ABSTRACT

@#The detachment and reattachment of inferior oblique (IO) have been done by ophthalmologists in inferomedial orbital wall fracture repair to avoid inducing a new diplopia in patients post-operatively. However, doing them in orbital wall reconstruction in patient who underwent maxillectomy for a malignancy has not been described yet. We describe a case where a disinserted inferior oblique was reattached after titanium mesh implantation to prevent diplopia after the surgery. This is the case of a 40-year-old male diagnosed with recurrent bilateral nasomaxillary ameloblastoma who underwent total maxillectomy of the right with removal of the inferomedial orbital wall, and detachment and reattachment of inferior oblique with no resulting inferior oblique palsy and diplopia from hypotropia or incyclotorsion. This is the first report, to our knowledge, to describe reattachment of IO in its approximate insertion during reconstruction of the inferomedial orbital wall after total maxillectomy from a malignancy with no resulting diplopia.

ABSTRACT

Abstract The purpose of this systematic review is to evaluate the safety of pre-endovascular abdominal aortic aneurysm repair (EVAR) embolization of aortic side branches - the inferior mesenteric artery and lumbar arteries. Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were followed. A search of MEDLINE and DIMENSION databases identified 9 studies published from 2011 to 2021 that satisfied the inclusion and exclusion criteria. These studies were analyzed to detect the incidence of embolization-related complications. A total of 482 patients underwent preoperative aortic side branch embolization, 30 (6.2%) of whom suffered some kind of minor complication. The only major complication observed was ischemic colitis in 4 (0.82%) patients, two (0.41%) of whom died after bowel resection surgery. Regarding these findings, aortic side branch embolization seems to be a safe procedure, with very low percentages of both minor and major complications.


Resumo O objetivo desta revisão sistemática foi avaliar a segurança da embolização de artéria mesentérica inferior (AMI) e artérias lombares (ALs) pré-correção endovascular de aneurisma da aorta abdominal. Foram realizadas pesquisas nas bases de dados MEDLINE e Dimensions. Foram encontrados 9 estudos publicados de 2011 a 2021 que atendiam aos critérios de inclusão e exclusão. Os estudos foram analisados ​​para definir a incidência de complicações relacionadas à embolização. No total, 482 pacientes foram submetidos a embolização de AMI e/ou ALs, dos quais 30 (6,2%) sofreram algum tipo de complicação menor. A única complicação importante observada foi colite isquêmica em 4 (0,82%) pacientes. Dois (0,41%) desses pacientes morreram após cirurgia de ressecção intestinal. Em relação a esses achados, a embolização de AMI e ALs parece ser um procedimento seguro, com um percentual muito baixo de complicações menores e importantes.

ABSTRACT

ABSTRACT Objective: To understand Primary Health Care nurses' role in treating Lower Urinary Tract Dysfunction. Method: Cross-sectional multi-methodological research, composed of quantitative and qualitative steps, independently and sequentially. Data collected remotely, through a questionnaire and focus group, analyzed using descriptive statistics and thematic analysis by Braun and Clarke, respectively. The project was approved under Opinion 22691119.0.0000.0030. Results: A total of 145 nurses participated in the study in the quantitative step and 20 in the qualitative step, working in Primary Health Care in Brazil. Of the 93.1% nurses who reported having already cared for people with Urinary Tract Dysfunction, only 54.4% provided guidance, mainly for training the pelvic floor muscles. Conclusion: Even though they have legal support and access to demand, nurses do not have the knowledge to offer conservative treatment for Lower Urinary Tract Dysfunction. Despite this, they were motivated to do so as long as they received specific training.


RESUMEN Objetivo: Comprender el papel del enfermero de Atención Primaria de Salud en el tratamiento de la Disfunción del Tracto Urinario Inferior. Método: Investigación transversal multimetodológica, compuesta por etapas cuantitativas y cualitativas, de forma independiente y secuencial. Datos recopilados de forma remota, a través de un cuestionario y un grupo focal, analizados mediante estadística descriptiva y análisis temático de Braun y Clarke, respectivamente. El proyecto fue aprobado bajo Opinión 22691119.0.0000.0030. Resultados: Participaron del estudio 145 enfermeros en la etapa cuantitativa y 20 en la cualitativa, actuando en la Atención Primaria de Salud en Brasil. Del 93,1% de los enfermeros que afirmaron haber atendido ya a personas con Disfunción del Tracto Urinario, sólo el 54,4% brindó orientación, principalmente para el entrenamiento de los músculos del suelo pélvico. Conclusión: Incluso con apoyo legal y acceso a la demanda, los enfermeros no tienen el conocimiento para ofrecer tratamiento conservador para la Disfunción del Tracto Urinario Inferior. Pese a ello, estaban motivados para hacerlo siempre que recibieran una formación específica.

Método: Pesquisa transversal multi-metodológica, composta por etapas quantitativa e qualitativa, de forma independente e sequencial. Dados coletados de forma remota, por meio de questionário e grupo focal, analisados por estatística descritiva e análise temática de Braun e Clarke, respectivamente. O projeto foi aprovado sob Parecer no. 22691119.0.0000.0030. Resultados: Participaram do estudo 145 enfermeiros na etapa quantitativa e 20 na qualitativa, atuantes na Atenção Primária à Saúde do Brasil. Dos 93,1% enfermeiros que referiram já terem atendido pessoas com Disfunção do Trato Urinário, apenas 54,4% prestaram orientações, sendo principalmente para treinamento da musculatura do assoalho pélvico. Conclusão: Mesmo possuindo respaldo legal e acesso à demanda, os enfermeiros não têm conhecimento para oferecer tratamento conservador para Disfunção do Trato Urinário Inferior. Apesar disso, mostraram-se motivados para tal atuação desde que recebam capacitação específica.

