ABSTRACT

Yi-Qi-Huo-Xue Decoction (YQHX) is the recombination of Dang-Gui-Bu-Xue Decoction (DBD), which is one of the well-known traditional Chinese Medicine (TCM) prescription, and has long been shown to have significant protective effects against myocardial ischemic injury. In previous studies, we found that YQHX could regulate lipid and glucose metabolism, promote angiogenesis, attenuate inflammatory response, and ameliorate left ventricular function in myocardial ischemia rat models. However, the underlying mechanism of how YQHX involves in lipid metabolism remains unclear so far. In this study, the underlying mechanism of YQHX in lipid metabolism disorders was elucidated in a myocardial ischemia rat model and a hypoxia-induced H9c2 cell injury model. YQHX (8.2 g·kg) and positive-control drug trimetazidine (10 mg·kg) were administered daily on the second day after left anterior descending (LAD) operation. At 7 days and 28 days after surgery, changes of cardiac morphology, structure, and function were evaluated by H&E staining and echocardiography, respectively. The plasma lipid levels and mitochondrial ATP content were also evaluated. Western blot and RT-PCR were used to determine the protein and mRNA expressions of AMPK, PGC-1α, CPT-1α, and PPARα. YQHX improved cardiac function and ameliorated lipid metabolism disorders. Furthermore, YQHX increased the expression of p-AMPK, PGC-1α, and CPT-1α without changing PPARα in ischemic rat myocardium. In vitro, YQHX activated the protein and mRNA expression of PGC-1α, CPT-1α, and PPARα in hypoxia-induced H9c2 cells injury, whereas AMPK inhibitor Compound c blocked the effects of YQHX. Taken together, the results suggest that YQHX reduces lipid metabolism disorders in myocardial ischemia via the AMPK-dependent signaling pathway.

ABSTRACT

Objective To evaluate the curative effect of the Duhuo-Jisheng decoction in treating knee osteoarthritis (KOA) with the deficiency of liver and kidney. Methods Using the random number table method, a total of 80 KOA patients were randomly divided into 2 group, 40 in each group. The treatment group were treated with Duhuo-Jisheng decoction, and the control group with glucosamine sulfate orally. The 2 groups were treated with 30 day. The blood sedimentation (ESR) of before and after treatment was detected. The level of C-reactive protein (CRP) was detected by latex enhanced immunization. The degree of knee joint pain was evaluated by VAS scale. The severity of the symptoms was evaluated by the visual scale of Osteoarthritis Index (Western Ontario Mac Master University Osteoarthritis index, WOMAC index). The changes of TCM symptoms were observed and the clinical effective rate was evaluated. Results The total effective rate of the treatment group was 95% (38/40), while the control group was 77.5% (31/40). The difference between the 2 groups was statistically significant (χ2=10.213, P=0.003). After treatment, the ESR (15.41 ± 4.23 mm/h vs. 17.6 ± 7.27 mm/h, t=-4.426), CRP (4.73 ± 1.28 mg/L vs. 6.12 ± 2.38 mg/L, t=-14.735) level in the treatemnt group were significantly lower than those in the control group (P<0.01). The VAS scores (2.5 ± 0.62 vs. 4.9 ± 0.8, t=-15.026), WOMAC index(12.1 ± 4.7 vs. 16.7 ± 6.7, t=-11.177) and TCM symptom score (6.5 ± 0.4 vs. 7.5 ± 0.4, t=-3.482) in the treatment group were significantly lower than those in the control group (P<0.01). Conclusions The Duhuo-Jisheng decoction is effective improve the symptom and quality of life of liver-kidney deficiency of KOA.

ABSTRACT

Origin of Chinese Characters (Shuowen Jiezi) records a large number of drug names, providing rich materials for the study of the history of pharmacy. But in the aftertime circulating process, errors were inevitable, making the ancient scholars disagree on the relationship between some drug names and their referents. This paper would give a textual research on medication names about chu, huo, huo, and hua. Relevant content was analyzed by using Shuowen Jiezi, Er Ya,Yu Pian and previous annotations of these exegesis books, as well as herbal literature. The relationship between the names and referents, as well as their lexical meanings were clarified by comparing textual content in specialized book about Chinese exegetics and previous herbal literature. Their medicinal efficacy was explained, and the value of herbalism in these exegesis books was explored.

