ABSTRACT

We aimed to evaluate ten-year outcomes of penile prosthesis (PP) implantation for the treatment of erectile dysfunction and to assess predictors of early prosthetic infection (EPI). We identified 549 men who underwent 576 PP placements between 2008 and 2018. Univariate and multivariate analyses were used to identify potential predictors of EPI. An EPI predictive nomogram was developed. Thirty-five (6.1%) cases of EPI were recorded with an explant rate of 3.1%. In terms of satisfaction, 82.0% of the patients defined themselves as "satisfied," while partner's satisfaction was 88.3%. Diabetes (P = 0.012), longer operative time (P = 0.032), and reinterventions (P = 0.048) were associated with EPI risk, while postoperative ciprofloxacin was inversely associated with EPI (P = 0.014). Rifampin/gentamicin-coated 3-piece inflatable PP (r/g-c 3IPP) showed a higher EPI risk (P = 0.019). Multivariate analyses showed a two-fold higher risk of EPI in diabetic patients, redo surgeries, or when a r/g-c 3IPP was used (all P < 0.03). We showed that diabetes, longer operative time, and secondary surgeries were the risk factors for EPI. Postoperative ciprofloxacin was associated with a reduced risk of EPI, while r/g-c 3IPP had higher EPI rates without an increased risk of PP explant. After further validation, the proposed nomogram could be a useful tool for the preoperative counseling of PP implantation.

ABSTRACT

Background: Current drugs used for sexual dysfunction have several limitations, necessitating the search for alternative medicine. Aim and Objective: The study investigated the effect of Citrullus lanatus ethanolic seed extract (CLESE) on the sexual behavior and reproductive hormones of adult male Wistar rats. Material and Methods: Twenty-four adult male Wistar rats were randomized into four groups (n=6). Group 1 received saline (1 ml/kg, p.o.) and served as normal control, groups 2 and 3 were given CLESE (500 and 1000 mg/kg, p.o.) for 14 consecutive days. Group 4 received Sildenafil Citrate (SC) (5 mg/kg, p.o.), which served as positive control drug and this was administered 4 h before sexual behavioral test only on days 7 and 14. Sexual behavioral tests were performed in a Plexiglas copulatory arena. Male sexual behavioral parameters were evaluated following one on one exposure of experimental male rats with oestrusinduced sexual seeking female rats. Thereafter, rats were sacrificed. Results: CLESE enhanced ibido by significantly (p < 0.05) increasing mounting and intromission frequencies relative to normal controls. CLESE also enhanced intromission and ejaculation latencies, but decreased mounting latency and post ejaculatory interval. Serum testosterone and luteinizing hormone were also significantly (p < 0.05) increased. The histological findings showed improved histoarchitechture of the testes and hypothalamic sections. Conclusion: The findings from this study showed that CLESE enhanced sexual behavior via mechanism related to increased serum testosterone and luteinizing hormone and may be useful for men with sexual dysfunction.

ABSTRACT

O presente artigo visa apresentar a ideia de congelamento como metáfora para a insensibilidade e a anestesia diante de "traumatismos precoces", no contexto da clínica da clivagem, de adoecimentos por "apassivação" e "patologias narcísico-identitárias". Para discutir o tema, é relatado brevemente o caso de uma paciente, aqui chamada Cecília, descrevendo seu adoecimento e defesas ativadas e destacando a "neutralização energética" diante de uma situação de desamparo extremo vivenciado na primeira infância. A aqui denominada clínica da cisão e dos adoecimentos por apassivação tem convocado os analistas a refletir acerca de outras estratégias terapêuticas, de modo que acompanhe não apenas as partes amortecidas, mas também os recursos egoicos de cada paciente.


The present article proposes the presentation of the idea of freezing as a metaphor for insensitivity and anesthesia against early trauma, understanding it within the context of a clinic of cleavage, deterioration due to apassivation and narcissistic pathologies. As an illustration, we present the report of a case of a patient, here called Cecília, describing her illness and activated defenses, highlighting the "energetic neutralization" of Roussillon, facing a situation of extreme helplessness experienced in early childhood. Faced to what we call the scission clinic and the illnesses by apassivation, we are called to reflect on the necessary therapeutic strategies that accompany not only the cushioned parts but also the egoic resources of the patient.


