ABSTRACT

Introducción: El riesgo de caídas es de carácter multifactorial, ya sea debido a factores extrínsecos o intrínsecos. Las caídas se consideran el principal mecanismo de lesión no intencional en los mayores. Objetivo: Determinar la prevalencia de caídas y factores de riesgo intrínsecos en personas adultas mayores de la ciudad de Barranquilla (Colombia). Materiales y métodos: Estudio descriptivo correlacional, realizado en 98 adultos mayores, tanto institucionalizados como en comunidad, residentes en Barranquilla (Atlántico), Colombia. Se aplicó una encuesta sobre factores de riesgo intrínsecos; el riesgo de caída se midió con la escala de Berg y se establecieron niveles de Vitamina D en sangre. Resultados: El promedio de edad fue de 75 años, con edades más avanzadas en población institucionalizada. La prevalencia general de caídas fue de 49 % en el último año. El 68,8 % de la población que presentó caídas fueron mujeres, con mayor proporción de caídas en los residentes en hogares geriátricos (52,1 %). Las mujeres provenientes de la comunidad se cayeron más que los hombres (91,3 %) lo cual muestra significancia estadística frente a las caídas (p = 0,002). El consumo de antihipertensivos fue el de mayor proporción (92 %), lo cual demuestra que es estadísticamente significativa para la población institucionalizada (p = 0,0224). El riesgo de caída alto y moderado fue mayor en la población institucionalizada (32 %). El 79,2 % de los individuos que presentaron caídas presentó niveles de vitamina D insuficientes. Conclusión: La población adulta mayor presenta una alta prevalencia de caídas, especialmente las mujeres y la población institucionalizada en centros geriátricos. Los hechos que presentaron significancia estadística fueron ser mujer y provenir de la comunidad, junto con el consumo de antihipertensivos en población institucionalizada.


Introduction: Fall risk can be related to multiple factors, whether they are of extrinsic or intrinsic nature. Falls are considered the main mechanism of accidental injury in the elderly. Objective: To determine the prevalence of falls and intrinsic risk factors in older adults in the city of Barranquilla (Colombia). Materials and methods: A descriptive correlation study was carried out on 98 institutionalized and community elderly adults from Barranquilla (Atlántico), Colombia. A survey on intrinsic risk factors was applied; the risk of falling was measured with the Berg scale and levels of Vitamin D in the blood were established. Results: The participant's average age was 75 years old, considering that the oldest ages were in the institutionalized population. The general prevalence of falls was 49% in the last year, 68.8% of the population that presented falls were women, and the highest proportion of falls occurred with residents of geriatric homes (52,1%). Women coming from the community fell more than men (91,3%) showing statistical significance compared to falls (p-value of 0,002). The consumption of antihypertensive drugs was the one with the highest proportion (92%), which showed statistical significance for the institutionalized population (p-value of 0,0224). The high and moderate risk of falls was higher in the institutionalized population (32%). Insufficient vitamin D levels (79,2% of those that fell) had a higher prevalence in women, without statistical significance. Conclusion: The elderly population has a high prevalence of falls, especially in women and in the population institutionalized in geriatric centers. The facts that presented statistical significance were being a woman and coming from the community, along with the consumption of antihypertensives in the institutionalized population.

