ABSTRACT

OBJECTIVE@#To investigate the effect of body mass index (BMI) on the short-term effectiveness of high tibial osteotomy (HTO) in the treatment of varus knee arthritis.@*METHODS@#The clinical data of 84 patients (84 knees) with varus knee arthritis treated with HTO between May 2016 and August 2020 were retrospectively analyzed. According to BMI, the patients were divided into normal group (32 patients in group A, BMI<25 kg/m 2), overweight group (27 patients in group B, BMI>30 kg/m 2), and obese group (25 patients in group C, BMI>30 kg/m 2). The BMI of groups A, B, and C were (23.35±0.89), (26.65±1.03), and (32.05±1.47) kg/m 2, respectively. There was no significant difference ( P>0.05) in gender, age, surgical side, disease duration, and preoperative Hospital for Special Surgery (HSS) score, visual analogue scale (VAS) score, knee range of motion, and hip-knee-ankle angle (HKA) between groups. The operation time, intraoperative dominant blood loss, and the decrease of hemoglobin on the 3rd day after operation were recorded and compared between groups. The improvement of knee joint function and pain status were evaluated by knee joint HSS score, knee range of motion, and VAS score before and after operation, and measuring the HKA of patients on X-ray film. During the follow-up, the X-ray films of the knee joint were reexamined to observe the position of the internal fixator and the healing of osteotomy.@*RESULTS@#All patients completed the operation successfully and were followed up 8-40 months (mean, 19.3 months). There was no significant difference in follow-up time, operation time, intraoperative dominant blood loss, and the decrease of hemoglobin on the 3rd day after operation between groups ( P>0.05). No operative complications such as severe vascular or nerve injury occurred. After operation, deep venous thrombosis of lower extremities occurred in 1 case in groups A and B respectively, and fat liquefaction of surgical incision occurred in 2 cases in group C. There was no significant difference in the incidence of perioperative complications between groups (3.1% vs. 3.7% vs. 8.0%) ( P=0.689). During the follow-up, there was no bone nonunion, plate fracture or loosening. At last follow-up, HSS score, VAS score, knee range of motion, and HKA significantly improved in the 3 groups when compared with those before operation ( P<0.05), but there was no significant difference in the differences of the above indexes between groups before and after operation ( P>0.05).@*CONCLUSION@#BMI does not affect the short-term effectiveness of HTO in the treatment of varus knee arthritis. HTO can be selected for overweight and obese patients after standard medical treatment is ineffective.

ABSTRACT

Objective:To explore the effect of knee arthroplasty on serum cytokines and quality of life in Kashin-Beck disease (KBD) patients with severe knee arthritis.Methods:In a prospective design, KBD patients with severe knee arthritis who underwent total knee arthroplasty in Quzhou Hospital of Traditional Chinese Medicine from June 2017 to June 2018 were selected as the study subjects. The levels of serum cytokines [interleukin-1β (IL-1β), tumor necrosis factor (TNF)-α] and nitric oxide (NO) were compared before operation, 6 months after operation and 1 year after operation. The scores of Western Ontario and McMaster University Osteoarthritis Index (WOMAC) and quality of life scale (KBDQOL) were evaluated.Results:The age of 34 KBD patients with severe knee arthritis was (60.4 ± 5.6) years old, ranged from 50 to 72 years old. There were 15 males and 19 females; the course of disease was (20.3 ± 10.5) years, ranged from 5 to 40 years. The levels of serum IL-1β [(16.49 ± 3.86), (12.08 ± 1.27) pg/ml], TNF-α [(9.28 ± 1.45), (6.52 ± 1.26) pg/ml] and NO [(63.58 ± 15.82), (43.84 ± 10.37) μmol/L] at 6 months and 1 year after operation were significantly lower than those before operation [(21.65 ± 5.08), (13.06 ± 2.15) pg/ml, (102.16 ± 20.74) μmol/L, P < 0.05], and the levels of the above indicators at 1 year after operation were significantly lower than those at 6 months after operation ( P < 0.05). The WOMAC dimensions and comprehensive scores of KBD patients at 6 months and 1 year after operation were significantly lower than that before operation ( P < 0.05), and the WOMAC dimensions and comprehensive scores at 1 year after operation were significantly lower than those at 6 months after operation ( P < 0.05). The social support, activity limitation, somatic function, mental status, overall health and total scores of patients at 6 months and 1 year after operation were significantly higher than those before operation ( P < 0.05). The overall health and total scores of patients at 1 year after operation were significantly higher than those at 6 months after operation ( P < 0.05). Conclusion:After total knee arthroplasty, the inflammatory reaction, clinical symptoms and quality of life of KBD patients with severe knee arthritis are significantly improved.

