ABSTRACT

OBJECTIVE@#To evaluate the effectiveness and safety of Zhenqi Buxue Oral Liquid (ZQ), progesterone capsules, and their combination in treating oligomenorrhea and hypomenorrhea with qi-blood and Kidney (Shen) essence deficiency.@*METHODS@#This was a prospective, randomized, multi-center controlled trial between June 2022 to December 2022. Ninety-six oligomenorrhea and hypomenorrhea patients with qi-blood and Shen essence deficiency were randomly assigned to receive ZQ (ZQ group, 29 cases), progesterone capsules (PG group, 32 cases), or the combined Chinese and Western medicine (COM group, 31 cases) at a ratio of 1:1:1. Patients in the ZQ or PG group took daily 10 mL twice a day of ZQ or 200 mg once a day of progesterone capsules for 10 consecutive days on day 15 of the menstrual cycle respectively, and patients in the COM group received the same ZQ combined with progesterone capsules. The treatment course lasted for 3 months and follow-up was performed at 1 and 3 months after the end of treatment. Primary endpoint was the menstrual Traditional Chinese Medicine Syndrome Scale (TCMSS) scores. Secondary endpoints included pictorial blood loss assessment chart (PBAC) scores, clinical efficacy rate, 36-item Short Form Health Survey (SF-36) scores, sex hormones and thickness of endometrium. Adverse events (AEs) were recorded.@*RESULTS@#TCMSS scores after 1- and 3-month treatment in all groups were significantly lower than those at baseline (P<0.05). Only TCMSS scores after 3-month treatment in the ZQ and COM groups continuously decreased compared with those after 1-month treatment in the same group (P<0.01). TCMSS scores after 3-month treatment in the ZQ and COM groups were significantly lower than those in the PG group (P<0.05, P<0.01). Compared with baseline, PBAC scores in the ZQ and COM groups after 3 months of treatment were also significantly higher (both P<0.01). The total effective rates of TCM syndrome of 3-month treatment were significantly improved in all groups compared with that after 1 month of treatment (P<0.05). The total effective rate of the COM group was the highest in the 3rd month of treatment and significantly higher than that of PG group alone (P<0.05). Compared with baseline, only the SF-36 scores of COM group were significantly improved after 3 months of treatment (P<0.05). No serious adverse reactions were observed after treatment.@*CONCLUSIONS@#The combination of ZQ and PG, or ZQ only had better effects on reducing TCMSS scores compared with PG, and COM showed the higher total effective rate compared with monotherapy. Besides, COM could effectively improve menstrual blood loss and quality of life. ZQ combined with PG may be an effective and safe option for oligomenorrhea and hypomenorrhea patients with qi-blood and Shen essence deficiency.

ABSTRACT

Objective To observe the clinical efficacy of therapy of nourishing kidney and liver for the treatment of hypomenorrhea with the syndrome of kidney deficiency and liver stagnation. Methods Fifty-one hypomenorrhea patients with the syndrome of kidney deficiency and liver stagnation were randomized into treatment group(N =25) and control group (N = 26). The treatment group was treated with oral use of Yangyin Shugan Capsules (mainly composed of Radix Bupleuri, Radix Paeoniae Alba, Radix Rehmanniae Preparata, and Fructus Ligustri Lucidi). The control group was given placebo capsules orally. After treatment for 8 weeks , the overall efficiency and therapeutic effect on menstruation of the two groups were evaluated according the changes of traditional Chinese medicine(TCM) symptom scores. Results(1) The total effective rate of the treatment group was 84.0%, and that of the control group was 15.4%, the difference being significant between the two groups (P 0.05). Conclusion Therapy of nourishing kidney and liver is effective for the treatment of hypomenorrhea with the syndrome of kidney deficiency and liver stagnation, and has obvious effect on improving menstrual volume and TCM symptoms.