El presente artículo propone la presentación de la idea de congelación como metáfora de la insensibilidad y la anestesia contra el traumatismo temprano, entendiéndola en el contexto de una clínica de escisión, deterioro por apassivación y patologías narcisistas. A modo de ejemplo, presentamos el relato de un caso de una paciente, aquí llamada Cecília, describiendo su enfermedad y las defensas activadas, destacando la "neutralización energética" del Roussillon, frente a una situación de extrema indefensión experimentada en la primera infancia. Frente a lo que llamamos la clínica de la cisión y las enfermedades por apassivación, estamos llamados a reflexionar sobre las estrategias terapéuticas necesarias que acompañan no sólo las partes amortiguadas sino también los recursos egoicos del paciente.


Cet article est de proposer la présentation de l'idée de gel comme une métaphore de l'insensibilité et l'anesthésie avant au début du traumatisme, le comprendre dans le contexte d'un clivage clinique des maladies par passivation et pathologies narcissiques identité.Comme illustration, nous présentons le rapport de cas d'un patient ici nommé Cecília, décrivant sa maladie et les défenses ont permis, en vedette dans la "neutralisation de l'énergie" du Roussillon, par rapport à une situation extrême d'impuissance vécu dans la première enfance. Transférer à ce que nous nommons que la répartition et les maladies cliniques par passivation, nous sommes appelés à réfléchir sur les stratégies thérapeutiques nécessaires qui accompagnent non seulement les parties, mais aussi les amorties caractéristiques egoic du patient.

ABSTRACT

The aim of our study was to evaluate the outcome of penile prosthesis implantation in patients with various comorbidities as a cause of erectile dysfunction (ED). The data of 181 patients who underwent surgery between 1998 and 2012 in two centers were evaluated. The mean age of the patients was 52.2 years (range: 31-71 years). The study group contained 162 patients (89.5%) with malleable prostheses and 19 (10.5%) with inflatable implants. All patients were re-evaluated 1 month later to assess prosthesis function and complications, and further re-examinations were performed if needed. Satisfaction was defined as having satisfactory intercourse and happiness with the device in general. The follow-up period was at least 12 months for each patient. The postoperative complication rate was 32% (n = 58). The number of complications with inflatable and malleable prostheses was 7 (3.9%) and 51 (28.1%), respectively. Overall, 21 prostheses (11.6%) had to be removed because of various complications and patient dissatisfaction. Patients with prior radical surgery had higher extraction rates ( = 14.606, P < 0.05, Chi-square test). The main reasons for removal were erosion (n = 11; 6.1%) and infection (n = 3; 2.1%). With respect to satisfaction during intercourse, we found that 104 (57.5%) patients described themselves as very satisfied with the prosthesis, while 21 (11.6%) were unsatisfied. The high explantation rate in patients with prior surgery was remarkable in our study. Our results revealed that a malleable prosthesis should not be the preferred type of implant for patients with prior surgery.

Introducción: El cáncer de próstata (CP) presenta una alta prevalencia. La radioterapia con intensidad modulada (IMRT) esuna nueva técnica con impacto poco conocido en la calidad de vida a largo plazo (CDVALP) de estos pacientes. El objetivo fue evaluar CDVALP en este grupo. Materiales y Método: Se incluyeron pacientes tratados entre 2000-2010, con más de 1 año de seguimiento. Se utilizó cuestionario EPIC, que fue completado directamente por el paciente. Se evaluaron dominios objetivos y subjetivos de hábitos urinarios, intestinales, hormonales y sexuales. Estudio aprobado por el comité de ética. Resultados: Se analizaron 45 encuestas. La mediana de edad fue 75 años (rango 55-90), seguimiento 4,4 años (rango 1,5-8,5). Tasa respuesta promedio fue de 96,9 por ciento. La toxicidad moderada-importante objetiva para dominio urinario fue: escape urinario 8,8 por ciento, disuria 4,4 por ciento, incontinencia 4,4 por ciento, uso pañales 2,2 por ciento, hematuria 6,6por ciento. En lo subjetivo, la polaquiuria fue "un problema grande" en 6,7 por ciento. La toxicidad intestinal objetiva moderada-importante fue: frecuencia deposición 8,9 por ciento, diarrea 4,4 por ciento, pujo 6,6 por ciento, incontinencia 4,4 por ciento, hematoquezia 0 por ciento. En el dominio hormonal: bochornos significativos 11,1 por ciento. En dominio sexual: compromiso importante del deseo 57,6 por ciento, erección 66,7 por ciento, orgasmo 55,6 por ciento, en globo 62,4 por ciento, Subjetivamente la disfunción eréctil se consideró "un problema grande" en 31,1 por ciento . En globo, la disfunción sexual subjetiva se consideró importante en 24,4 por ciento y moderada en 17,8 por ciento. Conclusiones: La radioterapia con IMRT se asocia a buena CDVALP en dominios urinario, intestinal, hormonal. La alta toxicidad sexual podría estar asociada a la edad avanzada de los pacientes tratados con radioterapia.(AU)