Resumen Se presenta el caso de un paciente con enferme dad tromboembólica venosa y contraindicación de anticoagulación en el cual se halló incidentalmente una duplicación de vena cava inferior, situación que determinó la necesidad del implante de dos filtros de protección embólica. Si bien esta anomalía vascular es de escasa presentación, es importante tener presente esta posibilidad para asegurarse de brindar una co rrecta protección a la hora del implante de filtros de vena cava inferior.


Abstract We present the case of a patient with venous throm boembolic disease and contraindication to anticoagu lation, where the incidental finding of a duplication of the inferior vena cava was made. This observation determined the need to implant two embolic protection filters. Although this vascular anomaly is rarely present, it is important to keep this possibility in mind to ensure that proper protection is provided when inferior vena cava filters are implanted.

ABSTRACT

Introduction: Two parameters of high-resolution esophageal manometry are used to observe the function of the esophagogastric junction (EGJ): the anatomical morphology of the EGJ and contractile vigor, which is evaluated with the esophagogastric junction contractile integral (EGJ-CI). To date, how these parameters behave in different gastroesophageal reflux disease (GERD) phenotypes has not been evaluated. Materials and methods: An analytical observational study evaluated patients with GERD confirmed by pH-impedance testing and endoscopy undergoing high-resolution esophageal manometry. The anatomical morphology of the EGJ and EGJ-CI was assessed and compared between reflux phenotypes: acid, non-acid, erosive, and non-erosive. Results: 72 patients were included (63% women, mean age: 54.9 years), 81.9% with acid reflux and 25% with erosive esophagitis. In the latter, a decrease in EGJ-CI (median: 15.1 vs. 23, p = 0.04) and a more significant proportion of patients with type IIIa and IIIb EGJ (83.3% vs 37.1%, p < 0.01) were found. No significant differences existed in the manometric parameters of patients with and without acid and non-acid reflux. Conclusion: In our population, EGJ-CI significantly decreased in patients with erosive GERD, suggesting that it could be used to predict this condition in patients with GERD. This finding is also related to a higher proportion of type III EGJ and lower pressure at end-inspiration of the lower esophageal sphincter in this reflux type.


Introducción: Para observar la función de la unión esofagogástrica (UEG) se utilizan dos parámetros de la manometría esofágica de alta resolución: la morfología anatómica de la UEG y el vigor contráctil, el cual se evalúa con la integral de contractilidad distal de la unión esofagogástrica (IC-UEG). Hasta el momento, no se ha evaluado cómo se comportan estos parámetros en los diferentes fenotipos de enfermedad por reflujo gastroesofágico (ERGE). Metodología: Estudio observacional analítico en el que se evaluaron pacientes con ERGE confirmado por pH-impedanciometría y endoscopia, llevados a manometría esofágica de alta resolución. Se evaluó la morfología anatómica de la UEG y la IC-UEG, y se comparó entre los diferentes fenotipos de reflujo: ácido, no ácido, erosivo y no erosivo. Resultados: Se incluyó a 72 pacientes (63% mujeres, edad media: 54,9 años), 81,9% con reflujo ácido y 25% con esofagitis erosiva. En este último grupo se encontró una disminución de la IC-UEG (mediana: 15,1 frente a 23, p = 0,04) y una mayor proporción de pacientes con UEG tipo IIIa y IIIb (83,3% frente a 37,1%, p < 0,01). No se encontraron diferencias significativas en los parámetros manométricos de los pacientes con y sin reflujo ácido y no ácido. Conclusión: En nuestra población, la IC-UEG estuvo significativamente disminuida en los pacientes con ERGE erosivo, lo que sugiere que podría ser utilizada como un predictor de esta condición en pacientes con ERGE. Este hallazgo también se relaciona con mayor proporción de UGE tipo III y menor presión al final de la inspiración del esfínter esofágico inferior en este tipo de reflujo.

ABSTRACT

La apicectomía consiste en la amputación de la fracción apical de la raíz de un diente lesionado y se considera la última opción terapéutica para mantener el diente afectado en boca. Con el objetivo de dilucidar el potencial terapéutico de la técnica quirúrgica, en el presente estudio presentamos el reporte de caso de un paciente con cuadro de hiperestesia asociada al nervio alveolar inferior debido a sobreobturación de la raíz mesial de un primer molar inferior izquierdo. Para resolver el caso realizamos apicectomía mediante abordaje convencional acompañado de medicación empírica enfocada a resolver cuadros de parestesia asociada al procedimiento. Durante los controles posteriores al procedimiento quirúrgico se pesquisa ausencia de parestesia y sintomatología dolorosa, por lo que consideramos la apicectomía como una buena opción de tratamiento en casos de sobreobturación apical que no pueden solucionarse mediante tratamiento no quirúrgico.


Apicoectomy consists of the amputation of the apical fraction of the root of an injured tooth and is considered the last therapeutic option to keep the affected tooth in the mouth. With the aim of elucidating the therapeutic potential of the surgical technique, in this study we present the case report of a patient with hyperesthesia associated with the inferior alveolar nerve due to overfilling of the mesial root of a left lower first molar. To solve the case we performed apicoectomy by conventional approach accompanied by empirical medication focused on resolving paresthesia associated to the procedure. During the controls after the surgical procedure we found absence of paresthesia and painful symptomatology, so we consider apicoectomy as a good treatment option in cases of apical overfilling that cannot be solved by non-surgical treatment.

