ABSTRACT

Introduction: Differential diagnoses between essential tremor and Parkinson's disease is challenging in some individuals, with both disorders sharing similarities. Considering these links, we hypothesized that both conditions have a similar profile for some antioxidant molecules, including 25-hydroxyvitamin D and bilirubin. Methods: We performed a cross-sectional study comparing serum levels of 25-hydroxyvitamin D and bilirubin in 31 ET patients, 38 PD, and 65 controls matched for age. We used the Fahn-Tolosa-Marin scale for the severity of tremors in the ET group. We used Hohen-Yahr and MDS-UPDRS part III scales in the PD group. In addition, we evaluated sociodemographic characteristics, including age, sex, ethnicity, years of study, duration of disease, and use of primidone. Results: We found no differences in serum levels for 25-hydroxyvitamin D or bilirubin subtype levels between the ET and PD groups. We found low levels of indirect bilirubin in the PD group compared to the controls. We did not find differences between ET and controls in all biomarkers of the study. Conclusion: ET and PD patients have similar profiles for 25-hydroxyvitamin D and bilirubin serum levels. The discovery of differences in oxidative stress biomarkers in both conditions, mainly low-cost substances available clinically, can assist in the differential diagnosis and, in the future, prognostication and better therapy management (AU).


Introdução: O diagnóstico diferencial entre tremor essencial (TE) e a doença de Parkinson (DP) é desafiador em alguns indivíduos com ambas as afecções apresentando algumas similaridades. Assim sendo, hipotetizamos que ambas têm perfil similar de algumas moléculas antioxidantes, incluindo 25-hidroxivitamina D e bilirrubina. Méto-dos: Realizamos um estudo transversal comparando os níveis séricos de 25-hidroxivitamina D e bilirrubinas em 31 indivíduos com TE, 38 com DP e 65 controles pareados por idade. A escala de Fahn-Tolosa-Marin foi usada para avaliação da gravidade do tremor no grupo com TE e Hohen-Yahr e UPDRS parte III na avaliação do grupo com DP. Também foram avaliadas as características sociodemográficas. Resultados: Não encontramos diferenças nos níveis séricos de 25-hidroxivitamina D ou bilirrubina entre os grupos TE e DP. Encontramos baixos níveis de bilirrubina indireta no grupo DP comparado aos controles. Não encontramos diferenças entre os grupos com TE e controles em nenhum dos biomarcadores do estudo. Conclusão: Pacientes com TE e DP apresentam níveis séricos semelhantes de 25-Hidroxivitamina D e bilirrubinas. Diferenças nos biomarcadores de estresse oxidativo em ambas as condi-ções, principalmente substâncias de baixo custo disponíveis na clínica, pode auxiliar no diagnóstico diferencial e, futuramente, no prognóstico e otimização terapêutica (AU).

ABSTRACT

Background Exposure to tobacco dust or noise is associated with the risk of cardiovascular disease in workers, but there are few studies on their effects on workers' serum bilirubin levels. Objective To analyze the effects of combined exposure to tobacco dust and noise on workers' serum bilirubin levels. Methods We selected 824 employees from a large cigarette factory in Wuhan. According to the status of occupational hazards on site, we divided the participants into a control group (n=149), a tobacco dust exposure group (n=198), a noise exposure group (n=299), and a tobacco dust and noise combined exposure group (n=178). We collected general information of the participants. We collected blood samples and measured serum bilirubin. We used chi-square test to compare between-group categorical indicators. We used analysis of variance to compare measurement data. Taking the control group as the reference category, we used generalized linear regression model to analyze serum bilirubin concentration across the three exposure groups and the control group. Results The abnormal rates of serum indirect bilirubin concentrations in the control group, the tobacco dust exposure group, the noise exposure group, and the tobacco dust and noise combined exposure group were 6.04% (9/149), 12.63% (25/198), 13.38% (40/299), and 17.42% (31/178), respectively, showing a clear increasing trend (P<0.05). The mean concentrations of indirect bilirubin in the tobacco dust and noise combined exposure group were significantly higher than that in the tobacco dust and the noise groups (P<0.05). The serum indirect bilirubin concentrations in the tobacco dust exposure group, the noise exposure group, and the tobacco dust and noise combined exposure group were 1.833, 1.774, and 1.634 times higher than those of the control group (P<0.05). Conclusion Occupational exposure to tobacco dust or noise may associate with elevated serum total bilirubin concentration in cigarette factory workers, mainly indirect bilirubin concentration. Serum indirect bilirubin anomaly is higher among workers simultaneously exposed to tobacco dust and noise.

