ABSTRACT

Introdução: Fotobiomodulação corresponde à exposição de tecidos biológicos a baixos níveis de luz vermelha e infravermelha, esta terapia favorece a reabilitação de diferentes tecidos e que pode ser utilizada para a melhora da prática clínica nas diferentes atuações da fisioterapia, como por exemplo, no tratamento dos acometimentos musculoesqueléticos e inflamatórios. Objetivo: Identificar os benefícios da fotobiomodulação empregados na reabilitação de pacientes nas diferentes patologias traumato-ortopédicas. Metodologia:Trata-se de uma revisão integrativa com busca online nas plataformas de dados: Medline e PubMed. Os descritores foram: fotobiomodulação, taumato-ortopedia e reabilitação. As línguas selecionadas foram: Português, Inglês e Espanhol, entre os anos de 2018 a 2022.Resultados:A descrição dos achados nos ensaios clínicos analisados mostra que a terapia de fotobiomodulação apresenta diferentes usos na prática clínica e que seu uso produz efeito analgésico, anti-inflamatório e regenerativo nos distúrbios musculoesqueléticos. Conclusões:De acordo com a revisão dos artigos, pôde-se perceber que a terapia por fotobiomodulação confirma seus benefícios e eficácia, portando, se fazendo positiva na atuação traumato-ortopédica, gerando resultados significativos quando comparada a outros recursos (AU).


Introduction: Photobiomodulationcorresponds to the exposure of biological tissues to low levels of red and infrared light, this therapy favors the rehabilitation of different tissues and can be used to improve clinical practice in different actions of physiotherapy, such as, for example, in the treatment of musculoskeletal and inflammatory disorders. Objective: To identify the benefits of photobiomodulation used in the rehabilitation of patients with different traumato-orthopedic pathologies. Methodology:This is an integrative review with online search on data platforms: Medline and PubMed. The descriptors were: photobiomodulation, thaumato-orthopedics and rehabilitation. The selected languages were: Portuguese, English and Spanish, from 2018 to 2022. Results: The description of the discovers in the analyzed clinical trials shows that photobiomodulation therapy has different uses in clinical practice and that its use produces analgesic, anti-inflammatory and regenerative effects in musculoskeletal disorders. Conclusions:According to the review of the articles, it could be seen that photobiomodulation therapy confirms its benefits and effectiveness, therefore, becoming positive in the trauma-orthopedic performance, generating significant results when compared to other resources (AU).

Objetivo: Identificar los beneficios de la fotobiomodulación utilizada en la rehabilitación de pacientes con diferentes patologías traumato-ortopédicas. Metodología: Esta es una revisión integradora con búsqueda en línea en plataformas de datos: Medline y PubMed. Los descriptores fueron: fotobiomodulación, taumato-ortopedia y rehabilitación. Los idiomas seleccionados fueron: portugués, inglés y español, entre los años 2018 a 2022. Resultados: La descripción de los hallazgos en los ensayos clínicos analizados muestra que la terapia de fotobiomodulación tiene diferentes usos en la práctica clínica y que su uso produce efectos analgésicos, antiinflamatorios y regenerador en trastornos musculoesqueléticos. Conclusiones: De acuerdo con la revisión de los artículos, se pudo apreciar que la terapia de fotobiomodulación confirma sus beneficios y efectividad, por lo tanto, tornándose positiva en el desempeño trauma-ortopédico, generando resultados significativos cuando se compara con otros recursos (AU).

ABSTRACT

Antecedentes. Ante la pandemia de COVID-19 el sistema de salud reasignó recursos económicos para la atención. Objetivo. Determinar el costo de la atención y el porcentaje del gasto en salud por COVID-19 en una unidad de medicina familiar de primer nivel de atención. Metodología. Estudio de costo y porcentaje de gasto en COVID-19 en una unidad de primer nivel de atención. Se identificaron los servicios generales y finales, para construir el costo fijo se utilizó la técnica de tiempos y movimientos, se identificaron el total de partidas presupuestales ejercidas en la unidad médica para cada uno de los servicios, para desagregar el gasto de los servicios generales a los finales se construyeron ponderadores. El costo variable se realizó con la técnica consenso de expertos y microcosteo. El costo promedio se relacionó con la productividad por servicio y con el total de pacientes atendidos por COVID-19, el resultado se relacionó con el presupuesto ejercido de la unidad. Resultados. El costo anual de la atención de COVID-19 en módulo respiratorio fue 158.597,25 dólares americanos, en medicina familiar fue 192.549,36 dólares americanos, el costo total ejercido en el año 2021 para atención de SARS COV 2 en una unidad de primera atención fue 351.146,61 dólares americanos. Esta cantidad representa el 9,6 % del gasto en salud. Conclusión. El costo en atención de COVID-19 y el porcentaje del gasto en salud en primer nivel de atención es elevado (AU)


Background. In the COVID-19 pandemic, the health system reallocated financial resources for care. Objetive. To determine the cost of care and the percentage of health spending due to COVID-19 in a first level care family medicine unit. Metodology. Study of the cost and percentage of spending on COVID-19 in a first-level care unit. The general and final services were identified, to construct the fixed cost, the technique of times and movements was used, the total budget items exercised in the medical unit for each of the services were identified, to disaggregate the expense of general services to the endings were constructed weights. Variable costing was performed using the expert consensus technique and microcosting. The average cost was related to productivity per service and to the total number of patients treated for COVID-19, the result was related to the budget used by the unit. Results. The annual cost of COVID-19 care in the respiratory module was 158.597,25 US dollars, in family medicine it was 192.549,36 US dollars, the total cost incurred in 2021 for SARS COV 2 care in a unit of first attention was 351.146,61 US dollars. This amount represents 9,6% of health spending. Conclusion. The cost of COVID-19 care and the percentage of health spending at the first level of care is high (AU)

