in the>3 000 m group(P<0.05).The incidence of CT-FFR≤0.80 and<0.70 in the>3 000 m group was higher than that in the 2 000-3 000 m group(χ2=4.782,28.118,P=0.029,<0.001).The comparison with the gold standard showed that this method has high sensitivity,specificity,and diagnostic consistency(P<0.001).Conclusion The coronary diagnosis system with AI technology has certain value in the system-atic evaluation of coronary artery characteristics and hemodynamic changes in CHD patients at high altitude.

Objective By using the computational fluid mechanic(CFD)method the tandem carotid artery stenosis(TCAS)was simulated on the model,and to compare the postoperative hemodynamic changes of different surgical procedures.Methods One patient with tandem stenosis of internal carotid artery(ICA)and common carotid artery(CCA)was selected.CFD technique was used to establish four three-dimensional(3-D)models of the carotid bifurcations,including one model of a real patient and three models of presumptive surgery.The hemodynamic analysis was performed with these models so as to explore the development mechanism of TCAS and to discuss the selection of suitable surgical plan.Results In tandem stenosis,the stenosis was preferentially formed in CCA and subsequently led to ICA stenosis.The local hemodynamic situation in TCAS was more complex and more risky than in single carotid artery stenosis.In tandem stenosis,the treatment of one stenosis site would affect the blood flow at the next stenosis site and cause restenosis or plaque rupture.Conclusion In treating patients with TCAS,CFD simulation examination should be performed when the surgical plan is formulated,which can help clinicians to predict the postoperative changes in blood flow and to choose the appropriate surgical plan.

Objective To investigate the effects of nalbuphine combined with dexmedetomidine on postop-erative recovery quality and pain in patients who undergoing laparoscopic bariatric surgery.Methods A total of 169 patients who underwent laparoscopic bariatric surgery at our hospital were included and divided into control group(group C),nalbuphine group(group N),dexmedetomidine group(group D),and nalbuphine combined with dexme-detomidine group(group ND)using randomised numerical table method.Group C received intravenous injection of saline,group N and group ND received intravenous injection of nalbuphine before the end of the surgery,and group D and group ND received pumping of dexmedetomidine before anesthesia induction and during surgery.Compare the postoperative recovery quality score(QoR-40),hemodynamics at different time points,visual analogue scale score(VAS),sedation-agitation scale(SAS),first time out of bed activity and exhaust time,and incidence of nausea and vomiting among four groups.Results The postoperative QoR-40 scores of patients in group ND were better than those in group C and group N(P<0.05),and the QoR-40 scores in group D were better than those in group C(P<0.05).MAP and HR were more stable during the awakening period in group ND and group D(P<0.05).Compared with group C,patients in all three groups had lower VAS scores and SAS scores(P<0.05)and consumed less remedial analgesic medication(P<0.05).In terms of adverse reactions,the incidence of postoperative nausea,vomiting and coughing in the group ND was lower than that in the group C(P<0.05).Conclusion The combination of nalbuphine and dexmedetomidine could improve the quality of postoperative recovery and pain in patients under-going laparoscopic bariatric surgery,reduce hemodynamic fluctuations during the patients′ recovery period,reduce the incidence of nausea and vomiting,and improve the patients′ prognosis.
ABSTRACT
Objective To explore the efficacy of early hyperbaric oxygen therapy(HBOT)combined with median nerve electrical stimulation(MNES)in the treatment of severe traumatic brain injury(sTBI)and its impact on hemodynamics,coma degree,and neurological function of patients.Methods A total of 78 patients with sTBI admitted to the General Hospital of Western Theater Command from March 2020 to October 2021 were selected as the research subjects.The patients were randomly divided into the control group and the observation group,with 39 patients in each group.The patients in both groups underwent basic treatments such as water,electrolyte and acid-base balance,nutritional support,anti-infection,and decompressive craniectomy.On this basis,patients in the control group received early HBOT,while patients in the observation group received both HBOT and MNES.Their clinical efficacy was compared between the two groups.Before and after treatment,dual-channel transcranial Doppler ultrasound was performed to detect hemodynamic indicators such as peak systolic blood flow velocity(Vs),mean blood flow velocity(Vm),and pulsatility index(PI)in the middle cerebral artery of patients in the two groups.The Glasgow Coma Scale(GCS)score was used to evaluate the degree of coma of patients in the two groups,the National Institutes of Health Stroke Scale(NIHSS)score was used to assess the neurological deficits of patients in the two groups,and the enzyme-linked immunosorbent assay was used to measure the levels of central nervous system specific protein(S100-β),glial fibrillary acidic protein(GFAP),and myelin basic protein(MBP).Complications during treatment of patients in the two groups were recorded,and their incidence was compared.Results The total effective rate of patients in the control and observation groups was 79.49%(31/39)and 92.31%(36/39),respectively.The total effective rate in the observation group was significantly higher than that in the control group(x2=8.971,P<0.05).There was no significant difference in Vm,Vs,and PI between the two groups before treatment(P>0.05).After treatment,the Vm and Vs in both groups were significantly higher than those before treatment,while the PI was significantly lower than that before treatment(P<0.05);and the Vm and Vs in the observation group were signifi-cantly higher than that those in the control group,while the PI was significantly lower than that in the control group(P<0.05).There was no significant difference in GCS and NIHSS scores between the two groups before treatment(P>0.05).After treatment,the GCS score in both groups was significantly higher than that before treatment,while the NIHSS score was significantly lower than that before treatment(P<0.05);and the GCS score in the observation group was significantly higher than that in the control group,while the NIHSS score was significantly lower than that in the control group(P<0.05).There was no significant difference in S100-β,GFAP,and MBP levels between the two groups before treatment(P>0.05).After treatment,the S100-β,GFAP,and MBP levels in both groups were significantly lower than those before treatment(P<0.05),and the S100-β,GFAP,and MBP levels in the observation group were significantly lower than those in the control group(P<0.05).During treatment,the incidence of complications in the control and observation groups was 23.08%(9/39)and 20.51%(8/39),respectively,showing no significant difference(x2=2.328,P>0.05).Conclusion Early HBOT combined with MNES shows good efficacy in treating sTBI,which can effectively improve the patients'hemodynamic level,alleviate the severity of coma,enhance neurological function,and promote early recovery of consciousness,without increased risk of complications.

