ABSTRACT

Background: Health care related expenditure is increasing with a rapid pace in which much proportion of expenses is on medicines. The expenditure can be reduced by prescribing generic medicines (Janaushadi). In India less prescription of generic medicines can be correlated to less knowledge. Hence this study was carried out among Interns who will be future practitioners to know their perception and practice about generic medicines. Objectives: To assess the Knowledge, Attitude and practice of Interns regarding generic medicines. Methodology: A cross sectional study was carried out among Interns using a pre-tested semi structured questionnaire containing questions related to their Knowledge, Attitude and their practice regarding generic medicines. Data was entered in MS Excel and analysed using SPSS version 20. Descriptive statistics were used. Results: Among the study participants, majority (90%) knew the correct meaning of the generic medicine. 97% of them felt that generic medicines can be prescribed instead of branded medicines. Majority (92%) of the study subjects felt that there should be one Janaushadi (generic medicines) store at every government hospital. Not many (19%) were aware of the law in India for prescribing generic medicines but majority (87%) knew about the Janaushadi Scheme of Government of India. 87% of the Interns had prescribed generic medicines to patients and among them they (85%) felt that generic medicines are easy to prescribe than branded medicines. Conclusion: This study showed that the Interns have good perception regarding generic medicines. Minor proportion of the study participants knew about the law in India for prescribing generic medicines. Majority of the participants have prescribed generic medicines to patients and felt that it is easy to prescribe generic medicines.

ABSTRACT

Background: Use of simulation-based learning methodologies enhance performance in both simulated resuscitations and real-life clinical situations. Retention of skill is very important for the MBBS doctors who hardly get any exposure to such learning before entering post-graduate courses. The major cause of early neonatal death is neonatal asphyxia, which can be prevented by neonatal resuscitation. Effective resuscitation at birth can prevent neonatal deaths to a greater extent. Training in cardio-pulmonary resuscitation is critical in determining successful outcomes in perinatal asphyxia. Hence the present study was done to check the retention of knowledge after Neonatal Resuscitation Program (NRP ) training. Methods: A prospective interventional study was carried out among the MBBS interns who started their internship program from March 2020 - February 2021 in ACSR Government Medical College, Nellore. All the Interns were given training in NRP as a part of internship orientation programme during the first week of internship. Out of 150 interns, 143 participated in the study. The interns were oriented to the study and a pretest questionnaire was administered to them to assess their knowledge and skill regarding NRP which was followed by NRP training by the faculty of pediatrics and neonatology. The same questionnaire was administered immediately at the end of the training, 1 and 6 months post NRP training for assessing their knowledge and skill. Results: As compared to pre intervention score, the mean scores of the immediate post intervention score and at 1 month after intervention the mean score was increased with respect to knowledge and skills. After 6 months the mean scores pertaining to knowledge and skill decreased which were statistically significant. Conclusions: A significant decrease in the level of knowledge and skills regarding neonatal resuscitation was observed. Therefore, maintaining resuscitation skill requires continuous practice, supplemented by simulation training as needed.

ABSTRACT

Background: Adverse drug reactions (ADRs) are one of the leading causes of death and are responsible of 5–10% of the hospital costs. Every drug having more or less adverse effects which may leads to medical and economic burdens on patients. Some ADRs are severe enough to increase the risk of death. Pharmacovigilance aims to detect and monitor the ADR, to identify the risk factors responsible of it, and to disseminate the knowledge required for the improvement of prescription of drugs. Aim and Objective: This study aims to assess the knowledge, attitude, and practice (KAP) of pharmacovigilance in MBBS interns of rural tertiary health center of Maharashtra. Materials and Methods: This was a cross-sectional study to evaluate the KAP regarding pharmacovigilance among MBBS interns. KAP questionnaire contained 20 questions along with options and correct answer. For pre-test, 20 min were given to them for answering all questions then a seminar (an educational intervention) was taken on pharmacovigilance and its importance and last, post-test was conducted in 20 min. Results: A total of 75 MBBS interns attended the CME on pharmacovigilance and filled the pre- and post-test questionnaire. Fifty-three (70.66%) participants were not known when the Pharmacovigilance Program of India (PvPI) started and 57 (76%) participants were unaware of regulatory body which is responsible for monitoring ADR in PvPI. Forty-seven (62.66%) participants had no clue that a serious adverse event should be reported within a stipulated time period to the regulatory body. Conclusion: Interns are new budding health care workers and they do not have sufficient knowledge of pharmacovigilance. Inclusion of pharmacovigilance in undergraduate curriculum, conducting CME for doctors, nurses, and pharmacist and motivation of doctors for reporting ADRs are some measures which can improve the spontaneous reporting system, patient safety, and Indian pharmacovigilance program.