ABSTRACT

OBJETIVO: Analisar a dosimetria do laser de baixa intensidade no processo de cicatrização de úlcera venosa. MÉTODO: Trata-se de um protocolo de revisão sistemática registrado no International Prospective Register of Systematic Reviews (PROSPERO) sob código de registro CRD420211256286. Serão realizadas buscas por evidências científicas em 11 bases de dados, utilizando os idiomas português, inglês e espanhol. A exportação das publicações seguirá as etapas de identificação e seleção dos estudos, e extração dos dados. As divergências serão resolvidas por consenso dos dois revisores, e caso persistam, um terceiro revisor será consultado para decidir sobre a inclusão do material. A ferramenta Risk of Bias 2 (RoB 2) será utilizada para avaliar o risco de viés dos estudos dos ensaios clínicos randomizados, ao passo que a ferramenta Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I) será utilizada para avaliar o risco de viés dos ensaios clínicos não randomizados. A análise crítica dos materiais selecionados quanto à dosimetria do laser de baixa intensidade para cicatrização de úlcera venosa resultará em uma síntese narrativa, sem metanálise.


OBJECTIVE: To analyze the dosimetry of low-level laser therapy in the healing process of venous ulcers. METHOD: This is a protocol for systematic review registered in the International Prospective Register of Systematic Reviews under registry code CRD420211256286. Articles will be searched in 11 databases using Portuguese, English, and Spanish languages. The export of publications will follow the steps of study identification, selection, and data extraction. Disagreements will be resolved by consensus among reviewers; if they persist, a third reviewer will be consulted to decide whether to include the material. The Risk of Bias 2 (RoB 2) tool will be used to assess the validity of randomized clinical trials, while the Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I) tool will be used to assess the risk of bias in non-randomized clinical trials. The critical analysis of selected materials on dosimetry of low-level laser therapy for venous ulcer healing will result in a narrative synthesis without meta-analysis.

ABSTRACT

ABSTRACT Objective: To measure the costs of preventive and therapeutic protocols of Photobiomodulation (PBM) for oral mucositis (OM) and their budgetary impact on Brazil's Ministry of Health (BMH). Material and Methods: A partial economic analysis was performed to estimate the costs using a bottom-up approach from a social perspective. Monetary values were assigned in Brazilian reais (BRL). The costs of the preventive protocol were calculated for five, 30, and 33 consecutive PBM sessions, depending on the antineoplastic treatment instituted. The costs of the therapeutic protocol were calculated for 5 or 10 sessions. The annual financial and budgetary impact was calculated considering the groups of oncologic patients with a higher risk of development of OM, such as those with head and neck and hematological cancer and pediatric patients. Results: The cost of a PBM session was estimated at BRL 23.75. The financial impact of providing one preventive protocol per year for all oncologic patients would be BRL 14,282,680.00, 0.030% of the estimated budget for hospital and outpatient care of the BMH in 2022. The financial and budgetary impacts of providing one treatment for OM for all patients in one year would be BRL 2,225,630.31 (0.005%, most optimistic scenario) and BRL 4,451,355.63 (0.009%, most pessimistic scenario). Conclusion: The budgetary impact of implementing PBM protocols in the Brazilian Healthcare System is small, even in a pessimistic scenario.

ABSTRACT

Introduction: during the global COVID-19 pandemic, non-invasive ventilation has become a widely utilized method for treating patients experiencing acute respiratory failure. Noninvasive pressure ventilation is frequently employed as a standard approach for managing acute respiratory failure resulting from COVID-19 pneumonia, as opposed to invasive ventilation methods. However, there is a lack of research on its effectiveness. Therefore, this study aimed to determine the risk of mortality among COVID-19 patients receiving non-invasive ventilation. Methods: a multi-centric retrospective cross sectional study was conducted on the records of 402 patients at the Eka Kotebe COVID-19 Center, St. Peter COVID-19 Care Center, and Millennium COVID-19 Treatment Center. The systematic random selection technique was employed in order to select the study unit, and data was extracted from patient charts using a pretested method and validated before being entered into Epi-data Manager 4.6 versions. Descriptive, bivariate, and multivariable analyses were performed using binary logistic regression in SPSS 25. In the multivariate logistic regression, a predictor variable was considered to have a significant connection if its p-value was less than 0.05 at a 95% confidence level. Results: four hundred and two patient records were reviewed during the study period and showed the mean patient´s age was 62.6 years, with male predominance. It revealed that 11.7% [CI: 8.7-15.2] of COVID-19 patients who received non-invasive positive pressure ventilation died, as being critical for COVID-19 patients was a main cause of noninvasive initiation. Patients over the age of 60 were more likely to die among those who received noninvasive ventilation for COVID-19 [AOR = 5.4 95% CI 1.32, 23.1]. Conversely, patients without diabetes were less likely to die [AOR = 0.23 95% CI 0.11, 0.48]. Moreover, patients with a tidal volume greater than 500 ml were more likely to pass away [AOR =2.2 95% CI 1.11,4.43], as were those who were on non-invasive ventilation (NIV) for more than 8 days [AOR = 0.24 95% CI.08, 0.81]. Conclusion: the significance of patients who were given non-invasive ventilators ended up dying. Age, diabetes, and high tidal volumes are linked to a higher risk of death. Non-invasive ventilation for over eight days showed a protective effect. Removing factors that caused NIV and ventilated COVID-19 patients' deaths may reduce mortality.