Objective To observe the reliability of regional 4D flow and whole heart 4D flow cardiac MRI(CMRI)for measuring hemodynamic parameters of left ventricle.Methods Heart ultrasonography and CMRI were prospectively obtained in 31 healthy subjects.Hemodynamic parameters of left ventricle were measured using heart ultrasound,3-chamber 4D flow CMRI(based on inflow and outflow channel of left ventricle)and whole heart 4D flow CMRI,respectively.Intra-class correlation coefficient(ICC)was performed to evaluate the consistencies of the measured left ventricle hemodynamic parameters among the above 3 methods.Results Good consistencies of peak systolic velocity in aortic supravalvular/subvalvular,E peak diastolic velocity of mitral valve,supravalvular/subvalvular aortic pressure and aortic valve pressure gradient(all ICC>0.75),while moderate consistency of A peak diastolic velocity of mitral valve(ICC=0.718)were found between heart ultrasound and 3-chamber 4D flow CMRI.Good consistencies of peak systolic velocity in aortic supravalvular/subvalvular,A peak diastolic velocity of mitral valve and supravalvular/subvalvular aortic pressure(all ICC>0.75),while moderate consistencies of E peak diastolic velocity of mitral valve and aortic valve pressure gradient(ICC=0.600,0.628)were found between heart ultrasound and whole heart 4D flow CMRI.Meanwhile,good consistencies of the above parameters were found between 3-chamber 4D flow CMRI and whole heart 4D flow CMRI(all ICC>0.75).Conclusion Measuring left ventricular hemodynamic parameters using local regional 4D flow and whole heart 4D flow CMRI were reliable,with good consistency with cardiac ultrasound.

