ABSTRACT

Organ transplantation has demonstrated its significant values by its excellent effectiveness in health reconstruction and life survival, where organ donation is a major component in promoting the development of organ transplantation in China. In recent years, an important progress has been made in organ transplantation in China with an annually increased organ donation rate. In spite of this, there is a serious fact confronted by us that the donated organ quantity is insufficient, which may be solved by further improvement of medical science and public health policy. According to the international experience, an incentive system may improve the organ donation rate effectively although the hidden ethic property of the incentive system itself may have an essentially conflict with the altruism contained in the organ donation. Therefore, in this article, the property of the incentive system, the interaction between organ donation and incentive system and the ethic justification of the system was reviewed, aiming to provide a reference for the further development of the organ donation and transplantation business in China.

ABSTRACT

This study collected the questionnaire from the new participants who are middle-aged and elderly people in project of incentives led by local governments. This study focuses on the exercise group and non-exercise groups proportion of pefered incentives and the correlation of perfered incentives in these two groups. This study analyzed 9,590 middle-aged and elderly people who participated in a health promotion project with incentives and answered a questionnaire during the period of participation. The incentives were included 4 types: (1.) regional gift cards; (2.) national gift cards; (3.) rewards points that could be used at department stores and convenience stores nationwide, and (4.) donations. The result shows that both non-exercise and exercise groups preferred financial incentives over non-financial incentives. By type of monetary incentive, non-exercise group were more likely to prefer national gift certificates 1.39 times (OR: 1.39, 95%CI: 1.03-1.89); significantly more likely to prefer rewards point 1.44 times (OR: 1.44, 95%CI: 1.07-1.95) over non-monetary incentives compared to exercise group. Among the middle-aged and elderly people who participated in a health promotion program with incentives, the majority of two groups showed the preference that financial incentives over nonfinancial incentives. Comparing monetary incentives, both groups preferred local gift certificates than national gift certificates and rewards point.

ABSTRACT

Objective:This study aims to explore the satisfaction and influencing factors of scientific research incentive mechanisms for young medical staff in a children′s hospital in Beijing, and to provide references for the management department to develop relevant incentive strategies.Methods:A satisfaction questionnaire survey of scientific research incentive mechanisms was carried out during April and May, 2022. Collected data were analyzed by rank sum test and ordered logistic regression.Results:The level of overall satisfaction of 339 young medical staffs was 81.42%. The satisfaction rate of incentive factors was 80.50% and hygiene factors was 77.51%.According to the findings, there were significant differences in educational backgrounds, professional titles and job types, hygiene factors, and incentive factors ( P<0.05). The contributing factors of the job satisfaction were professional title, hygiene factors and incentive factors. Conclusions:The overall satisfaction with the incentive work of the young research staff surveyed was relatively at high-level. It is necessary to improve tailed hygiene factors to meet the characteristics young medical staff, such as setting up full-time scientific research working hours, exploring the establishment of diversified of scientific research sharing mechanisms to increase the utilization of the scientific research platform. It is important to enhance incentive factors to establish a long-term incentive mechanism for young personnel, such as setting up interdisciplinary research projects, exploring the establishment of a recommendation mechanism for outstanding young talents, in order to provide them with more opportunities for further intensive training.

ABSTRACT

Objective:To improve the clinical ability of interns by establishing the evaluation index system focusing on the training of clinical competence, and to construct the performance incentive mechanism based on the index system.Methods:The subjective evaluation method and expert consultation method were used to construct 5 first-class indexes and 15 second-class indexes. And the indexes and score setting were consulted by experts through questionnaires and in-depth interviews.Results:According to the feedback results of expert consultation, the scores of teaching ward-rounds and small lectures in the index system were need to adjust. At the same time, the minimum graduation standards were set to determine whether to participate in the final examination.Conclusion:The scores of teaching ward-rounds and small lectures have been raised, the scores of case discussion lowered, and the final examination scores are not included in the minimum standards. After the establishment of the initial index system, a series of supporting measures, including two-way selection of tutors, postgraduate reexamination, and other policies, have been carried out for performance incentives. At the same time, new teaching methods, student-oriented teaching activities, and formative evaluation have been adopted to actively improve the clinical competence of interns.