ABSTRACT

Abstract Background Previous studies have suggested that systemic metabolic abnormalities are closely related to psoriatic arthritis (PsA). Gamma-glutamyl transpeptidase (GGT) and indirect bilirubin (IBIL), two essential active substances in hepatic metabolism that have been demonstrated as an oxidative and anti-oxidative factor respectively, have been proved to be involved in oxidative stress damage and inflammation in several human diseases. However, their role in PsA remains unclear. Methods In this retrospective comparative cohort study, a case group of 68 PsA patients and a control group of 73 healthy volunteers from the Third Hospital of Hebei Medical University were enrolled. Serum GGT, IBIL, GGT/IBIL ratio and C-reactive protein (CRP), a well applied bio-marker of systemic inflammatory in PsA, were compared between the two groups. Furthermore, the relationship of GGT, IBIL and GGT/IBIL with CRP were explored in PsA patients. Finally, the patients were divided into high inflammation group and low inflammation group according to the median value of CRP. Multivariate logistic regression analyses were used for the association of systemic inflammation level with GGT, IBIL and GGT/IBIL. Results Compared with healthy controls, PsA patients exhibited significantly higher serum GGT, GGT/IBIL, and CRP levels and lower IBIL levels. Serum GGT and GGT/IBIL were positively correlated with CRP, whereas IBIL were negatively correlated with CRP. Binary logistic regression analysis revealed that serum GGT was a risk factor for high CRP in PsA, whereas IBIL was a protective factor. Furthermore, GGT/IBIL was a better indicator of high CRP condition in PsA patients than either GGT or IBIL alone, as determined by the receiver operating characteristic curves. Conclusion GGT and IBIL may participate in the pathogenesis of PsA. Additionally, GGT, IBIL and the balance of the two may reflect systemic inflammation mediated by oxidative stress events related to metabolic abnormalities to a certain extent.

ABSTRACT

Background: Exchange transfusion (ET) is required in hyperbilirubinemic hospitalized neonates.Methods: The study center at SMS and associated hospitals. The blood bank is licensed and fully equipped. Fall in serum bilirubin minimum of 40 cases was required as the sample size of the present study.Results: The Sex ratio in the neonates undergoing Exchange transfusion was 65% (26) Males and 35% (14) females. The mean values of Hb (g/dl), Hematocrit and Direct bilirubin (mg/dl) of male and female before and after exchange transfusion, were non-significant.Conclusion: Exchange transfusion was required more commonly in male neonates.  The mean values of change in Hb (g/dl), Hematocrit and Direct bilirubin (mg/dl) of males and females before and after exchange transfusion, were non-significant. The mean values of change in Indirect bilirubin (mg/dl) in both males and females before and after exchange transfusion was highly significant.  The mean values of change in Total bilirubin (mg/dl) in males and in females were significant.

ABSTRACT

Objective To evaluate the curative effect of compound nutrient assisted phototherapy on neonatal jaundice.Methods Neonatologists at seven hospitals participated in the study.A total of three hundred and twenty full-term newborns with high indirect bilirubin admitted to hospital from September 2017 to January 2018 were selected.One hundred and sixty-six cases in the observation group,and one hundred and fifty-four cases in the control group,all enrolled neonates were given single-sided,conventional intensity phototherapy.Observation group took compound nutrient at the same time.The average gestational age,age,birth weight of two groups before treatment were not significantly different.Serum total biilirubin,indirect bidirubin,liver function (ALT,AST) and phototherapy time were monitored before treatment and 3 days after treatment.Results The serum total bilirubin in the observation group was significantly lower than that of the control group after 3 days of treatment[(196.7 ± 57.2) μmol/L vs (216.5 ± 54.6) μmol/L],(t=3.17,P<0.01).The indirect bilirubin in the observation group was significantly lower than that of the control group after 3 days of treatment [(176.3 ± 54.3) μmol/L vs (197.2 ± 52.9) μmol/L],(t=3.50,P<0.01).The time of phototherapy of the children in the observation group was significantly shorter than that of the control group[(19.8 ± 14.4)d vs (22.9 ± 13.3) d],(t =2.00,P < 0.01).Rash,fever,bronze disease,spilled milk,vomiting,abdominal distention,diarrhea,constipation,liver damage etc.were no significant difference the observation group and the control group(P > 0.05).Conclusion Compound nutrients had good efficacy and safety in adjuvant phototherapy for neonatal high indirect bidirubin.