ABSTRACT

【Objective】 To explore the quality of autologous platelet-rich plasma prepared by manual and automatic method and to study the clinical injection therapeutic effects in patient with knee arthritis. 【Methods】 28 patients with knee arthritis in Orthopedics Department of our hospital were enrolled. PRP was prepared manually in 12 patients and automatically in 16. The whole blood of 50~70 mL was drawn from 12 patients and prepared into PRP of 10 mL manually by centrifugation. The 20~30 mL PRP was collected by automatic apheresis. Plt, RBC, WBC and other related indexes of PRP were detected after collection. Among the 28 patients, 12 in the manual group received injection twice, while 16 in the automatic group 4 episodes, with an interval of 15~20 days. The treatment effect was evaluated after the last treatment. 【Results】 The PRP prepared by two methods can both reach the required quality standard of platelet counts..The patient′s pain symptoms relieved significantly after injection treatment and the effect was remarkable. There was no significant difference in clinical efficacy between the two groups. 【Conclusion】 The PRP prepared by manual and automatic methods can be used for injection treatment for patient with knee arthritis. The clinical therapeutic effects were good and worth promotion and application.

@#Introduction: Fluorosis has been associated with an increased risk of degenerative changes in the knee. Multiple studies have found an association between arthritis and elevated fluoride levels. We aim to delineate if elevated fluoride level has any direct correlation with the degree of radiological grading and clinical symptoms in knee arthritis. Materials and Methods: A cross-sectional study of 80 knee arthritis patients was conducted from February 2017 to April 2018. Serum fluoride levels were measured and patient’s pain scores, WOMAC scores and radiological grading were correlated with the elevated fluoride levels. Results: In our study, 30 out of 80 patients had increased serum fluoride level. Statistically significant differences were noted in VAS score, WOMAC score and Kellgren and Lawrence radiological grades between patients with normal serum fluoride level and those with elevated fluoride level. Conclusion: There is an increased risk of knee arthritis in patients with elevated blood fluoride levels and patients with increased fluoride levels are associated with more severe symptoms and radiographic disease.

ABSTRACT

Objective To investigate the efficacy of arthroscopic synovectomy on refractory knee arthritis complicated with popliteal cyst.Methods Patients diagnosed as rheumatoid arthritis (RA) or spondyloarthritis (SPA) with refractory knee arthritis who underwent knee arthroscopic synovectomy in our hospital from 2010 to 2017 were enrolled,including 20 patients (16 RA,4 SpA) with popliteal cyst.Clinical data,RA disease activity score (DAS28),SpA back pain score,etc,were collected to evaluate the efficacy of knee surgery.Results Erythrocyte sedimentation rate (ESR) [58(17,79)mm / 1h vs.19(9,30)mm/1h,P< 0.001],C reactive protein (CRP) [3.72(0.92,8.14) mg/L vs.0.85(0.10,3.08) mg/L,P<0.001],rheumatoid factor [64.6(20.2,193.3) vs.20.5(10.0,58.4),P<0.001],DAS28 score(4.67±1.25 vs.2.81±1.23,P<0.001),knee joint discomfort score [5(4,6) vs.2(1,3),P<0.001] and the volume of knee joint effusion by ultrasound (P<0.05) in 95 RA patients were significantly decreased compared to those before operation.ESR [27(12,54)mm/1h vs.20 (16,28) mm/1 h,P<0.001],CRP [3.27(1.06,6.95) mg/L vs.1.41(0.34,3.03)mg/L,P<0.001],knee discomfort score [2(0,5) vs.1(0,3),P<0.05],back pain visual analogue score (VAS) [5(4,5) vs.2(1,3),P<0.001],and the volume of knee joint effusion by ultrasound (P<0.001) in 58 SpA patients were significantly lower than those before the operation.The rate [16.84%(16/95) vs.6.32%(6/95),P=0.023] and grading (P=0.007) of popliteal cyst in RA were decreased after the operation.No statistically difference was observed in the rate [6.90% (4/58) vs.5.17%(3/58),P=0.697] of popliteal cyst in patients with SpA,yet with a trend of decrease in 4 patients.Conclusion This study provide evidence that knee arthroscopic synovectomy has a good effect for refractory knee arthritis,which can reduce disease activity,improve joint symptoms and decrease the grading of popliteal cyst.

