ABSTRACT

Postoperative maxillary cyst is a late complication that occurs after maxillary sinus radical surgery. The only curative treatment is surgery, and endoscopic fenestration is currently performed in most cases. However, in some cases, it is difficult to form a permanent fenestration. Here, we report a case of postoperative maxillary cyst which was reduced by administration of kumibinroto and could avoid surgery. The case was a 58-year-old man. He received bilateral maxillary sinus radical surgery 40 years ago. He had swelling of his left cheek for half a year and took an antibacterial drug at a nearby doctor, but he was referred to our department because there was no improvement. We diagnosed him with a postoperative maxillary cyst based on CT findings, but it was considered difficult to form a permanent fenestration due to the small contact area between the nasal cavity and the cyst. He refused to puncture from the oral cavity, so we planned endoscopic surgery and administered kumibinroto to him. After 6 weeks, the swelling of the left cheek disappeared. After 12 weeks, CT showed shrinkage of the cyst, and surgery could be avoided. No further cyst shrinkage was observed, and we reduced the dose of kumibinroto after 20 weeks. He is now being followed up.

ABSTRACT

Cough-variant asthma is the most common cause of chronic cough. It may progress to classic asthma and therefore requires adequate treatment intervention. We experienced two cases of cough-variant asthma that was refractory to standard drug therapy but improved after additional administration of kumibinroto. In Case 1, the patient was a 46-year-old woman receiving hormone therapy after breast cancer surgery. She developed cough-variant asthma 5 years ago, and though she has received drug therapy, the symptoms aggravated. Cough continued to appear at night without improvement of asthma symptoms. However, the symptoms improved with subsequent administration of kumibinroto. In Case 2, the patient was a 47-year-old woman who developed cough-variant asthma after childbirth, which repeatedly worsened and improved. The cough-variant asthma aggravated after change of workplace and did not improve with drug therapy. However, asthma symptoms improved with oral administration of kumibinroto and hangekobokuto, and subsequent administration of kumibinroto alone. In both patients, the asthma was seasonal. These cases suggest that kumibinroto administration may contribute to the improvement of cough-variant asthma.

ABSTRACT

Kumibinroto is a Kampo formulation which was first produced by Sohaku Asada (1815-1894) and which has since been used for patients with stasis of body fluids presenting beriberi like symptoms. Previously, we reported that tympanic sounds on right side of the abdomen might be an indication for the use of kumibinroto. In this study, having both symptoms with stasis of body fluids, and tympanic sounds on right side of the abdomen were used as a target when prescribing kumibinroto. To the best our knowledge, the present study was the first to demonstrate that the effects of kumibinroto give significant improvement in dermatological disorders such as chronic prurigo, cutaneous pruritis and chronic urticaria, and susceptibility to chills. These findings suggest that tympanic sounds on right side of the abdomen may an extremely useful indicator for kumibinroto, and position it for practical use for various disorders, for which it has not been formerly used.

ABSTRACT

Kumi-binro-to is a Kampo formulation produced by Sohaku Asada. It has been used for patients with stasis of body fluids presenting beriberi-like symptoms. The cases in this report where Kumi-binro-to was effective had symptoms similar to the cases where it was ineffective, but they had different distributions of tympanic sound in the abdomen.<br>We investigated the efficacy of four weeks of treatment with Kumi-binro-to in 30 cases having both symptoms of stasis of body fluids and tympanic sound in the abdomen. Kumi-binro-to was effective in 20 of 25 cases that had the tympanic sound in the right iliac, flank and subphrenic regions. However, Kumi-binro-to was not effective in 5 cases where the tympanic sound was restricted to the epigastrium, left iliac, flank and subphrenic regions. Tympanic sound on the right side of the abdomen might be one of the indications for Kumi-binro-to.

ABSTRACT

A total of 20 chronic headache cases in which other Kampo medicines were ineffective or hardly effective were treated with Kumi-binro-to extract, and the effect of this medicine was investigated.<br>At the start of administration, we investigated the following items: any deficiency or excess, presence or absence of stagnation of vital energy (facial erythema, ophthalmologic injection, bitter taste in the mouth, etc.) and disease caused by contaminated water (tendency of edema, sound of fluctuating liquid in the region of the stomach, decreased urine volume) as well as the presence or absence of grasping pain (pressure pain) in the gastrocnemius muscle which is a characteristic symptom as a target of this drug. The efficacy of Kumi-binro-to was assessed by a pain score after weeks from the start of administration.<br>Kumi-binro-to was markedly effective in 5 cases, effective in 11 cases and ineffective in 4 cases, indicating that the ratio of markedly effective and effective cases accounted for 80% of the patients treated. There was no adverse reaction. When the characteristics of 16 cases assessed as markedly effective or effective were investigated, disease caused by contaminated water and stagnation of vital energy was observed in 13 and 14 of the 16 cases respectively. The grasping pain in the gastrocnemius muscle that is a characteristic target of this drug was noted in only one case.<br>The above result suggests that Kumi-binro-to promotes diuresis to eliminate wetness-evil and regulates vital energy, and is effective against chronic headache, especially when the disease caused by contaminated water is accompanied with stagnation of vital energy.