Introducción: La degeneración combinada subaguda (DCS) es un trastorno caracterizado por la degeneración difusa de la sustancia blanca a nivel del SNC, que afecta específicamente los cordones posteriores y laterales de la médula espinal, con pérdida de la mielinización periférica y central. De manera frecuente, las manifestaciones clínicas son parestesias y debilidad generalizada causada por deficiencia de vitamina B12. Presentación del caso: Paciente masculino de 79 años, con cuadro clínico de 3 meses de evolución de limitación funcional para la marcha acompañado de desorientación. Al examen físico evidenció desorientación, cuadriparesia e hiporreflexia, con niveles séricos bajos de vitamina B12, RM cervical con focos hiperintensos en el segmento C3/C6 y endoscopia de vías digestivas altas con atrofia de la mucosa gástrica. Presentamos un caso clínico de DCS. Discusión: Este es un caso de DCS que se manifiesta por medio de una alteración neuropsiquiátrica, con una presentación inicial inespecífica que comprende deterioro de la marcha, movimientos anormales con afectación cognitiva y psiquiátrica dada por alucinaciones visuales y desorientación. Su sospecha es importante en pacientes con factores de riesgo por medio del conocimiento de la patología, para una adecuada sospecha diagnóstica y una instauración oportuna de reposición vitamínica, la cual presenta una excelente respuesta. Conclusión: La DCS es un trastorno en el que se evidencia anemia con deficiencia de vitamina B12, des-mielinización del tejido nervioso y en muchos casos signos sugestivos de atrofia gástrica, y para ello es crucial la detección temprana de esta enfermedad por medio de la determinación de niveles séricos de vitamina B12, asociado a síntomas neurológicos, para así lograr su adecuado diagnóstico y tratamiento.


Introduction: Subacute combined degeneration (DCS) is a disorder characterized by diffuse degeneration of white matter at the CNS level, specifically affecting the posterior and lateral cords of the spinal cord, also with loss of peripheral and central myelination, frequently the clinical manifestations are paresthesias and generalized weakness caused by vitamin B12 deficiency. Case presentation: A 79-year-old male patient with a 3-month history of functional limitation for walking accompanied by disorientation. On physical examination, he revealed disorientation, quadriparesis, and hyporeflexia, with low serum levels of vitamin B12, cervical MRI with hyperintense foci in segment C3/C6, and upper digestive tract endoscopy with atrophy of the gastric mucosa. We present a clinical case of DCS. Discussion: This is a case of DCS that manifests itself through neuropsychiatric alteration with a nonspecific initial presentation with gait impairment, abnormal movements with cognitive and psychiatric affectation given by visual hallucinations and disorientation. Its suspicion is important in patients with risk factors. risk through knowledge of the pathology for an adequate diagnostic suspicion and a timely establishment of vitamin replacement for which it presents an excellent response. Conclusion: DCS is a disorder where anemia with vitamin B12 deficiency, demyelination of the nervous tissue and in many cases signs suggestive of gastric atrophy are evident, for which early detection of this disease is crucial through the determination of serum levels of vitamin B12 associated with neurological symptoms, in order to achieve its proper diagnosis and treatment.

ABSTRACT

The etiology of subacute combined degeneration (SCD) of the spinal cord is closely associated with vitamin B12 (VitB12) deficiency. The clinical manifestations of SCD are complex and vary substantially. Due to some SCD patients with atypical manifestations and concomitant autoimmune disorders, the probability of misdiagnosis and missed diagnosis is still relatively high in the early stage. We report the cases of two patients who were missed or misdiagnosed at another hospital because of the normal initial VitB12 level and partial overlap of clinical manifestations, finally diagnosed as SCD with atypical manifestations and concomitant autoimmune disorders, pharyngeal-cervical-brachial Guillain-Barre syndrome in Case 1 and SCD with autoimmune thyroiditis in Case 2. After undergoing corresponding treatment, death was reported in Case 1 and improvement in Case 2. Analysis of the clinical manifestations and investigation of the underlying pathogenesis in such patients could help improve the rate of early diagnosis and allow timely treatment of SCD, thereby preventing disease progression and poor clinical outcomes.