Objective Understand the effects of low-resistance elastic band exercise on lower limb function and health-related quality of life in elderly with degenerative knee arthritis. Methods The 79 patients with degenerative knee arthritis were randomly divided into observation group (40 cases) and control group (39 cases). In the observation group, low-resistance elastic bands were exercised for 12 weeks and 2 times per week. In the control group, routine care measures. The following limb function assessment scales and health-related quality of life scales were evaluation tools, and statistical analysis was performed based on the measured data. Results There was no significant difference in the score of gait balance, isotonic contraction, knee muscle strength of lower extremity function before intervention between the two groups(P>0.05). The score of gait balance, isotonic contraction, knee muscle strength of lower extremity function after intervention was(4.63 ± 0.07),(4.57 ± 0.08),(4.53 ± 0.44)points in the observation group, and(3.60±0.23),(3.59±0.12),(3.86±0.09)points in the control group, and there was significant difference between the two groups(t=1.982, 1.968, 2.356, all P<0.05). There was no significant difference in the score of joint activities, body function, social interaction, emotional state, mental health of quality of life before intervention between the two groups(P>0.05). The score of joint activities, body function, social interaction, emotional state, mental health of quality of life after intervention was(4.48± 0.12),(4.57±0.22),(4.38±0.57),(4.29±0.29),(4.33±0.41)points in the observation group, and(3.46± 0.52),(3.48±0.25),(3.59±0.97),(3.45±0.83),(3.51±0.52)points in the control group, and there was significant difference between the two groups(t=2.405-3.564, all P<0.01). Conclusions For elderly, low-resistance elastic band exercise is easy to learn in operation, safety and practicality are also higher, as a reference to improve the lower limb function and health-related quality of life in elderly patients with degenerative knee.

ABSTRACT

PURPOSE: This study examined the effects of gender, age, underlying disease, duration after onset of symptoms, preoperative invasive procedures, bacterial culture of joint fluid, and stage of infection by the Gächter classification on the prognosis of patients with infectious knee arthritis who underwent arthroscopic surgery. MATERIALS AND METHODS: From June 2014 to December 2016, 51 patients who underwent arthroscopic surgery for infective knee arthritis were enrolled in this study. The average follow-up period was 14.2±2.1 months (range, 12–20 months). The subjects were 27 men (52.9%) and 24 women (47.1%), with an average age of 55.1±17.6 years (range, 13–84 years). A preoperative evaluation of the joint aspiration with a count of more than 50,000 leukocytes and a polymorphonuclear leukocyte count of 95% or more was performed. All patients underwent arthroscopic surgery and postoperative continuous joint irrigation. RESULTS: The initial mean value of the C-reactive protein decreased from 9.55±6.76 mg/dl (range, 1.51–31.06 mg/dl) to a final mean of 0.74±1.26 mg/dl (range, 0.08–6.77 mg/dl); the mean duration of C-reactive protein normalization was 27.6±18.9 days (range, 8–93 days). Among the 51 patients who received arthroscopic surgery and antibiotics, 44 patients (86.3%) with infectious knee arthritis completed treatment with improved clinical symptoms, such as fever, pain, and edema, and the C-reactive protein decreased to less than 0.5 mg/dl. Finally, 5 cases were treated with two or more arthroscopic operations, and 2 cases were converted to arthroplasty after prosthesis of antibiotic-loaded acrylic cement. CONCLUSION: The duration of surgery after the onset of symptoms and the stage according to the Gächter classification are important prognostic factors for predicting the successful treatment of infectious knee arthritis. On the other hand, the other factors were not statistically significant. Nevertheless, patients with bacteria cultured from the joint fluids appear to reflect the treatment period because the period of normalization of the C-reactive protein is shorter than that of the control group.