ABSTRACT

OBJECTIVE To provide ideas and inspiration for promoting the development of the generic drug industry and legal system of China. METHODS With text analysis, empirical research and other methods, the competitive generic drug incentive system in the United States was combed, its implementation effect and successful experience were analyzed, and the reference was provided for the implementation of relevant systems in China. RESULTS The competitive generic drug incentive system in the United States had achieved a satisfactory effect in three aspects: encouraging generic drug applications, accelerating the listing of generic drugs, and promoting drug price reduction, which was ultimately conducive to improving drug accessibility by publishing List of Off-Patent,Off-Exclusivity Drugs without an Approved Generic, while accelerating the development and review of generic drugs with insufficient competition and giving market exclusivity periods. CONCLUSIONS China’s reference to the competitive generic drug incentive system in the United States meets the needs of the current situation of domestic drug supply and also has the basis for industry practice. Therefore, we can refer to it, improve the system of releasing the catalog of encouraged generic drugs to be imitated, optimize the approval process for them, and authorize the exclusive period incentive to them.

ABSTRACT

Identificou e analisou possíveis implicações da participação de três professores licenciados em Educação Física (duas mulheres e um homem) em programas de incentivo à docência no desenvolvimento profissional em escolas de educação básica. Realizou-se uma pesquisa qualitativa, a partir de entrevista semiestruturada com professores licenciados em Educação Física. A análise dos dados mostrou que os impactos das vivências nos programas de incentivo à docência relacionam-se com a percepção do real papel de ser docente, com a tomada de consciência mais crítica e ampliada a partir do caráter colaborativo e coletivo nas discussões temáticas, nos planejamentos de atividades de ensino e pesquisa. Verificou-se ainda que os programas influenciaram no reconhecimento, por parte dos professores, da importância da continuidade do processo formativo, com ênfase para novas formas na abordagem dos conteúdos, através da retomada das metodologias de ensino, o repensar do binômio teoria-prática no processo ensino e aprendizagem, além da inclusão na seleção dos conteúdos trabalhados.


It identified and analyzed possible implications of the participation of three Physical Education teachers (two women and one man) in programs to encourage teaching in professional development in basic education schools. A qualitative research was carried out, based on semi-structured interviews with teachers licensed in Physical Education. Data analysis showed that the impacts of experiences in teaching incentive programs are related to the perception of the real role of being a teacher, with a more critical and expanded awareness of the collaborative and collective character in the thematic discussions, in the planning of teaching and research activities. It was also verified that the programs influenced the recognition, by the teachers, of the importance of the continuity of the formative process, with emphasis on new ways of approaching the contents, through the resumption of teaching methodologies, the rethinking of the theory-practice binomial in teaching and learning process, in addition to inclusion in the selection of contents worked on.


Identificó y analizó posibles trascendencias de la participación de tres profesores licenciados en Educación Física (dos mujeres y un hombre) en programas de fomento de la docencia en el desarrollo profesional en escuelas de educación básica. Se realizó una investigación cualitativa, basada en entrevistas semiestructuradas a docentes licenciados en Educación Física. El análisis de los datos mostró que los impactos de las experiencias en programas de incentivos docentes están relacionados con la percepción del rol real del ser docente, con una conciencia más crítica y ampliada del carácter colaborativo y colectivo en las discusiones temáticas, en la planificación de la enseñanza y Actividades de investigación. También se verificó que los programas influyeron en el reconocimiento, por parte de los profesores, de la importancia de la continuidad del proceso formativo, con énfasis en nuevas formas de abordar los contenidos, a través de la reanudación de las metodologías de enseñanza, el replanteamiento de la teoría-práctica binomio en el proceso de enseñanza y aprendizaje, además de la inclusión en la selección de contenidos trabajados.