Background: Prostate cancer has a high prevalence. Intensity modulated radiation therapy (IMRT) is a new technique with little known impact on quality of life (QOL) in the long term of these patients. The objective was to evaluate QOL in this group. Materials and Methods: We included patients from 2000 to 2010, with more than 1 year follow-up. EPIC questionnaire was completed by the patient directly. We evaluated objective and subjective domains urinary and bowel habits, hormonal and sexual symptoms. Study approved by the ethics committee. Results: 45 surveys were analyzed. The median age was 75 years (range 55-90); follow up was 4.4 years (range 1.5-8.5). Average response rate was 96.9 percent. Toxicity moderately important objective for urinary domain was: urinary escape 8,8 percent, dysuria 4,4 percent, incontinence 4.4 percent, use diapers 2,2, hematuria 6,6 percent. In the subjective, polakiuria was "a big problem" in 6,7 percent. Objective Intestinal toxicity moderately important was: deposition rate 8,9. percent, diarrhea 4.4 percent, incontinence 4.4 percent, hematochezia 0 percent. In the hormonal domain clinical significant hot flashes in was 11.1 percent. In sexual domain: significant desire dysfunction in 57.6 percent, erection 66.7, orgasm 55.6 percent, Sexual overall 62.4 percent, subjectively erectile dysfunction was considered "a big problem" in 31.1 percent. Overall, subjective sexual dysfunction was considered important by 24.4 percent and moderate in 17.8 percent. Conclusions: IMRT radiation therapy is associated with good CDVALP in domains urinary, bowel, hormonal. The sexual high toxicity may be associated with older age of patients treated with radiotherapy.(AU)

ABSTRACT

Objective To investigate the clinical efficacy of three yin points acupuncture plus medicinal spreading moxibustion in treating erectile dysfunction.Methods Sixty patients with erectile dysfunction meeting the inclusion criteria were randomly allocated to two groups, 30 cases each. The control group took sildenafil at 1 hr before sexual intercourse. The treatment group received three yin points acupuncture plus medicinal spreading moxibustion, 8 days as one course of treatment, for three consecutive courses with 2 days of rest between the courses. The erectile function was scored in the two groups before and after treatment. The clinical therapeutic effects were compared.Results The total efficacy rate was 93.3% in the treatment group and 80.0% in the control group; there was no statistically significant difference between the two groups (P>0.05). The marked efficacy rate was 83.3% in the treatmentn group and 60.0% in the control group; there was a statistically significant difference between the two groups (P0.05).Conclusions Both acupuncture-moxibustion and sildenafil have a therapeutic effect on erectiledysfunction. Sildenafil produces a rapid and stable effect. Acupuncture and moxibustion produce a slower effect but have an advantage in the marked efficacy rate. Acupuncture and moxibustion are safe, of fewer contraindications and suitable for most of the population.