ABSTRACT

Kingella kingae es un colonizador habitual de la orofaringe en niños pequeños. Aunque en la mayoría de ocasiones la colonización es asintomática, puede dar lugar a infección invasiva. Este microorganismo ha sido crecientemente identificado como agente causal de infección osteoarticular en niños. Los datos limitados de artritis séptica por K. kingae y la dificultad para su identificación en los medios de cultivo habituales, pueden ocasionar retraso diagnóstico y en el inicio del tratamiento. Presentamos dos casos clínicos de lactantes con artritis séptica por K. kingae diagnosticada mediante PCR en líquido articular. El primero de ellos se trata de una mujer de 20 meses con sospecha inicial de sinovitis transitoria de cadera derecha, que acude a urgencias por cojera de 10 días de evolución. Ingresa tras 48 horas de fiebre y tumefacción de rodilla derecha, instaurándose tratamiento empírico con cloxacilina y cefotaxima tras diagnóstico de artritis séptica. Se obtiene PCR positiva para K. kingae en líquido articular. El segundo es un varón de 20 meses que acude a urgencias por dolor, tumefacción y limitación a la flexión de rodilla izquierda de 2-3 semanas, que en las últimas 24 horas se acompaña de fiebre de hasta 38.5ºC. Cuadro catarral previo. Ingresado una semana antes por el mismo motivo, con analítica sin alteraciones y análisis del líquido articular normal. De nuevo se decide ingreso repitiendo las pruebas complementarias e iniciando tratamiento empírico con cloxacilina y cefotaxima. Identificación de K. kingae mediante PCR en segunda muestra de líquido articular. Ambos casos presentaron buena evolución sin secuelas posteriores.


Kingella kingae is a common colonizer of the oropharynx in young children. Although most colonization is asymptomatic, it can lead to invasive infection. This microorganism is increasingly recognized as a cause of osteoarticular infections in children. The limited data of septic arthritis due to K. kingae and the difficulty to identify this organism in routine cultures, can delay the diagnosis and treatment. Two cases of infants with septic arthritis by K. kingae diagnosed by PCR are described. The first one is about a 20-month-old woman with initial suspicion of transient synovitis of the right hip, who came to the emergency department with limping in the last 10 days. She is admitted to hospital because of fever and inflammation on the right knee for 48 hours. We initiated empirical treatment with cefotaxime and cloxacillin after diagnosis of septic arthritis. Positive PCR for K. kingae was obtained in joint fluid. The second case is about a 20-month-old male who came to the emergency room due to pain, swelling and limitation to left knee flexion, 2-3 weeks of evolution, accompanied by fever for the last 24 hours (38.5ºC). He had the antecedent of catarrhal picture previously. He had been admitted to hospital the previous week because of the same symptoms, with blood and joint fluid exams without alterations. He is admitted again with empirical treatment with cloxacillin and cefotaxime after repeating the laboratory exams. K. kingae was identified by PCR in the second sample of joint fluid. Both cases presented had good evolution without subsequent sequels.