ABSTRACT

Objective To study the effects of modified Duhuo-Jisheng decoction for treatment of paresthesia after transforaminal endoscopic discectomy in patients with lumbar disk herniation. Methods A total of 89 lumbar disk herniation patients with paresthesia after transforaminal endo-scopic discectomy were divided into the control group (n=44) and the treatment group (n=45) by random number table method. The control group were treated with mecobalamin capsule, and the treatment group treated modified Duhuo-Jisheng decoction. The two groups were treated for 4 weeks. The VAS scale was used to evaluate the pain level of the lower extremities, and the nylon rope test score was used to evaluate the numbness of the lower extremities; and the Oswestry Disability Index was used to evaluate lumbar function; and the World Health Organization Quality of Life (WHOQOL-BREF) was used to evaluate the quality of life. The clinical efficacy of the two groups was compared. Results The total effective rate was 93.3% (42/45) in the treatment group and 75.0% (33/44) in the control group. There were significant differences between the two groups (χ2=5.641, P<0.05). After treatment, the VAS score and ODI score in the treatment group were significantly lower than those in the control group (t=11.440, 8.976, P<0.01). The nylon rope test score and WHOQOL-BREF score were significantly higher than those of the control group (t=10.524, 6.221, P<0.01). Conclusions The modified Duhuo-Jisheng decoction can significantly improve pain and numbness of lower extremities, lumbar function and quality of life in lumbar disk herniationpatients with paresthesia after transforaminal endo-scopic discectomy.

ABSTRACT

[bjective]The paper summarizes the academic thoughts and clinical experience of Professor HAN Xinmin for treating Hp related anorexia in preschool children.[Methods] By learning from Professor HAN Xinmin, recording the relevant cases and analyzing typical cases, to sum up the knowledge of etiology and pathogenesis, treatment characteristics, as well as list one proved case.[Results] Professor HAN Xinmin holds that this disease is caused by exogenous damp heat, and the pathogenesis is damp heat invading the spleen and stomach. There are two kinds of syndromes: dampness heat trapping spleen and spleen deficiency dampness retention. The treatment emphasizes the clearing away heat and dampness and moving the spleen for good appetite. Huo Pu Xia Ling Decoction and Yun Pi Kai Wei Decoction have achieved a satisfactory clinical curative effect. [Conclusion] Therapies of Professor HAN Xinmin in treating Hp related anorexia in preschool children have significant effect, which has the value of popularization and application.

ABSTRACT

Objective To observe the clinical efficacy of Yang Xin Huo Xue(nourishing the heart and activating blood flow) needling method plus Western medication in treating chronic heart failure due to coronary heart disease (CHD).Method Eighty patients with CHD-induced chronic heart failure were randomized into a treatment group and a control group, to respectively receiveYang Xin Huo Xueneedling method plus Western medicationor Western medication alone for intervention. The heart function was estimated and brain natriuretic peptide (BNP) was detected before and after the intervention in both groups, and the change of heart function was observed.Result Yang Xin Huo Xueneedling method plus Western medication achieved a satisfactory therapeutic efficacy in treating chronic heart failure, and its total effective rate was significantly different from the rate in the control group (P<0.05); compared with the control group, the BNP value declined more significantly in the treatment group, and the difference was statistically significant (P<0.05).Conclusion Yang Xin Huo Xueneedling method plus Western medication can produce a satisfactory efficacy in treating CHD-induced chronicheart failure.

[Methods]Summarize Pro.HUO Jiege 's medication basis and rules through collating the prescription of HUO's clinical experiences, combining with research on the literarure of TCM. Discuss the unique views and characteristics of TCM in reducing adverse reactions caused by western medicine treatment, and expound the prevention and treatment of recurrence and metastasis through typical cases.[Results]Pro.HUO Jiege thinks that the etiology and pathogenesis relate to insufficient natural endowment, seven emotions and cancerous toxin. Pro.HUO is expert in treating breast cancer through regulation of Qi, giving consideration to both heart and kidney.In terms of using medication,he is experienced in treating lymph edema after breast cancer operation, toxic reaction induced by chemoradiotherapy,endocrine treatment-related menopausal syndrome and distant metastasis of breast cancer. He usually invigorates Qi and strengthens the spleen, nourishes yin and blood and uses a small amount of attacking poison to help ward off evil spirit. And the case in this article has obtained good treatment results.[Conclusion] Pro.HUO Jiege is good at using traditional Chinese medicine in the treatment of breast cancer,which makes fairly good clinical effect,and his experience is worth learning and promotion.