ABSTRACT

PURPOSE: In patients with erectile dysfunction, it is important to differentiate psychogenic from organic causes. Penile Doppler ultrasonography is a relatively inexpensive and minimally invasive tool for this purpose. This study was conducted to evaluate the causes of erectile dysfunction in an adult male population, using penile Doppler ultrasonography. MATERIALS AND METHODS: A retrospective study was conducted in a single center. All patients who presented with complaints of erectile dysfunction and underwent penile Doppler ultrasonography between July 2014 and June 2016 were included in this study. All examinations were performed using GE Voluson S6 and GE Logiq P5 devices. Following baseline scans, an intracavernosal injection of 20 µg of prostaglandin E1 was given. Peak systolic and end diastolic velocities were measured in each cavernosal artery. Patients with a peak systolic velocity of 5 cm/s was considered to indicate venous incompetence. RESULTS: Out of 97 patients (mean age, 37.09±11.59 years; range, 19~69 years), 50 patients (51.5%) had normal findings, 24 patients (24.7%) had arterial insufficiency, 15 patients (15.5%) had a venous leak, and 8 patients (8.2%) patients had arterial insufficiency with a venous leak. Psychogenic erectile dysfunction was significantly higher among patients aged ≤40 years, while arterial insufficiency with or without a venous leak was significantly higher among patients aged >40 years (p=0.022). CONCLUSIONS: A majority of the studied individuals demonstrated no organic cause of erectile dysfunction, thus confirming a high prevalence of the psychogenic etiology, particularly in relatively young individuals.

ABSTRACT

From a Darwinian perspective we live to reproduce, but in various situations genetic males elect not to reproduce by choosing medical treatments leading to infertility, impotence, and, in the extreme, emasculation. For many men, infertility can be psychologically distressing. However, for certain genetic males, being infertile may improve their quality of life. Examples include (1) men who seek vasectomy, (2) individuals with Gender Dysphoria (e.g., transwomen, and modern day voluntary eunuchs), (3) most gay men, and (4) men treated for testicular and prostate cancer. Men who desire vasectomy typically have a Darwinian fitness W >1 at the time of their vasectomies; i.e., after they have their desired number of offspring or consider themselves past an age for parenting newborns. In contrast, prostate and testicular cancer patients, along with individuals with extreme Gender Dysphoria, do not necessarily seek to be sterile, but accept it as an unavoidable consequence of the treatment for their condition undertaken for survival (in case of cancer patients) or to achieve a better quality of life (for those with Gender Dysphoria). Most gay men do not father children, but they may play an avuncular role, providing for their siblings' offspring's welfare, thus improving their inclusive fitness through kin selection. In a strictly Darwinian model, the primary motivation for all individuals is to reproduce, but there are many situations for men to remove themselves from the breeding populations because they have achieved a fitness W ≥1, or have stronger medical or psychological needs that preclude remaining fertile.

Purpose: The proposal of this study was to determine the prevalence and the associated factors of erectile dysfunction (ED) among hemodialysis (HD) patients. Materials and Methods: This was a cross-sectional study based on data collected from HD male patients. Clinical, demographic and laboratory data of all patients were collected in three HD clinics from December 2010 to June 2011. Patients answered questions of erectile function domain from International Index of Erectile Function. Data were evaluated by descriptive analysis and by univariate (ULRA) and multivariate logistic regression analysis (MLRA). Results: Three hundred and five patients participated of the study. The prevalence of ED was 68.19%. ED was associated with diabetes (DM), benign prostatic hyperplasia, glomerulonephritis as cause of chronic renal failure (CRF), smoking habits, lower creatinine levels (ULRA), use of calcium channel blocker (MLRA), aging, lower education level, alcohol consumption, DM (as cause of CRF) and coronary insufficiency (ULRA and MLRA). Conclusions: ED was highly prevalent in the HD men. It was independently associated with aging, current use of alcohol, long alcohol use (even for those who do not drink more), lower education level, diabetes as cause of CRF, coronary insufficiency and use of channel blockers calcium. .

ABSTRACT

Objective:To study the clinical effect of Bailing capsules in the treatment of diabetic impotence. Methods: Totally 136 cases of diabetic impotence were randomly divided into the observation group and the control group with 68 cases each. The obser-vation group was treated with Bailing capsules and the control group was with tadalafil tablets. The effect of the two groups was com-pared, and the changes in IIEF-5 score, serum testosterone ( T) and zinc ( Zn2+) before and after the treatment were studied as well. Results:The total effective rate of the observation group was 83. 82% while that of the control group was 67. 65%, and the difference was statistically significant (P0. 05), while IIEF-5 score was increased significantly (P0.05), while the serum testosterone (T) and IIEF-5 score were increased significantly in the observation group (P <0. 05). After the treatment, the symptoms in the two groups were both improved with increased IIEF-5 score, T and Zn2+, and the ob-servation group was with more obvious improvement. Conclusion:The effect of Bailing capsules in the treatment of diabetic impotence is promising, which is worthy of further study.