ABSTRACT

ObjectiveTo investigate the current status of first aid knowledge among middle-aged and elderly residents aged 50 and above in a community in Shanghai, in order to provide reference for improving the self-rescue and mutual aid capabilities of middle-aged and elderly residents. MethodsA multi-stage stratified random sampling method was used to conduct a survey on 335 residents aged 50 and above in a community in Shanghai using a self-made survey questionnaire. The current situation and related factors of emergency knowledge level of residents aged 50 and above in the community were comprehensively analyzed. ResultsFirst aid knowledge level of 335 residents aged 50 and above was low, and the rate of high cognitive level was only 24.18%. Univariate analysis showed that male residents had a higher awareness rate than female residents (P=0.044), while residents aged 70 and above and 60‒ had lower awareness rates than residents aged 50‒ (P<0.05). Residents with chronic diseases had a higher awareness rate than those who did not (P=0.031). Residents with family members suffering from chronic diseases had a higher awareness rate than those without (P<0.001). Experience of first aid training affected residents’ awareness of first aid (P=0.003). ConclusionThe level of first aid knowledge among middle-aged and elderly residents aged 50 and above in the community is low. Age, the presence of chronic diseases in family members, and emergency training are independent and relevant factors that affect the awareness of first aid knowledge among middle-aged and elderly residents. The government and relevant institutions should explore the establishment of a standardized emergency response training system, implement classification and grading for middle-aged and elderly groups with different characteristics, and provide targeted emergency training to strengthen their self-rescue and mutual aid capabilities and improve the success rate of pre-hospital emergency care.

ABSTRACT

Objective To investigate the expression of neutrophil extracellular traps (NETs) and phagocytic function in the peripheral blood of patients with hepatic alveolar echinococcosis (HAE), and to examine their correlations with clinical inflamma tory indicators and liver functions. Methods A total of 50 patients with HAE admitted to Department of Hepatobiliary and Pancreatic Surgery, The Affiliated Hospital of Qinghai University from August 2022 to June 2023 were enrolled, while 50 age- and gender-matched healthy individuals from the Centre for Healthy Examinations of the hospital during the same period served as controls. The levels of NETs markers neutrophil myeloperoxidase (MPO) and neutrophil elastase (NE) were measured using enzyme-linked immunosorbent assay (ELISA). Peripheral blood neutrophils were isolated using density gradient centrifugation, stimulated in vitro using phorbol 12-myristate 13 acetate (PMA), and the levels of MPO and citrullination histone H3 (CitH3) released by neutrophils were quantified using flow cytometry. The phagocytic functions of neutrophils were examined using flow cytometry. In addition, the correlations of MPO and NE levels with clinical inflammatory indicators and liver biochemical indicators were examined using Spearman correlation analysis among HAE patients. Results The peripheral blood plasma MPO[(417.15 ± 76.08) ng/mL vs. (255.70 ± 80.84) ng/mL; t = 10.28, P < 0.05], NE[(23.16 ± 6.75) ng/mL vs. (11.92 ± 3.17) ng/mL; t = 10.65, P < 0.05]and CitH3 levels[(33.93 ± 18.93) ng/mL vs. (19.52 ± 13.89) ng/mL; t = 4.34, P < 0.05]were all significantly higher among HAE patients than among healthy controls, and a lower phagocytosis rate of neutrophils was detected among HAE patients than among healthy controls[(70.85 ± 7.32)% vs. (94.04 ± 3.90)%; t = 20.18, P < 0.05], and the ability to produce NETs by neutrophils was higher among HAE patients than among healthy controls following in vitro PMA stimulation. Pearson correlation analysis showed that the phagocytosis rate of neutrophils correlated negatively with platelet-to-lymphocyte ratio (PLR), neutrophil-to-lymphocyte ratio (NLR), interleukin-6 (IL-6) level and C-reactive protein (CRP) level (rs = −0.515 to −0.392, all P values < 0.05), and the MPO and NE levels positively correlated with inflammatory markers NLR, PLR, CRP and IL-6 (rs = 0.333 to 0.445, all P values < 0.05) and clinical liver biochemical indicators aspartic transaminase, alanine aminotransferase, direct bilirubin and total bilirubin among HAE patients (rs = 0.290 to 0.628, all P values < 0.001). Conclusions Excessive formation of NETs is found among HAE patients, which affects the phagocytic ability of neutrophils and results in elevated levels of inflammatory indicators. NETs markers may be promising novel biomarkers for early diagnosis, monitoring, and severity assessment of liver disease.

