ABSTRACT

Objective:To explore the effect of home-based rehabilitation training on a patient with posterior shoulder dislocation with reverse Hill-Sacks lesion. Methods:According to the preoperative and postoperative evaluation and the operation way, a four-phase home-based rehabilitation training plan was developed for the patient, which included the training of muscle strength and range of motion, and proprioceptive sensation training and so on. The patient was followed up in rehabilitation department regularly. The range of motion was measured, and were also evaluated with Manual Muscle Test, Visual Analogue Score (VAS) and modified Rowe Score. Results:Six months after operation, the active range of motion of flexion, extension, adduction, abduction and external rotation of his shoulder reached to the normal range, the active internal rotation reached to level L3, the muscle strength recovered to the normal level, the VAS decreased from six preoperative to 0 postoperative, and the modified Rowe Score improved from 16 preoperative to 91. Ten months after operation, his active internal rotation reached to level L1, and the modified Rowe Score was 93. Conclusion:Individualized remote guidance and reasonable home-based rehabilitation training might be effective on posterior shoulder dislocation with reverse Hill-Sacks lesion.

ABSTRACT

Home-based rehabilitation can effectively reduce the clinical complications of patients with spinal cord injury and accelerate the recovery of neurological function, as well as reduce the heavy economic burden on healthcare system. Home-based rehabilitation has shown effectiveness in multiple studies. Neurofeedback could relieve the neuropathic pain of 40% home patients with spinal cord injury. Wearable technology could monitor and promote the recovery of hand motor function. Household portable stimulators and surface electrodes could inhibit unnecessary bladder activity and improve urinary incontinence. Strengthening stretching training at home could reduce shoulder pain in patients with chronic spinal cord injury. Using functional electrical stimulation at home could improve the muscle function of patients with spinal cord injury due to loss of neurotrophic muscle atrophy. There are still some disadvantages of home-based rehabilitation, such as complicated operation, expensiveness and not convenient to carry for training equipment, which need to be further developed.

ABSTRACT

Objective:To explore the effects of home-based telerehabilitation guidance on physical function and activities of daily living for stroke patients. Methods:From September, 2015 to March, 2018, 240 stroke recovering patients from three community health service centers in Fengtai District, Beijing for outpatient rehabilitation were randomly divided into control group (n = 120) and tele-rehab group (n = 120). Both groups accepted routine rehabilitation for six months, while the tele-rehab group received telerehabilitation guidance in addition. They were assessed with Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI) before and after treatment. Results:The scores of FMA and MBI improved after treatment in both groups (|t| > 14.536, P < 0.001), and improved more in the tele-rehab group than in the control group (|t| > 2.200, P < 0.05). Conclusion:Home-based telerehabilitation guidance may enhance the effect of community-based outpatient rehabilitation for stroke patients.

ABSTRACT

Objective:To explore the effect of home-based rehabilitation on children with congenital muscular torticollis (CMT). Methods:From January, 2016 to January, 2018, 60 children with CMT were randomly divided in to control group (n = 30) and observation group (n = 30). Both groups received comprehensive physical therapy; the observation group received home-based rehabilitation in addition. Three months after treatment, the sternocleidomastoid thickness, severity classification of CMT, and clinic effect were compared. Results:After treatment, the sternocleidomastoid thickness decreased in both groups (t > 9.862, P < 0.001), and was better in the observation group than in the control group (t = 5.468, P < 0.001); the severity classification of CMT decreased in both groups (|Z| > 5.857, P < 0.001), and was better in the observation group than in the control group (Z = -2.226, P < 0.05), as well as the clinical effect (U = 2359, P = 0.018). Conclusion:Home-based rehabilitation combined with comprehensive physical therapy is superior to comprehensive physical therapy only.