ABSTRACT

A Imaginação Motora (IM) é a representação mental de um ato motor sem a execução real do movimento, e ativa as mesmas áreas cerebrais do movimento real, mesmo na presença de paralisia, perda de membro ou visão, podendo ser utilizado no processo de conservação e estimulação de engramas cerebrais no processo de recuperação motora de um membro paralisado. Método: Homem, 34 anos, hemiplegia direita pós Acidente Vascular Cerebral (AVC) isquêmico. Realizou exercícios com profissional de Educação Física, duas vezes/semana, 50 minutos/sessão, durante 19 semanas, além do programa convencional de reabilitação multidisciplinar. A intervenção baseou-se na IM para flexão e extensão do joelho do lado paralisado, seguida da tentativa do mesmo movimento ativo. Resultados: Amplitude de movimento ativa (ADM_A) dos flexores do joelho direito iniciou em 217° com carga mínima do equipamento (5 kg). Em seguida, o profissional solicitava ao paciente que imaginasse que estava realizando o movimento e depois tentasse realizá-lo. Após 19 semanas, a ADM_A foi de 112°. Conclusão: Ganhos em ADM_A de 8,48° para a flexão de joelho do hemicorpo paralisado representa uma diferença mínima clinicamente importante em pacientes pós-AVC. A IM aumenta a demanda cognitiva nas áreas motoras cerebrais, aumentando a plasticidade, resultando em ganhos motores que impactam no prognóstico de capacidade e funcionalidade, justificando seu uso como método de treinamento na recuperação pós-AVC. A IM associada ao treinamento de força na reabilitação contribui para a recuperação de sequelas pós-AVC.


Motor Imagination (MI) is the mental representation of a motor act without the actual execution of the movement. It activates the same brain areas as real movement, even in the presence of paralysis, missing limb or vision, and can be used in the process of conserving and stimulating brain engrams in the process of motor recovery of a paralyzed limb. Method: We report a 34-year-old patient with right hemiplegia due to ischemic stroke. He performed exercises with a Physical Educator professional, twice a week, 50 minutes/session, for 19 weeks, in addition to the conventional multidisciplinary rehabilitation program. The intervention was based on MI for flexion and extension of the knee on the paralyzed side, followed by the attempt of the same active movement. Results: Active range of motion (ROM_A) of the right knee flexors started at 217° with the minimum equipment load (11 lbs). Then, the professional asked the patient to imagine that he was performing the movement and then try to perform it. After 19 weeks, ROM_A was 112° Conclusion: The ROM_A gain of 8.48° for knee flexion of the paralyzed hemibody represents a clinically important minimal difference in post-stroke patients. MI increases the cognitive demand on the brain's motor networks, increasing plasticity, resulting in motor gains that impact the prognosis of capacity and functionality, justifying its use as a training method in post-stroke recovery. MI associated with strength training in rehabilitation contributes to the recovery of post stroke sequelae.

Objetivo: mapear os cuidados pós-operatórios em reconstrução com retalhos cirúrgicos de ferida traumática em membro inferior. Método: revisão de escopo desenvolvida de acordo com as recomendações do Joanna Briggs Institute Reviewer's Manual em bases de dados referenciais, portais de informação e literatura cinzenta. Foram traçadas duas estratégias de busca para amplo alcance das publicações. Resultados: identificados dez cuidados pós-operatórios nas reconstruções com retalhos cirúrgicos em membro inferior relacionados a momentos específicos desta fase. Sendo categorizados em: 1) Cuidados no pós-operatório imediato, 2) Cuidados no pós-operatório mediato e 3) Transição do Cuidado. Conclusão: embora as reconstruções com retalhos cirúrgicos sejam um tratamento consolidado, a assistência pós-operatória ainda é incipiente quanto aos cuidados recomendados. Não há consenso sobre a implementação dos cuidados no manejo pós-operatório. O monitoramento dos retalhos cirúrgicos, clínico ou por dispositivos, foi o único cuidado contemplado em todas as publicações selecionadas, considerado essencial no pós-operatório independente da fase(AU)


Objective: to map postoperative care in reconstruction with surgical flaps of a traumatic wound in the lower limb. Method: scope review developed according to the recommendations of the Joanna Briggs Institute Reviewer's Manual in reference databases, information portals and gray literature. Two search strategies were designed for the wide reach of publications. Results: ten postoperative care procedures were identified and related to specific moments in this phase. Being categorized into: 1) Care in the immediate postoperative period, 2) Care in the mediate postoperative period and 3) Transition of Care. Conclusion: although reconstructions with surgical flaps are a consolidated treatment, postoperative care is still incipient in terms of recommended care. There is no consensus on the implementation of care in postoperative management. The monitoring of surgical flaps, clinical or by devices, was the only care considered in all selected publications, considered essential in the postoperative period, regardless of the phase(AU)


Objetivo: mapear los cuidados postoperatorios en la reconstrucción con colgajos quirúrgicos de una herida traumática en miembro inferior. Método: revisión del alcance desarrollada según las recomendaciones del Joanna Briggs Institute Reviewer's Manual (Manual del Revisor del Instituto Joanna Briggs) en bases de datos referenciales, portales de información y literatura gris. Se diseñaron dos estrategias de búsqueda para el amplio alcance de las publicaciones. Resultados: se identificaron diez procedimientos de cuidados postoperatorios relacionados con momentos específicos de esta fase. Siendo categorizados en: 1) Atención en el postoperatorio inmediato, 2) Atención en el postoperatorio mediato y 3) Transición de la Atención. Conclusión: si bien las reconstrucciones con colgajos quirúrgicos son un tratamiento consolidado, los cuidados postoperatorios aún son incipientes. No existe consenso sobre la implementación de los cuidados en el manejo postoperatorio. El seguimiento de los colgajos quirúrgicos, clínico o por dispositivo, fue el único cuidado abordado en todas las publicaciones seleccionadas, considerado fundamental en el postoperatorio, independientemente de la etapa(AU)

