ABSTRACT

Introduction: The purpose of this study was to clarify the relationship between the available service information on small-scale multifunctional in-home nursing care (KANTAKI) and its operational status via text mining.Methods: We obtained nationwide textual information on KANTAKI from the Nursing Care Service Information Disclosure System and the Ministry of Health, Labour, and Welfare, and analyzed the characteristics of the word usage using KH Coder. The number of users and employees and the implementation of services were compared among the facilities that used terms relating to medical dependency and end-of-life care, which are KANTAKI characteristics, and with other facilities.Results: The facilities that used terms relating to medical dependency and end-of-life care showed significantly more users requiring nursing care level 5 and more full-time nursing staff than those not using such terms. Moreover, regarding service provision, the rate of procedures was significantly higher in 11 of 12 items, except for stoma.Conclusion: The facilities that use terms relating to medical dependency and end-of-life care in their information have more users and provide a greater variety of services. In the future, educational support is required to enable facility managers to understand the services and translate them into their work.

ABSTRACT

Introduction: This study examined the association between the use of home nursing care and the consultation time of emergency home visits among patients using home health care. Methods: Participants were recruited from three urban clinics in Japan between September 1 and November 30, 2019. Univariate analysis and multivariate negative binomial regression analysis considering institutional clusters were performed on the relationship between the two. Results: A total of 278 patients were included in the analyses. The use of home nursing care was significantly associated with a decrease in the time spent during emergency home visits in both univariate and multivariate analyses (P < 0.018 and P < 0.001, respectively). The multivariate analysis estimated the mean reduction in consultation time to be 10.3 minutes (95% confidence interval, 9.9-10.8). Conclusion: The use of home nursing care reduced the consultation time in emergency home visits. This study suggested that home nursing care can reduce the burden on physicians providing home visits, but further studies are needed to improve collaboration.

É importante almejar qualidade de vida ou bem-viver aos idosos que já atingiram 80 ou mais anos de idade, com autonomia e independência. Com o avançar da idade, entretanto, torna-se mais frequente a dependência dos cuidados da família por parte dos idosos. OBJETIVO: Avaliar a dinâmica da família, a qualidade e o estilo de vida de idosos da quarta idade e de seus familiares cuidadores. Método: Estudo descritivo do qual participaram 100 idosos e seus cuidadores. Os dados foram coletados por meio do Questionário do Perfil da Família Cuidadora, o Family APGAR, Whoqol-Old, Whoqol-Breve e o Estilo de vida de Nahas. RESULTADO: A amostra de idosos caracterizou-se por ser a maioria mulher, com idade média de 84 anos, portadora de hipertensão arterial e cardiopatias. Os cuidadores (67%) viviam na casa dos próprios idosos, eram predominantemente mulheres, entre filhas e netas, sendo 20% idosos cônjuges de ambos os sexos. Embora os cuidadores tenham referido boa saúde e qualidade de vida, seu estilo de vida atingiu nível regular. Na ótica dos idosos, a família apresenta alta disfuncionalidade, embora tenham uma melhor qualidade de vida em alguns domínios e facetas. CONCLUSÃO: Com base na pesquisa, foi possível identificar a necessidade de redirecionar as políticas públicas e sociais a favor da rede de apoio à população idosa mais envelhecida, extensiva às famílias cuidadoras.


It is important to get the quality of life or well being to the elderly who reached the fourth age - 80 years or more - with autonomy and independence. However, as people grow old, reliance on family care for the elderly becomes more frequent. OBJETIVE: This study aimed to assess family dynamics, quality of life and lifestyle of elderly at the fourth age and of their family caregivers. Method: Descriptive study involving 100 elderly and their caregivers. Data were collected by applying the Family Caregiver Profile Questionnaire, Family APGAR, Whoqol-Old, Whoqol-Bref and Nahas Lifestyle. RESULTS: The elderly sample comprised mostly women, with mean age of 84 years, mostly of all with hypertension and heart diseases. Caregivers (67%) lived in the elderly's household, were mostly women, daughters and granddaughters; 20% were elderly caregivers partners from both sexes. Although caregivers have reported good health and quality of life at a good level, their lifestyle was regular. According to the elderly, families have high dysfunctionality, although they had better levels of quality life in some areas and facets. CONCLUSION: Based on the research it was possible to identify the needs of redirecting to the public and social policies, in favor of a supporting network to the aged population and their family.

