ABSTRACT

Wumei Pill is a classic insect repellent prescription, which is used for ascaris lumbricoides and diarrhea. It is considered to be a representative prescription for the treatment of simultaneous occurrence of cold and heat syndromes. Its prescription is mainly composed of sour drugs ( Mume Fructus and bitter wine) and pungent drugs ( Aconiti Lateralis Radix Praeparataaconite, Cinnamomi Ramulus, Sichuan pepper, Angelicae Sinensis Radix, Zingiberis Rhizoma, and Asari Radix et Rhizoma), supplemented by bitter drugs ( Coptidis Rhizoma and Phellodendri Chinensis Cortex) and sweet drugs ( Ginseng Radix et Rhizoma and honey). From the point of view of "Tang-Ye-Jing-Fa Map", the role of liver is to disperse, whose deficiency leads to limb syncope, and excess leads to the full of hypochondrium and abdominal pain. The pungent herbs can tonify the liver-deficiency, the sour herbs can dispel the liver-excess, and sweet herbs can relive them both. The role of spleen is to moderate, whose deficiency leads to fatigue and weakness, and excess leads to the vomiting and diarrhea. The sweet herbs can tonify the spleen-deficiency, the pungent herbs can dispel the spleen-excess, and bitter herbs can relive them both. Therefore, the function of Wumei Pills is located in the liver and spleen, giving consideration to the heart and lung. It is used for the deficiency and excess mixed syndrome of liver and spleen, which is mainly characterized by chest and flank abdominal pain, limb chills, diarrhea, muscle and pulse contracture and the symptoms of the parts along the liver meridian. It is commonly used in the treatment of ascaris, diarrhea, impotence, depression. At the same time, it can also be used for the treatment of diseases about heart and lung, and cough, anxiety and other diseases.

ABSTRACT

Liver fibrosis is the leading cause of the morbidity and mortality of liver diseases worldwide and has become one of the key issues in the research on chronic liver diseases among domestic and foreign scholars. An increasing amount of evidence has shown that gut microbiota dysbiosis plays a crucial role in the development and progression of liver fibrosis, and improvement of gut microbiota dysbiosis has become a new target for anti-fibrotic treatment. At present, traditional Chinese medicine has attracted much attention in improving gut microbiota. Studies have shown that "harmonizing liver and spleen" plays an important role in reducing fibrosis degree and delaying the progression of liver fibrosis. Therefore, based on the theory of "harmonizing liver and spleen", the achievements in liver-gut axis and gut microbiota by modern molecular biological techniques can provide a theoretical basis for the treatment of liver fibrosis from "harmonizing liver and spleen" in clinical practice, as well as new directions and ideas for anti-fibrotic treatment with traditional Chinese medicine.

ABSTRACT

Objective:To investigate the clinical manifestations of Epstein-Barr virus infection in children and the efficacy of interferon combined with ganciclovir. Methods:A total of 252 children with Epstein-Barr virus infection who received treatment in Liaocheng Maternal and Child Health Hospital from June 2018 to February 2020 were included in this study. They were randomly assigned to undergo treatment either with ganciclovir alone (control group, n = 126) or interferon combined with ganciclovir (experimental group, n = 126). General condition, clinical manifestation, clinical outcomes, and clinical efficacy were compared between the two groups. Results:The 252 children with Epstein-Barr virus infection were divided into four groups according to different age brackets: infancy (3.97%), early childhood (53.57%), preschool (28.97%), school age (13.49%). Children at the early childhood and preschool ages accounted for high proportions. Their clinical manifestations included fever, pharyngeal congestion, cervical lymph node swelling, and pharyngeal pain. Children with hepatosplenomegaly accounted for the highest proportion (44.12%) among those at the school age, and children with binocular edema accounted for the highest proportion (10.37%) among those at the early childhood age. The time to defervesce, eyelid edema, and lymph node regression in the experimental group were (3.55 ± 1.58) hours, (3.82 ± 1.17) hours, and (9.55 ± 1.60) hours respectively, which were significantly shorter than those in the control group [(4.40 ± 1.80) hours, (5.33 ± 1.58) hours, (10.44 ± 1.66) hours, t = 3.64, 2.47, 2.67, P < 0.001, P = 0.024, 0.009]. The total response rate was significantly higher in the experimental group than in the control group [96.03% (107/126) vs. 84.92% (121/126), χ2 = 9.03, P = 0.003]. Conclusion:Epstein-Barr virus infection has different clinical manifestations in children at different ages. Interferon combined with ganciclovir is more effective on Epstein-Barr virus infection than ganciclovir alone.