ABSTRACT

A parestesia do nervo alveolar inferior é uma alteração neurossensorial, que provoca distúrbios de sensibilidade em determinadas áreas da face, essa situação pode ocorrer em diversos procedimentos cirúrgicos odontológicos. Na qual pode-se citar diversas técnicas de tratamento para tal complicação. Sob esse viés, a aplicação do laser de baixa intensidade na região afetada, vem obtendo resultados amplamente satisfatórios, apresentando uma melhora eficaz em sua recuperação sensorial ao decorrer do seu tratamento, além de ser uma via terapêutica de protocolo e método simples, baixo custo e não invasivo. Visto isso, este trabalho teve por objetivo compreender a eficácia da terapia com laser de baixa potência para o tratamento da parestesia orofacial do nervo alveolar inferior em complicações pós-operatórias em procedimentos cirúrgicos na odontologia. O estudo aqui presente baseia-se numa revisão de literatura narrativa e qualitativa, dentre as quais as bases de dados utilizadas foram: Google Scholar, PubMed e Portal BVS. A busca resultou no total de 456 artigos, dentre os quais após filtragem em leitura dos títulos, dos resumos e suas metodologias, foram selecionados 21 artigos para leitura íntegra e produção desta literatura. Portanto, por mais que haja divergência quanto ao protocolo da laserterapia para as diferentes classificações da parestesia do NAI, a literatura aponta evidências quanto a sua melhora sensorial, alguns autores mencionam através da Escala visual analógica que os lasers possuíram resultados significativos ao decorrer do tratamento(AU)


Inferior alveolar nerve paresthesia is a sensorineural alteration, which causes sensitivity disorders in certain areas of the face, this situation can occur in several dental surgical procedures. In which one can cite several treatment techniques for such a complication. The application of low-intensity laser in the affected region has obtained largely satisfactory results, showing an effective improvement in its sensory recovery during the course of its treatment, in addition to being a therapeutic route with a simple protocol and method, low cost and noninvasive. Given this, this study aimed to understand the effectiveness of low-power laser therapy for the treatment of orofacial paresthesia of the inferior alveolar nerve in postoperative complications in surgical procedures in dentistry. The present study is based on a review of narrative and qualitative literature, among which the databases used were: Google Scholar, PubMed and Portal VHL. The search resulted in a total of 454 articles, among which after filtering by reading the titles, reading the abstract and their methodologies, 19 articles were selected for full reading and production of this literature. Therefore, as much as there is divergence regarding the laser therapy protocol for the different classifications of IAN paresthesia, the literature points to evidence regarding its sensory improvement, some authors mention through the visual analogue Escava that the lasers had significant results during the treatment(AU)

ABSTRACT

La parálisis aislada del músculo oblicuo inferior constituye la parálisis menos frecuente de todas las que afectan a los músculos extraoculares. Se realiza la presentación de una paciente de seis años de edad con antecedentes de salud que fue traída por su madre a la consulta de oftalmología pediátrica y estrabismo por presentar una desviación ocular desde los primeros meses de nacida. Se planteó como posibles diagnósticos un síndrome de Brown del ojo izquierdo o una parálisis del músculo oblicuo inferior izquierdo, se confirmó el segundo diagnóstico con el test de ducción forzada. Existen varias opciones quirúrgicas para estos pacientes. El objetivo fue determinar si se logra el alineamiento quirúrgico en la posición primaria de mirada con la intervención quirúrgica propuesta de recesión del recto superior derecho 5 mm. La paciente se encuentra alineada en posición primaria de mirada, sin limitación de las versiones y sin tortícoli.


Isolated palsy of the inferior oblique muscle is the least frequent of all palsies affecting the extraocular muscles. We present a six-year-old female patient with a medical history who was brought by her mother to the pediatric ophthalmology and strabismus clinic because she presented an ocular deviation since the first months of her life. A Brown's syndrome of the left eye or left inferior oblique muscle palsy was proposed as possible diagnoses, the second diagnosis was confirmed with the forced duction test. There are several surgical options for these patients. The objective was to determine if surgical alignment in the primary gaze position is achieved with the proposed surgical intervention of right superior rectus recession 5 mm. The patient is aligned in the primary gaze position, without limitation of the versions and without torticollis.

Objetivo: Relacionar atividade física, características demográficas e clínicas, ajuste à prótese e condições de saúde relacionada à qualidade de vida de adultos com amputações de membro inferior. Método: Participaram do estudo 70 indivíduos com amputações de membro inferior de ambos os sexos. Os dados foram coletados por meio de ficha para caracterização dos participantes, do Physical Activity Scale for Individual with Physical Disabilities (PASIPD), da Trinity Amputation and Prosthesis Experience Scales­Revised (TAPES-R) e do Short Form Health Survey (SF-12). Os dados foram analisados por meio de estatística descritiva e inferencial visando correlações, comparações e associações. Foi adotado p menor ou igual a 0,05. Resultados: Os resultados demonstraram que os participantes tiveram gasto energético de 26,93 MET h/d e maiores médias de condições de Saúde Relacionado à Qualidade de Vida no domínio saúde mental, ainda apresentaram maiores médias relacionados ao Ajuste à Prótese no domínio Social (3,56) com Grau de Ajuste acima da média (6,42). Conclusão: Os participantes do estudo apresentaram bom nível de atividade física, bom ajuste à prótese, boa saúde física e mental, melhor adaptação à amputação e participação. Além disso, também foi observado uma pior relação na adaptação entre pessoas com amputação acima do joelho, quando comparadas às amputações abaixo do joelho.