ABSTRACT

Optimizing the salary system of general practitioners and establishing a compensation incentives mechanism fitting its needs, would contribute effectively to the development of the general practitioner system in China. Fully leveraging the salary incentive system, the United Kingdom(UK) effectively improved the working enthusiasm of general practitioners and its quality of primary medical services, and limited its medical expenses as well. The authors expounded the incentive mechanism of general practitioners′ salary in the UK from such dimensions as salary model and salary structure, and introduced the implementation effect. Based on Robbins′ comprehensive incentive theory, the authors introduced the internal logic of the UK′s general practitioners′ salary incentive mechanism. Based on the UK experiences, along with existing problems in China, the authors recommended on setting up value-based salary levels, optimizing the salary structure, and establishing a dynamic adjustment mechanism. These ideas are expected to help optimize the salary incentive mechanism for general practitioners in China.

ABSTRACT

Integration of healthcare and prevention is key path to achieving the pattern of " macro healthcare, macro health". However, it is hard at present to fully integrate healthcare and prevention in China, mostly due to the lack of effective incentive compatibility strategies. Based on the incentive compatibility theory, this study proposed a multi-level principal-agent chain of healthcare and prevention integration. Taking the incentive transmission among stakeholders in the principal-agent chain as the analysis framework, the dilemma and causes of incentive incompatibility of healthcare and prevention integration were studied.Combined with the proctice of healthcare and prevention integration in Sanming city, the incentive compatibility strategies of healthcare and prevention integration were proposed at the level of government, medical institutions, medical staff, public health staff and residents. These strategies refer to coordinating the use of medical insurance funds and public health funds, strengthening organization management as well as assessment and incentive, rewarding health promotion management points, and setting up health science popularization teams.

Objective:According to the requirements of high-quality development of public hospitals, to explore the innovative incentive path for medical youth based on the two-factor theory, and provide a reference for promoting the high-quality development of public hospitals.Methods:Using the literature analysis method, the two-factor theory, hospital scientific research incentive mechanism, and scientific research incentive mechanism for young talents were investigated. Meanwhile, combining the two-factor theory and practical experience, the problems that existed in the innovation incentive policy of public hospitals for young medical talents were analyzed, and the corresponding countermeasures were proposed to build the innovation incentive path of young medical talents under the two-factor theory.Results:Based on analyzing the demand characteristics of young medical talents, managers should distinguish health care factors and incentive factors, and implement incentives from both aspects. Provide incentives through improving the personal sense of achievement, creating a personal growth environment, and promoting professional titles to stimulate young talents' innovation motivation; implement health care factors from aspects of working conditions, material benefits, salary levels, etc.Conclusions:As a new concept of development, high-quality development is not only reflected in scientific and technological innovation-driven, but also in the innovation of management mechanisms so that institutional innovation becomes the driving force for high-quality development.

ABSTRACT

There are a large number of patients with rare diseases in China, and most of them are faced with problems such as no medicine to cure, and have drugs outside of China but not inside. This paper combed the incentive policies of orphan drugs in the United States and the European Union, including orphan drugs legislation and setting up special management institutions, orphan drugs qualification certification, government funding, tax reduction and agreement assistance in the research and development process, providing accelerated listing channels in the examination and approval process, giving market monopoly period in the circulation stage, and giving priority to review. On this basis, it also explored the incentive measures of non-profit organizations in the research and development of orphan drugs, including providing financial support, clinical research and so on. From the perspective of guaranteeing the right to life and health of patients with rare diseases, the fairness and accessibility of medication, and the subjects’ right to know and privacy in clinical trials, this paper provides reference for perfecting the incentive mechanism of orphan drugs research and development in China.