Este artigo trata do caso de Pierre, um rapaz que apresenta dificuldade nos seus relacionamentos amorosos, angústias de morte e crises desencadeadas por situações fobogênicas: apresentações públicas, viagens, relações sexuais. Além disso, ele se impõe verificações obsessivas. A rememoração revela-se ineficaz por conta de uma clivagem dos afetos. As lembranças permanecem intelectuais. A análise vai ajudálo na organização de uma vida melhor, no desaparecimento das fobias que o debilitavam e dos sintomas obsessivos, à custa de um distanciamento relativo das relações afetivas.


This article presents Pierre’s case, a young man with difficulties in love relations, death anxiety, and crisis triggered by phobia-generating situations: public presentations, trips, sexual relations. Also he imposes himself obsessive verifications. Recalling is inefficient due to affect splitting. Memories remain intellectual. Analysis will help him in the organization of a better life, in the disappearance of phobias that handicapped him, and of the obsessive symptoms, due to a relative distancing from affective relations.


Este artículo trata del caso de Pierre, un muchacho que presenta dificultad en sus relaciones amorosas, angustias de muerte y crisis desencadenadas por situaciones fobogénicas: presentaciones públicas, viajes, relaciones sexuales. Además, él se impone verificaciones obsesivas. La rememoración se revela ineficaz debido a un clivaje de los afectos. Los recuerdos permanecen intelectuales. El análisis lo ayudará a una mejor organización de la vida, al desaparecimiento de las fobias que lo debilitaban y de los síntomas obsesivos, a costo de un distanciamiento relativo de las relaciones afectivas.

Objective: To explore influence of telmisartan on sexual function in male patients with hypertension. Methods: A total of 150 male patients with hypertension were enrolled and randomly divided into telmisartan group (n=75) and benazepril control group (n=75). Blood pressure, international index of erectile function (IIEF) and quality of sex were observed and compared between two groups. Results: After treatment, there was significant decrease in blood pressure in both groups (P0.05); compared with benazepril control group, there were significant increase in IIEF score [(11.28±1.32) scores vs. (19.48±2.49) scores], sex life frequency [(2.09±0.22) times/month vs. (6.31±0.85) times/month] and erectile proportion (62.7% vs. 82.7%), P<0.05 all in telmisartan group; Erectile function (was divided into normal, mild disability, moderate disability and severe disability, χ2=6.322, P=0.0412) and subjective sexual desire (was divided into strong, middle and weak, χ2=7.493, P=0.0326) of telmisartan group were also significantly better than those of benazepril control group. Conclusion: Telmisartan can improve sexual function and quality of sex life based on guaranteed antihypertensive effect.

PURPOSE: The mean platelet volume (MPV) is a marker of the platelet activity and is reported to increase in vascular diseases. We aimed to investigate the association between MPV and vasculogenic erectile dysfunction (ED). MATERIALS AND METHODS: MPV and platelet (PLT) levels were measured in 50 cases of ED and 40 healthy controls. The diagnosis of vasculogenic ED was based on a detailed sexual history, physical examination, laboratory assessment, and color Doppler ultrasonography. The results are given as mean+/-standard deviation of the mean. RESULTS: The mean ages of the patient and the control groups were 53.70+/-12.39 years (range 24~77 years) and 53.85+/-9.5 years (range 30~73 years), respectively (p=0.947). The MPV and PLT values were significantly higher in the patients with ED than those of the controls (7.49+/-1.4), (6.85+/-1.2), (262.97+/-68), (252.89+/-82) respectively, p0.05), and there was no correlation between MPV and either age of patients (p=0.905) or duration of ED (p=0.583). CONCLUSIONS: The platelet count and MPV was detected to be increased in patients with vasculogenic ED. This finding suggests a role for platelets in the pathogenesis of vascular complications and that the MPV would be useful in monitoring disease progression.

