ABSTRACT

We report a case of qi-deficiency fever that was successfully treated with hochuekkito. A 26-year-old woman presented to our clinic with a complaint of pain of her fingers. Clinical findings revealed no evidence of rheumatoid arthritis. Since she wanted treatment for the recent mild fever, additional examination was performed. Infection, malignant tumor, and collagen disease were ruled out as probable cause of the fever. Clinically, she was diagnosed with qi-deficiency fever. After treatment with hochuekkito, her body temperature normalized, indicating that treatment with hochuekkito was effective. Hochuekkito was originally prescribed for treatment of qi deficiency due to yin fire ; however, it is currently used for treatment of other diseases. In this report, we described a case using hochuekkito.

ABSTRACT

We herein report a case of 91-year-old woman with antiresorptive agent-related osteonecrosis of the jaw (ARONJ) developed during treatment for osteoporosis with intravenous ibandronate. Although she was treated with several antibiotics and had an incision for the drainage of a pus discharge at a dental clinic for two months after the onset of ARONJ, the discharge persisted. We then added hochuekkito to her treatment, which resulted in a gradual decreased in pus discharge and decrease in wound size and the wound resolved after approximately one month. ARONJ is considered a refractory disease, however, it was successfully treated with hochuekkito in this patient. ARONJ is also a rare condition and to our knowledge, this is considered to be the first case of ARONJ, which has been successfully treated with Kampo medicine.

ABSTRACT

The number of patients with taste disorder and halitosis has recently increased in Japan. Some of these patients have no abnormalities identified in blood tests and no apparent dental problems, so they are diagnosed with unidentified complaints. In this paper, we report 2 patients who were diagnosed with psychogenic taste disorder and pseudohalitosis. Kampo medical examination found their symptoms to be related to qi deficiency. They were prescribed 5g of hochuekkito extract per day, and their symptoms were relieved. These results suggest that hochuekkito is effective against unidentified complaints such as psychogenic taste disorder and pseudohalitosis.

ABSTRACT

We administered Japanese Kampo medicine for seven patients with Grade 3 or more idiopathic sensorineural hearing loss who could not be administered enough amounts of steroids for some reasons. They all exhibited deficiency patterns, and which were all dual deficiency patterns of qi and yin. All patients were administered hochuekkito and hachimijiogan. Six patients were cured, and one patient was recovered.

ABSTRACT

Teikosan is an herbal medicine that was developed from the Japanese herbal (Kampo) medicine hochuekkito. Unlike hochuekkito, teikosan does not include Zizyphi fructus and Zingiberis rhizoma. Instead, Cnidii rhizoma, Scutellariae radix, Coptidis rhizoma, Angelicae dahuricae radix, and shakusekishi are included as constituents of teikosan. The clinical indication of teikosan is anal prolapse with pain that is more severe than that considered appropriate for hochuekkito treatment, but there have been few clinical reports before. Here, we describe a case of anal prolapse with anal pain, which was successfully treated with teikosan. A case was a 47-year-old woman whose chief complaint was anal prolapse of internal hemorrhoids with pain. She was considered as indication for hochuekkito, but her symptoms including anal pain were so severe that we prescribed teikosan, especially because Angelicae dahuricae radix, Scutellariae radix, Coptidis rhizome in teikosan were expected to improve her anal pain. After the administration of teikosan, her symptoms including anal pain have much improved. Teikosan should be considered as a therapeutic option for anal prolapse with severe anal pain or inflammation.

ABSTRACT

We experienced a case of paranoid schizophrenia whose negative symptoms such as fatigue and depression were successfully treated with kamishoyosan and hochuekkito. The combination of medical herbal extracts did not affect positive symptoms. Herbal medicine may play not only a supporting role in treating mental disorders in general but also an important role in improving negative symptoms of schizophrenia.