Purpose To observe the right ventricle and left ventricle blood pool T2 map in chronic thromboembolic pulmonary hypertension(CTEPH)and healthy controls,and to analyze the value of T2 mapping technique in evaluating CTEPH.Materials and Methods A total of 42 patients with CTEPH and 42 healthy volunteers had been prospectively recruited from January 2020 to January 2022 in China-Japan Friendship Hospital.All CTEPH patients underwent cardiac magnetic resonance with T2 mapping and right heart catheterization.Cardiac magnetic resonance was performed on healthy controls.Diastolic T2 mapping was performed in cardiac magnetic resonance,and then the ratio of right ventricular to left ventricular T2 values(RVT2/LVT2)between the CTEPH group and the healthy group was calculated and compared.Meanwhile,the correlation between RVT2/LVT2 and hemodynamic parameters in the CTEPH group was analyzed.Results RVT2/LVT2 in the CTEPH group was significantly lower than that in the healthy group(0.74±0.16 vs.0.86±0.12;t=3.673,P<0.001).RVT2/LVT2 in CTEPH group was negatively correlated with pulmonary vascular resistance(r=-0.534,P<0.001);while it was positively correlated with cardiac index,right atrium oxygen saturation,right ventricle oxygen saturation and pulmonary arteries oxygen saturation(r=0.600,0.603,0.648,0.582,P<0.001).Conclusion RVT2/LVT2 in the CTEPH group is positively correlated with right cardiac oxygen saturation and negatively correlated with pulmonary vascular resistance.T2 mapping may be a noninvasive evaluation of hemodynamics in CTEPH.

Objective:To clarify the changes of intrahepatic ultrasound hemodynamics before and after hepatic artery thrombosis (HAT) after liver transplantation (LT), providing early warning and anticoagulation guidance to clinicians.Methods:The clinical data of patients who underwent liver transplantation at Zhongshan Hospital of Fudan University between June 2006 and October 2022 were retrospectively analyzed, 47 patients with a diagnosis of HAT confirmed by DSA (digital subtraction angiography) were included in the HAT group, and 71 patients without vascular complications were included in the non-HAT group. Differences in peak flow velocity (PSV), resistance index (RI), and portal vein velocity (PVV) were compared between the two groups. Logistic regression analysis was used to determine the relationship between postoperative PSV decline and HAT occurrence, while ROC curve were used to determine the critical value and evaluate the diagnostic efficacy. Patients with HAT were divided into well-treatment group and poor-treatment group according to whether the blood flow was restored after multiple surgeries or thrombolytic treatments. The changes of early intrahepatic hemodynamics after surgical or thrombolytic therapy were compared between the two groups.Results:①A decrease in PSV of the transplanted hepatic artery was measured 1 d before HAT, and PSV<0.39 m/s predicted thrombus formation with a sensitivity of 0.70, specificity of 0.86, and the AUC was 0.83. ②After treatment, PSV in the HAT group increased immediately, approaching the normal level on the 2nd day. In the well-treatment group, PSV and PVV reached normal levels on the first day after treatment, which were significantly higher than the corresponding values in the poor-treatment group ( P=0.030, 0.021). Conclusions:In the early stage after liver transplantation, a PSV<0.39 m/s is related to the occurrence of HAT thrombosis 1 d later. A significant increase in PSV on the first day after treatment indicates a good treatment response, and there is no need for further DSA re-examination or increasing the number of thrombolysis.

Objective To construct a nomogram model for predicting the risk of in-hospital death in CHF patients by using noninvasive hemodynamic monitoring combined with age,DBP,CRP and renal insufficiency(serum creatinine≥ 442 μmol/L).Methods A total of 223 elderly patients with acute onset of CHF admitted in First,Second Medical Centre of Chinese PLA General Hos-pital from September 2022 to March 2023 were recruited in this study.According to their clinical outcomes,they were divided into survival group(196 cases)and death group(27 cases).Based on the in-hospital death and other related indicators,a nomogram model was constructed to predict the risk factors of in-hospital death in CHF.Results Noninvasive hemodynamic mornitoring indi-cated that the death group had significantly higher LVEF and LCWI values but lower LVEDV value than the survival group(P<0.05,P<0.01).Multivariate logistic regression analysis showed that age(OR=1.131,95%CI:1.052-1.213,P=0.001),DBP(OR=0.932,95%CI:0.882-0.982,P=0.011),CRP(OR=1.171,95%CI:1.021-1.352,P=0.024),LVEDV(OR=0.984,95%CI:0.962-0.992,P=0.011)and renal insufficiency(OR=5.863,95%CI:1.351-1.731,P=0.004)were independent risk factors for the short-term prognosis of the elderly CHF patients.The AUC value of the nomogram model was 0.902(95%CI:0.819-0.948,P<0.05),and calibration curve analysis showed the C-index was 0.902,indicating accurate predictive perform-ance.Conclusion Age,DBP,LVEDV,CRP and renal insufficiency are independent risk factors for the short-term prognosis of the elderly CHF patients.

