ABSTRACT

SUMMARY: Parietal emissary foramina (PEF) are small holes, which are localized between the middle and posterior thirds of the parietal bone posterior surface close to the sagittal suture. PEF are important structures that protect the parietal emissary vein, which passes through it. During neurosurgery procedures, parietal foramina (PF) knowledge is crucial. This work aimed to evaluate presence and location of the PF in the skull of an adult human. Moreover, measure the distance amidst PF and the sagittal suture's midline to ascertain its clinical repercussions. 74 adult human skulls, without gross pathology, were observed for the PF's existence. The PF's and sagittal suture's midline distance were measured. According to the PF patterns of presence, five groups were distributed. Finally, specimens were photographed and subjected to statistical analysis. The PF was absent in 7 skulls (9.5 %). There were 9 skulls (12.2 %) exhibited central parietal foramen where the parietal foramen lies on the sagittal suture. 17 skulls (23 %) showed right unilateral parietal foramen, whereas 15 skulls (20.3 %) demonstrated left unilateral parietal foramen. The final 26 skulls (35.1 %) exhibited bilateral parietal foramen. This descriptive study supplies valuable information of PF variations, which is crucial for neurosurgeons in modifying surgical techniques and procedures to alleviate injury to PF-emerging structures such as emissary veins.


Los forámenes emisarios parietales (FEP) son pequeños orificios que se localizan entre los tercios medio y posterior de la superficie posterior del hueso parietal, cerca de la sutura sagital. Los FEP son estructuras importantes que protegen la vena emisaria parietal, que lo atraviesa. Durante los procedimientos de neurocirugía, el conocimiento de los forámenes parietales (FP) es crucial. Este trabajo tuvo como objetivo evaluar la presencia y ubicación del FP en el cráneo de hombres adultos, además, medir la distancia entre el FP y la línea mediana de la sutura sagital para conocer su repercusión clínica. Se examinaron 74 cráneos humanos adultos, sin patología grave, para determinar la existencia del FP. Se midió la distancia de la línea mediana de la sutura sagital y del FP. De acuerdo con los patrones de presencia del FP, se distribuyeron en cinco grupos. Finalmente, los especímenes fueron fotografiados y sometidos a análisis estadístico. El PF estaba ausente en 7 cráneos (9,5 %). Hubo 9 cráneos (12,2 %) que presentaban un PF central localizándose en la sutura sagital. 17 cráneos (23 %) presentaban un FP unilateral derecho, mientras que 15 cráneos (20,3 %) se observó un FP unilateral izquierdo. Los 26 cráneos restantes (35,1 %) exhibieron FP bilaterales. Este estudio descriptivo proporciona información valiosa sobre las variaciones del FP, que es fundamental para los neurocirujanos en el momento de modificar las técnicas y los procedimientos quirúrgicos para aliviar las lesiones de las estructuras emergentes del FP, como las venas emisarias.

SUMMARY: The foramen magnum is an important topographic opening which connects cranial cavity and spinal canal. The analysis of the bone material established that there are differences in the shape of the foramen magnum between individuals. The aim of this study was to determine sex based on shape and size of foramen magnum using geometric morphometrics method. A study was performed on three-dimensional models (3D models) of 214 human skulls of known sex and known age (141 male skulls and 73 female skulls). The skulls are located at the museum of Medical Faculty, University of Sarajevo. Skulls belong to Bosnian population from the mid-twentieth century. All examined skulls were scanned with a laser scanner to obtain their 3D models. On 3D models of the examined skulls, four landmarks were marked on foramen magnum. Analysis of sex determination was performed using the MorphoJ program. Results of this study showed that there are sex differences in the shape and size of the foramen magnum. Sex determination based on the shape and size of the foramen magnum was showed 65.25 % accuracy for male and 63.01 % accuracy for female using geometric morphometrics method. Examination of the effect of size of foramen magnum on sexual dimorphism of shape of foramen magnum showed a statistically significant effect. Sex determination based just on the shape of foramen magnum using geometric morphometrics method was possible with 62.41 % accuracy for male and 58.90 % accuracy for female on examined sample. Sex differences on shape and size of foramen magnum were found using geometric morphometrics method on three-dimensional models of the examined skulls. The percentage of accuracy was higher for male based on the shape and size of the foramen magnum than for female.


