ABSTRACT

@#AIM: To investigate the relationship between the macular hemorrhage and refractive abnormalities,and analyze the occurrence probability and the factors of refractive abnormalities in high-risk neonates. <p>METHODS: From January 2017 to January 2018, a total of 1 229 neonates with high-risk factors invided into 3 groups, according to the different parts of retinal hemorrhage, examined by RetCamⅢ device. All the subjects were checked the refractive status by autorefractor when they were 1 to 1.5 years old. The abnormalities were checked their refractive status under using of 1% atropine oculentum after 7d. The relationship between the macular hemorrhage and refractive abnormalities, and the occurrence probability and the factors of refractive abnormalities were analyzed.<p>RESULTS: Among 205 eyes which were retinal hemorrhage irrelated macula 6 eyes had refractive abnormality(2.93%)in group A. 57 eyes which were macular hemorrhage 17 eyes had refractive abnormality(29.82%)in group B. 2 196 eyes which were no retinal hemorrhage 40 eyes had refractive abnormality(1.82%)in group C. It had significant difference between groups A and B, groups B and C(<i>P</i><0.001), but had no significant difference between groups A and C(<i>P</i>=0.27). Multivariate Logistic regression analysis showed that macular hemorrhage and neonatal hypoxic ischemic encephalopathy were the independent risk factors for refractive status.<p>CONCLUSION: The incidence of refractive abnormality was high in macular hemorrhage. Macular hemorrhage and neonatal hypoxic ischemic encephalopathy were the independent risk factors for refractive abnormality,and we should strengthen the observation and intervention of such new-borns.

ABSTRACT

Brain injury is the main cause of neonatal death and neurological dysfunction in infants,the sequelae of which are more common,and bring serious harm to the society and the family of the patients.Therefore,early detection of brain damage in high-risk infants,evaluation of its severity and guidance for further treatment have become a hot topic in recent years.People are coming to realize the importance of continuous EEG monitoring of newborns with encephalopathy.Amplitude-integrated electroencephalogram (aEEG) is easy to operate,intuitive and easy to read.It is suitable for prolonged tracing and is suitable for continuous monitoring of brain function in neonatal intensive care unit.This article reviews the aEEG application in clinic,including the newborn with hypoxic ischemia induced brain injury,selection of appropriate patient,hypothermia treatment,neonatal convulsions,premature brain cerebral injury,premature brain maturity assessment,other high-risk infants brain damage diagnosis and efficacy evaluation.

ABSTRACT

PURPOSE: To investigate the effects of a warm water-filled bag on body temperature, Oxygen demand, respiratory rate and blood sugar in high risk neonates. METHOD: The infants were assigned to two groups, 16 in the experimental and 20 in the control group. Infants in the experimental group was received 40~42 degrees C warm water-filled bag therapy for 2 hours under radiant warmers. Infants in the control group were placed under the same kind of radiant warmers but without the warm water-filled bag. The following were measured every 30 minutes: rectal temperature, O2 demand (FiO2), and respiratory rate. Blood sugar was measured hourly. RESULT: Body temperatures increased significantly in the experimental group (P=0.0138), and there was a difference according to time (P=0.0001). Significant difference were found between the two groups for FiO2 (P=0.0180). There was no difference in respiratory rates between the two groups and rates according to time for the experimental group were also not significant (P=0.3569). No significant difference between two groups was found for blood sugar(P=0.2152), but there was a significant difference according to the times (P=0.0001). CONCLUSION: This results of this study indicate that a warm-filled bag is an effective means of resorting body temperature and reducing O2 demand in high risk neonates.

ABSTRACT

Objective To investigate the character 、difference and clinical meaning of distortion product otoacoustic emissions(DPOAE) and auditory brainstem respons(ABR) in high risk neonates with.Methods DPOAE and ABR were recorded with amplaid MK 22 and GSI60 in 200(400ears) high risk neonates, then compared the results.Results Of 200(400ears) newborns, the presence of DPOAE was 64%(256/400 ears), the presence of ABR was 88.25%(353/400 ears). The co-negativity and co-positivity of DPOAE and ABR were 61.50%(246/400 ears)and 9.25%(37/400 ears). The false negative and false positive of DPOAE were 3.91%(10/256 ears)and 74.31%(107/144 ears).Conclusion The DPOAE test is simple,quick,harmless,sensitive and objective. Because it only reflects the function of the cochlea and has high false positive, we can't make the diagnosis of hearing loss immediately when newborns don't pass the DPOAE test.The ABR test is a reliable method for neonatal hearing screening. The combination of DPOAE and ABR in high risk neonatal hearing screening could increase the accuracy and reliability of the test and retest results.