ABSTRACT

Objective: To explore the positive rate of intrinsic factor antibody (IFAb) and level of vitamin B(12) (VitB(12)) in normal physical examination population and the possible relation between IFAb, VitB12 and sex, age, number of RBC, HGB and MCV. Methods: A total of 1 427 people who came to Peking Union Medical Colleague Hospital (PUMCH) for physical examination were enrolled. There were 758 males with average age of (52.5±14.5) years-old and 669 females with average age of (50.3±14.3) year-old. Beckman DxI800 automatic biochemical-immune analyzer and corollary reagents were used to analyze the level of serum IFAb and VitB(12). The results in different sex, age were documented and their correlation with the value of whole blood cell count was tested later on. Results: Among the 1 427 normal subjects, 66 (4.63%) were positive for IFAb. The positive rate for IFAb in the population≥40 years-old was higher than those<40 years-old (5.66% vs 1.48%, χ(2)=7.46, P=0.006). The deficiency rate of VitB(12) in the population<40 years-old, 40-59 years-old and ≥60 years-old was 2.22%, 2.51% and 5.50%, respectively (χ(2)=8.55, P=0.014). There were no difference between people with different sex in the positive rate of IFAb (5.15% for males and 4.04% for females, χ(2)=0.99, P=0.320) or in the deficiency rate of VitB(1)2 (3.83% for males and 2.69% for females, χ(2)=1.44, P=0.230). The results of multiple linear regression showed that HGB level of IFAb positive subjects was 3.05 g/L lower on average than those of IFAb negative, but IFAb had no effect on both RBC and MCV. There was no correlation between VitB(1)2 deficiency and HGB, RBC and MCV. Conclusion: The positive rate of IFAb and deficiency rate of VitB(1)2 increase as age increases. But the presence of VitB(12) deficiency is later than the positive findings of IFAb. IFAb showed some effects on the level of HGB, which may compensate the limitations of VitB(12) detection to some extent. It is necessary to check the IFAb and level of VitB(12) in people with middle or old ages.

ABSTRACT

<p><b>Objective: </b>To identify the clinical and epidemiological characteristics of pernicious anemia (PA) in patients treated at Miyagami hospital on Tokunoshima Island.</p><p><b>Methods: </b>We evaluated 14 patients with PA who were enrolled and treated from March 2009 to May 2014.</p><p><b>Results: </b>The majority of patients were older than 50 years of age and primarily consisted of elderly women. Routine medical examinations revealed macrocytic anemia in 6 of the 14 patients (43%), although they were clinically asymptomatic and subsequently diagnosed with PA. The average number of patients per year was reported to be 3 (95% confidence interval [CI]: 1.25-3.42).</p><p><b>Conclusions: </b>The annual incidence of PA per 100,000 individuals on Tokunoshima Island is possibly much higher than that previously reported in Japan. An increase in the elderly population, a negligence of the disease, and an ethnic and regional diversity may explain this discrepancy. As the annual incidence of PA may be higher than that previously reported, anemia must be carefully evaluated in the differential diagnosis of PA.</p>

ABSTRACT

Objective To investigate serum levels of β-human chorionic gonadotrophin (β-HCG) and progesterone (P) in early pregnant women,and their relation to luteal maintenance therapy for early pregnancy protection.Methods One hundred and thirty five infertility women treated in Department of Gynecology and Obstetrics of Beijing Chaoyang Hospital from January 2007 to December 2011.Total 150pregnancy cycles,including 84 with intrauterine insemination (IUI) and 66 with natural conception,were divided into two groups:normal intrauterine pregnancy group (group A,n =80) and early pregnancy loss group (group B,n =70).Medical history,ultrasonic findings,serum female hormone,P and β-HCG levels at early pregnancy stage were analyzed.Results The age of group A and group B was (30.0 ± 3.9) years and(30.7 ± 4.9) years,respectively (P > 0.05).The follicle-stimulating hormone/luteinizing hormone in group A and group B was 1.57 ±0.96 and 1.56 ± 1.08 ; the estradiol levels in two groups were (152 ±66) pmol/L and (147 ± 69) pmol/L,respectively (both P > 0.05).There were no differences in dominant follicles and endometrial thickness between groups A and B (P > 0.05).Ovulation promotion and luteal support treatments were adopted in both groups:50% (40/80) of cycles in group A received ovulation promotion,73% (58/80) of cycles received luteal support,while 44% (31/70) and 76% (53/70) received in groups B,respectively.The levels of serum progesterone in group A during 14-21 d,22-27 d and ≥ 28 d after ovulation were higher than those in group B at each time points (P < 0.0l).There were no significant differences in serum progesterone levels between women with luteal support treatment and those without luteal support treatment in both groups (P > 0.05).Conclusion Dynamic monitoring of serum progesterone level in early pregnancy may be used as an auxiliary index for prediction of pregnancy outcome,but it may not be an indicator for luteal maintenance therapy.

