ABSTRACT

ABSTRACT Bowel preparation is mandatory prior to elective colonoscopy and their effectiveness is closely related to the quality of the examination. There are many preparations on the market and there is no consensus on which is best. This study aimed to compare three solutions for colon preparation in patients undergoing colonoscopy. We conducted a prospective study with 61 patients divided randomly into three groups: one that received a standard dose of macrogol, another received a standard dose of 10% mannitol and another received a standard dose of sodium picosulphate. Patients and examining endoscopists responded to questionnaires for compiling data. In the results we noticed that 10% mannitol, despite being less tolerated by the patient when compared to sodium picosulphate, presents better results in colonic cleaning, being therefore superior in this regard. Macrogol was considered as an intermediate in relation to the other two preparations. As for tolerability, preference is given to sodium picosulphate as best tolerated, followed by mannitol and by macrogol, which is poorly tolerated by the patient. We conclude that as the main objective of bowel preparation in colonoscopy is the quality of colonic cleaning, 10% mannitol was superior to the other preparations studied.


RESUMO O preparo intestinal é mandatório antes da realização das colonoscopias eletivas e sua eficácia está intrinsecamente relacionada à qualidade do exame. Existem diversos preparos no mercado e não há consenso sobre qual é melhor. Este estudo teve como objetivo comparar três soluções para preparo de cólon em pacientes submetidos à colonoscopia. Foi realizado um estudo prospectivo com 61 pacientes distribuídos de forma randomizada em três grupos: um recebeu macrogol, outro manitol a 10% e outro picossulfato de sódio em doses padrão. Os pacientes e os endoscopistas examinadores responderam a questionários para compilação de dados. Nos resultados notamos que o manitol a 10%, apesar de ser menos tolerado pelo paciente quando comparado ao picossulfato de sódio, apresenta melhores resultados na limpeza colônica, sendo, portanto, superior neste quesito. O macrogol foi considerado como intermediário em relação aos outros dois preparos. Quanto à tolerabilidade, a preferência recai sobre o picosulfato de sódio como o mais bem tolerado, seguido pelo Manitol; macrogol foi o menos tolerado pelo paciente. Concluímos que, como o principal objetivo do preparo intestinal na colonoscopia é a qualidade da limpeza colônica, o manitol a 10% mostrou-se superior aos demais preparos estudados.

ABSTRACT

Introduction: Recently, ointment bases have been considered to play more important roles in wound healing than those played by their main agents. There has been an increasing interest in the use of macrogol (MAC) because it is considered to accelerate the epithelialization of pressure ulcers. We compared the effectiveness of MAC and vaseline (VAS) in healing d2 (partial-thickness skin loss) sacral pressure ulcers.Methods: Twenty-five cases of d2 sacral pressure ulcers, which occurred in patients in long-term-care beds in our hospital, were initially included in this study. VAS was applied to d2 sacral pressure ulcers in 13 cases (VAS group), and MAC was also applied in 12 cases (MAC group). After VAS or MAC was applied, all pressure ulcers were covered with non-adhesive, water-absorbing gauzes. One case of in-hospital mortality was excluded from the MAC group; therefore, 13 cases in the VAS group and 11 cases in the MAC group were included in the final analysis. The healing periods were compared between the two groups.Results: There were no significant differences in the patients' background between the two groups. The healing period (days) in the MAC group (8 [7-11.5]) was significantly shorter than that in the VAS group (16 [12-32]) (P=0.029).Conclusion: MAC may accelerates the healing process of d2 sacral pressure ulcers.

ABSTRACT

Background Macrogol 15 hydroxystearate (HS15) is a novel soft non-ionic surfactant and is widely used to solubilize the poorly soluble drugs due to high drug loading and enhancing permeability ability to hydrophobic drug.However,the delivering effects to cornea of HS15 on terbinafine hydrochloride (TH),a insoluble antifungal agents,is unclear.Objective This study was to investigate the promoting corneal absorption effects of HS15 micelles (HNMs) on TH.Methods TH-HNMs was prepared by a co-solvent method.The hydrodynamic droplet size,polydispersity index,and Zeta potential of TH-HNMs were measured by using a Zetasizer.The shape of the micelles was observed under the transmission electron microscope.The high performance liquid chromatography (HPLC) was employed to detect the in vitro cumulative releasing level of TH in the TH-HNMs and drug entrapment efficiency.TH-HNMs was topically adninistered in eyes of 5 healthy male New Zealand rabbits to evaluate the ocular irritation response.Ninety rabbits were randomized into experimental group and control group,and 50 μl of 0.5% TH-HNMs and 0.5% oily TH were topically administered in the right eyes of the animals in the experimental group and contral group,respectively.The animals were sacrificed 5,15,30,60,90,120,180,240,360 minutes after eye dropping by over anesthetization way and the corneas were harvested,and the TH content in the cornea was detected using HPLC.The study protocal was approved by Life Science Ethic Committee of Henen Eye Hospital.Results The average size and polyolis persital index of TH-HNMs were 13.32 nm and 0.046,respectively,and its average Zeta potential was-0.133 mV.The drug entrapment efficiency was 100%.The release level of TH from the micelles presented a pH-dependent manner.The release level of TH was (95.20±3.20)% in the phosphate buffer with pH 5.0 and (0.17± 0.01)% in the phosphate buffer with pH 7.4.The ocular irritation score was 2,and no visible damage was found around experimental eyes after instillation of TH-HNMs.The peak content of TH in the rabbit cornea 5 minutes was (20.26±2.26)pg/g in the experimental group,which was significantly higher than (1.40± 0.44)μg/g in the control group (t =18.926,P=0.000).The area under the curve (AUC)0.360min of drug concentration-time curve in the experimental group was 1 292.25 μg/(g · min),which was 15.6 times more than the control group.Conclusions TH-HNMs is an ideal agent with a simple preparing process,high drug entrapment efficiency,small size and low ocular irritation.Compared with oily TH,TH-HNMs can effectively enhance the corneal absorption of TH.

ABSTRACT

Objective: To improve the traditional preparation procedure of effervescent tablets in order to raise stabilization. Methods: The orthogonal design was used for improvement of process. Results: The optimum procedure condition was A 3B 2C 1. That is adding 7.5% citric acid, 11.25% sodium bicarbonate encapsulated by 5% PEG. Conclusion: The new procedure is superior to the traditional procedure, and it is suitable for the requirement of production on a large scale.

ABSTRACT

Objective: To establish the optimum preparation process for Xuesaitong Dropping Pills(total saponin of Radix Notoginseng). Methods: On the basis of orthogonal design, preparation conditions were selected among sort of polyglycol (as substance), consumption of polyglycol and processing temperature of usoline (as cooling agent). Results: Under the condition of using polyglycol 6000, and processing temperature at 10~15 ?C , the quality of Xuesaitong Dropping Pills is good.

ABSTRACT

Objective: To establish the optimum preparation process for Suxiaoxintong Dropping Pills (Cortex moutan, Rhizoma Chuanxiong, Borneolum Syntheticum). Methods: The preparation conditions, such as the different kinds of primary substance, cooling agent, the proportion between the extracts and primary substance etc. were studied with L 18 (3 7) orthogonal design. Results: Six batches of the products prepared with the process on the experimental result were conformed to the standard of the Chinese pharmacopoeia. Conclusion: It has been proved that the process is available.