Objective:To analyze the electrocardiographic characteristics of patients with chronic pulmonary artery stenosis (PAS), and to explore their relationship with disease severity indicators.Methods:The study was a retrospective case-series analysis. Patients with chronic PAS admitted to Gansu Provincial Hospital from January 2018 to July 2021 were enrolled. The clinical data and the results of electrocardiography, transthoracic echocardiography, right cardiac catheterization, N-terminal B-type natriuretic peptide (NT-proBNP) measurement and 6-min walking distance test of patients were analyzed. The linear regression model or logistic regression model was used to analyze the relationship between electrocardiographic characteristics and the disease severity in patients with chronic PAS.Results:Sixty-three patients aged (62.1±9.7) years including 43 females (68.3%) were enrolled in the study. Among them, 62 patients (98.4%) had (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ)<1.5 mV, and no patients had V 5lead R: S ratio to V 1 lead R: S<0.04 and V 6 lead R: S ratio<0.4. There were 55 patients (87.3%), with flat or inverted T-waves in V 1, and 10 patients (15.9%) with flat or inverted T-waves in all precordial leads (V 1-V 6). There were 18 patients (28.6%) with flat or inverted T-waves in inferior leads (Ⅱ, Ⅲ, aVF). Multiple liner regression analysis showed that Max R V1, 2+Max S I, aVL-S V1 combined with the number of flat or inverted T-waves in limb leads was independently correlated with atrial area ( R2=0.290, P=0.002); R V1+S V5 was independently correlated with right ventricular area ( R2=0.257, P=0.001); R peak V 1 combined with the number of flat or inverted T waves in precordial leads was independently correlated with tricuspid annular plane systolic excursion ( R2=0.407, P<0.001); (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ) combined with the number of flat or inverted T waves in precordial leads was independently correlated with NT-proBNP ( R2=0.504, P<0.001); Max R V1, 2+Max S I, aVL-S V1 were independently correlated with right atrial pressure ( R2=0.803, P=0.036); (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ) were independently correlated with mean pulmonary artery pressure ( R2=0.302, P<0.001); R aVRcombined with the number of flat or inverted T-waves in precordial leads was independently correlated with cardiac index ( R2=0.173, P=0.003); (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ) was independently correlated with pulmonary vascular resistance ( R2=0.173, P=0.002); R peak V 1 combined with the number of flat or inverted T-waves in precordial leads was independently correlated with mixed vein oxygen saturation ( R2=0.302, P<0.001). Conclusion:The vast majority of patients with chronic PAS have (R 1+S Ⅲ)-(S Ⅰ+R Ⅲ)<1.5 mV and flat or inverted T-wave in V 1 lead, and some characteristic electrocardiographic manifestations are correlated with indicators of disease severity.

@#Objective To establish a personalized Stanford type B aortic dissection numerical simulation model, and using computational fluid dynamics (CFD) numerical simulation to obtain the hemodynamic behavior and law of the type B aortic dissection at different stages of development. Methods Based on the theory of three-dimensional model reconstruction, we used CT images of a patient with type B aortic dissection in the Xiamen Cardiovascular Hospital of Xiamen University, relevant medical image processing software to reconstruct a personalized aortic three-dimensional model, and CFD to reconstruct the model which was simulated in fluid mechanics. Results The three-dimensional reconstruction model could intuitively observe the changing trend of the false cavity at different stages of the dissection development. Through fluid mechanics simulation, the blood flow rate, pressure, wall shear stress, vascular wall Von Mises stress and other parameters at different stages of the dissection development were obtained. Conclusion The hemodynamic behavior and law of relevant parameters in the development stage of aortic dissection are analyzed. The combination of the values of relevant parameters and clinical medical detection and diagnosis can well predict the development of the disease, and finally provide more theories and methods for the scientific diagnosis of aortic dissection.