ABSTRACT

OBJECTIVE: Evaluate the association of Helicobacter pylori infection with anti-parietal cell antibodies (APCA) and anti-intrinsic factor antibodies (AIFA) and their impact on vitamin B12 serum level. PATIENTS AND METHODS: One hundred patients (M/F: 43/57; age 46.5 ± 17.5 years) who underwent upper gastrointestinal endoscopy at King Abdullah University Hospital, Irbid, Jordan were enrolled in the study. The patients were grouped as H. pylori-infected (n = 81) or H. pylori negative (n = 19) by histopathological examination. Fasting serum vitamin B12 levels, antiparietal cell antibodies and anti-intrinsic factor antibodies for patients and controls were determined. RESULTS: Anti-parietal cell antibodies and anti-intrinsic factor antibodies were positive in 9.9% and 18.5% of H. pylori-positive patients respectively. None of the H. pylori negative subjects had anti-parietal cell antibodies or anti-intrinsic factor antibodies. Serum vitamin B12 level was lower in the H. pylori-infected patients (275 ± 70.4 pg/mL) than in controls (322.9 ± 60.7 pg/mL; p 0.05). H. pylori was positive in 94% of the low-vitamin B12 group compared with 64.6% of the normal-vitamin B12 group (p 0.5). CONCLUSION: Patients with H. pylori infection are more likely to have anti-parietal cell antibodies and anti-intrinsic factor antibodies. There was an association between H. pylori infection and lower vitamin B12 levels. H. pylori infection might be a significant factor in the pathogenesis of autoimmune gastritis.

ABSTRACT

Objective To analyze the clinical characteristic of autoimmune gastritis (AIG).Methods From January 1990 to April 2010,the clinical data of 55 AIG patients were retrospectively analyzed,which included hemoglobin,lactate dehydrogenase (LDH),α-hydroxybutyrate dehydrogenase (α-HBDH),gastrin,intrinsic factor antibody (IFA),parietal cell antibody (PCA),gastrointestinal endoscopy examination and 24-hour esophageal pH recording.Another 31 megaloblastic anemia (MA) patients were selected as control.Statistical analysis was performed by independent-samples t test.Results Among 55 AIG patients,49 patients were associated with MA,and three out of four cases were identified of IFA.About 43.8% (21/48) patients were PCA positive.Before treatment,the levels of LDH and α-HBDH of AIG patients with MA were (1045.50±853.46)U/L and (853.71±824.23) U/L which significantly increased,than those of patients without MA [(166.67±41.03) U/L,(133.67±27.90) U/L],the differences were statistically significant (t=-4.665,-2.120,both P<0.05),however there was no significant difference when compared with the control group [(1047.52±1028.31) U/L,(1050.23±1264.37) U/L,both P>0.05)].A total of 46 patients underwent gastroendoscopy examination,63.0% (29/46) patients had gastric body atrophy while gastric antrum not involved; 34.8% (16/46) patients had neither gastric body nor antrum atrophy; seven patients gastric mucosal showed intestinal metaplasia and one patient showed intestinal metaplasia with atypical hyperplasia and 2.2% (1/46) presented both the antrum and the body atrophy.Conclusions The levels of LDH and α-HBDH increased in AIG patients might be related with MA caused marrow in-situ hemolysis.IFA is recommended as a routine test for AIG.There is still some limitations of AIG diagnosis according to histopathological features of gastric endoscopy specimen.The clinical features should be taken into consideration.