Objective:To observe the therapeutic effect ofcapsaicin at different concentrations on chronic knee arthritis pain model in mice.Methods:Choosing 50 healthy adult male Kunming mice builded chronic knee arthritis pain model by injecting 0.01 mL CFA (Complete Freund's Adjuvant,CFA) into left joint cavity.The model would be succeed in building after 3 weeks.The successful model mice were divided into five groups randomly (n=10):The first experimental group (saline group),the second experimental group (capsaicin excipient group),the third experimental group (0.5 % of capsaicin),the fourth experimental group (3 % of capsaicin) and the fifth experimental group (8 % of capsaicin).All of the mice would be observed the time of withdrawal latencies from the thermal heated surface after administration of one,four and seven hours,and thermal withdrawal time within 60 days after the injection.Results:①The physiological saline group compared with excipient group,the thermal withdrawal time had no statistically significant difference (P>0.05)after administration of one,four and seven hours,and thermal withdrawal time within 60 days.②The acute pain duration of the third group would disappear after capsaicin injection 7 hours,four hours for the fourth group,and one hour for the fifth group.③The duration of analgesia of the third group,lasted for 18.9± 1.1 days;The analgesia time of the fourth group lasted for 33.7± 1.0 days;The analgesia time of the fifth group lasted for 58.2± 1.2 days.Conclusions:Capsaicin has analgesic effects on chronic knee pain model in mice induced by CFA,and the days of analgesia increases with the concentration of capsaicin.

ABSTRACT

OBJECTIVE:To observe the efficacy and safety of Shenqi jiangtang granule in the adjuvant treatment of type 2 dia-betes knee arthritis. METHODS:62 patients with type 2 diabetes knee arthritis were randomly divided into control group(31 cas-es) and observation group (31 cases). Control group received hypoglycemic and basic treatment for arthritis,including diet con-trol,exercise therapy and health education,as well as 0.25 g Metformin hydrochloride tablet with a meal,3 times a day + 50 mg Acarbose tablet with a meal,3 times a day,chewing;patients with arthritis pain 100 mg Aspirin enteric-coated tablet after a meal, once a day (chewing or breaking apart was prohibited). Observation group additionally received 3 g Shenqi jiangtang granule half an hour before a meal with 50 ml warm water,3 times a day. The treatment course for both groups was 6 months. Clinical effica-cy,and fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PG),glycated hemoglobin(HbA1c),interleukin-1β(IL-1β),IL-6 before and after treatment,and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was significantly higher than control group,the difference was statistically significant(P0.05). After treatment,FPG,2 h PG,HbA1c,IL-1β and IL-6 in 2 groups were significantly lower than before,and observation group was lower than control group,the differences were statistically significant(P0.05). CONCLUSIONS:Based on conventional treatment,Shenqi jiangtang granule shows obvious efficacy in the adjuvant treatment of type 2 diabetes knee arthritis.,it can reduce blood glucose and inflammation cy-tokine levels,mild symptoms of adverse reactions.

[Objective]To analyse the clinical effect of total knee resurface replacement treatment for advanced knee gouty arthritis. [Method]From March 2001 to July 2007,nine patients with advanced gouty knee arthritis(total 10 knees) were treated with total knee resurface replacement with posterior stability prosthesis in the author's research group.Among them,eight patients had unilateral knees and one had bilateral knees.Following-up data were successfully obtained.[Result]All the patients were followed up for at least one year.HSS Score system revealed that six knees were rated as excellent,two as good and two as fair.Either preoperative or follow-up HSS scores was analysed by T-test and their significant differences were observed(P