Objective To investigate the effects patterned structure located on the surface of mechanical valve leaflet exerted on the hydrodynamic properties of valve prostheses.Methods Bileaflet aortic mechanical valves of GKS 21A and 23 A were randomly selected as the control group,hydrodynamic performance of each valve were assessed,as per ISO5840 by calculating mean transvalvular pressure(MTP),regurgitation fraction(REG),effective orifice area(EOA)and effective orifice area(eLoss).Then,laser etching was applied to construct a parallel-groove array pattern on leaflet surface perpendicular to the direction of the blood flow.The valve was subjected a-gain to the same test,and the data obtained were recorded and analyzed.Results In the pulsating flow test,the MTP of the experimental group was smaller than that of the control group at the same flow rate and size.The RE in GKS 21A experimental group was smaller than that in control group,and the RE in GKS 23A experimental group was larger than that in control group.The EOA in the experimental group was larger than that in the control group at the same flow rate and size.The eLoss in the GKS 21A experimental group was smaller than that in the control group,and the eLoss in the GKS 23A experimental group was larger than that in the control group.Conclusion The parallel-groove array pattern on the surface of the leaflet affected the hemodynamic performance of the valve prostheses.

Objective To investigate the hemodynamic effects of enhanced external counterpulsation(EECP)on cerebral arteries with different stenoses.Methods Zero-dimensional/three-dimensional multiscale hemodynamic models of cerebral arteries with different stenoses were constructed.Numerical simulations of the EECP hemodynamics were performed under different counterpulsation modes to quantify several hemodynamic indicators of the cerebral arteries.Among them,the mean time-averaged wall shear stress(TAWSS)downstream of the stenosis was in the range of 4-7 Pa,a low percentage of TAWSS risk area,and high narrow branch flow were considered to inhibit the development of atherosclerosis and create a good hemodynamic environment.Results For cerebral arteries with 50%,60%,70%,and 80%stenosis,the hemodynamic environment was optimal in counterpulsation mode when the moment of cuff deflation was 0.5,0.6,0.7,and 0.7 s within the cardiac cycle.Conclusions For 50%stenotic cerebral arteries,the counterpulsation mode with a deflation moment of 0.5 s should be selected.For 60%stenotic cerebral arteries,the counterpulsation mode with a deflation moment of 0.6 s should be selected.For 70%or 80%stenotic cerebral arteries,the counterpulsation mode with a deflation moment of 0.7 s should be selected.As stenosis of the cerebral arteries increases,the pressure duration should be prolonged.This study provides a theoretical reference for the EECP treatment strategy for patients with ischemic stroke with different stenoses.

Objective:To investigate the therapeutic effect of sequential therapy with butylphthalein on acute cerebral infarction and mild-to-moderate increases in middle cerebral artery blood flow in patients.Methods:The clinical data of 92 patients with acute cerebral infarction and mild-to-moderate increases in middle cerebral artery blood flow who received treatment at the Xiaogan Hospital Affiliated to Wuhan University of Science and Technology from January 2018 to October 2021 were retrospectively analyzed. These patients were divided into a study group and a control group using a random number table method. The control group was given an intravenous infusion of butylphthalein sodium chloride injection, while the study group took oral butylphthalein soft capsules after intravenous infusion of butylphthalein sodium chloride injection. The baseline data, hemodynamics, neurological function, and clinical outcomes were compared between the two groups. At 90 days after treatment, the National Institutes of Health Stroke Scale (NIHSS), the Activity of Daily Living Scale (ADL), and the modified Rankin Scale (mRS) were used to evaluate clinical outcomes. Transcranial Doppler ultrasound (TCD) examination was performed to evaluate hemodynamic changes.Results:A total of 92 patients completed all the observation indices as required, including 48 patients in the study group and 44 patients in the control group. There were no significant differences in demographics, vascular risk factors, laboratory results, NIHSS score, ADL score, or arterial hemodynamics of the diseased brain between the two groups (all P > 0.05). At 90 days after treatment, the NIHSS score in the study group was significantly lower than that in the control group [(4.00 ± 1.95) points vs. (4.91 ± 2.08) points; t =-2.16, P = 0.033]. The ADL score in the study group was significantly higher than that in the control group [(82.71 ± 9.56) points vs. (76.25 ± 11.47) points; t = 2.94, P = 0.004]. The good rate of outcomes in the study group was significantly higher than that in the control group [70.83% (34/48) vs. 50.00% (22/44); χ2 = 4.18, P = 0.041]. There were significant differences in the peak systolic velocity [(152.33 ± 9.58) cm/s vs. (157.41 ± 11.77) cm/s; t = 2.27, P = 0.025] and the mean velocity [(90.00 ± 8.30) cm/s vs. (94.45 ± 9.07) cm/s; t = -2.46, P = 0.016] of the middle cerebral artery between the study and control groups. The difference in pulsitility index between the two groups was not statistically significant [(0.97 ± 1.06) vs. (1.01 ± 1.21); t = 1.69, P = 0.093]. Compared with the poor outcome group, patients in the good outcome group had lower NIHSS and ADL scores after discharge (both P < 0.001), and the proportion of patients who received sequential therapy with butylphthalein in the good outcome group was higher [(60.70% (34/56) vs. 38.90% (14/36); χ2 = 4.18, P = 0.041]. Conclusion:Sequential therapy with butylphthalein can reduce neurological deficits, promote neurological function recovery, improve the hemodynamics of diseased blood vessels, and greatly improve daily living activities in patients with acute cerebral infarction complicated by mild to moderate increases in middle cerebral artery blood flow.

