ABSTRACT

OBJECTIVE: To compare dorsal-and-medial hybrid approach and medial-to-lateral approach in laparoscopic right hemicolectomy with complete mesocolic excision(CME). METHODS: Patients undergone laparoscopic right hemicolectomy in Department of Gastrointestinal Surgery in Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from July 2017 to April 2018 were prospectively included.Patients were divided into two groups:dorsal group and medial group. Clinical and pathological data were collected and compared between the two groups. RESULTS: There were 35 patients in medial group and 40 patients in dorsal group. No significant differences were found between the two groups in baseline characteristics,perioperative outcomes and pathological results. No significant difference was found between the two groups in length of bowel(24 cm vs. 22 cm),A line distance(9.8 cm vs.9.4 cm),B line distance(9.0 cm vs. 8.5 cm),area of mesentery(112.4 cm~2 vs. 109.0 cm~2),total lymph node count(19 vs.19),lymph node adequate ratio(97.1% vs. 97.5%)and CME ratio(80% vs. 85%). Obesity was found to be an independent risk factor of CME ratio(P=0.019). CONCLUSION: Dorsal-and-medial hybrid approach and medial-to-lateral approach are comparable in safety,feasibility and effectiveness in laparoscopic right hemicolectomy with complete mesocolic excision. Randomized clinical trials with larger sample size are needed.

ABSTRACT

Objective To investigate the outcomes of hybrid treatment of pulmonary valve's balloon valvuloplasty via right ventricle without cardiopulmonary bypass for pulmonary atresia with intact ventricular septum(PA/IVS) and critical pulmonary stenosis(PS) in neonates.Methods The clinical features and data of 3 children(1 case of PA/ IVS and 2 cases of critical PS),who underwent hybrid treatment of balloon valvuloplasty,were recorded and retrospectively analyzed.All patients' right ventricle grow well,tricuspid valve Z score >-2,and underwent pulmonary valve balloon valvuloplasty via fight ventricle without cardiopulmonary bypass.Results All the 3 children underwent pulmonary valve balloon valvuloplasty via fight ventricle without cardiopulmonary bypass,and there was no death.The mean arterial blood oxygen saturation after operation went up,and there was no complication related to balloon valvuloplasty.The children were followed up for 3 to 6 months,and all the patients had mild pulmonary stenosis,but they did not need any treatment.Conclusions Pulmonary valvuloplasty with hybrid technique is effective in treatment of selective PA/IVS children(fused leaflets of pulmonary valve) and critical PS in neonates.Hybrid therapy may provide adequate pulmonary blood flow and promote further growth of tricuspid valve and right ventricular cavity with less trauma and more safety.

ABSTRACT

Background: Computer aided teaching (CAT) became popular in medical education but there is debate among the teachers regarding application of computer in a large group lecture class. Aim of the study: The present study was aimed to find out the acceptance and use of CAT in a conventional large group lecture class along with black board assistance. Material and methods: CAT was conducted in a large group lecture class of embryology. Related lined diagrams were drawn on the blackboard in front of the students. Students were given a Likert’s type three response closed end questionnaire to express their opinion at the end of the concerned system of embryology. Result: Ninety-nine percent of the students expressed that the teacher should guide them on how to draw a lined diagram. Ninety-nine percent mentioned that emphasis should be given on the applied aspect of the topic. Ninety-six percent of students’ opinions were that at the end of each lecture, summary should be either given as a hand out or be allowed to take notes. Ninety-three percentage of the students mentioned that computer aided teaching is beneficial, 86% expressed that hybrid approach of teaching, provoked further study of embryology, 91% mentioned that the teacher should inform which books to be used as reference for further studies. Conclusion: The study result suggests that CAT is applicable in a large group lecture class. It is better to adopt hybrid approach of teaching for equal involvement of teacher and students. It is necessary to the inform students regarding the importance of concerned topics and expert teacher give the students proper guidance.