ABSTRACT

Revisão sistemática acerca da efetividade da remuneração por desempenho na melhoria de indicadores de processo/resultado em programas e serviços de atenção primária à saúde. Realizou-se busca na PubMed, Scopus, Web of Science, SciELO e Biblioteca Virtual de Saúde, resultando em 22 estudos analisados quanto ao tipo de estudo, objetivo, qualidade da evidência e principais achados. Constatou-se que a remuneração por desempenho apresentou impacto na atenção clínica às doenças, acessibilidade aos serviços, melhoria nos processos de acompanhamento e rentabilidade da utilização e apresenta-se como estratégia potencial à indução de melhorias na qualidade nos serviços de saúde, ainda que consideradas suas limitações.


A systematic review of the effectiveness of pay for performance in the improvement process of indicators/results in primary health care programs and services. A search was conducted in PubMed, Scopus, Web of Science, SciELO, and Virtual Health Library, resulting in 22 studies analyzed as to the type of study, objective, evidence quality, and main findings. It was found that the pay for performance impacted clinical care to disease, accessibility to services, improving the monitoring of processes, and profitability of use and presents itself as a potential strategy to induce improvements in the quality of health services, although considering its limitations.

ABSTRACT

Neste artigo pretende-se refletir sobre os gastos tributários do governo federal, especificamente acerca da aplicação dos recursos da Lei de Incentivo ao Esporte (Lei nº 11.438/2006). A investigação se apoiou nos documentos oficiais publicados pelo governo federal e, dessa forma, se caracteriza como uma pesquisa documental, de cunho qualitativo e quantitativo. Os dados coletados são do período inicial (agosto de 2007 a dezembro de 2015) e do segundo período (2016 a 2018) de vigência da referida Lei. A análise e a discussão dos resultados se concentram no debate sobre as categorias: fundo público, gastos tributários e o financiamento do esporte. Observou-se que há uma baixa captação dos recursos e uma concentração na sua destinação por região, manifestação esportiva, modalidade, proponentes e na realização de eventos.(AU)


The present investigation intends to reflect on the expenditures of the federal government, specifically on the application of the resources of the Sports Incentive Law (Law nº 11.438/2006). The investigation was based on official documents published by the federal government and is characterized as documentary research, to qualitative and quantitative nature. The date is from the initial period of validity of the referred Lax, (August 2007 to December 2015) and the second period (2016 to 2018). The analysis and discussion of the results focus on the debate on the categories of public funds, tax expenditures and sports financing. It was observed that there is a low level of fundraising and a concentration on its destination by region, Sporting event, modality, proponents and events. (AU)

Resumen Introducción y objetivo: Los procesos de aprendizaje y motivación han sido centrales para la comprensión de los mecanismos que subyacen al tabaquismo. En particular, existe evidencia creciente sobre la importancia de valores motivacionales de incentivo para entender el inicio y mantenimiento del tabaquismo. El objetivo general de este experimento fue evaluar el papel de la nicotina aguda sobre el valor de incentivo de una recompensa natural, (comida) asociada con un estímulo ambiental, (palanca experimental). Método: Se utilizaron ratas Wistar. Se administró nicotina (0.4 mg/kg) de manera aguda en momentos específicos del entrenamiento, utilizando un procedimiento de diez sesiones de adquisición y cuatro sesiones extinción en una tarea pavloviana de automoldeamiento. El diseño experimental incluyó tres grupos, el grupo control de solución salina y grupos de nicotina durante la adquisición y la extinción. Resultados: Se encontró que la administración aguda de nicotina, de manera específica y en comparación con los otros dos grupos experimentales, resultó en un efecto de retardo durante la fase de extinción, y que una administración similar de nicotina no resultó en efectos observables durante el desempeño comportamental en adquisición. Conclusiones: Estos resultados apoyan el papel de la nicotina como fortalecedora del valor de incentivo de las claves ambientales durante la extinción para una tarea de automoldeamiento.