Objective:To evaluate the clinical efficacy of ultrasound-guided superior laryngeal nerve block combined with cricothyroid membrane puncture for awake tracheal intubation in difficult airways.Methods:A total of 80 patients scheduled for general anesthesia surgery owing to difficult airways at Marine Police Corps Hospital of Chinese People's Armed Police Force from August 2019 to February 2023 were included in this prospective study. These patients were randomly divided into a control group and an observation group ( n = 40 per group). Both groups of patients underwent fiberoptic bronchoscope-guided awake tracheal intubation through the nasal route. Before intubation, the control group received surface anesthesia combined with cricothyroid membrane puncture anesthesia, while the observation group received superior laryngeal nerve block combined with cricothyroid membrane puncture anesthesia. During the intubation process, hemodynamic indicators of the patients were monitored, the duration of the intubation operation was recorded, and the degree of sedation and cough response was evaluated. Before and after intubation, the levels of serum stress response factors were determined, and the satisfaction level of patients with the intubation operation was investigated. Results:At the time points T1 (immediately after the tracheal tube enters the glottis) and T2 (after the tracheal tube enters the trachea), the heart rate in the observation group was (84.08 ± 8.25) times/minute and (82.64 ± 7.81) times/minute, respectively, and the mean arterial pressure in the observation group was (92.57 ± 8.32) mmHg (1 mmHg = 0.133 kPa) and (103.27 ± 9.16) mmHg, respectively, which were significantly lower compared with those in the control group ( t = 5.92-12.56, all P < 0.001). At T1 and T2, there was no significant difference in blood oxygen saturation (SpO 2) between the observation and control groups ( t = 0.34-0.43, P = 0.652-0.726). The duration of the intubation operation in the observation group was (8.96 ± 1.15) minutes, which was significantly shorter than (11.47 ± 1.39) minutes in the control group ( t = 6.84, P < 0.001). The Ramsay sedation score during intubation in the observation group was 3 (2,3) points, which was significantly higher than 2 (1,3) points in the control group ( Z = 9.26, P < 0.001). The cough response score in the observation group was 1 (0,1) point, which was significantly lower than 1 (0, 2) points in the control group ( Z = 4.37, P < 0.001). The serum levels of stress response factors norepinephrine and adrenaline in the observation group were significantly lower than those in the control group ( t = 14.58-25.94, both P < 0.05). The overall patient satisfaction in the observation group was 97.50%, which was significantly higher than that in the control group ( χ2 = 4.51, P = 0.034). Conclusion:Superior laryngeal nerve block combined with cricothyroid membrane puncture can be used for anesthetic management of awake nasal tracheal intubation in difficult airways. This combined approach can improve patient hemodynamic stability and sedation, shorten the duration of intubation, reduce stress responses during intubation, and enhance patient satisfaction.