ABSTRACT

OBJECTIVE: We wanted to assess the role of the popliteal lymph nodes for differentiating rheumatoid arthritis (RA) from osteoarthritis (OA), and we also wanted to investigate the relationship between the popliteal lymph nodes and the inflamed synovial volume (ISV) by using contrast enhanced (CE), fat suppressed, three dimensional-fast spoiled gradient echo (3D-FSPGR) MR imaging. MATERIALS AND METHODS: Contrast enhanced 3D-FSGPR MR imaging of 94 knees (21 with RA and 73 with OA) was analyzed. A lymph node was defined as being 'observed' if it could be seen in at least two planes of the three orthogonal reformatted planes. The number of observed lymph nodes, the mean of the smallest dimension of each lymph node and the existence of central fatty change were recorded. The OA group was graded according to the ISV calculated by a segmentation method: grade I was 40 cm3. Statistical analysis of the number and the mean size of the popliteal lymph nodes among the four groups (the RA group and the grade I-III OA groups) was performed. RESULTS: The prevalence of the observed popliteal lymph nodes was significantly different between all the OA groups or between the grade III OA group and the RA group (p < 0.0001, 0.0001, respectively). The popliteal lymph node was observed in 32 out of 73 OA cases, whereas it was visible in all of the 21 RA cases. The number (mean+/-standard deviation) of lymph nodes in the grade I OA group, the grade II OA group, the grade III OA group and the RA group was 1.2+/-0.4, 1.2+/-0.4, 1.3+/-0.5, and 2.7+/-1.1, respectively. The mean size (mean+/-standard deviation) of the lymph nodes was 3.8+/-1.0 mm, 3.6+/-1.1 mm, 4.1+/-0.8 mm, and 5.4+/-1.3 mm, respectively. The incidence of central fatty changes was significantly lower in the RA group than in all the OA groups and the grade III OA group. When differentiating RA from OA, and when the differentiation was confined to the RA group and grade III OA group, respectively, the criteria of the number of lymph nodes, their size, their central fatty change and a combination of all these three criteria showed statistical significance (Az values for the former were 0.869, 0.847, 0.776, and 0.942; Az values for the latter were 0.855, 0.799, 0.712, and 0.916). The number and mean size of the lymph nodes correlated with the ISVs (r = 0.49, p < 0.001; 0.50, 0.001, respectively). CONCLUSION: The number, size and central fatty changes in the popliteal lymph nodes observed on the MR images might serve as simple and useful markers in differentiating RA disease from OA disease. These markers would be particular helpful in cases of severe synovial enhancement where the ISVs of both RA and OA overlap. The number and mean size of the lymph nodes also correlated well with the ISV.

ABSTRACT

PURPOSE: Objective measurements of knee of flexor and extensor strength using isokinetic equipment and function after total knee arthroplasty (TKA) were evaluated until postoperative 12 months. MATERIALS AND METHODS: The control group constituted of 15 persons who were of same age and weight without knee problems. Most of the patients (70%) were operated on both knees simultaneously. The patient group consisted of constituted with 20 patients (34 cases). Isokinetic testing (Cybex) of knee flexor and extensor strength of knee and functional evaluation by HSS score was performed preoperatively and at 3, 6, 12 months postoperatively. RESULTS: The peak torque of the knee extensor and flexor muscle in the patient group was decreased by 17-41% compared to the control group. Especially, the peak torque of extensor was more decreased than flexor muscles. At postoperative 12 months, the peak-torque values of hamstring and quadriceps were able to attain the same strength levels of the preoperative knee. According to the grade of the Hospital for Special Surgery knee rating scales by Insall, 28 cases (82 %) in the patient group could obtain good result at postoperative one year. CONCLUSIONS: Exercises to increase the strength of extensor should be emphasized in the rehabilitation programs after TKA

ABSTRACT

To determine the therapeutic effect of closed-needle irrigation of knee for pain from osteoarthritis(OA), we conducted a randomized, single blind, 8 week prospective trial study. A comparison was done between the closed-needle irrigation of knee and standard conservative treatment in 30 patients with non-end stage OA of the knee. Statistically significant differences (P<0.05) favoring closed-needle irrigation of knee over conservative treatment were noted by the Visual Analog Scale for pain in these patients. Overall assessments on effectiveness of therapy by patients(Patient's global assessment) and physician(Knee rating scale) were also statistically superior among closed-needle irrigation group. We conclude that the closed-needle irrigation of knee for the treatment of pain from osteoarthritis is an effective treatment and is superior than the traditional conservative treatment in therapeutic effect.