Abstract Introduction and goal: Learning and motivational processes have been central for a modern understanding of tobacco addiction. There is evidence that supports the importance of incentive motivational processes for the maintenance of tobacco addiction. The main goal of the present experiment was to evaluate the effects of acute nicotine on the incentive value of a natural reward, (food) paired with an environmental cue (pressing lever). Method: Wistar rats were used. Accute nicotine (0.4 mg/kg) was administered on key sessions, using a pavlovian autoshaping procedure involving ten acquisition and four extinction sessions. The experimental design included three groups, a saline administration control group and groups with specific nicotine administration during either acquisition or extinction. Results: We found that acute administration of nicotine, in contrast with saline only or previous nicotine administration during acquisition, had an enhancing effect on responding for the environmental cue during autoshaping extinction, but we did not find evidence that acute nicotine affected acquisition performance. Conclusion: Our results are consistent with a role of nicotine enhancing the incentive value of stimuli during extinction from a pavlovian autoshaping task.

Since the establishment of the voluntary blood donation system, the rate of voluntary blood donation in China has increased significantly. But in recent years, blood donation has entered a period of slow growth, while the demand for clinical blood continues to increase, resulting in a seasonal and periodic shortage of blood. Various incentives have been used to promote blood donation in order to ensure the supply of blood for clinical use. However, the actual effectiveness of incentives to promote blood donation is unclear. This article aims to analyze the advances in incentives of voluntary blood donation, and discuss the existing problems, in order to lay a foundation for further research on more economical and effective promotion measures to establish a more perfect promotion measures management system.

ABSTRACT

Objective:To better understand the satisfaction with medical worker′s scientific research, raise the management and service levels of scientific research management department, and stimulate hospital workers devote to scientific research.Methods:This paper analyzes the main factors that affect the scientific researchers′ job satisfaction in a tertiary hospital in Shanghai based on questionnaire survey. SPSS 26.0 was used to test the reliability and validity of the scale. Two independent sample t-test, one-way ANOVA and LSD-T test are used to explore the factors that affect the scientific research enthusiasm of staffs in different positions.Results:The incentive measures of scientific research in hospital can be summarized into three different aspects: material/spiritual incentive mechanism, scientific research management system and personnel training system, the average scores were: X-≈3.97, X-≈ 4.09, X-≈ 3.84, according to which the scientific research management system has the highest satisfaction.Conclusions:The overall satisfaction of staff in the hospital needs to be improved. According to the characteristics of different staff, tailored research management measurements should be developed to mobilize the enthusiasm of hospital staff in scientific research, enhance their satisfaction, and further improve the scientific research capacity of the hospital.

ABSTRACT

The sustainable development of the teaching staff is crucial to the overall advancement of medical education reform under the background of medical and educational collaboration. With the support of the Shanghai Key Teachers' Teaching Incentive Program, Shanghai Jiao Tong University School of Medicine has established the interdisciplinary and inter-school teaching teams for undergraduate students and broken though the "last mile" of fully implementing the medical teaching reform of organ-system integrated curriculum in the country. Meanwhile, we also has reformed hourly remuneration system, formulated rules and regulations corresponding to each link, and improved the supervision mechanism, which has consolidated the mechanism of medical and educational collaboration, promoted the development of community for teachers and comprehensively improved the quality of medical education.

ObjectiveTo explore the effect of incentive spouse collaborative psychological intervention on delivery outcomes of primipara. MethodsFrom July 2019 to July 2020, 300 primiparas with an uneventful pregnancy were recruited and divided into the control group and the intervention group, with 150 cases in each group. The control group was managed by routine nursing care, and the intervention group was managed by incentive spouse collaborative psychological intervention. The delivery outcomes in two groups, including delivery route, postpartum blood loss, and perineal laceration were compared. ResultsAfter intervention, the cesarean section rate, the 2-hour postpartum blood loss and perineal laceration in the intervention group were less common than those in the control group and the difference was statistically significant (P<0.05). The first, second and total stages of labor duration in the intervention group were shorter than those in the control group and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of neonatal asphyxia between these two groups (P>0.05). In terms of vaginal delivery experience scores of the two groups, the intervention group was better than the control group and the difference was statistically significant (P<0.05). ConclusionThe mode of incentive spouse collaborative psychological intervention can reduce the rate of cesarean section, the amount of bleeding 2 hours after delivery, and perineum injury. It can shorten the labor process, and effectively improve the delivery outcome of primiparas.