ABSTRACT

Background: Beta thalassemia continues to be a significant burden to Western India particularly Saurashtra region of Gujarat. Since cost of treatment is high emphasis must be shifted from treatment to prevention that includes mass screening as most effective tool including RBC indices & peripheral blood smear. These tests have limited availability, re quire sophisticated equipments and are expensive. Thus, there is need for simple, low cost and reliable test which can be used in absence of sophisticated equipments. The present study has evaluated the validity of such test: indirect bilirubin and urine urobilinogen. Study had two objectives: 1) Estimation of indirect bilirubin, urobilinogen and Shine & Lal Index. 2) Comparing specificity and sensitivity of above test with HbA2 electrophoresis. Methods: The present study was conducted on 100 (n=100) subjects in blood bank, department of pathology, government medical college Rajkot, Gujarat, India. In first group 50 subjects (Thalassemia minor) were selected while in second group 50 (n2=50) normal individuals from hospital staff were selected. Complete-haemogram, serum-direct, indirect and total bilirubin, urine urobilinogen and their sensitivity and specificity were calculated. Results: Of the 50 cases in test group, 41 had higher Indirect Bilirubin level (>0.7 mg/dl), 35 had high urobilinogen level (>1 mg/dl). In control group out of 50 cases, 3 had high indirect bilirubin levels, 4 had high urobilinogen levels. Indirect-bilirubin had sensitivity of 82%, specificity of 94%. Urobilinogen showed sensitivity of 70% & specificity of 92%. Conclusion: Indirect bilirubin and urine-urobilinogen is a valuable, cost-effective screening test for beta-thalassemia-trait with sensitivity & specificity comparable to RBC indices.

Objective To investigate the serum lactate dehydrogenase (LDH),total bilirubin (TBIL)and indirect bilirubin (IBIL)and homocysteine (Hcy)levels change in megaloblastic anemia (MA)and myelodysplastic syndrome (MDS)in the differential diagnosis of the clinical application value.Methods Hitachi 7 600 automatic biochemical analyzer was applied to the determination of 40 patients with MA,30 with MDS patients and 40 healthy physical examination (normal control group)of serum LDH,TBIL,IBIL and Hcy levels,and application SPSS16.0 statistical software for data processing were retrospectively analyzed.Results Ma group of LDH,TBIL,IBIL and Hcy four biochemical index level was significantly higher than that of MDS group and normal control group (P0.05).MA group of LDH,TBIL,IBIL,Hcy higher positive rate higher than that of MDS group,LDH and Hcy was most significant,four biochemical indexes between the two groups compared to increase the positive rate of difference was statistically significant (P<0.01).Conclusion Joint detection of LDH,TBIL,IBIL and Hcy four biochemical index level of the differential diagnosis of MA and MDS has certain clinical value.

ABSTRACT

Objective To investigate the changes of plasma indirect bilirubin(IDB) and its role in the pathogenesis of obstructive sleep apnea hyponea syndrome(OSAHS).Methods Plasma IDB levels were detected in 30 healthy controls and 20 patiemts with OSAHS by improved J G method with 7170A automatic biochemistry analytical apparatus.Plasma IDB levels in OSAHS were compared before and an overnight or a month after CPAP treatment.ResultsPlasma IDB levels in controls were significantly higher than those in OSAHS [(16 2?5 0) ?mol/L vs (8 0?3 5)?mol/L, P 0 05] and markedly increase after a month CPAP ventilation [(8 0?3 5) ?mol/L vs (14 0?5 0)?mol/L, P