Objective: To relate physical activity, demographic, and clinical characteristics, fit to the prosthesis and health conditions related to the quality of life of adults with lower limb amputations. Method: 70 individuals with lower limb amputations of both sexes participated in the study. Data were collected using a form to characterize the participants, the Physical Activity Scale for Individual with Physical Disabilities (PASIPD), the Trinity Amputation and Prosthesis Experience Scales­Revised (TAPES-R) and the Short Form Health Survey (SF-12). Data were analyzed using descriptive and inferential statistics aiming at correlations, comparisons, and associations. A p lower than or equal to 0.05 was adapted. Results: The results showed that the participants had an energy expenditure of 26.93 MET h/d and higher averages of Health Conditions Related to Quality of Life in the mental health domain, they still had higher averages related to Prosthesis Fit in the Social domain (3.56) with Adjustment Degree above average (6.42). Conclusion: Study participants had a good level of physical activity, good fit to the prosthesis, good physical and mental health, better adaptation to the amputation and participation. In addition, a worse relationship was observed in the adaptation between people with amputations above the knee, when compared to amputations below the knee.

Introducción. La acalasia es un trastorno motor del esófago caracterizado por la ausencia de peristalsis y la alteración en la relajación del esfínter esofágico inferior. La cardiomiotomía laparoscópica de Heller más funduplicatura parcial es el tratamiento estándar. La mejoría sintomática ha sido bien documentada, pero no hay suficiente información objetiva respecto a los cambios fisiológicos y radiográficos luego del procedimiento. Métodos. Estudio de cohorte bidireccional de pacientes llevados a cardiomiotomía laparoscópica de Heller, entre los años 2018 y 2021, en el Hospital Universitario San Vicente Fundación de Medellín, Colombia. Se describen variables demográficas y clínicas. Se realizaron puntaje sintomático de Eckardt, manometría esofágica y radiografía de esófago en el pre y postoperatorio. Se hizo comparación de síntomas, presión basal del esfínter esofágico inferior, presión de relajación integrada y diámetro del esófago antes y después de la intervención. Resultados. Se incluyeron 24 pacientes. El 63 % fueron mujeres y la edad promedio fue de 44 años. Los valores promedio preoperatorios vs postoperatorios fueron: puntaje de Eckardt 10,6 vs 1,4 puntos (p<0,001), presión basal de 41,4 vs 18,1 mmHg (p=0,004) y presión de relajación integrada de 28,6 vs 12,5 mmHg (p=0,001). El diámetro del esófago no presentó cambios. No hubo correlación de síntomas con los cambios de presión del esfínter esofágico inferior. El tiempo de seguimiento fue de 20 meses. Conclusiones. La cardiomiotomía de Heller es un procedimiento altamente efectivo para el tratamiento definitivo de la acalasia, logrando una mejoría subjetiva y objetiva basada en síntomas y en parámetros de manometría, respectivamente


Introduction. Achalasia is a motor disorder of the esophagus characterized by the absence of peristalsis and impaired relaxation of the lower esophageal sphincter. Laparoscopic Heller ́s cardiomyotomy plus partial fundoplication is the standard treatment. Symptomatic improvement has been well documented, but there is insufficient objective information regarding physiologic and radiographic changes after the procedure. Methods. Bidirectional cohort study of patients underwent laparoscopic Heller ́s cardiomyotomy between 2018 and 2021 at the San Vicente Fundación University Hospital in Medellín, Colombia. Demographic and clinical variables are described. Eckardt symptom score, esophageal manometry, and esophageal radiography were performed pre and postoperatively. A comparison of symptoms, baseline lower esophageal sphincter pressure, integrated relaxation pressure, and esophageal diameter before and after intervention were performed.Results. 24 patients were included. 63% were women and the average age was 44 years. The preoperative vs. postoperative mean values were: Eckardt score 10.6 vs. 1.4 points (p<0.001), basal pressure of 41.4 vs. 18.1 mmHg (p=0.004) and integrated relaxation pressure of 28.6 vs. 12.5 mmHg (p=0.001). The diameter of the esophagus did not present changes. There was no correlation of symptoms with lower esophageal sphincter pressure changes. The follow-up time was 20 months. Conclusions. Heller cardiomyotomy is a highly effective procedure for the definitive treatment of achalasia, achieving subjective and objective improvements, based on symptoms and manometry parameters, respectively

ABSTRACT

Objetivo: Describir las características craneofaciales, dentoalveolares, de tejido blando, vía aérea y el patrón de actividad muscular determinadas a través de los estudios cefalométricos y electromiográficos de individuos incompetentes labiales y con presencia de anomalías dentomaxilares de 7 a 12 años de edad. Materiales y método: Cuarenta y seis participantes con incompetencia labial fueron sometidos a una toma de radiografía lateral de perfil para el análisis cefalométrico. Para el estudio electromiográfico se consideró el patrón de actividad de los músculos Orbicular superior de los labios, orbicular inferior de los labios y temporal anterior en funciones: reposo, fonoarticulación, deglución, máximo apriete labial. Resultados: Se observó clase II esqueletal y molar, retrusión mandibular, biprotrusión incisal, biprotrusión labial, disminución de vía aérea superior. La mayor actividad muscular fue observada en máximo apriete labial. Conclusión: Los niños y niñas con incompetencia labial y anomalías dentomaxilares presentan alteraciones en las características craneofaciales, dentoalveolares, de tejido blando, vía aérea y actividad muscular determinadas a través de los estudios cefalométricos y electromiográficos.