ABSTRACT

BackgroundBariatric surgery has emerged as an important tool in the management of obesity. Some patients undergoing bariatric surgery are prone to develop emotional abnormalities and have abnormally elevated concentrations of inflammatory factors level in peripheral blood, whereas current domestic research focusing on the impact of preoperative emotional states and peripheral blood inflammatory factors level on weight loss effect remains limited. ObjectiveTo explore the correlation of preoperative emotional abnormalities with the effectiveness of bariatric surgery in obese patients, and to provide theoretical basis for improving the clinical efficacy of bariatric surgery. MethodsEighty-one obese patients scheduled for bariatric surgery at gastrointestinal surgery Department of West China Hospital, Sichuan University from December 30, 2022 to June 30, 2023 were enrolled and assessed using Hamilton Depression Scale-17 item (HAMD-17) and Hamilton Anxiety Scale (HAMA). Patients who scored 7 or above on HAMD-17 or HAMA or had a history of previous depression or anxiety diagnoses were classified into emotional abnormality group (n=34), and samples who scored less than 7 on HAMD-17 and HAMA and were free of history of previous depression and anxiety diagnoses were set as non-emotional abnormality group (n=47). The data were collected by the self made questionnaire. Patients were subjected to complete the assessment of Beck Scale for Suicide Ideation-Chinese Version (BSI-CV), Eating Disorder Inventory (EDI) and Pittsburgh Sleep Quality Index (PSQI). Laboratory tests including peripheral blood C-reactive protein (CRP) and interleukin-6 (IL-6). Body weight and height assessed in the early morning after an overnight fasting period were recorded in all participants at 1- and 6-month after surgery through outpatient clinic visits or telephone follow-up. Pearson correlation coefficient was used to examine relationship among body mass index (BMI), preoperative emotional states and peripheral blood inflammation mediators. ResultsAmong 81 obese patients, 62 completed the study, including 27 cases in emotional abnormality group and 35 cases in non-emotional abnormality group. Emotional abnormality group scored higher on BSI-CV (current), BSI-CV (worst), EDI and PSQI, and detected higher levels of CRP and IL-6 compared with non-emotional abnormality group (Z=2.677, 2.975, t=3.573, 4.035, 1.990, 2.799, P<0.05 or 0.01). For BMI, there was no significant group effect and time×group interaction effect (P>0.05), but a significant time effect (F=227.740, P<0.01). Within emotional abnormality group, BMI at the baseline, 1- and 6-month after surgery showed a positive correlation with IL-6 level (r=0.419, 0.510, 0.559, P<0.05 or 0.01), BMI at 6-month after surgery was positively correlated with HAMD-17 total score (r=0.390, P<0.05), and ΔBMI% at 6-month after surgery was negatively correlated with HAMD-17 total score (r=-0.421, P<0.05). Within non-emotional abnormality group, baseline BMI was positively correlated with IL-6 level (r=0.338, P<0.01). ConclusionThe short-term effect of bariatric surgery may be comparable in obese patients with or without emotional abnormalities, while it cannot be ruled out whether the outcome of bariatric surgery is related to the severity of preoperative depression.

@#Objective To compare the differences of safety and immunogenicity of DTaP-IPV-Hib-HepB hexavaccine,DTaPIPV-Hib pentavaccine plus HepB single vaccine or DTaP-IPV-HepB pentavaccine plus Hib single vaccine,so as to provide a reference for the marketing and use of hexavaccine in China.Methods Randomized controlled trials(RCTs)of DTaP-IPVHib-HepB hexavaccine,DTaP triple vaccine,Hib,IPV and HepB vaccines published at home and abroad were searched.The safety and immunogenicity of the hexavaccine were evaluated by Meta-analysis using Revman 5.4.1 software.Results A total of 7 articles,8 RCTs and 3 429 subjects were included. Meta-analysis of safety showed that there was no significant difference in the incidence of injection site and systemic adverse reactions after vaccination with hexavaccine and pentavaccine plus single vaccine(P > 0. 05)except for induration at the inoculation site and crying. Meta-analysis of immunogenicity showed no significant difference in antibody indexes after vaccination with hexavaccine and pentavaccine plus single vaccine(P > 0. 05).Conclusion The safety and immunogenicity of DTaP-IPV-Hib-HepB hexavaccine in basic immunity was comparable to that of the control vaccine,and might be applied to infants and young children to prevent related diseases. However,due to the limitations of the quantity and quality of included studies,the above conclusions still depend on the further development of larger sample,multicenter and high-quality

ABSTRACT

Objective: General practitioners in rural clinics are required to deal with musculoskeletal problems, but they often lack the confidence. We aim to confirm changes in their confidence in orthopedic practices after using tele-education.Materials and Methods: We conducted tele-education in orthopedic practices from June 1, 2022, to November 30, 2022. Using a teleconference system, the first author, an orthopedic specialist, provided tele-education training to an independent general practitioner in a rural clinic. We adopted a 7-point Likert scale to assess the general practitioner’s confidence levels. In pre- and post-research, the counts and confidence levels in the scale were assessed for 18 types of orthopedic practices each month. Furthermore, we interviewed the general practitioner to examine the factors influencing their confidence.Results: The confidence levels increased for all measurement items. The most experienced orthopedic practice was “Advising on daily care for musculoskeletal problems”, with confidence levels increasing from 3 to 6. The least experienced orthopedic practice was “Manipulative reduction of radial head subluxation”, with confidence levels increasing from 4 to 5. The factors that influenced the change in confidence levels were regular feedback and unrestricted availability of consultations.Conclusion: Tele-education in orthopedics may enhance general practitioners’ confidence in orthopedic practices.