ABSTRACT

Objective: To study risk factors for non-alcoholic fatty liver disease (NAFLD) and vascular erectile dysfunction (ED) in patients with metabolic syndrome (MS). Methods: A total of 18 096 subjects were selected from people undergoing physical examination from 2008 to 2009 in northern cities of China by random cluster sampling method, and analyzed the risk factors for NAFLD and ED. Results: The 18 096 cases with age 18~76 (46.8±10.1) years old,containing 10 096 (55.79%) males and 8 000 (44.21%) females. Awareness rate of MS was 8.33% and prevalence rate of MS in healthy adults was 21.18%. Most common components of MS were hyperuricemia (27%, 4838/18096), obesity and overweight (21%), hypertension (20%,) and dyslipidemia (17%) in turn. Body mass index (BMI, kg/m2) and waist/hip ratio (WHR) of all MS subgroups from high to low were ED group [(28.9±1.1), (1.26±0.03)], overweight or obesity group [(27.5±2.3), (1.31±0.03)], prediabetes group [(26.8±2.6), (1.03±0.03)] and hypertension group [(26.1±1.3), (0.90±0.04)] in turn. A total of 3 721 MS patients (20.56%)complicated with NAFLD; By means of NAFLD complicated by MS as dependent variable, Logistic regression analysis indicated that increased ALT, waist circumference(WC), age, DM family history, LDL-C and BMI (β=1.004~0.479, P=0.000~0.016 in turn) were risk factors for NAFLD, and physical exercise and occupational physical work were protective factors for NAFLD. There were 106 ED males and its prevalence rate was 1.04%; Logistic regression analysis indicated that age, WC, LDL-C, DM family history and BMI (β=0.681~0.238, P=0.000~0.018 in turn) were risk factors for ED, and educational degree, physical exercise and occupational physical work were protective factors for ED. Conclusion: Risk factors for NAFLD and ED in MS were closely correlated with MS. It’s a new path to prevent and treat NAFLD and ED through correcting risk factors of MS.

Chao Yuan-fang's The Origin and Indicators of Disease(Zhu Bing Yuan Hou Lun) established a foundation for the treatment for impotence from the kidney.Most doctors in all the past dynasties thought that impotence was mainly caused by the kidney vacuity.Some doctors in the Jin and Ming dynasties treated impotence from the liver.In the Qing dynasty,Chen Shi-duo treated impotence from the heart and Han Shan-zheng treated impotence by nourishing yin.The article considers that impotence should be treated from the kidney vacuity,liver depression and blood stasis with the age,constitution,occupation aad region of the patients according to the common patterns of impotence in the modern times.

ABSTRACT

PURPOSE: We performed a retrospective study to analyze the effectiveness of implantable penile prostheses in the treatment of erectile dysfunction. MATERIALS AND METHODS: This study included 249 patients who received implants between 2001 and 2008. A total of 139 patients who underwent penile prosthesis implantation were interviewed. RESULTS: Approximately half of patients had previously used oral drugs before implantation of the prosthesis. About 45 percent had diabetes, 25.9 percent had previously undergone radical prostatectomy (RP), and 64 percent had hypertension. Exchange was performed in 5.7 percent for fracture, inadequate size, or extrusion. A total of 24.5 percent of men had immediate postoperative pain, 7.9 percent had local infection, and 8.6 percent had other complications. Patients who had previously undergone RP were 3.2 times more likely to experience a postoperative complication than patients who had not (p = 0.061). Eighty-nine (64 percent) patients returned to having sex as they had before being diagnosed with ED. Ninety-two of the men (66.2 percent) had sexual intercourse one to two times per week. One hundred twenty patients (86.3 percent) rated their level of satisfaction as good, excellent or very good, which was similar to the percentage of partners. The mean follow-up was 40 months. CONCLUSION: Higher rates of postoperative infections and mechanical problems with the implant were found in this study as compared to other studies, which was probably associated with the relative lack of experience of the trainees who were performing the surgeries. Patients with a history of RP or diabetes mellitus prior to implantation were at higher risk of postoperative complications.