ABSTRACT

To establish mathematical model of Xin’an Wang’s internal medicine for treating epigastric pain through the data mining technique, and provide a more sufficient scientific basis for systematically discussing the rule of traditional Chinese medicines in treating epigastric pain. From the “Wang Zhongqi case” and “Wang Renzhi case”, outpatient medical records of Professor Letao Wang and Professor Jian Wang were used to select 310 cases of medical records in the diagnosis and treatment of epigastric pain according to inclusion criteria. Then we extracted the information of prescription, symptoms and signs, and established “information database of Xin’an Wang’s medical cases of internal medicine for treating epigastric pain”. Using frequency analysis, calculation of high-frequency drugs, and application of IBM Modeler 14.1 by SPSS Apriori algorithm for correlation analysis to extract the core prescription and then establish the mathematical model of Wang’s doctors treating epigastric pain. Wang’s doctors in Xin’an often treat the stomach and spleen with the same treatment of liver and spleen. Wang’s physician treated epigastric pain with both liver and spleen. Wang’s physician assisted the spleen with Tongyang method, or relieved phlegm, dredged liver and Qi to Tongyang, or Xinwen Sanhan to Tongyang, or Jianpi Huashi to Tongyang, or Huoxue Xingqi to Tongyang and Huoxue Sanjie method and Huoxue Huayu method.

ABSTRACT

This article presents that the pathology effects between liver and spleen should be divided into hepatic excess transmitting into the spleen, hepatic asthenia implicating injure to spleen and spleen disease influence the liver.Hepatic excess transmitting into the spleen emphasizes the view of excess to transmit while deficiency not to transmit. The excess syndromes of liver in terms of stagnation of liver-qi,hepatic qi,hepatic fire, hepatic yang, hepatic wind, hepatic coldness,dampness-heat of the liver and stagnant qi with drink, phlegm and blood stasis should be divided into three kinds: native-qi stagnant itself, evolving to pathogenic factor and direct attack induced evil occurring inside, they are all excess disease transmitting into the spleen. The hepatic asthenia implicating injure to spleen should distinguish the liver function and liver substance.Deficiencies of liver function are deficiency of liver-qi and deficiency of liver-yang;Deficiencies of liver substance are deficiency of liver-blood and deficiency of liver-yin.Deficiency of liver-qi induces the spleen pertaining to earth can not be abreaction, deficiency of liver-yang mostly implicates injure to splenic yang, deficiency of liver-blood can not be excessive mother element and having stagnation of liver-qi with deficiency of the spleen, deficiency of liver-yin involves the yin of spleen and stomach mostly with asthenic liver-yin causing excessive pyrexia and having sign of stomach-fire.The spleen disease influence the liver mainly presents spleen-earth obstructed and hepatic qi stagnation and spleen disease occurred evil to invade liver.

ABSTRACT

BACKGROUND: We analyzed the clinical characteristics of trauma involving the liver and/or the spleen to evaluate the safety of nonoperative management. METHODS: A retrospective study was conducted on 78 cases who were treated with operative (37 cases) and nonoperative (41 cases) management at Wonkwang University Hospital from January 1995 to June 1998. Nonoperative management was done in hemodynamically stable patients. RESULTS: Three cases in the operative group (OG) and 5 cases in the nonoperative group (NOG) were children. Causes of injury were traffic accidents (66.6%), falls (15.4%), penetrating injuries (11.4%), and assaults (6.4%). Associated intraabdominal injuries were renal contusions (42%), pancreatic injuries (18%), mesenteric tearing (12%), diaphragm ruptures (9%). Associated extraabdominal injuries were chest injuries (56.5%), long bone fractures (15.2%), pelvic bone fractures (9.8%), and CNS injuries (8.7%). Mean total blood requirements were 7.0 units in the OG and 1.2 units in the NOG. Complications were 35 cases in the OG (pleural effusion, 6 cases; intraabdominal abscess, 4 cases; pneumonia, 4 cases; biloma, 3 cases) and 15 cases in the NOG (pleural effusion, 5 cases; pneumonia, 5 cases; intraabdominal abscess, 2 cases). Causes of 8 delayed operations were bowel perforations (small bowel 2 cases, and colon, 1 case), bile peritonitis (2 cases), and hemodynamically instability (3 cases). The hospitalization period was 16 days in the OG and 14 days in the NOG. Six patients in the OG died and the causes were multiple organ failures (4 cases), hypovolemic shock (1 case), and ARDS (1 case). No patient died in the delayed operation cases and the NOG, even in major injury cases aquired respiratory distress syndrom. CONCLUSIONS: Nonoperative management of traumatic liver and/or spleen injury is safe and can be tried initially under the conditions of stable hemodynamics, feasible abdominal CT, and feasible immediate operation.