ABSTRACT

OBJECTIVE To provide reference for improving the rational use of antimicrobial drugs in primary township medical institutions. METHODS Based on the county prescription pre-review center, a team led by anti-infective clinical pharmacists constructed the management mode for the use of antimicrobial drugs in county-level medical communities with clinical pharmacists as the main team by finding out the main problems in the use of antimicrobial drugs in primary township medical institutions, providing feedback on the problems, organizing relevant training for the problems, improving the customization rules of the prescription pre-review software, implementing the automatic interception and pharmacist online prescription review and other measures. Data on the use of antimicrobial drugs were collected and compared in the 15 primary township medical institutions between January-June in 2022 (before the implementation of the mode) and January-June in 2023 (after the implementation of the mode). RESULTS Compared with before the implementation of the mode, the utilization rate of antimicrobial drugs in outpatients of primary township medical institutions decreased from 24.97% before the implementation of the mode to 19.39% after the implementation of the mode; the utilization rate of antimicrobial injection in outpatients decreased from 66.10% to 46.80%; the utilization rate of intravenous drip of antimicrobial drugs in outpatients decreased from 52.33% to 40.35%; the rates of combined use of antimicrobial drugs in outpatients decreased from 12.70% to 8.19%; the reasonable rate of antimicrobial prescribing in outpatients increased from 55.28% to 73.93%. After the implementation of the mode, the proportion of antimicrobial prescriptions for each diagnosis was basically the same as before; the defined daily dose system (DDDs) and proportion of a few antimicrobial drugs changed compared with before according to the anatomical therapeutic chemical classification of drugs, among which DDDs of lincomycin, gentamicin and other drugs declined significantly; DDDs of antimicrobial drugs for each classification was basically the same as before according to AWaRe classification. CONCLUSIONS The management mode of the use of antimicrobial drugs led by anti-infective clinical pharmacists is constructed in the prescription pre-reviewing center of county-level medical communities, which can effectively improve the rational use of antimicrobial drugs in the primary township medical institutions.

The specimens were collected during the years 2014 to 2017, using gillnets with mesh sizes ranging from 30 to 120mm between opposite knots. Through the analysis of stomach contents, the preference in the consumption of items of animal origin was observed. Juveniles consumed insects in greater proportions (IAi% = 50%), while adults consumed fish (IAi%=99,98%). Despite the large supply of food items available in the high-water period, juveniles were the only ones to consume items of allochthonous origin, such as insects. Juveniles presented a different dietary strategy and dietary composition to adults. Juveniles were omnivores with a generalist strategy, while adults were piscivores with a specialist strategy. Thus, the food composition, niche breadth, trophic position and feeding strategy of Acestrorhynchus falcirostris change due to the stage of development.


Resumo O objetivo deste estudo foi determinar a ecologia trófica de juvenis e adultos de Acestrorhynchus falcirostris durante o período de enchente e cheia em seis lagos de ilha adjacentes ao rio Solimões. Assim, investigamos: i) a posição trófica, através do nicho trófico fracional; ii) a largura de nicho; iii) sobreposição de nicho; iv) a estratégia alimentar. ABSTRACT

Lip necrotizing lesions are painful, complex to heal, and negatively impact the patient's orofacial condition. The present study reports a clinical case of a necrotizing lesion on the lower lip in a 38-year-old male patient, previously healthy, five days after receiving the first dose of the SARS-COV-2 vaccine, with an initial diagnosis of labial herpes zoster, with secondary infection and unsatisfactory response to clinical treatment. After debridement of the necrotic tissue, followed by a combination of antimicrobial photodynamic therapy (aPDT), photobiomodulation therapy (PBMT) and hydrofiber dressing with silver, the patient evolved with complete healing of the lip lesion and satisfactory restoration of orofacial functions after 7 days. According to the present case report, this combination of phototherapy modalities suggests to be a promising tool for the treatment of necrotizing labial lesions.

El profesional de cultura física contribuye, con aportes importantes, al perfeccionamiento de los programas; aplica a la educación el ejercicio físico desde las primeras edades y favorece la formación integral. El objetivo de este trabajo consistió en diseñar una metodología para el desarrollo de las cualidades coordinativas básicas en la Educación Física del nivel inicial 2. Para ello, se realizó un estudio de diseño preexperimental de control mínimo, con dos años de duración (2020-2021). La muestra seleccionada respondió a un muestreo intencional estratificado, no probabilístico. Se emplearon métodos de los niveles teórico y empírico, tales como: análisis y síntesis, inductivo-deductivo, sistémico-estructural funcional, medición, experimento, criterio de expertos y estadística descriptiva, así como técnicas de investigación entre los que destacan, análisis documental, observación estructurada, entrevista y encuesta. La metodología se realizó mediante la implementación de ejercicios físicos. Participaron 229 niños del nivel inicial 2; a partir del diagnóstico realizado se determinó que el equilibrio dinámico (46,3 %) y la coordinación dinámica general (43,7%) fueron evaluados de regular, además de dificultades en todos los patrones de movimiento, la motricidad gruesa, la fina, los reflejos, las reacciones y patrones posturales. El estudio mostró que se puede mejorar el desarrollo de las cualidades coordinativas básicas en la Educación Física del nivel inicial 2.

A metodologia foi realizada por meio da execução de exercícios físicos. Participaram 229 crianças do nível inicial 2; A partir do diagnóstico realizado, constatou-se que o equilíbrio dinâmico (46,3%) e a coordenação dinâmica geral (43,7%) foram avaliados como regulares, além de dificuldades em todos os padrões de movimento, motricidade grossa e fina, reflexos, reações e padrões posturais. O estudo mostrou que o desenvolvimento das qualidades coordenativas básicas pode ser melhorado na Educação Física no nível inicial 2.