ABSTRACT

Background: Newly qualified medical practitioners in South Africa (SA) are part of the frontline health care workers who face Africa's most severe coronavirus disease 2019 (COVID-19) pandemic. The experiences of interns during the pandemic reflect SA's preparedness to respond in a crisis and inform strategies that could be adopted to balance training and service in resource-challenged contexts. Aim: To explore the strengths, weaknesses, opportunities and threats posed during the first wave of the COVID-19 pandemic as reflected on by interns within the clinical training platforms in SA. Setting: Public hospitals in KwaZulu-Natal. Methods: An online questionnaire consisting of eight open-ended questions based on the SWOT framework related to personal and professional perspectives to clinical training during the COVID-19 pandemic was developed using SurveyMonkey. All data were collected remotely via social media platforms. Data were thematically analyzed. Results: Forty-six interns reflected on personal and systemic challenges as the major threats and weaknesses in intern training during the COVID-19 pandemic. Extrapolating on strengths and opportunities, there were three overarching learnings interns reflected on. These related to being a medical professional, communities of practice and the development and enhancement of clinical and non-clinical competencies. Existing challenges in the environment exacerbated the threats posed by COVID-19 and innovative strategies related to improving support, feedback, broadening the intern curriculum and online training. Conclusion: Although the clinical environment where interns learn and work is often stressful and overpowered by high service burdens, there are unique opportunities to enhance self-directed learning and graduate competencies, even in the midst of the COVID-19 pandemic.

ABSTRACT

@#Due to restrictions brought by the COVID-19 pandemic, occupational therapy (OT) programs in the Philippines postponed face-to-face internship indefinitely. While guidelines encourage the use of the different alternative strategies in emergency remote learning, many Filipino clinical supervisors are apprehensive about online internship to prepare interns for clinical practice. In response to the growing concerns regarding online internship, an online forum was organized and attended by 23 clinical supervisors who shared their experiences and reflections. The online forum included sharing of speakers from major practice settings, breakout sessions, and sharing of insights from the breakout sessions. Qualitative data were collected and analyzed. Four themes emerged: issues and challenges in using telehealth as part of OT internship; maximizing technology in OT internship; re-envisioning competencies of students and internship supervisors towards the quality of client care, and; potentialities for the future of OT internship. Components of online internship will stay and must be further developed even after the pandemic. Through the forum, clinical supervisors can achieve collective goals in order to effectively educate OT interns amid unprecedented times.

ABSTRACT

Background@#Andragogy, the theory of adult learning is relevant to medical education especially in the clinical internship learning context. It focuses on the students, faculty, and patients interacting together while developing competencies as future practitioners. Medical interns are active adult learners, enhancing their development of knowledge, skills, and positive attitudes, and pursuing professionalism as they participate in case-based discussions (CBD).@*Objectives@#The study sought to identify the basic principles of andragogy while medical interns participate in case-based discussions including the end-of-course satisfaction. It also determined the selected demographic factors associated with the andragogic principles. @*Methodology@#Respondents included 80 interns by convenience sampling who rotated at the Department of Ophthalmology and Visual Sciences in a tertiary referral hospital. The study used a combination of descriptive quantitative and qualitative research design. A valid instrument using the Adult Learning Principles Design Elements Questionnaire (ALPDEQ) was used to measure the medical intern's andragogic orientation as well as end-of-course satisfaction. A direct, non-participant observation of case-based discussions conducted at the department was done by the researcher and research associate. @*Results and Conclusion@#The occurrence of andragogic principles specifically motivation, experience, need to know, readiness, and self-directedness was observed during the conduct of case-based discussion. There was no association between principles of andragogy and demographic factors such as age, gender, civil status, medical school, preparatory medical course, and place of origin. CBD is an effective learning strategy, which provides medical interns adequate venues to be self-directed and apply the principles of andragogy in a workplace-based setting.

ABSTRACT

@#Objective. This study aimed to assess the knowledge, attitude, and practice on the Basic Life Support (BLS) among the Philippine General Hospital medical interns, compare the scores between trained and untrained interns, and determine the proportion of respondents with updated BLS training. Methods. This was a cross-sectional study using a thirty-five item questionnaire on the knowledge, attitude, and practice of medical interns at the Philippine General Hospital on the BLS. Results. A total of 262 interns were included in the study. Although the mean knowledge scores of untrained and trained medical interns did not differ significantly (7.00 vs. 7.30, p=0.1637), more than 20% committed mistakes on questions on the following: jaw-thrust, rescue breaths, defibrillation, and CPR check. Generally, interns followed the BLS guidelines in their practice and had a positive attitude towards BLS. However, only 37% of trained participants had training within the past two years. Conclusion. Most medical interns have a fair knowledge on BLS regardless of training status and adhere to them in practice. Since the training duration did not affect their knowledge scores, initial training in medical school and continuous exposure in the wards may have provided adequate BLS reinforcement. Recertification may not be needed during their formative years in medical school.

ABSTRACT

The current approach to management of caries has evolved towards minimal invasive dentistry comprising of early diagnosis of incipient lesions, remineralization and conservative dental treatment with minimal intervention if required. Despite the focus of the dental curriculum on minimal invasive dentistry, not much of it is adopted into practice by the students. We wanted to assess the knowledge, attitude, and skills of minimal invasive dentistry amongst the dental interns of Dakshina Kannada district. MethodsThe data was gathered through an online questionnaire developed and validated by experts. ResultsA total of 244 dental interns responded to the online questionnaire survey. Majority was aware of the principles of minimal invasive dentistry mainly through lectures, although not many had practiced it.