ABSTRACT

A 31-year-old woman started to have a yellowish-white scab with exfoliation on her lip after delivery. Her exfoliative cheilitis did not improve with kigikenchuto nor juzentaihoto for treating deficiency of vital energy and lack of blood. After she developed upset stomach, her prescription was changed to hochuekkito which supports the digestive system. Her symptom improved dramatically. The characteristic of exfoliative cheilitis consists of chronic inflammation of lips which is considered to be yang-syndrome, therefore hochuekkito was effective as it covers yang-syndrome as well as yin-syndrome. It is suggested that psychological factors play a role for a pathogenesis of exfoliative cheilitis. We consider that antistress effect of bupleurum root and sedative effect of citrus unshiu peel in hochuekkito might also be key factors.

ABSTRACT

<p>Objective:The aim of this study was to examine the efficacy and safety of hochuekkito to address reduced activities of daily living (ADL), nutritional status, and immunity in patients with severe conditions, represented by a functional independence measure (FIM) total score of ≤ 40.</p><p>Methods:Thirty-one patients who were undergoing rehabilitation for hemiplegia after cerebrovascular disease were randomized into 2 groups:those treated with hochuekkito (TJ-41 group) and those treated without hochuekkito (control group). Their conditions were observed for 24 weeks, focusing on items such as ADL and incidence of inflammatory complications.</p><p>Results:The FIM total score markedly improved after treatment in both groups, but the changes in the score between admission and discharge were similar between the two groups. The incidence of inflammatory complications was significantly lower in the TJ-41 group (P = 0.049). Among the patients with a motor FIM score of ≤ 20, those in the TJ-41 group showed a tendency of increase in total lymphocyte count after treatment. No adverse drug reactions were observed during the study period.</p><p>Conclusion:These results suggest that hochuekkito is effective in reducing the incidence of inflammatory complications in patients undergoing rehabilitation for cerebrovascular or other diseases.</p>

ABSTRACT

We present seven cases of insomnia successfully treated with hochuekkito. Two patients showed improvement of their insomnia after taking hochuekkito before going to bed, and three patients showed improvement after taking hochuekkito twice per day. The other two patients could sleep better after adding hochuekkito to other Kampo formulations. All these patients were light sleepers, and became easily tired, excessive sleepy after meals, and had daytime sleepiness. However, they had no gastrointestinal symptoms, such as appetite loss. Five of the seven patients reported waking up feeling better after taking hochuekkito. Two other Kampo formulations, sansoninto and kihito, were also given to patients with deficient constitution, who complained of insomnia. Sansoninto and kihito are formulae that compensate for qui and blood deficiency. Kihito contains more herbs with beneficial effects on “spleen and stomach”, and “heart” functions more than sansoninto, and therefore, kihito may be preferred for patients with a more deficient constitution. The reason why our patients were able to sleep more deeply and wake up smoothly with hochuekkito may be that they exhibited remarkable qui deficiency, showing general fatigue, excessive sleepiness after meals, and daytime sleepiness, but without the symptoms of blood deficiency, such as palpitations or uneasiness, being easily frightened or forgetful, or showing anemia or bleeding.

ABSTRACT

A main symptom of chronic fatigue syndrome (CFS) is fatigue which continues for more than 6 months and does not improve with rest, leading to a marked decrease in quality of life. Other problems include fever, sleep disorder, and headache. Etiologically, a preceding viral infection, immunological disorder, and changes in the central nervous system, especially in hippocampus have been reported. However, the precise pathogenesis of CFS has not been elucidated, and neither diagnostic markers nor effective treatments have yet to be discovered. Here, we report a case of CFS, successfully treated with Kampo medicines.<br>The patient was a 16-year-old high school student, who had received medical therapy for one year under the diagnosis of CFS. His chief complaint was a continuing fever and strong malaise after influenza infection. At his visit to our outpatient clinic, we confirmed that his symptoms met the criteria for CFS. A combination treatment with Kampo medicine (sanoshashinto) and duloxetine improved his malaise and fatigue in 4 weeks, but fever and anorexia remained. The addition of hochuekkito to the combination dramatically ameliorated his symptoms. This case suggests that Kampo medicines would contribute to the effective treatment of CFS, which is refractory to Western medicines.