The physical culture professional contributes, with important contributions, to the improvement of the programs; applies physical exercise to education, from the earliest ages and promotes comprehensive training. The objective of this work was to design a methodology for the development of basic coordinative qualities in Physical Education of the initial level 2. To this end, a pre-experimental design study of minimum control was carried out, lasting two years (2020-2021). The selected sample responded to a stratified, non-probabilistic intentional sampling. Methods from the theoretical and empirical levels were used, such as: analysis and synthesis, inductive-deductive, systemic-structural-functional, measurement, experiment, expert judgment and descriptive statistics, as well as research techniques, among which stand out documentary analysis, structured observation, interview and survey. The methodology was carried out through the implementation of physical exercises. 229 children from initial level 2 participated. Based on the diagnosis made, it was determined that dynamic balance (46.3 %) and general dynamic coordination (43.7 %) were evaluated as fair, in addition to difficulties in all movement patterns, gross and fine motor skills, reflexes, reactions and postural patterns. The study showed that the development of basic coordinative qualities can be improved in Physical Education at initial level 2.

Introduction: Scientific studies in Brazil grew around 10.7% compared to previous years. However, the level of quality of evidence has been decreasing. The aim in our study is to examine the meeting abstracts of the Brazilian congress of coloproctology and analyze the level of evidence in trends and variables. Methods: A descriptive bibliometric study, working with secondary data to review scientific abstracts in the annals of the coloproctology congress from 2015 to 2019. Results: A total of 1756 abstracts of the Brazilian Congress of Coloproctology were analyzed for 5 years (2015-2019). There was a higher trend of abstracts presented with lower levels of evidence (level of evidence 5: 52.3% and 3: 30%), being the majority composed of case reports (49.4%) and retrospective studies (30.4%). The last two years analyzed (2018: 55.2% and 2019: 59.3%) had a predominance above average of case reports. From 2017 to 2019 there was a significant decrease in the number of level 2 evidence studies (18.10%,11.80% and 5.50%), while the number of studies with level 5 evidence showed an increase (45.60%, 56.60% and 61.40%). Statistical analysis occurred in only 17%, with an important decrease for the last two years (2018: 13.6%; 2019: 12.1%). Conclusions: Although the data of this study is from the Brazilian coloproctology point of view, they are important for the global scientific community, as they allow a quantitative evaluation of the relative contribution from the level of evidence of Brazilian coloproctology researchers to the scientific scenario. Fundamento: El nivel de conocimiento sobre lesiones premalignas en la población geriátrica es un factor significativo que incide en la prevención, evitando la progresión a un cáncer bucal. Objetivo: Identificar el nivel de conocimiento de los pacientes geriátricos sobre las lesiones premalignas bucales. Metodología: Entre septiembre de 2019 y junio de 2021 se realizó un estudio observacional descriptivo transversal en el Consultorio Médico de la Familia # 12 del municipio Cabaiguán, provincia Sancti Spíritus. La población estuvo constituida por 158 pacientes geriátricos y mediante un muestreo probabilístico aleatorio simple la muestra quedó conformada por 80. Se aplicaron métodos del nivel teórico, empírico, estadístico y matemático. Las variables utilizadas fueron edad, sexo, escolaridad, nivel de conocimiento sobre factores de riesgo y signos clínicos, conocimientos sobre el autoexamen bucal y la conducta a seguir. Resultados: Se constató que el nivel de conocimientos sobre factores de riesgo fue regular y sobre signos clínicos y forma y frecuencia del autoexamen bucal fue mal. La mayoría de la muestra en estudio conocía donde acudir y afirmó haber recibido información al respecto. Conclusiones: En su mayoría el nivel de conocimientos sobre lesiones premalignas se observó entre regular y mal con un descenso significativo del nivel de conocimientos en los pacientes de edades más avanzadas.


Background: The knowledge level about premalignant lesions in the geriatric population is a significant factor that influences on prevention, avoiding progression into mouth neoplasms. Objective: To diagnose the knowledge level in geriatric patients about buccal premalignant lesions. Methodology: A cross-sectional descriptive observational study was conducted between September 2019 and June 2021 at the Family Medical Office # 12 of Cabaiguán municipality, Sancti Spíritus province. The population was constituted by 158 geriatric patients and by means of a simple random probabilistic sampling, the sample was made up by 80. Theoretical, empirical, statistical and mathematical methods were applied. The used variables were age, sex, schooling, risk factors and clinical signs knowledge, knowledge of buccal self-examination and conduct to be followed. Results: The knowledge level about risk factors was found to be regular and on clinical signs and form and frequency of buccal self-examination was bad. The majority of the sample under study knew where to go and affirmed that they had received information about it. Conclusions: Mostly the knowledge level of premalignant lesions was observed to be regular to bad with a significant decrease in the knowledge level in more elderly patients.