ABSTRACT

To determine the approximate incidence and clinical features of pernicious anemia in a Korean population, we retrospectively analyzed clinical data for patients with pernicious anemia who were diagnosed between 1995 and 2010 at five hospitals in Chungnam province. Ninety-seven patients were enrolled, who accounted for 24% of patients with vitamin B12 deficiency anemia. The approximate annual incidence of pernicious anemia was 0.3 per 100,000. The median age was 66 (range, 32-98) yr, and the male/female ratio was 1.25. Anemia-associated discomfort was the most common symptom (79.4%), followed by gastrointestinal and neurological symptoms (78.4% and 38.1%, respectively). Pancytopenia was found in 36 patients (37.1%), and autoimmune disorders were found in 15 patients (15.5%). Antibody to intrinsic factor was detected in 62 (77.5%) of 80 patients examined, and antibody to parietal cells was detected in 35 (43.2%) of 81 patients examined. Of the 34 patients who underwent tests for Helicobacter pylori, 7 (12.5%) were positive. The anemia-associated and gastrointestinal symptoms resolved completely in all patients after intramuscular injection of cobalamin, whereas neurological symptoms remained in some. In conclusion, pernicious anemia is less frequent in Koreans than in Western populations; however, the clinical features of this disorder in Koreans do not differ from those of Western cases.

ABSTRACT

BACKGROUND: Pernicious anemia is the most common cause of vitamin B12 deficiency in western populations, but to date, only case reports or small series dealing with this malady have been reported in Korea. This study describes the clinical characteristics of pernicious anemia in Koreans. METHODS: We retrospectively analyzed the clinical data for twenty-two Korean patients with pernicious anemia who were diagnosed during the period from 1995 to 2004 at Chungnam National University Hospital. RESULTS: Only two patients were diagnosed before 2000. The median age of the patients was 66 years and the male/female ratio was 1.8. Anemia-associated discomfort was the most common symptom (95.5%); this was followed by gastrointestinal and neurological symptoms (77.2% and 50.0%, respectively). Autoimmune disorders were found in five patients (22.7%). The median hemoglobin level was 7.0g/dL (range: 3.1~11.8g/dL) and pancytopenia was found in 12 patients (54.5%). The median serum vitamin B12 Level was 26pg/mL (range: 12~189pg/mL). Fifteen (78.9%) and eight (42.1%) of the 19 patients who underwent tests for antibodies were positive for anti-intrinsic factor and anti-parietal cell antibody, respectively. Nineteen of 21 patients who were treated with intramuscular cobalamin recovered from their cytopenia within 3 months. The gastrointestinal symptoms resolved completely for all the patients, while neurological symptoms remained for some of the patients. CONCLUSION: This study shows that the clinical features of pernicious anemia in Koreans are not different from those of the western cases. In addition, pernicious anemia is not a rare finding in Korea.

ABSTRACT

The purpose of this study was to evaluate the effect of intrinsic factor and extrinsic factor for growth of the mandibular condylar cartilage of 4 day-old rats In a serum-tree medium for 1, 4, 7,14 days. They were compared with normal growth in vivo and with growth of spheno-occipital synchondrosis in serum-free medium. The cellular kinetics of cartilages were evaluated by autoradiography of tritiated thymidine. 1. Condylar cartilage was enlarged with rounded head on day 14 of experiment while in vivo the rounded-headed shape changed into functionally flattened appearance. 2. On day 14 of experiment, a severe reduction of the proliferative zone and a considerable increase of the hypertrophic zone were observed while in normal control group endochondrol bone formation and bone marrow were observed. 3. The proliferative activity in the proliferative zone of condylar cartilage detected by 3H-thymidine incorporation was lower than that of normal control group and decreased more than that of spheno-occipital synchondrosis, but it continued during the 14 days of culture. 4. The continued maintenance of condylar cartilage and morphologic change were disturbed in this culture system, but cell division within the proliferative zone was continued and probably linked to intrinsic factor.

ABSTRACT

Megaloblastic anemia following gastrectomy is due to the total absence or inadequate secretion of intrinsic factor and is manifested by megalobastic changes in bone marrow, blood cells, and other proliferative cells. In Korea, detailed description and precise analyses of the cases of megaloblastic anemia following gastrectomy are relatively rare in contrast to the potential of its incidence from gastrectomy due to many causes or to the importance of its clinical significance. Here, we present the case of a 51-year old man who had undergone a total gastrectomy with esophagojejunostomy and incidental splenectomy due to early gastric cancer and developed megalobastic anemla 7 years after surgery. After gradual improvement of clinical and hematologic features with treatment of parenteral vltamin Bl2, he was followed-up with vitamin B12 maintenance therapy.
