ABSTRACT

Introduction: Automated body fluid (BF) analysis is gradually replacing the traditional methods of cell counting in all BFs. This study was done to analyze the high-fluorescence (HF)-BF parameter generated on Sysmex XN-1000 and study its correlation with the presence of malignant cells in the body fluids. A correlation between manual and automated differential counts was also done. Materials and Methods: A total of 1985 samples including 797 ascitic fluids (AF), 532 pleural fluids (PF), and 656 cerebrospinal fluids (CSF) were run on Sysmex XN-1000 in BF mode and cytopathology was available for 924 BFs including 389 AF, 379 PF, and 156 CSF. Both manual and automated methods were used for cell differential and cell morphology. Results: Of the 924 samples with corresponding cytopathology, malignancy was found in 59 samples. The HF-BF%/100 WBCs (24.8 ± 72.5) and HF-BF#/?L (329.86 ± 932.35) for malignant BF samples were found to be significantly higher than the nonmalignant samples (4.41 ± 8.1) and (19.57 ± 61.91), respectively. Receiver–operator-characteristic curve cutoffs for all BF for percentage and absolute HF-BF were 2.85%/100 WBCs and >12/?L. A good correlation was found between the manual and automated WBC differential counts in all fluids except CSF with total count <5/?L. Conclusions: BFs can be reliably analyzed on automated analyzers. HF-BF parameter is helpful in identifying malignant samples but cannot be totally relied upon. If HF-BF%/# are above the lab-generated cutoffs, microscopy should be done. A complete validation study on HF-BF parameter in BF mode is desired to set the standards for the analysis of serious effusions.

ABSTRACT

Objective To study the clinical significance of reticulocyte parameters change before and after chemotheraphy in acute leukemia.Methods Thirty-two chemotherapeutic patients suffered from acute leukemia were improved after chemotherapy.The percentage of reticulocyte(RET%),reticulocyte absolute value(RET#),low fluorescent reticulocyte percentage(LFR),middle fluorescent reticulocyte percentage(MRF),high fluorescent reticulocyte percentage(HFR),white blood cells(WBC) and absolute neutrophils count (ANC) and immature reticulocyte fraction(IRF) were detected by using the Sysmex 2000 automatic blood cell analyzer.Results IRF,MFR and HFR after chemotherapy in the patients with leukemia were always declined,reached the lowest on 7 d of chemotherapy,began to recover on 14 d after chemotherapy,and recovered to the level before chemotherapy on 21 d of chemotherapy.LFR began to increase on 7 d of chemotherapy,and began to decrease on 14 d of chemotherapy.WBC and RET# were always decreased on 7,14 d of chemotherapy,and began to recover on 21 d of chemotherapy.Conclusion IRF,MFR,HFR and LFR are the sensitive indicators reflecting the bone marrow recovery and are earlier than WBC and RET# by 7 d.

ABSTRACT

Objective To investigate the examination of tumor cells in pleural effusion by body fluid mod-el of Sysmex XN-9000 automated hematology analyzer ,and set the cutoff value of HF-BF# and HB-FB%for screen-ing tumor cells in pleural effusion. Methods One hundred and six pleural effusion specimens were detected by Sysmex XN-9000,and malignant samples were confirmed by TCT(Thinprep Cytology Test)and immunohistochem-istry method. Results Of 106 specimens,44 cases of HF-BF count were positive,and 12 samples were confirmed malignant by TCT and immunohistochemistry method. The cutoff value of HF-BF#(× 106/L)and HF-BF%/100WBC were 3.5 and 1.55 respectively. The detection sensitivity,specificity,positive predictive value,negative predictive value,accuracy of HF-BF# were 66.7%,40.1%,27.3%,91.7% and 63.2% respectively and those of HF-BF%were 83.3%,40.6%,27.3%,91.7%and 63.2%respectively. Conclusions HF-BF# and HF-BF%of body fluid model of Sysmex XN automated hematology analyzer are effective for screening tumor cell in pleural effusion ,and all the screening samples must be confirmed by pathology methods.

ABSTRACT

To study the value of high-fluorescent cell for the screening of malignant hydrothorax or ascites. The clinical diagnosis results and hydrothorax or ascites specimens of 400 outpatients registered from January to December in 2014 were collected, and high-fluorescent cells were counted with Sysmex XE-5000 blood cell analyzer, and then stained for microscopy. Receiver operating characteristic (ROC) curve was used to explore the sensitivity, speci-ficity, positive likelihood ratio and negative likelihood ratio of high-fluorescent cell for screening malignant hydrothorax or ascites. The numbers of high-fluorescent cells of the cancer patients were significantly higher than those of the non-cancer patients, with Z equal to -7.372 and P less than 0.05. The values of the area under curve (AUC), sensitivity, specificity, positive likelihood ratio and negative likelihood ratio were 0.801, 70.67%, 84.31%, 4.50 and 0.35 respective-ly. Detection of high-fluorescent may be used to screen malignant hydrothorax or ascites, and further exami-nation and follow-up have to be performed in case the number of high-fluorescent cells in the hydrothorax or ascites is not less than 68.
