ABSTRACT

Acute nonvariceal upper gastrointestinal bleeding (ANVUGIB) is one of the most common medical emergencies that associated with high mortality rate. Surgical operation is usually required in patients when conservative and endoscopic therapy fail to control bleeding. With the advances in catheter-based techniques and newer embolic agents, transcatheter arterial embolization (TAE) is gradually applied in treatment of ANVUGIB and has got similar effect to surgical operation in control of bleeding and fewer associated complications. The advances of TAE in treatment of ANVUGIB were reviewed in this article.

ABSTRACT

Objective To discuss the value of combining superselective mesenteric angiography (SSMA)with methylene blue in-jection for localization of the Obscure Gastrointestinal Bleeding (OGIB).Methods 12 patients presenting with OGIB had preopera-tive localization of the bleeding site by SSMA.The microcatheter was remained inside the bleeding artery branch after angiography. Then the methylene blue was injected via the microcatheter for localization during exploratory laparotomy and the dyed intestine was then removed. Results The detected rate of bleeding sites by using preoperative SSMA was 100%.The lesions in total 12 patients were found quickly during the exploratory laparotomy. The length of the removed intestine was 9 -12 cm (median,10 cm). After followed up for 10-24 months (median,18 months),all patients with OGIB had no recurrence. Conclusion The method of SSMA combined with the use of methylene blue injection is a safe and effective procedure for the localization of OGIB and could be beneficial for increasing the surgical success rate of hemostasis.

ABSTRACT

Objective To discuss the value of emergency angiography and embolization treatment of hemorrhage of digestive tract. Methods Give emergency angiography and embolization treatment for 15 patients with hemorrhage of digestive tract.Male is 12.Famale is 3.Adopt to Seldinger technic and method of coaxial duct, the head of ducts accesses to the region of bleeding, with gelatin foam or suture to embolism. Results All 15 patients were sucessful embolismed and get objective of hemostasis.Angiography appeared contrast medium overflowing and the humens, mucosa development. All patients were not bleeding again. Conclusion Clinic efficiency of emergeney ;embolization treatment for hemorrhage of digestive tract is assurance and explanation the principle and events of attention.
