ABSTRACT

Objective To analyze the growth and development status of children with severe β-thalassemia major (β-TM) in Bazhong area and its correlation with hemoglobin concentration (HGB) and serum ferritin (SF) level, and to provide a theoretical basis for the diagnosis and treatment of children with β-TM in Bazhong area. Methods A total of 292 children with β-TM admitted to Bazhong Central Hospital from January 2019 to December 2020 were selected. The Z-score method was used to evaluate the growth and development of the children. According to the growth and development of the children, they were divided into the normal group (normal growth and development, n=163) and delayed group (growth and development delay, n=129). Another 60 healthy children were selected as the control group. The levels of HGB, serum SF, free triiodothyronine (FT3), free tetraiodothyroxine (FT4), and thyroid stimulating hormone (TSH) were compared among the three groups of children, and clinical data such as age, sex and age of onset were collected from the case system. Univariate analysis and logistic regression were used to analyze the independent risk factors of growth and development delay in β-TM children. Pearson correlation was used to analyze the correlation between growth retardation and HGB and serum SF levels in β-TM children. Results The serum SF and TSH levels in the delayed group were significantly higher than those in the normal group and the control group, while the levels of HGB and serum FT4 were significantly lower than those in the normal group and control group (P2000 ng/mL were risk factors for growth retardation in β-TM children (P2000 ng/mL, the height, weight, and HGB and serum SF levels should be monitored regularly.

ABSTRACT

Background: One of the most common endocrine disease is hypothyroidism which is usually associated with mental retardation, lack of concentration, motor dysfunction, memory deficits, visual, and hearing impairment. These symptoms suggest the involvement of nervous system, where the processing efficiency and sensory motor association is affected, which can be assessed by reaction time. Aims and Objective: The aim of the study was to compare the auditory reaction time in controls, newly diagnosed hypothyroid patients and in hypothyroid patients after attaining euthyroid status. Materials and Methods: Auditory reaction time was recorded from 35 subjects from general population, who formed the control group and 35 newly diagnosed hypothyroid patients who were selected from OBG and Medicine department, RRMCH, Bangalore. These patients were followed up and reaction time recording was repeated after they attained euthyroid status. Results: Statistically significant increase in ART values was observed in newly diagnosed hypothyroid patients compared to controls. The values were significantly reduced in hypothyroid patients after the treatment on attaining euthyroid status. There was no significant difference in both ART values between controls and hypothyroid patients after the treatment on attaining euthyroid status. Conclusion: The present study showed a significantly prolonged auditory reaction time in hypothyroid patients which improved with treatment. We also demonstrated a positive correlation between ART and thyroid stimulating hormone levels. Hence, we conclude that auditory reaction time tests could be added to the routine thyroid dysfunction tests to know the early recognition of the neuronal involvement, prompt treatment, and assess prognosis of hypothyroid patients.Department of Obstetrics and Gynaecology, GMERS Medical College, Gandhinagar, Gujarat, India

ABSTRACT

Objective To observe the correlation of mood changes and serum levels of thyroid hormone in patients with diabetes and depression mood before and after intensive insulin therapy. Methods 340 patients with type 2 diabetes (T2DM) were enrolled, of which 187 cases were given intensive insulin therapy for 3 months and the other 153 cases were treated with oral hypoglycemic. All patients received depression measurement and tests of blood glucose, glycated hemoglobin (HbA1c), serum thyroid hormones before and after treatment. The data were analyzed statistically. During the treatment, fasting plasma glucose was controlled in ( 4.4 ~ 6. 1 )mmoL/L and 2-hour postprandial plasma glucose was controlled in (4.4 ~ 8.0) mmol/L. Results Thyroid hormone levels in the diabetic patients with depression mood ( T4 ( 6. 1 ± 1.4 ) μg/dl, T3 ( 0.98 ± 0.26) ng/ml ) were all lower than that in without depression mood ( T4 ( 8.4 ± 1.7 ) μg/dl, T3 ( 1.51 ± 0.29 ) ng/ml ) (P < 0. 01 ), and TSH ( 4.5 ± 1. 1 ) were higher than that in without depression mood ( 1.9 ± 0.9 ) uIU/ml (P < 0.01 ) before insulin treatment. After insulin therapy depression degree( 0.51 ± 0.12) were decreased significantly (0.68 ± 0. 21 )(P<0.01 ) ,and thyroid hormone levels were significantly increased (T3 (1.38 ± 0. 28 )ng/ml,T4 (7.7 ±1.5 )μg/dl ),and TSH levels were significantly reduced( (2.1 ± 1.2 ) uIU/ml) (P < 0. 01 ) than that before insulin therapy in the patients combined with depression mood. Depression degree was significantly negative correlated with T4 and T3 ( r= - 0.468, - 0.511, P < 0.05 ) and significantly positive correlated with TSH ( r = 0.583, P< 0.01 ) before treatment. Decline of depression degree were significantly positive correlated with elevation of T3, T4 levels ( r =0. 395 ,0. 337, P<0. 05 ) and were significantly negative correlated with decrease of TSH levels( r= -0. 239, P<0.05 ) after insulin treatment. However, depression degree and thyroid hormone levels did not significantly change before and after in oral antihyperglycemic agents groups. Conclusion Improvement of depression was associated with thyroid hormone levels in type 2 diabetic patients after insulin therapy.

ABSTRACT

BACKGROUND: Success of transplantation is critically dependent upon the quality of the donor organ and optimal management. Recently, hormonal replacement therapy has been reported to result in rapid recovery of cardiac function and enable significantly more organs to be transplanted, while some other studies show conflicting results. The aim of this study is to comprehensively evaluate changes in basal circulating hormonal levels of the brain-dead organ donors. METHODS: We reviewed the records of all brain-dead patients between January, 2004, and June, 2007. Hemodynamic variables, plasma hormone levels were recorded at following time points: admission to the ICU (T1, baseline), 30 minutes (min) after first apnea test (T2), 30 min after second apnea test (T3), before operation for harvesting (T4). Hormonal measurements included cortisol, adrenocorticotrophic hormone, triiodothyronine (T(3)), thyroxine, free thyroxine, thyroid-stimulating hormone, growth hormone, and testosterone. RESULTS: Nineteen patients were included in this study. Comparisons of hemodynamic parameters and hormonal levels to baseline values revealed no significant changes throughout the study period. When the patients were divided into 2 groups according to the requirement of norepinephrine (either>0.05 or 0.05microgram/kg/min of norepinephrine had T(3) level below the normal range at significantly more time points of measurement (7 vs. 0). CONCLUSION: In this comprehensive assessment of hormonal levels in brain-dead organ donors, we could not observe any significant changes during the ICU stay. Replacement therapy of T(3) may be considered in patients requiring >0.05microgram/kg/min of norepinephrine.

ABSTRACT

Opioid peptides are implicated in the control of gonadotropin and prolactin secretion. The role of opioid antagonist naloxone and its effects on plasma gonadotropin, prolactin, testosterone levels and testicular hyaluronidase, acid phosphatase, [3H]uridine and thymidine incorporation, RNA, DNA and protein concentrations were evaluated in rats after administration of naloxone beginning day 1 through 21 and autopsied on 45, 60 and 90 days of age. Plasma gonadotropin and testosterone levels were significantly elevated after naloxone treatment. Testicular hyaluronidase and acid phosphatase activity increased till 60 days post treatment and declined thereafter. Concentrations of RNA and protein did not change significantly but the concentration of DNA declined at 45 and 60 days of age. These results suggest that endogenous opioid peptides exert regulatory influence on gonadotropin secretion which in turn control the testicular function in the male rat.