ABSTRACT

AIM To observe the clinical efficacy of Maixuekang Capsules (hirudin) combined with female progestational hormone treating blood-stagnated cryptogenic hypomenorrhea and their mechanism of action.METHODS Ninty eligible patients were randomly and equally divided into treatment group (Maixuekang Capsules combine with female progestational hormone) and control group (female progestational hormone),treated for three menstrual cycles,the menstrual blood volume,blood D-dimer,maximum platelet aggregation rate,the thickness of endometrium,spiral artery blood flow peak systolic velocity (PSV),resistance index (RI) and pulsatility index (PI) were recorded during the three months period.RESULTS The menstrual blood volume of the two groups were all increased post-treatments (P < 0.05),the treatment group was superior to the control group (P < 0.05);blood D-dimer and maximum platelet aggregation rate of the treatment group had reduced post-treatments (P < 0.05),there was no difference in the two values of the control group between post-treatment and pre-treatment (P > 0.05);the thickness of endometrium of the two groups were both increased after the treatment (P < 0.05),while there was no remarkable statistical difference between the two groups (P > 0.05);RI in the treatment group had remarkable reduction post-treatments (P < 0.05),but there was no remarkable difference of RI in the control group (P > 0.05);there was no statistical difference of PSV and PI in both groups before and after the treatment (P > 0.05).CONCLUSION The combination treatment of Maixuekang Capsules and female progestational hormone can obviously increase menstrual blood volume,improve uterine blood perfusion and endometrial microcirculation.It has more advantages than use hormones alone.

ABSTRACT

[Objective] The article analyzed the application of 'Disease Prevention'theory in the hypomenorrhea and provided treatment way for posterity doctor treating the disease. [Method] In this chapter it elaborated the pathogenesis of hypomenorrhea about traditional Chinese medicine and modern medicine. At the same time, it elaborated hypomenorrhea's prevention and treatment from the following aspects:prevention of disease which did not appear, prevention of disease which appeared from worsening,and prevention of disease which was cured from reoccurring to explore the treatment effect and advantage.[Result] The hypomenorrhea was harmful and its incidence rate to a trend of annual increase in recent years. To put'Disease Prevention'theory in the whole diagnosis process had a significant clinical curative effect. [Conclusion] In this paper, starting from the'Disease Prevention'theory treatment of the disease has some practical value, which is worthy of popularization and application.

Case group SS genotype rate was 66%,control group 51%,OR=1.865(95%CI:1.055~3.298).The SS and LL allele genotypes frequency was compared in two groups and the difference was significant(?2=4.634,P=0.031). Conclusion:ER? gene CA repeat polymorphism is related to unknown aetiological hypomenorrhea. SS allele genotype may be the risk factor of unknown aetiological hypomenorrhea.

ABSTRACT

Objective:To investigate the association of a(TA)n dinucleotide repeat polymorphism with unknown aetiology hypomenorrhea.Methods:100 normal menstruation person(control group) and 100 hypomenorrhea patients(case group)were recruited.they all came from southen-western China.The(TA)n dinucleotide repeat in ERa gene upstream hypervariable region were purifed,cloned and sequence analysed.we observe the ERa gene(TA)n dinucleotide repeat polymorphism distribution in the two groups.Results:All samples exhibted 7 different alleles.The(TA)n dinucleotide repeats were 11~17,The most prevalent allele was TA14 repeat in control group.The distribution of the different alleles of the(TA)n dinucleotide repeat sequence in the group and control group has significant difference(?2=13.412,P =0.037).Fisher's exact test was applied to compare the frequency of each a1lele between two groups.The frenquency of the TA13 allele was found to be significantly greater in hypomenorrhea group(P =0.006),and the frenquency of the TA15 allele was less in the group(P =0.033).Conclusion:A(TA)n dinucleotide repeat sequence which is in ERa gene upstream hypervariable region was associated with unknown aetiology hypomenorrhea.The TA13 allele was found to be associated with unknown aetiology hypomenorrhea and may be a risk factor;whereas TA15 allele may be a protective factor for patients.

ABSTRACT

Objective To study the relationship between the polymorphism of estrogen receptor ? gene (ER?)and unknown aetiological hypomenorrhea in the women from Southwestern China. Methods Totally 100 hypomenorrhea and 100 eumenorrhea women from Southwestern China served respectively as case group and control group. Restriction fragment length polymorphism (RFLP) was used to analyze the first intron incision enzyme PvuⅡ and XbaⅠin ER? gene. Short tandem repeat (STR) in hypervariable region of the gene was depurated, cloned and sequenced. The distribution of the gene genotype was studied in case group and control group. Results P genotypic frequency was 47.5% in case group, and 30.5% in control group (OR 1.810, 95%CI 1.113-2.765,P=0.012). While X genotypic frequency was 20.5% in case group and 32.0% in control group (OR 0.641, 95%CI 0.361-0.898, P=0.036). PvuⅡ and XbaⅠ restriction fragments were distributed in polymorphism in the 2 groups. The TA13 allele frequency was higher in case group than in control group (P