OBJECTIVE:To learn from pedia tric drug pricing and price incentive policy in Japan ,and to provide reference for the improvement of pediatric drug price system in China. METHODS :The drug pricing strategy of Japan was analyzed ,and price incentive policy of pediatric drugs in Japan and its implementation status and effect were summarized. The suggestions of perfecting the price system of pediatric drugs in China were put forward . RESULTS & CONCLUSIONS :The pricing strategy of medical insurance drugs in Japan included two types ,such as access pricing of medical insurance list and price adjustment of drugs in medical insurance list. Among them ,price incentive policy for pediatric drugs mainly included the premium for initial pricing of new pediatric drugs ,the premium for drugs with newly added pediatric indication included in medical insurance list and other measures(such as eligible drugs could obtain indirect price compensation ,and children ’s factors could be considered in health technology assessment ). Overall ,these measures accelerated the R&D and marketing of Japanese pediatric drugs ,and effectively promoted the development of pediatric drugs in Japan. The author suggests that our country should carry out health technology assessment which reflects the particularity of pediatric drugs ,give appropriate direct price incentives to the payment price of pediatric drugs ,and reasonably set the price of drug specification suitable for children and give price incentives so as to promote the development of pediatric drugs in China.

ABSTRACT

This study aimed to evaluate the association between student characteristics and recruitment strategies in the adherence of college students to a web-based alcohol intervention. Participants were 46,329 Brazilian students aged from 18 to 30, who consumed alcohol during the past three months. Three recruitment strategies were implemented: open invitations, and personally-addressed invitations with or without non-monetary incentives. We evaluated the educational, sociodemographic, motivational, and alcohol consumption effects on adherence using logistic regression models. Women (aOR = 1.09 [1.04; 1.14]), students with higher income (aOR = 1.32 [1.21; 1.45]), and more motivated students (aOR = 1.04 [1.03; 1.05]) were more adherent to the intervention, as well as those reporting binge drinking (aOR = 1.26 [1.19; 1.33]) and alcohol hazardous use (aOR = 1.11 [1.05; 1.18]). The use of incentives was the main factor associated with adherence (aOR = 3.69 [2.46; 5.55]). Our results may help the development of future web-based interventions related to alcohol use.


O presente estudo teve como objetivo avaliar a influência do perfil dos estudantes e das estratégias de recrutamento na adesão de universitários a uma intervenção via web sobre o uso de álcool. Participaram da pesquisa 46.329 estudantes brasileiros com idade entre 18 e 30 anos e relatando consumo de álcool nos últimos três meses. Utilizaram-se três estratégias de recrutamento: convite aberto e convite pessoalmente dirigido com e sem incentivo não monetário. Avaliouse o efeito de características educacionais, sociodemográficas, motivacionais e de uso de álcool sobre a adesão por meio de modelos de regressão logística. Mulheres (aOR = 1,09 [1,04; 1,14]), estudantes com maior renda (aOR = 1,32 [1,21; 1,45]) e mais motivados (aOR = 1,04 [1,03; 1,05]) foram mais aderentes, bem como universitários que relataram binge (aOR = 1,26 [1,19; 1,33]) e uso de risco (aOR = 1,11 [1,05; 1,18]). O emprego de incentivos foi o principal fator associado à adesão (aOR = 3,69 [2,46; 5,55]). Os resultados podem auxiliar no desenvolvimento de futuras intervenções sobre o uso de álcool via internet.