ABSTRACT

A disfunção erétil tem fatores de risco similares aos das doenças cardiovasculares, mas também pode ser um fator de risco independente para tais doenças. As evidências atuais consideram a disfunção erétil como uma desordem primordialmente de origem vascular e, mais do que isso, como um marcador precoce das doenças cardiovasculares. Apesar disso, o potencial da disfunção erétil como um sinal para alertar os médicos para uma possível manifestação precoce de doenças cardiovasculares mais graves tem sido pouco explorado na prática clínica diária.

ABSTRACT

A disfunção erétil ou "incapacidade de obter e manter ereção peniana" é condição antiga na história da humanidade. No entanto, ainda é uma entidade muito frequente nos consultórios médicos que merece atenção, abordagem e tratamento específico.Apesar do grande avanço da medicina, a história clínica e exame físico são partes fundamentais no diagnóstico e conduta desses pacientes. Durante anamnese detalhada, as causas etiológicas podem ser facilmente identificadas e direcionar a forma de tratamento mais adequada.Nos últimos anos vários novos métodos terapêuticos foram desenvolvidos e outros, utilizados no passado, foram aperfeiçoados. Dentre esses, os principais são: os inibidores da 5-fosfodiesterase (5FDE), avaliação psicológica, injeções intracavernosas e próteses penianas.Com certeza, a medicação via oral utilizada sob demanda mudou a história da disfunção erétil e se tornou o tratamento mais utilizado e efetivo para esse grupo de pacientes. Por esse motivo, especialistas no assunto se referem à história da disfunção erétil como "era pré e pós-inibidores da 5FDE".

ABSTRACT

Objective: The objective of the present study was to verify if there is any relation between diabetes mellitus and male infertility. Methods: the spermograms of 43 non-diabetic subjects and 12 diabetic patients (type 1 and 2) aged 20-60 years were compared. Spermiological findings in diabetic patients were compared with those of normal individuals of the same age. Serum testosterone, prolactin, follicle-stimulant hormone, luteinizing hormone, glucose and glycosilated hemoglobin were assayed in diabetic patients. Results: Six diabetic patients (four type 1 and two type 2) presented chronic complications attributed to neuropathy and vascular insufficiency. No difference was observed in the semen characteristics (odor, color, viscosity and pH) between the control group and the diabetic patients. There were no differences between seminal concentrations and percentage of motile spermatozoa during the first hour of observation in the two groups (p < 0.05). Impotence was reported by four diabetic patients (33.3%). Erectile failure was associated with diabetic microangiopathy and neuropathy. There were no controls with impotence. No significant hormonal changes were found in the diabetic patients. Conclusions: The present results suggest that neuropathy and vascular insufficiency may be implicated in sexual dysfunction in type 1 and 2 diabetic patients, without significantly affecting the hypothalamic-pituitary-gonadal axis.


Objetivo: O objetivo do presente estudo foi verificar se há relação entre diabetes mellitus e infertilidade masculina. Métodos: Foi estudado espermograma de 43 indivíduos não-diabéticos e 12 diabéticos (tipo I e tipo II) com idade variando entre 20 e 60 anos. Os diabéticos também foram submetidos à dosagem hormonal de testosterona, prolactina, hormônio folículo-estimulante e hormônio luteinizante, além da e glicemia de jejum e hemoglobina glicosilada. Resultados: Foram encontrados seis pacientes diabéticos (quatro tipo I e dois pacientes tipo II) com complicações crônicas decorrentes da vásculo- e neuropatia diabéticas. A análise do líquido seminal dos voluntários, não-diabéticos e diabéticos, não mostrou diferenças entre as características dos sêmens: odor, cor, viscosidade e pH. Também não houve diferença entre as concentrações seminais e a porcentagem de espermatozoides móveis durante a primeira hora de observação entre os dois grupos (p > 0,05). Foi relatada impotência por quatro diabéticos (33,3%) com complicações crônicas do diabetes, secundárias à neuropatia e à angiopatia diabética. Não foi encontrada impotência em nenhum paciente do grupo não-diabético. Não foram encontradas alterações hormonais significativas nos diabéticos. Conclusões: Os resultados sugerem que a neuropatia e insuficiência vascular podem estar relacionadas à disfunção sexual em pacientes diabéticos tipo I e II, sem, entretanto, comprometimento do eixo hipotálamo-hifofisário-gonadal.