ABSTRACT

Objective To explore the clinical effect of Sophora powder combined with Xianfang-Huoming decoction for ulcerative colitis.Methods A total of 80 UC patients with TCM syndrome of excessive noxious heat were randomly divided into 2 groups (40 patients each group). The Western medicine group(control group) were treated with Mesalazin Enteric-coated tablets, and the Traditional Chinese medicine(observation group) group were treated with Sophora powder combined with Xianfang-Huoming decoction. The tumor necrosis factor-α(TNF-α)and Interleukin-8(IL-8) were detected. The Mayo index scores were conducted, and the curative effect and the recurrence rate were observed.ResultsAfter treatment, the level of TNF-α (66.11 ± 3.72 ng/Lvs. 92.61 ± 4.32 ng/L,t=29.398,P﹤0.01), IL-8 (53.11 ± 2.72 ng/L vs. 69.31 ± 1.32 ng/L,t=33.888,P﹤0.01)in the observation groupwere significantly lower than those in the control group. The Mayo index score (2.07 ± 0.57 vs. 3.30 ± 0.93, t=7.132,P﹤0.01) in the observation group was significantly lower than that in the control group. The total effective rate (92.5% vs. 85.0%,χ2=1.127, P=0.288) in the observation group was not significantly different from that in the control group. The recurrence rate (5.4% vs. 26.5%,χ2=6.005,P=0.013) in the observation group was significantly lower than that in the control group.Conclusions The Sophora powder combined withXianfang-Huoming decoction can treat the UC patients with TCM syndrome of excessive noxious hest, and the its recurrence rate is low.

ABSTRACT

This study was aimed to observe the clinical effect of Du-Huo Ji-Sheng (DHJS) decoction with low frequency ultrasonics in treatment of knee osteoarthritis (KOA) by evaluating the visual analogue scale (VAS), Lysholm score and the expression levels of transforming growth factor-β (TGF-β) and matrix metalloproteinase-3 (MMP-3). A total of 60 KOA outpatients were randomly divided into the treatment group and the control group, with 30 cases in each group. Patients in the treatment group were treated with DHJS decoction and low frequency ultrasonics therapy, while patients in the control group were treated with ibuprofen and low frequency ultrasonics. The treatment was given once a day for two weeks. The pretreatment and posttreatment VAS, Lysholm score and expression levels of TGF-β and MMP-3, which were detected by ELISA, were recorded. The results showed that compared with the control group, DHJS decoction with low frequency ultrasonics therapy could alleviate pain as well as improve the activity of joint function among KOA patients. It could also increase the content of TGF-βin serum, and suppress the level of MMP-3. It was concluded that low frequency ultrasonics could enhance the transdermal absorption of DHJS decoction, relieve the clinical symptoms of KOA patients.

ABSTRACT

Objective To observe the effect of heat-producing needling (Shao Shan Huo) and cool-inducing needling (Tou Tian Liang) on the gait cycle after operation for gluteus contracture. Method Sixty-six patients after operation for gluteus contracture were randomized into a treatment group and a control group, 33 in each group. The control group was intervened by rehabilitation, while the treatment group was by acupuncture in addition to the rehabilitation training. The topical points Zhibian (BL 54), Huantiao (GB 30), and Juliao (GB 29) were treated with reducing cool-inducing needling technique; Biguan (ST 31), Futu (ST 32), Zusanli (ST 36), Fenglong (ST 40), Xuehai (SP 10), Diji (SP 8), and Yanglingquan (GB 34) were treated with reinforcing heat-producing needling technique. The motor time parameters were observed before and after intervention.Result After intervention, the supporting phase of the affected side, supporting phase of the healthy side, gait cycle, swinging phase of the affected side, swinging phase of the healthy side, and bilateral supporting phase of the treatment group were significantly different from that of the control group (P<0.05).Conclusion Heat-producing and cool-inducing needling techniques can effectively shorten the gait cycle after operation for gluteus contracture, and can improve the walk function.

ABSTRACT

Objective To establish a method for the determination of aristolochic acid A in Jiuwei-Qianghuo pill. Method The HPLC system consisted of the Phenomenex Luna C18(4.6 mm × 250 mm, 5μm)column, the mobile phase consisted of acetonitrile and 0.05% H3PO4, gradient elution flow rate was 1.0 ml/min, the column temperature was 35℃, the UV detector was set at 250 nm.Results The linear response range was 0.014-0.280μg/ml(r=0.999 9). The detection limit and quantitation limit of aristolochic acid A in Jiuwei-Qianghuo pill was 1.5 and 4.5 ng/ml. The average recovery of aristolochic acid A was 96.3%. Conclusions The method was high sensitivity, accurate, repeatable and high secificity. It can be used as an inspection method for safe use of Jiuwei-Qianghuo pill.