ABSTRACT

Introduction: Today, Health care nursing is dominating only at the hospital but it is deficient at home. Family doctors and nurses actively move around families and receive home calls at the clinic, make assistances but services to prevent from illness, to issue health education and to provide home health care nursing, can’t provide current requirements. Thus, we have chosen this title in order to evaluate health status of citizens who are living in the some district of Ulaanbaatar city and to define demands of home health care nursing.Goal: To search population health who are being served at the some Family Clinic and home health care nursing in Ulaanbaatar city.Оbjectives:1. To make compared research to health status of families which are living in apartments and ger districts.2. To identify faced problems to population health status3. To define needs of people who want home health care nursingMaterials and Methods: We used descriptive model in our research work. We included total 670 people from 156 families which are served of Ulaanbaatar city. We used SPSS 17 software to the processing. Results: 1178 residents of 266 households live in the territory of Songinokhairkhan district, 7th khoroo, and Chingeltei district are studied in our survey. 21% of them stand for 21-30 years old and a minimum amount or 2% of them stand for the people aged more than 71 years old only. 11.7% of all people who included to the research needs to get home health care nursing. Also we considered illnesses of people who have home cares and 1% has gastric ulcer, 6% have paralysis due to hemorrhage in the brain, 1% have cardiovascular disease and 1% has tumor. 28 families or 16.5% offered to move around families to examine aged people, infants and cases, 12.2% offered to give an advice about preventing from illness. 5,7% offered to provide home health care nursing to people who have grave illness and to promptly arrive on home calls.Conclusions:1. 45.5% of families who live in apartments and 70%of families who live in ger districts considered that they have problems in relation to health during the one year and hence 85% of them feel respiratory diseases and it is related to air pollution near to the city.2. The people who live in the ger districts and apartments hypertension and cardiovascular diseases are occupying major percent and it is attracted our attentions.3. 11.7% of families who live in ger districts at our research, need to get home health care nursing. Also we searched knowledge level of nurses for people who have home health care and 87.6% evaluated that they have medium or less knowledge.

ABSTRACT

PURPOSE: The purpose of this study was to compare the costs and benefits of home nursing care services between public health centers (PHC) and private hospitals. METHOD: Participants were 105 patients who had received home nursing care services from a private hospital or public health center. From a societal perspective, the researcher identified the costs and benefits of the services using performance data and calculated the net benefit and benefit/cost ratio. RESULT: The net benefit of the home nursing care service based in the PHC was 165.9 million won and benefit/cost ratio was 2.0, while the net benefit of the home nursing care services by the private hospital was 141.1 million won and benefit/cost ratio was 1.7. Both types of programs were economically validated. CONCLUSION: Home nursing care services were basically efficient as the results showed a positive net benefit. A cost-benefit analysis indicated that the PHC-based home nursing care services were more efficient than that of the private hospital. With limited human resources and management standards in public health centers, results suggest the need for a more systematic management of the home nursing care service to improve the health of this vulnerable community population.

ABSTRACT

1) A questionnaire survey was performed concerning at-home palliative care.A total of788responses were received from medical personnel and members of the general public.<BR>2) The results clearly showed that medical personnel had a lower-than-expected awareness of palliative care. However, 25%of the respondents answered that palliative care could be performed in a home-care setting.<BR>3) Further improvements in the education of medical personnel about palliative care are essential to ensure that patients and their families will be offered palliative care.

ABSTRACT

PURPOSE: The purpose of this study was to analyze functional independence and need for home nursing care in stroke patients. METHOD: This was a descriptive study. The subjects comprised 117 stroke patients who were supposed to discharge in less than a week. The instruments used for this study were the functional independence measure (FIM) and the need assessment for home nursing care of stroke patients. The data were analyzed using frequency, mean, t-test, ANOVA, and Pearson correlation coefficient. RESULTS: Pearson correlation analysis revealed that there were negative correlations between subcategories of the FIM and the need for home nursing care in stroke patients. Especially, in case of the subjects who recorded lower scores at self-care they showed higher needs for home nursing care in the domain of physical problem and rehabilitation. CONCLUSION: For the operation of the home nursing care, the protocol for home nursing care is needed to the stroke patients living at home. The FIM instrument is recommended as a useful scale in order to assess the disability for the stroke patients and the need for home nursing care because this one has correlation with the scale of need for home nursing care.

ABSTRACT

PURPOSE: The purpose of this study was to understand the meaning of experiences in home visiting nursing service among the elderly in community provided by nursing students. METHOD: This is a phenomenological study to investigate the experience of community-based patients. Data were collected through in-depth interviews from the 4th of March to the 15th of December 2003. Individual interviews were conducted with 8 patients on subjective experiences. Data were analyzed through Colaizzi's method, in which meaningful statements were extracted. RESULT: Four categories were identified from nine theme clusters. The four categories were 'thankfulness', 'dependence', 'sorrowfulness' and 'the sense of being'. CONCLUSION: These results show the importance of visiting nursing service for the elderly in community.

ABSTRACT

PURPOSE: The purpose of study is to develop a home nursing care project model suitable for Korea's heath care system and policy directions for expansion and establishment of home nursing care. METHOD: The first, status of home nursing care program in medical institution and public health center was evaluated respectively in view of structure, process and performance. And then, issues of the program were analyzed in view of accessibility, safety, and sufficient supply. The second, demand projection of home nursing care according to income level and technical level of service that is needed to the subject was tried. In addition, the level of supply for home nursing care at present was estimated. The third, home nursing care program in advanced countries (Japan and U.S.A.) was examined. RESULT AND CONCLUSION: Community-based home nursing care program is developed into public-operation model and private-operation model from above the sub-subject result. Functional network for referral system among related institutions is built up to meet various needs, regardless of accessibility to distance and economy. And prior settlements and policy directions for expansion and establishment of home nursing care are suggested.