Objective: To describe craniofacial, dentoalveolar, soft issue and airway features, and the muscular activity, determined through a cephalometric and electromyographic study in individuals with lip incompetence and dentomaxillary anomalies aged 7 to 12 years. Methods: Forty-six participants with lip incompetence underwent lateral profile radiography for cephalometric analysis. For the electromyographic study, the activity of the superior orbicularis oris, inferior orbicularis oris and anterior temporalis muscles was considered in the following functions: rest, speaking, swallowing, and reciprocal compression of the lips. Results: Skeletal and molar class II, mandibular retrusion, labial biprotrusion, incisal biprotrusion, and upper airway dysfunction were found. The highest muscular activity was observed in reciprocal compression of the lips. Conclusion: Children with lip incompetence and dentomaxillary anomalies have alterations in the craniofacial, dentoalveolar, soft issue, and airway features, and in the muscular activity , determined through a cephalometric and electromyographic study.

Purpose: The purpose of this study was to evaluate the onset of dissociated vertical deviation (DVD) and inferior oblique overaction (IOOA), their subsequent development, and their correlation with pre and postoperative parameters. Methods: Medical records of patients with infantile esotropia who underwent surgery between 2005 and 2017 were retrospectively reviewed. DVD and IOOA were measured before and after surgery. Patients were divided into two groups based on horizontal and vertical deviation at the time of presentation: those with infantile esotropia only (group A) and patients with infantile esotropia who developed vertical deviation (group B). Results: Out of a total of 102 patients, DVD occurrence was seen in 53 patients (51.9%) and IOOA was seen in 50 patients (48.04%). DVD was seen in 22 patients at the time of initial examination and in 31 patients postoperatively. IOOA at presentation was seen in 45 patients (44.1%) and 5 patients (8.8%) postoperatively. No statistical difference was found in the age of surgery, angle of deviation, mean follow?up, and mean refractive error within both groups. The postoperative motor outcome was statistically comparable between the two groups (P = 0.29). Sensory outcomes of fusion (P = 0.048) and stereopsis (P?value = 0.00063) were better in group A. Conclusion: No correlation was found between the age of occurrence and development of vertical deviation with refractive error, angle of deviation, age, or type of surgery. We found that motor outcomes are not affected but sensory outcomes are affected in patients with vertical deviations. This indicates that DVD and IOOA are developed due to inherent disruption of fusion and stereopsis

ABSTRACT

Objetivo: Describir los resultados de la técnica de entrecruzamiento del orbicular, descrita por profesores eméritos del Pando Ferrer, en el entropión senil. Métodos: Se realizó un estudio observacional descriptivo longitudinal retrospectivo con pacientes atendidos en el Instituto Cubano de Oftalmología Ramón Pando Ferrer entre enero del 2021 a junio de 2022. La muestra estuvo constituida por pacientes operados con entropión senil con la técnica de Martínez Rodríguez modificada. Se les realizó examen ocular completo con seguimiento a las 24 h, 7 y 15 días, al mes y a los tres meses posteriores a la cirugía. Las variables utilizadas fueron edad, sexo, lateralidad, tiempo de cirugía y complicaciones. Resultados: La edad media fue de 79,26 años, con predominio del sexo masculino (69,56 %) en 23 pacientes y 26 párpados; siendo 20 casos unilaterales y 3 bilaterales. Se reportó una complicación de hipocorreción (3,84 %) y un tiempo medio de 11,9 minutos. Conclusiones: La técnica descrita es un abordaje efectivo demostrado en la baja frecuencia de complicaciones, sencillo y poco invasivo para entropión senil sin laxitud horizontal grave.


Objective: To describe the results of the orbicularis crosslinking technique, described by emeritus professors from Pando Ferrer institute, in senile entropion. Methods: A retrospective, longitudinal, descriptive and observational study was carried out with patients who received attention at Instituto Cubano de Oftalmología Ramón Pando Ferrer between January 2021 and June 2022. The sample consisted of patients with senile entropion operated on with the modified Martinez-Rodriguez technique. They underwent a complete ocular examination with follow-up at 24 hours, 7 and 15 days, one month and three months after surgery. The used variables were age, sex, laterality, surgery time and complications. Results: The mean age was 79.26 years, with a predominance of the male sex (69.56 %) in 23 patients and 26 eyelids; there were 20 unilateral cases and 3 bilateral ones. One complication of hypocorrection (3.84 %) and a mean time of 11.9 minutes were reported. Conclusions: The described technique is an effective approach for senile entropion, demonstrated according to the low frequency of complications, as well as simple and minimally invasive and without severe horizontal laxity.