ABSTRACT

Objective] To observe the clinical efficacy of using Qu Feng Huo Luo Xie Zhuo tang to treat the chronic gouty arthritis of Tan Zhuo Zu Zhi Zheng. [Methods] From June 2013 to December 2014, 116 patients who got chronic gouty arthritis of Tan Zhuo Zu Zhi Zheng were chosen from the orthopedic outpatient cases in our hospital. All patients were equally divided into treatment group and control group by using random number table. The 58 patients of treatment group were treated with Qu Feng Huo Luo Xie Zhuo tang while the other 58 patients of the control group were treated with allopurino for 2 months. We analyzed the value of the Blood Uric Acid(BUA) and clinical efficacy before treatment and 2 months after treatment.[Results] The results showed that value of the BUA of two groups had no significant difference( P>0.05) before treatment, after 2 months, the value of the BUA of treatment group was lower than control group, the value of the BUA of two groups had significant difference( P<0.05).The rate of effective cases in treatment group was 87.9% while the control group was 68.9%, the clinical efficacy of the treatment group was better than the control group.There was statistical difference in the results of treatment between the groups( P<0.05).[Conclusion] Using Qu Feng Huo Luo Xie Zhuo tang to treat the chronic gouty arthritis of Tan Zhuo Zu Zhi Zheng could get good effect,and it was better than allopurinol which was often used in western medicine.It was worth to be researched and observed in clinic.

ABSTRACT

Objective To evaluate the short-term and long-term therapeutic efficacies of LU’s heat-producing needling (Shao Shan Huo) in treating cervical spondylotic radiculopathy (CSR). MethodTwo hundred patients with CSR were randomized into a treatment group and a control group, 100 in each group. The treatment group was intervened by using heat-producing needling method, while the control group was by regular acupuncture technique with even reinforcing-reducing manipulations. For both groups, the treatment was given once a day, 10 sessions as a treatment course. The Pain Rating Index (PRI), Present Pain Intensity (PPI), and Visual Analogue Scale (VAS) were adopted for observation before intervention and after two treatment courses. The clinical efficacies were compared between the two groups.Result Right after the intervention and in the 3-month follow-up, the VAS, PPI, and PRI scores were significantly different from that before intervention in both groups (P<0.01). There were significant differences in comparing the VAS, PPI, and PRI scores between the two groups after intervention and in the 3-month follow-up (P<0.01). The VAS, PPI, and PRI scores in the treatment group right after intervention were markedly different from that in the 3-month follow-up (P<0.01). The total effective rate was 98.0% in the treatment group versus 79.0% in the control group, and the difference was statistically significant (P<0.01).Conclusion LU’s heat-producing needling is an effective way in treating CSR, as it can significantly reduce pain intensity, and improve other symptoms and body signs.

ABSTRACT

Objective:To observe the effect of complex reinforcing [Shao Shan Huo (Mountain-burning Fire)] and reducing [Tou Tian Liang (Heaven-penetrating Cooling)] manipulations on motion and balance of the lower limbs in patients with subacute combined degeneration of the spinal cord (SCD). Methods:A total of 100 SCD cases were randomly allocated into an observation group and a control group by their visit sequence, 50 cases in each group. On the basis of Western medical treatment, cases in the observation group were also treated with acupuncture therapy plus complex reinforcing-reducing manipulation; whereas cases in the control group were only treated with the same Western medical treatment as those in the observation group. Before and 2 months after treatment, kinematic parameters including muscle force, muscle tone and range of motion of the hip and knee joints were measured. In addition, balancing parameters including the path length of center of pressure (COP), peripheral area, COP path length per unit area and rectangular area were also measured before and after treatment. Results:After treatment, there were statistical intra-group differences in COP path length, peripheral area, COP path length per unit area, left-right offset, rectangular area, coefficient of stability and coefficient of weight distribution (P<0.01). There were statistical inter-group differences in muscle force, muscle tone and range of motion of hip and knee joints (P<0.01). The total effective rate was 83.3% in the observation group, versus 60.0% in the control group, showing a statistical difference (P<0.05). Conclusion:Complex reinforcing-reducing manipulations can improve the lower limb motion and balance in SCD patients.