ABSTRACT

The purpose of this study is to evaluate the efficacy of hochuekkito for late-onset hypogonadism (LOH) patients. We administered hochuekkito 7.5 g/day for 8 weeks to 47 patients with LOH whose AMS scale was more than 27. We assessed the patients' symptom change with the AMS, SHIM, SDS, BDI, and SF-36. We measured their endocrine profiles and levels of their cytokines. At the end of study, 31 of 47 patients were evaluable. No significant difference in subjective symptoms was seen with any questionnaire after 8 weeks hochuekkito administration. However, hochuekkito significantly increased free testosterone and decreased ACTH/cortisol levels. Thus we believe hochuekkito is beneficial for the treatment of LOH.

ABSTRACT

Continuous suppressive antiviral medication for the treatment of repeated genital herpes is often carried out, but few treatments with Kampo have been reported. Here, we report experience with a case in which the repeated recurrence of genital herpes was treated successfully with hochuekkito.<br>A 34-year-old woman had experienced pregnancy twice and delivered twice. She had also had repeated recurrences of genital herpes from several years earlier, and been treated at a nearby doctor. Her recurring herpes did not resolve, however, and she visited our department. In addition to these recurrences, general fatigue and appetite loss were observed, which were regarded as Qi deficiency, and she was started on hochuekkito 7.5 g/day. The general fatigue disappeared two weeks after the treatment. Herpes recurrences also decreased gradually, and symptoms were no longer observed after medication was stopped less than a year later.<br>In this case, genital herpes recurrences were decreased, while symptoms such as general fatigue and appetite loss were also improved. Here, genital herpes recurrence was regarded as Qi deficiency, which was possible to treat with hochuekkito.

ABSTRACT

It is difficult to treat arthralgia of unknown cause. We report a case of polyarthralgia treated effectively with hochuekkito.<br>The case was a 48 year-old woman who had polyarthralgia for three months. Her joints were not swollen and reddish. There were no remarkable signs of rheumatoid arthritis or any other immune diseases. She also complained of fatigability. She was diagnosed as patterns of qi deficiency and qi stagnation. After prescribing hochuekkito for fatigability, her arthralgia as well as fatigability substantially decreased. In this case, it is suggested that hochuekkito was effective as a qi-tonifying formulation.

ABSTRACT

We report a case of pneumothorax successfully treated with hochuekkito.<br>The patient was a 56-year-old female, who was diagnosed with right-sided pneumothorax, and who improved after resting cure. However, her condition did not completely resolve. About three years later, we prescribed hochuekkito because her pneumothorax became worse with general fatigue. Five days after taking hochuekkito, the pneumothorax improved in her chest X-ray. But it became worse again, and finally she underwent an endoscopic operation. This case suggests the possibility that hochuekkito can improve pneumothorax temporally. There is no literature on Kampo therapy as it applies to pneumothorax, and we believe this case to be the first report of value on the subject.

ABSTRACT

We experienced 2 cases in which Kampo (Chinese medicine) treatment was effective for septic osteoarthritis. Case 1 : a 34-year-old female. Septic coxarthritis developed with no left hip joint abnormality being noted, while DIC (disseminated intravascular coagulation) from MRSA pneumonia was treated. Surgeries were carried out five times, but there was no wound closure, and exudate discharge continued. At first, she was treated with Hochuekkito, did not run a fever, and good granulation tissue was formed in the wound. After a change to Senkinnaitakusan, exudate quantity decreased. Moreover, after an external fixation operation and being treated with Juzentaihoto, epithelization progressed, and the wound eventually closed. Case 2 : a 79-year-old female. One year and 6 months after osteosynthesis with compression hip screw was carried out for a femoral neck fracture, operation scar complications and large quantities of exudate were seen, so we diagnosed her with late onset septic osteomyelitis. She gained weight with Juzentaihoto treatment, while a decrease in exudates and fistula closure with Astragali Radix and Ginseng Radix were confirmed by MRI, and her nutritional state improved. Generally speaking, septic arthritis and osteomyelitis are difficult to treat, but Kampo medicines were curatively effective for these diseases, particularly when increasing Astragali Radix and Ginseng Radix quantity.