ABSTRACT

Objetivo: Comparar dos técnicas de cirugía reconstructiva para lesión en miembro inferior con exposición ósea y, a través de ella, diferenciar que la técnica de VAC® (Vacuum Assisted Closure, cierre asistido con presión negativa) es una alternativa con beneficio de recuperación potencial sin alteraciones significativas que pudieran llevar a un compromiso funcional. Materiales y métodos: Estudio de tipo analítico con corte prospectivo, cuantitativo y longitudinal, en el que se desarrolló la terapia con el uso del sistema de VAC® y de colgajo gemelar medial en todos los pacientes de la Clínica Stella Maris que presentaron heridas traumáticas de miembro inferior con exposición ósea de tercio medio tibial durante el periodo 2019. Resultados: Se evidenció que la medición con la escala funcional de la marcha (FAC, por sus siglas en inglés) fue mejor en los pacientes con la técnica de VAC® (dado que el 50 % tiene grado V) respecto a la técnica de colgajo (50 % en grado IV); las diferencias fueron estadísticamente significativas (p < 0,05). Se apreció que el tiempo de cierre fue mayor en la técnica de VAC® debido al proceso de regeneración progresiva hasta llenar o cubrir la zona completa de la lesión; por otro lado, se evidenció la diferencia de la intensidad del dolor posoperatorio entre las dos técnicas: de moderado a intenso con la técnica de colgajo y leve, en su mayoría, con la técnica de VAC®. Conclusiones: El sistema de aspiración VAC® es eficiente para la cobertura ósea en defectos traumáticos del tercio medio tibial anterior, por lo que constituye una alternativa con potencial beneficio de recuperación sin alteración de estructuras anatómicas, ya que brinda mejores resultados funcionales y menores complicaciones. Es una opción útil que actúa de forma segura porque estimula el cierre de la herida y minimiza las necesidades de un tratamiento quirúrgico.


Objective: To compare two reconstructive surgery techniques for lower limb injury with exposed bone and demonstrate that the VAC® (vacuum-assisted closure) negative pressure wound therapy is an alternative for potential recovery showing no significant changes that could lead to functional compromise. Materials and methods: An analytical, prospective, quantitative and longitudinal study conducted with all the patients of Clínica Stella Maris with traumatic injuries of the lower limb and exposure of the middle third of the tibia treated with the VAC® system and the medial calf flap in 2019. Results: The measurement obtained with the functional ambulation categories (FAC) scale showed better results among the patients treated with the VAC® technique (since 50 % got grade V) than those who underwent the flap technique (50 % got grade IV), being the differences statistically significant (p < 0.05). It was observed that the time to closure was longer with the VAC® technique due to the progressive regeneration process consisting of the complete filling or coverage of the lesion area. On the other hand, the difference in the postoperative pain intensity between the two techniques was evident, being moderate to intense with the flap technique and mild, for the most part, with the VAC® technique. Conclusions: The VAC® suction system is effective for bone coverage in traumatic defects of the anterior middle third of the tibia. It is an alternative for potential recovery that does not change the anatomical structures because it provides better functional results and fewer complications. It is a useful and safe option that stimulates wound closure and minimizes the need for surgical treatment.

Esta es la descripción más frecuente según los autores consultados. El objetivo fue analizar las variaciones en el origen y distribución de las arterias sigmoideas mediante disección. Se utilizaron 13 preparados cadavéricos formolizados al 10 %. Se disecó la cavidad abdominal para identificar a las arterias sigmoideas. Se evidenció su bifurcación paralela al colon sigmoideo. Se lo delimitó mediante reparos palpables. Patrón I: 4 casos (30,8 %). Variante de la arcada sigmoidea como rama colateral de la arteria mesentérica inferior. Tipo Ia: 1 caso (25 %). Sin asociaciones. Tipo Ib: 1 caso (25 %). Asociada al tronco sigmoideo. Tipo Ic: 2 casos (50 %). Asociada a arterias sigmoideas accesorias. Patrón II: 6 casos (46,2 %). SUMMARY: The sigmoid arteries are branches of the inferior mesenteric artery and supply the sigmoid colon. They originate from the trunk of the sigmoids. This is the most frequent description according to the consulted authors. Introducción: La baja incidencia del leiomiosarcoma de la vena cava inferior dificulta tanto la estandarización del diagnóstico como el tratamiento. Objetivo: Presentar el manejo realizado en nuestro centro de un paciente que desarrolló un leiomiosarcoma de vena cava inferior, una patología de baja incidencia y que las posibilidades de realizar un rescate quirúrgico son muy bajas. Resultados: Se presenta el caso de un paciente de 54 años con una tumoración sólida en porción infrarrenal y yuxtarrenal de vena cava inferior de 71 × 76 × 117 mm compatible con leiomiosarcoma de vena cava, con infiltración de uréter derecho que ocasiona uropatía obstructiva derecha grado I-II sin alteración de la función renal, que fue resecada y reconstruida mediante prótesis sin complicaciones. Discusión: Se discute la fisiopatología, el diagnóstico y manejo en relación con el caso presentado. Conclusión: la baja incidencia de estos tumores dificulta tanto la estandarización del diagnóstico como del tratamiento, aunque la cirugía sigue siendo el tratamiento de elección.


Introduction: The low incidence of leiomyosarcoma of the inferior vena cava hinders both the standardization of diagnosis and treatment. Objective: To present the management carried out in our center of a patient who developed an inferior vena cava leiomyosarcoma, a low incidence pathology with uncertain surgical rescue. Results: 54-year-old patient with a solid tumor in the infrarenal and juxtarenal portions of the inferior vena cava of 71 × 76 × 117 mm compatible with leiomyosarcoma of the vena cava, with infiltration of the right ureter that causes right obstructive uropathy grade I-II without kidney function changes; tumour was resected and continuity reconstructed with a prosthesis without complications. Discussion: The pathophysiology, diagnosis and management are commented. Conclusion: the low incidence of these lesions makes it difficult to standardize both diagnosis and treatment, although surgery remains the treatment of choice.