ABSTRACT

Objectives: This was a single-subject study, aimed to demonstrate different vocal demand situations that are typical for primary school and teacher's vocal demand response under two acoustical conditions, with and without voice amplification, during five working days. Methods: The long-term voice dosimetry with Vocal Holter Med (PR.O. Voice Srl) was carried out on a 49-year-old female teacher with voice disorders during daily teaching activities. A sound field amplification system (SFAS) PentaClass Runa was installed in the classroom. Voice dosimetry was provided under two different acoustical conditions: without SFAS (2 days) and with SFAS (3 days). Results: Phonation time percentage, sound pressure level (SPL), SPL SD, fundamental frequency (F0), F0 SD, cycle, and distance doses were investigated in seven communication scenarios (lessons, group/individual classes, sports lessons in the gym and schoolyard, breaks, lunch breaks, and other activities). The median scores of all voice parameters differed significantly between different vocal demand contexts. The significant statistical difference in the vocal demand response was in the communication situations with and without SFAS. In addition, the number of children, reverberation time, and ambient air relative humidity impacted voice SPL and the cycle dose. Conclusions: Lessons, sports lessons held in the gym or schoolyard, breaks, and lunch breaks were considered as high vocal demand communication situations requiring higher voice intensity and fundamental frequency, higher phonation time percentage, cycle, and distance doses. Group/individual work and other teacher activities during the day, unrelated to direct work with students, were categorized as low vocal demand communication scenarios.


Objetivos: Este fue un estudio de sujeto único, cuyo objetivo fue demostrar diferentes situaciones de demanda vocal típicas de la escuela primaria y la respuesta vocal de los docentes bajo dos condiciones acústicas, con y sin amplificación de voz, durante cinco días laborables. Métodos: Se llevó a cabo dosimetría vocal a largo plazo con Vocal Holter Med (PR.O. Voice Srl) durante las actividades diarias de enseñanza en una docente de 49 años con trastornos de la voz. Se instaló un sistema de amplificación de campo sonoro (SFAS) PentaClass Runa en el aula. La dosimetría vocal se realizó bajo dos condiciones acústicas diferentes: sin SFAS (2 días) y con SFAS (3 días). Resultados: Se investigaron el porcentaje de tiempo de fonación, el nivel de presión sonora (SPL), SPL SD, la frecuencia fundamental (F0), F0 SD, ciclos y dosis de distancia en siete escenarios de comunicación diferentes (clases, clases grupales/individuales, clases de educación física en el gimnasio y el patio de la escuela, recreos, almuerzos y otras actividades). Las puntuaciones medias de todos los parámetros vocales diferían significativamente entre los diferentes contextos de demanda vocal. La diferencia estadísticamente significativa en la respuesta a la demanda vocal se observó en las situaciones de comunicación con y sin SFAS. Además, el número de niños, el tiempo de reverberación y la humedad relativa del aire ambiente afectaron al SPL de la voz y la dosis de ciclo. Conclusiones: Las lecciones, las clases de educación física en el gimnasio o el patio de la escuela, los recreos y los almuerzos se consideraron situaciones de comunicación de alta demanda vocal, que requerían una mayor intensidad y frecuencia fundamental de la voz, un mayor porcentaje de tiempo de fonación y dosis de ciclo y distancia más altas. El trabajo grupal/individual y otras actividades del profesor durante el día no relacionadas con el trabajo directo con los estudiantes se categorizaron como escenarios de comunicación de baja demanda vocal.

Introducción: Las radiografías dentales son una de las exposiciones médicas más frecuentes a la radiación ionizante. El uso de radiación ionizante está asociado a un riesgo probable de desencadenar efectos biológicos adversos y posibles daños a la salud del paciente. Para evitar que los pacientes reciban dosis innecesariamente altas durante estas exposiciones, la Comisión Internacional de Protección Radiológica recomienda la utilización de los niveles de referencia para diagnóstico, como una herramienta efectiva de ayuda a la optimización de la protección radiológica de los pacientes. Objetivo: Estimar los niveles de referencia para diagnóstico en radiografía dental intraoral y panorámica en la ciudad de Bogotá, D. C. Metodología: Se evaluaron los parámetros de exposición radiográficos de los equipos y la calidad de imagen en 68 equipos de radiografía dental periapical y 23 equipos de radiografía panorámica. Se estimaron las magnitudes dosimétricas de kerma incidente en aire (Kai) en equipos intraorales para la radiografía de un maxilar molar de un adulto y el producto kerma aire-área (PKA) en equipos de radiografía panorámica en un examen de un adulto estándar. Resultados: El tercer cuartil de la distribución de kerma incidente en aire para radiografía intraoral fue de 3,3 mGy y del producto kerma aire-área para radiografía panorámica fue de 103,9 mGycm2. En la distribución de frecuencias de kerma incidente en aire para radiografía intraoral, el porcentaje más alto de equipos estuvo en el rango de 2,0-3,0 mGy. En la distribución de frecuencias del producto kerma aire-área para los equipos de radiografía panorámica, el porcentaje más alto de equipos estuvo en el rango de 60 a 80 mGycm2. Discusión: Las instituciones consideradas para establecer los Niveles de Referencia para Diagnóstico en este estudio contaron con una adecuada calidad de la imagen evaluada con un maniquí dental, pero las variaciones en las dosis de radiación entre instituciones señalan la necesidad de implementar herramientas que contribuyan a la optimización de las prácticas. Conclusiones: Se recomienda usar los valores de los niveles de referencia para diagnóstico encontrados en esta investigación para optimizar la protección radiológica en las exposiciones radiológicas dentales, y se espera que este estudio sirva de base para nuevas investigaciones en las demás ciudades del país.