El foramen magno es una importante abertura topográfica que conecta la cavidad craneal y el canal espinal. El análisis del material óseo estableció que existen diferencias en la forma del foramen magno entre individuos. El objetivo de este estudio fue determinar el sexo en función de la forma y el tamaño del foramen magno utilizando morfometría geométrica. El estudio se realizó en modelos tridimensionales (modelos 3D) de 214 cráneos humanos de sexo y edad conocidos (141 cráneos masculinos y 73 cráneos femeninos). Los cráneos se encuentran en el museo de la Facultad de Medicina de la Universidad de Sarajevo. Los cráneos pertenecen a población bosnia de mediados del siglo XX. Todos los cráneos examinados fueron escaneados con un escáner láser para obtener sus modelos 3D. En los modelos 3D de los cráneos examinados, se marcaron cuatro puntos de referencia en el foramen magno. El análisis de determinación de sexo se realizó utilizando el programa MorphoJ. Los resultados de este estudio mostraron que existen diferencias de sexo en la forma y el tamaño del foramen magno. La determinación del sexo basada en la forma y el tamaño del foramen magno mostró una precisión del 65,25 % para los hombres y del 63,01 % para las mujeres utilizando morfometría geométrica. El examen del efecto del tamaño del foramen magno sobre el dimorfismo sexual de la forma del foramen magno mostró un efecto estadísticamente significativo. La determinación del sexo basada solo en la forma del foramen magno utilizando morfometría geométrica fue posible con una precisión del 62,41 % para los hombres y del 58,90 % para las mujeres en la muestra examinada. Se encontraron diferencias de sexo en la forma y el tamaño del foramen magno utilizando morfometría geométrica en modelos tridimensionales de los cráneos examinados. El porcentaje de precisión fue mayor para los hombres en función de la forma y el tamaño del foramen magno que para las mujeres.

ABSTRACT

Background: Understanding of thoroughly explained anatomical basis is must for various surgical approaches at the craniovertebral junction. High mortality and morbidity are anticipated during the surgical procedures when undertaken without in depth anatomical knowledge. With so much clinical importance in this area, our study will present a thorough understanding in terms of skull. Objectives: The aim of the present study is to evaluate the various shapes of foramen magnum in dry adult human skulls of Indian population and to find out their clinical correlation. Materials and methods: 347 dried adult human skull base obtained from the Department of Anatomy, Geetanjali Medical College and Hospital, Udaipur, Rajasthan and other medical colleges were used in the present study. All the dry adult human skulls were observed from outer side at their base by naked eyes to determine the shape of foramen magnum. It was classified into one of the following shapes: - Oval, round, tetragonal, egg shaped, hexagonal, pentagonal and irregular. Results: The shape of the foramen magnum in dry skulls were oval in 44.95%, round in 30.84%, hexagonal in 9.23%, irregular in 6.63%, pentagonal in 5.19%, tetragonal in 1.73 %, and egg shaped in 1.44%. Conclusion: Inferences of the present study in the form of variations in the shapes of foramen magnum will be useful to the neurosurgeons, radiologists, orthopedics, anthropologists, forensic experts as well as anatomists. Further these data can be used as an anatomical reference for the researchers.

ABSTRACT

The pterion is a topographic point on the lateral aspect of the skull where frontal, sphenoid, parietal and temporal bones form the H or K shaped suture. This is an important surgical point for the lesions in anterior and middle cranial fossa. This study was performed on 50 dry skulls from Serbian adult individuals from Department of Anatomy, Faculty of Medicine in Novi Sad. The type of the pterion on both sides of each skull was determined and they are calcified in four types (sphenoparietal, frontotemporal, stellate and epipteric). The distance between the center of the pterion and defined anthropological landmarks were measured using the ImageJ software. Sphenoparietal type is predominant with 86 % in right side and 88 % in left side. In male skulls, the distance from the right pterion to the frontozygomatic suture is 39.89±3.85 mm and 39.67±4.61 mm from the left pterion to the frontozygomatic suture. In female skulls the distance is 37.38±6.38 mm on the right and 35.94±6.46 mm on the left. The shape and the localization of the pterion are important because it is an anatomical landmark and should be used in neurosurgery, traumatology and ophthalmology.


El pterion es un punto topográfico en la cara lateral del cráneo donde los huesos frontales, esfenoides, parietales y temporales forman la sutura en forma de H o K. Este es un punto quirúrgico importante para las lesiones en la fosa craneal anterior y media. Este estudio se realizó en 50 cráneos secos de adultos serbios del Departamento de Anatomía de la Facultad de Medicina de Novi Sad. Se determinó el tipo de pterión en ambos lados de cada cráneo y se calcifican en cuatro tipos (esfenoparietal, frontotemporal, estrellado y epipterico). La distancia entre el centro del pterion y los puntos de referencia antropológicos definidos se midió utilizando el software ImageJ. El tipo esfenoparietal es predominante con 86 % en el lado derecho y 88 % en el lado izquierdo. En los cráneos masculinos, la distancia desde el pterion derecho hasta la sutura frontocigomática es 39,89 ± 3,85 mm y 39,67 ± 4,61 mm desde el pterion izquierdo hasta la sutura frontocigomática. En los cráneos femeninos, la distancia es 37,38 ± 6,38 mm a la derecha y 35,94 ± 6,46 mm a la izquierda. La forma y la localización del pterion son importantes debido a que es un indicador anatómico y debe usarse en neurocirugía, traumatología y oftalmología.