A hipoplasia da veia cava inferior é uma patologia rara que integra o conjunto de anomalias do desenvolvimento da veia cava inferior. A sua incidência situa-se entre 0,3%-0,5% na população saudável e 5% nos adultos jovens sem fatores de risco para trombose venosa profunda, sendo considerada um importante fator de risco para o desenvolvimento de trombose dos membros inferiores. O principal objetivo deste trabalho é reportar a conduta obstétrica de um caso clínico de uma grávida diagnosticada com hipoplasia da veia cava inferior, prévia à gravidez. Trata-se de um caso clínico, de uma grávida, primigesta, com 37 anos, com hipoplasia da veia cava inferior e heterozigotia para o gene MTHFR677 diagnosticadas, na sequência de uma trombose venosa bilateral dos membros inferiores e do segmento infrarrenal da veia cava inferior. A gravidez foi seguida em consulta hospitalar na nossa instituição, tendo a grávida sido medicada com enoxaparina em dose profilática e ácido acetilsalicílico, com um período pré natal que decorreu sem intercorrências. Às 37 semanas e 6 dias de gestação, deu entrada no Serviço de Urgência de Obstetrícia por rotura prematura de membranas. Intraparto foram utilizadas meias de compressão pneumática intermitente, tendo o parto ocorrido às 38 semanas de gestação por via vaginal (parto eutócico), do qual nasceu um recém-nascido do sexo feminino, com 2620g e índice de Apgar 9/10/10. O presente caso clínico demonstra que em situações de hipoplasia da veia cava inferior com um seguimento obstétrico adequado é possível a realização de um parto vaginal, possibilitando um desfecho obstétrico favorável (AU).


Hypoplasia of the inferior vena cava is a rare condition that belongs to the group of developmental anomalies of the inferior vena cava. It has an incidence between 0.3% and 0.5% in the healthy population and 5% in young adults without risk factors for deep venous thrombosis, being considered an important risk factor for the development of lower limb thrombosis. This study aims to report the obstetric conduct of a clinical case of a pregnant woman diagnosed with hypoplasia of the inferior vena cava prior to pregnancy. This is a clinical case of a pregnant woman, primigravid 37 years old, with hypoplasia of the inferior vena cava and heterozygosity for MTHFR677, diagnosed following a bilateral venous thrombosis of the lower limbs and the infrarenal segment of the inferior vena cava. The pregnancy was followed up in our institution. The pregnant woman was medicated with a prophylatic dose of low molecular weight heparin and acetylsalicylic acid with an uneventful prenatal period. At 37 weeks and 6 days of gestation, she was admitted to the Obstetrics Emergency Service due to premature rupture of membranes. Intermittent pneumatic compression sockings were used intrapartum, and at 38 weeks of gestation, a female newborn was vaginally delivered (eutocic delivery) with 2620g and an Apgar score of 9/10/10. The present clinical case demonstrates that in situations of hypoplasia of the inferior vena cava with an adequate obstetric follow-up, it is possible to perform a vaginal delivery, enabling a favourable obstetric outcome (AU).

ABSTRACT

Nas cirurgias odontológicas é possível que ocorra alguns acidentes ou complicações que podem interferir no dia a dia do paciente. A lesão do nervo alveolar inferior é uma complicação decorrente de cirurgias orais que podem causar um distúrbio de sensibilidade transitória ou persistente, na região do lábio inferior e na região delimitada do forame mentoniano e hemi-arco da mucosa. O diagnóstico da parestesia pode ser feito através de testes mecanoceptivos e nocioceptivos em que o profissional escolherá para qual melhor se adapte no paciente. O objetivo do presente trabalho foi abordar por meio de uma revisão de literatura as formas de diagnóstico e tratamento da parestesia do nervo alveolar inferior decorrentes de cirurgias orais. As bases de dados utilizadas para confecção desta revisão são encontradas nas bibliotecas virtuais eletrônicas: BVS (Biblioteca Virtual em Saúde), LILACS (Literatura Latino-Americana e do Caribe em Ciências da Saúde) e Pubmed. Como critério de inclusão foram selecionados artigos publicados de 2012 a 2022 em língua inglesa, portuguesa e espanhola. Após a leitura do título e resumo dos resultados da pesquisa e aplicação dos critérios de inclusão e exclusão dentre eles foram excluídos 883, pois não se enquadravam no nosso critério de inclusão. No final, foram selecionados 13 estudos por meio de base de dados, que serviram de base para esta revisão. Concluímos que as formas de tratamento para a parestesia na literatura, são um pouco escassas e conflitantes, mas relata que o uso da laserterapia e acupuntura tem sido uma forma de tratamento com um bom índice de sucesso, e para os casos que não sejam suficientes tais tratamentos, pode-se optar por uma cirurgia(AU)


In dental surgeries it is possible to have some accidents or complications that can interfere with the patient's day. Injury to the inferior alveolar nerve is a complication resulting from oral surgeries that can cause a disturbance of sensitivity that can be transient or persistent in the region of the lower lip and in the delimited region of the mental foramen and hemiarch of the mucosa. The diagnosis of paresthesia can be made through mechanoceptive and nocioceptive tests that the professional will choose, which best suits the patient. The objective of the present work is to approach, through a literature review, the forms of diagnosis and treatment aimed at inferior alveolar nerve paresthesia resulting from oral surgeries. The databases used for this review are found in the virtual electronic libraries: VHL (Virtual Health Library), LILACS (Latin American and Caribbean Literature on Health Sciences) and Pubmed. As inclusion criteria, articles published from 2012 to 2022 in English, Portuguese and Spanish were selected. After reading the title and summary of the research results and applying the inclusion and exclusion criteria, 883 were excluded, as they did not meet our inclusion criteria. In the end, 13 studies were selected from the database, which served as the basis for this review. We conclude that the forms of treatment for paresthesia in the literature are a little scarce and conflicting, but it reports that the use of laser therapy and acupuncture has been a form of treatment with a good success rate, and for cases that are not enough, such treatments, one can opt for surgery(AU)