Introduction: Dental X-rays are one of the most frequent medical exposures to ionizing radiation. The use of ionizing radiation is associated with a probable risk of triggering adverse biological effects and possible damage to the patient's health. To prevent patients from receiving unnecessarily high doses during these exposures, the International Commission on Radiological Protection recommends the use of diagnostic reference levels as an effective tool to help optimize radiological protection for patients. Objective: To estimate diagnostic reference levels in intraoral and panoramic dental radiography in the city of Bogotá, D.C. Methodology: In 68 periapical dental radiography equipment and 23 panoramic radiography equipment, the radiographic exposure parameters of the equipment and image quality were evaluated. The dosimetric magnitudes of incident air kerma (Ka,i) in intraoral equipment for the radiography of a maxillary molar of an adult and the air kerma-area product (PKA) in panoramic radiography equipment in a standard adult examination were estimated. Results: The third quartile of the incident air kerma distribution for intraoral radiography was 3,3 mGy and the air kerma-area product for panoramic radiography was 103,9 mGycm2. In the frequency distribution of incident air kerma for intraoral radiography, the highest percentage of equipment was in the range of 2,0-3,0 mGy, and in the frequency distribution of the air kerma-area product for equipment of panoramic radiography, the highest percentage of the equipment was in the range of 60 to 80 mGy cm2. Discussion: The institutions considered to establish the diagnostic reference levels in this study had an adequate quality of the image evaluated with a dental phantom, but the variations in radiation doses between institutions indicate the need to implement tools that contribute to the optimization of the practices. Conclusions: It is recommended to use the values of the diagnostic reference levels found in this research to optimize radiological protection in dental radiological exposures, and it is expected that this study will serve as a basis for further research in other cities of the country.

ABSTRACT

Objetivo:Analisar as evidências disponíveis na literatura acerca da utilização da fotobiomodulação no tratamento de intercorrências mamárias pós-parto. Método: Revisão integrativa dos manuscritos nas bases de dados National Library of Medicine National Institutes of Health (PubMed), Web of Science, Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs), Cumulative Index to Nursing and Allied Health Literature (CINAHL), EMBASE e Scopus. Foram incluídos artigos originais redigidos em português e inglês, analisados de acordo com o nível de evidência e qualidade metodológica. Resultados: A amostra constituiu-se de cinco artigos. Com base na análise dos resultados, foram elaboradas três categorias temáticas: tipos de fotobiomodulador utilizados nas intercorrências mamárias, efeitos do fotobiomodulador na cicatrização das intercorrências mamárias e efeitos do fotobiomodulador na redução da dor mamilar. Tanto o fotobiomodulador de light-emitting diode (LED) quanto o de laser foram eficazes no tratamento dos traumas mamilares, pois a aplicação proporcionou aceleração do processo cicatricial. A terapia com laser de baixa intensidade foi considerada eficaz para tratar lesões mamilares em lactantes com dor, oferecendo alívio e prolongando o aleitamento materno exclusivo. O protocolo de laser de uma única aplicação não foi eficaz na redução da dor em mulheres com mamilos danificados. Conclusão: Este estudo buscou mitigar as lacunas existentes entre assistência e pesquisa em enfermagem, uma vez que contribui para o cuidado pautado na prática baseada em evidências.


Objective:To analyze the evidence available in the literature about the use of photobiomodulation in the treatment of breast complications in the postpartum. Method: Integrative review of manuscripts in National Library of Medicine National Institutes of Health (PubMed), Web of Science, Latin American and Caribbean Health Sciences Literature (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINAHL), EMBASE, and Scopus databases. Original articles written in Portuguese and English were included, analyzed according to the level of evidence and methodological quality. Results: The sample consisted of five articles. Based on the analysis of the results, three thematic categories were created: types of photobiomodulator used in breast complications, effects of photobiomodulators on healing of breast complications, and effects of photobiomodulators on reducing nipple pain. Both light-emitting diode (LED) and laser photobiomodulators were effective in the treatment of nipple trauma, as the application accelerated the healing process. Low-intensity laser therapy was found to be effective in treating nipple injuries in nursing mothers with pain, providing relief and prolonging exclusive breastfeeding. The single application laser protocol was not effective in reducing pain in women with damaged nipples. Objetivo:Objetivo: analizar la evidencia disponible en la literatura sobre el uso de la fotobiomodulación en el tratamiento de las complicaciones mamarias después del parto. Método: revisión integradora de manuscritos en las bases de datos PubMed, Web of Science, LILACS, CINAHL, EMBASE y Scopus. Se incluyeron artículos originales escritos en portugués e inglés, analizados según nivel de evidencia y calidad metodológica. Resultados: la muestra estuvo compuesta por cinco artículos. Con base en el análisis de los resultados, se crearon tres categorías temáticas: tipos de fotobiomoduladores utilizados en las complicaciones mamarias, efectos de los fotobiomoduladores en la curación de las complicaciones mamarias y efectos de los fotobiomoduladores en la reducción del dolor en los pezones. Tanto el fotobiomodulador LED como el LÁSER resultaron efectivos en el tratamiento del trauma del pezón, ya que su aplicación aceleró el proceso de cicatrización. Se descubrió que la terapia con láser de baja intensidad es eficaz en el tratamiento de lesiones en los pezones en bebés con dolor, proporcionando alivio y prolongando la lactancia materna exclusiva. El protocolo LASER de aplicación única no fue efectivo para reducir el dolor en mujeres con pezones dañados. Conclusión: Este estudio buscó mitigar las brechas existentes entre el cuidado de enfermería y la investigación, ya que contribuye al cuidado basado en la práctica basada en la evidencias.