The piriform aperture is an anatomical structure generally pear-shaped, formed by some facial bones. The knowledge of its morphological presentation is of significant importance for performing a surgical procedure. This study aimed to analyze the morphometry and shape of the piriform aperture in human skulls, considering the sexual dimorphism of this structure. One hundred (100) human skulls were evaluated. The measurements were made with a digital caliper rule. Two parameters were analyzed: the height of the piriform aperture (R-ANS) - distance between the bottom edge of internasal suture to the anterior nasal spine; width (PA-W) - the longest distance in a transverse plane. Its form was evaluated according to seven types described in the literature and the sex differentiation (based on the Vanrell frame). For data analysis between sexes, we used the Student's t-test (p<0.05; CI: 95 %). In this study it was observed that the height (R-ANS) of the piriform aperture in males (31.4 mm) was higher than in females (29.4 mm), without significant differences. The width (PA-W) had equal means values for both sexes (25.7 mm). Regarding the shape of the pyriform aperture, it was found that the type I (pear) is the most common in males (43.6 %) and in women the type VII (rounded) is the predominant type (36 %). When sexes were evaluated in a combined manner, it was observed that the most common was the type I (pear - 39.1 %) and the less common were type III (diamond - 0.0 %), type II (inverted heart - 1.6 %) and type IV (inverted heart - 3.1 %). Knowledge of these morphometric data and piriform aperture formats is essential for surgical procedures involving this anatomical region. The results, particularly those related to the shape of the piriform aperture in women, may contribute to future work related to this facial structure, leading to better surgical decisions.


La apertura piriforme es una estructura anatómica generalmente en forma de pera, formada por algunos huesos faciales. El conocimiento de su presentación morfológica es de importancia en los procedimientos quirúrgicos. Este estudio tuvo como objetivo analizar la morfometría y la forma de la apertura piriforme en cráneos humanos, teniendo en cuenta el dimorfismo sexual de esta estructura. Se evaluaron cien cráneos humanos y las mediciones se realizaron con una regla de pinza digital. Se analizaron dos parámetros: la altura de la apertura piriforme - distancia entre el margen inferior de la sutura internasal y la espina nasal anterior; ancho de la distancia mayor en un plano transversal. Su forma fue evaluada según siete tipos descritos en la literatura y la diferenciación de sexo (basada en el marco de Vanrell). Para el análisis de datos entre los sexos, se utilizó la prueba t de Student (p <0,05; IC: 95 %). En este estudio se observó que la altura (R-ANS) de la apertura piriforme en los hombres (31,4 mm) era más alta que en las mujeres (29,4 mm), sin diferencias significativas. El ancho (PA-A) tenía valores medios iguales para ambos sexos (25,7 mm). En cuanto a la forma de la apertura piriforme, se encontró que el tipo I (pera) es el más común en los hombres (43,6 %) y en las mujeres el tipo VII (redondeado) es el tipo predominante (36 %). Cuando se evaluaron los sexos en conjunto, se observó que el más común fue el tipo I (pera - 39,1 %) y los menos comunes fueron el tipo III (diamante - 0,0 %), tipo II (corazón invertido - bajo la espina nasal 1,6 %) y tipo IV (corazón invertido a nivel de la espina nasal- 3,1 %). El conocimiento de estos datos morfométricos y formatos de apertura piriforme es esencial para los procedimientos quirúrgicos que involucran esta región anatómica. Los resultados, en particular los relacionados con la forma de la apertura piriforme en las mujeres, pueden contribuir al trabajo futuro relacionado con esta estructura facial, lo que llevará a mejores decisiones quirúrgicas.

ABSTRACT

Objective To establish a human-head phantom with realistic skull anatomy and resistivity distribution in order to provide an accurate experimental platform for brain electrical impedance tomography(EIT).Methods Firstly a skull model with 3-layer structure was constructed with double-nozzle 3D printer,and every layer had its specific resistivity verified on the accuracy. Then brain parenchyma with its resistivity was modelled using 3D printer and cerebrospinal fluid and scalp were mimicked using NaCl solution;after the whole phantom was assembled,imaging test using EIT was performed.Results The skull model was similar to the realistic one in terms of anatomy and resistivity distribution;the EIT experiment on the new phantom showed similar results to simulation.Conclusion The proposed phantom has realistic skull anatomy,resistivity distribution and multi-layer anatomical structure, which reflects the features of skull resistivity and thus is suitable for experiments on brain EIT.

ABSTRACT

Cranial capacity constitutes one of the most important characters for determining the racial differences. However, few studies exist that have evaluated the sexual dimorphism of cranial capacity and its significance in determining the sex of an individual. A craniometric study was conducted to differentiate sex from intracranial volumes of dry human skulls. The study was done on 160 dry human skulls (100 males and 60 females) in the department of forensic medicine, Gandhi Medical College, Bhopal (M.P.), during the year 2005 – 2006. Skulls chosen for the present study were of adult age only. Dry, hard mustard seed of uniform size were used to fill the cranial cavity to determine the capacity. Mean cranial capacity of male skulls was found to be 1302.95 + 108.8 c.c. (range 1070 – 1560 c.c.), while in female skulls the mean cranial capacity was found 1179.92 + 97.08 c.c. (range 1000 – 1420 c.c.). A highly significant difference (p value <0.01) was observed between the intracranial volume of male and female skulls were compared. This data is expected to be of Clinical, Forensic, Anthropological and Genetic Importance

Thisstudy presents its clinical considerations and applications. The presence of other anatomical variations was notverified.