ABSTRACT

@#Objective To observe the effect of comprehensive rehabilitation combined with home-based direction on spastic cerebral palsy.Methods 42 children with spastic cerebral palsy were divided into treatment group (n=21) and control group (n=21). The treatment group accepted comprehensive rehabilitation and home adjusted. Their parents participated in the rehabilitation and received home-based direction.The control group resigned the rehabilitation for poverty or other reasons, without home adjusted. They were assessed with Gross Motor Function Measure (GMFM-88) and Activities of Daily Living of Cerebral Palsy before and 6 months after treatment. Results The scores of GMFM-88 and Activities of Daily Living of Cerebral Palsy improved in the treatment group after treatment (P<0.05), and improved more in the treatment group than in the control group (P<0.05). Conclusion Rehabilitation training combined with home-based direction could improve the gross motor function and activities of daily living in children with spastic cerebral palsy.

ABSTRACT

BACKGROUND: We were interested in getting feedback from physical therapists practicing in individualized home health programs about the present situation and the problems of home-based rehabilitation therapy services. METHODS: We recruited 110 physical therapists practicing in individualized home-visiting health programs offered at health centers. They each completed a 15-item self-administered questionnaire including personal information, therapy environments, characteristics of patients, treatment outcomes and suggestions. RESULTS: Of the 110 respondents, 66 (60%) were engaged in home-based rehabilitation therapy services with other duties. Sixty (54.5%) indicated that >25% of patients improved with home-based therapy and 90% of patients were satisfied with the services they received. Eighty-three (75.5%) respondents indicated that the amount of therapy provided was insufficient, 82 (74.5%) thought that there were no measurement tools to use except for the rehabilitation therapy services records and 27 (24.6%) had evaluation meetings with other medical service members for >50% of their patients. CONCLUSION: The majority of the physical therapists in our study indicated that the home-based rehabilitation therapy services satisfied the needs of patients and improved their functional abilities. However, amount of therapy, use of measurement tools for outcome evaluations and comprehensive team approach were indicated as insufficient. We conclude that increasing the number of physical therapists and systematizing the home-based rehabilitation therapy services would improve the present situation.

ABSTRACT

Due to the limitation of length of stay, hospital costs, medical conditions, and other restrictions, the time in the hospital for rehabilitation therapy is limited for patients with stroke in the convalescent stage. They need to continue rehabilitation treatment at home after being discharged from hospital. The provision of rehabilitation for stroke patients is one of professional courses in community care. Community nurses guide for home-based rehabilitation through a variety of guidance methods plays a positive role in improving the quality of life of the patients.

ABSTRACT

Objective To investigate the effects of ultrashortwave therapy associated with home-based rehabilitation on lung function and quality of life (QOL) in chronic obstructive pulmonary disease (COPD) patients. Methods Ninety patients with stable COPD were equally and randomly divided into a combination group (received uhrashortwave therapy combined with home-based rehabilitation and regular treatment), a rehabilitation group (received home-based rehabilitation and regular treatment), and a control group ( received regular treatment). Spirometry and diaphragm function were measured, and St George's respiratory questionnaire (SGRQ) was administered for QOL assessment at the beginning and after 6-month of treatment. Results FEVI% pred and FEV1% increased and average SGRQ scores decreased in the combination group and rehabilitation group. FEVI% pred and SGRQ scores improved most in combination group (all P < 0.05 ). FEV1% pred, FEV1 % and SGRQ scores in control group did not show any obvious change. Diaphragm function in all groups did not change significantly. Conclusions Ultra-shortwave therapy combined with home-based rehabilitation might have combinative effects in improving lung function and QOL of COPD patients.

ABSTRACT

@#Objective To explore the effect of home-based rehabilitation on cerebral palsy.Methods 60 children with cerebral palsy were assigned to hospital-based rehabilitation group and hospital-home-based rehabilitation group and treated for 1 year.Every child was evaluated before and after the treatment with Bayley Scales of Infant Development(BSID).The Mental Development Index(MDI) and Physical Development Index(PDI) were compared between these two groups.Results After one year,the MDI and PDI improved in both groups(P<0.05),while those of the hospital-home-based rehabilitation group was better than of the hospital-based rehabilitation group(P<0.05).Conclusion Home-based rehabilitation plays an important role in the rehabilitation of cerebral palsy children.