ABSTRACT

Patulous Eustachian tube (patulous ET) is not a rare disease, for which the prevalence rate is considered to be higher than 5%. Autophonia and ear fullness are common symptoms of patulous ET. Various treatments in cluding drug therapies and surgical procedures are attempted, although they do not give satisfactory results.<br>In oriental medicine, patulous ET is considered a Qui deficiency and/or blood deficiency. And Kamikihito is known as a popular Kampo prescription for patulous ET. Here we report the effect of hochuekkito on Patulous ET. Hochuekkito was administered to 10 patients, and complete improvement was had in 4 cases, partial im provement in 1 case and 5 cases had no change. Elevated ET tonus, gains in fatty tissue surrounding the ET, and influence on psychogenic factors were speculated as mechanisms by which hochuekkito brought about its effect.<br>Through this report, we expect that hochuekkito can be an alternative treatment for patulous ET.

Aim : Hochuekkito is a Japanese herbal medicine that is known to be useful for the treatment of uterine prolapse and urinary incontinence. In this study, we investigated the effects of hochuekkito prescriptions in female patients with stress urinary incontinence (SUI).Methods : 13 women were enrolled in this study. They each took 7.5 g of hochuekkito powder prior to a meal three times a day, for 4 weeks. The effects were evaluated before and after treatment with a visual analogue scale (VAS), a incontinence-quality-of-life index (IQOL), and voiding diaries. Moreover, pad tests and uroflowmetry measures were done before and after treatment.Results : VAS and IQOL scores were not improved significantly. Although the mean number of SUI episodes per one day, and the number of pads used decreased after treatment as compared to before, these were not significantly different. Micturition frequency and uroflowmetry parameters did not change after treatment. However, in patients whose Body Mass Index (BMI) was < 25, subjective symptoms significantly improved after treatment. No side effects were observed in this study.Conclusion : Hochuekkito was a useful treatment for slender female patients with SUI.

ABSTRACT

It is essential to prolong the term as possible in the treatment of threatened premature delivery. We report a case of threatened abortion successfully treated with hochuekkito. The patient was 31 year-old pregnant woman with lower abdominal pain. She was diagnosed to be at the risk of premature delivery on 21 weeks and 5 days of gestation. Intravenous ritodrine hydrochloride was started and she had to stay in bed. Severe side effects of ritodrine hydrochloride such as palpitation, tachycardia, tremor, nausea, and loss of appetite were appeared and she was consulted to our department on 23 weeks and 1 day of gestation. Those symptoms were markedly improved after administration of hochuekkito extract. She delivered a male infant of 1230g birth weight on 28 weeks of gestation. Hochuekkito suppressed severe side effects, and enabled to continue the infusion of ritodrine hydrochloride, suggesting the usefulness of Kampo therapy in the treatment of threatened premature delivery.

ABSTRACT

Female organ ptosis, especially uterine prolapse, is a delibilitative, dysfunctional condition of the supporting pelvic floor system of ligaments, connective tissues and muscles which manifests during pregnancy, delivery, or post-menopause. The Japanese Kampo herbal formula “hochuekkito” has been reportedly applied to uterine prolapse based on its putative ability to rejuvenate body vitality (Qi) and raise declined Qi. We assessed the efficacy of hochuekkito for uterine prolapse based on observations of its effect on clinical and objective symptoms. We administered hochuekkito to 17 patients (62.6 ± 7.1years) diagnosed with modern techniques as having uterine prolapse. The results showed that it was very effective in 6 cases (35.7%), effective in 9 cases (52.9%), and ineffective in 2 cases (11.8%). 10 patients with pessaries were all found to have been treated effectively (very effective : 4, effective : 6). In 6 patients whose compliance was below 90%, the percentage of very effective cases was 75%. The effectiveness score of hochuekkito with pelvic exercise (1.5 ± 0.5) and with pessaries (1.4 ± 0.5) was significantly higher than that with hochuekkito alone (0.8 ± 0.8) (p < 0.05). Finally, for uterine prolapse and bladder prolapse, the addition of pelvic exercise and pessaries to hochuekkito brought about more prompt improvement than that with hochuekkito alone. The increased efficacy with the addition of pelvic exercise or a pessary also helped strengthen atonic pelvic muscles, thus synergistically working together with the effects of hochuekkito.