Objective: To observe the effect of complex reinforcing-reducing manipulations of acupuncture on flexion and extension angles of hip and knee in patients after surgical treatment of gluteus maximus contracture. Methods: A total of 66 cases following surgery of gluteus maximus contracture were randomly allocated into an observation group and a control group by the random digits table, 33 in each group. In addition to basic treatment, cases in the observation group were treated with reducing manipulation [Tou T ian Liang (Heaven-penetrating Cooling)] on Zhibian (BL 54), Huantiao (GB 30) and Juliao (GB 29) on the affected side and reinforcing manipulation [Shao Shan Huo (Mountain-burning Fire)] on Biguan (ST 31), Futu (ST 32), Zusanli (ST 36), Fenglong (ST 40), Xuehai (SP 10), Diji (SP 8) and Yanglingquan (GB 34). Patients in the control group only received the same basic treatment as the observation group. The treatment was done once a day, 30 d for a course and a 1-week interval between two courses. Then the flexion and extension angles of hip and knee were evaluated using the Lokomat full automatic robot gait evaluation system after 3 courses of treatment. Results:There were intra-group statistical differences in hip flexion angle on foot followed (HFA-FF), the maximum of hip flexion angle (MAX-HFA), the maximum of hip extension angle (MAX-HEA), knee flexion angle on foot followed (KFA-FF), the maximum of knee flexion angle on stance phase (MAX-KFA-TP) and the maximum of knee flexion angle on swing phase (MAX-KFA-WP) in the observation group and in HFA-FF, MAX-HEA and KFA-FF in the control group (P Conclusion: The complex reinforcing-reducing manipulations of acupuncture can effectively improve the hip/knee functions following surgery of gluteus maximus contracture.

ABSTRACT

This study was aimed to investigate the effects of Shen-Y uan Y i-Qi Huo-Xue (SYYQHX) capsule com-bined with early percutaneous coronary intervention (PCI) on near-term quality of life (QOL) in unstable angina (UA) patients. Seventy-eight patients diagnosed with UA were selected and randomly divided into the treatment group and control group, with 39 patients in each group. Early PCI was undergone after coronary angiography. Before PCI, the control group was given routine western medication. The treatment group was given routine western medication plus SYYQHX capsule, three pills once, three times daily. The treatment lasted for 30 days. The QOL scores were evalu-ated among patients from both groups before and after treatment in order to determine the efficacy on angina, electro-cardiogram (ECG) and traditional Chinese medicine (TCM) main symptom. The results showed that compared to pre-treatment, the scores of physical limitation (PL), angina stability (AS), angina frequency (AF), and treatment satisfac-tion ( TS ) were significantly increased after treatment ( P < 0 . 05 ) . Compared with the control group , after treat-ment with SYYQHX capsule, the AF, duration time and symptoms of palpitation, fatigue and shortness of breath were obviously improved (P< 0.05). The scores of AS, AF and TS in the treatment group were significantly increased (P < 0.05). It was concluded that SYYQHX capsule combined with early PCI can improve the near-term QOL and TCM main symptoms among UA patients.

ABSTRACT

This study was aimed to investigate the effect of Yi-Qi Wen-Yang (YQWY) and Huo-Xue Li-Shui (HXLS) decoction on changes of insulin resistance (IR) model and the myocardial tissues of rats with congestive heart failure (CHF) pathology. CHF rat models were established on Wistar male rats through injection of doxorubicin hydrochlo-ride into rat tail vein. Wistar male rats models were randomly divided into the model group, western medicine group, low dose decoction group, middle dose decoction group, and high dose decoction group. After 4-week gavage, 3 mL vein blood was taken from the angular vein sinus for the determination of blood glucose and serum insulin, and the calculation of IR. Finally, the rats were sacrificed. And then, the heart was removed to make HE slice and observe the pathological change of myocardium. The results showed that compared with the model group, YQWY and HXLS decoction can improve the IR level among CHF rats (P<0.05). Among them, the effects of the high dose and middle dose group were obvious. At the same time, this decoction can improve the myocardial cells in CHF rats in myocar-dial cells of the high dose group. And its morphology change was close to the digoxin group. It was concluded that YQWY and HXLS decoction can reverse IR and improve ventricular remodeling among CHF rats to a certain extent.

[Objective]To introduce the clinical experience of Pro.Huo Jiege treating lung cancer in the elderly. [Methods]Through copying prescription with the teacher and carding records, and studing on Chinese literature, sum up Pro.Huo Jiege ’s unique insights for the etiology, pathogenesis, principles and methods, and explains it by one case. [Results] Pro.Huo Jiege thinks that the treatment of elderly lung cancer should base on syndrome differentiation, regulate spleen and stomach and nutrition to other organs at the same time.[Conclusion]The treatment of lung cancer in the elderly patients by Traditional Chinese medicine has achieved some effect in clinical practice and provides guidance in the clinical practice.
