ABSTRACT

Objective:To investigate the clinical efficacy of ultrasound-guided mammotomy versus conventional surgery in the treatment of benign breast tumors. Methods:Sixty patients with benign breast tumors who received treatment in Jiamusi Central Hospital from September 2018 to September 2019 were included in this study. They were randomly assigned to undergo either conventional surgery (control group, n = 30) or ultrasound-guided mammotomy (treatment group, n = 30). Intraoperative blood loss, operative time, incision length, and the incidence of complications were compared between the two groups. Results:Intraoperative blood loss, operative time, and incision length in the treatment group were (4.25 ± 1.23) mL, (15.36 ± 3.21) minutes, (0.41 ± 0.05) cm, respectively, which were significantly lower or shorter than those in the control group [(10.75 ± 2.12) mL, (29.68 ± 7.23) minutes, (2.9 ± 0.8) cm, t = 14.526, 9.915, 17.015, all P < 0.05]. In the treatment group, the incidence of complications was 3.33% (local hematoma n = 1, skin depression n = 0, flap necrosis n = 0, postoperative bleeding n = 0), which was significantly lower that 23.00% in the control group (local hematoma n = 2, skin depression n = 1, flap necrosis n = 1, postoperative bleeding n = 3, χ2 = 5.192, P < 0.05). Conclusion:Compared with the traditional treatment, ultrasound-guided minimally invasive mastectomy is more effective in the treatment of benign breast tumors.

ABSTRACT

We report one case of unilateral breast cancer treated with contralateral prophylactic mastectomy simultaneously and confirmed pathologically as synchronous bilateral primary breast carcinoma.The clinical characterization and diagnosis of bilateral primary breast cancer combined with a review of literature were discussed to serve as a reference for early diagnosis and treatment of the disease.

ABSTRACT

OBJECTIVES: Vacuum-assisted breast biopsy (VABB) is a widely used technique for the diagnosis of breast lesions. It is carried out with local anesthesia, but procedural pain and stress are still problematic. Dexmedetomidine is a α-2 receptor agonist that can sedate without significant respiratory depression. The study aimed to report the effectiveness of sedation with monitored anesthesia care (MAC) using dexmedetomidine in VABB. METHODS: This was a retrospective chart review of patients who received VABB under MAC with dexmedetomidine. Forty-seven patients during the period of February 2015 to July 2016 were included. We collected data on patient characteristics, infusion drug and dose, induction to incision time, anesthetic, operation, and recovery time and other complications and vital signs. RESULTS: The mean operating time was 50.1 ± 24.9 minutes, and the anesthetic time was 71.2 ± 28.3 minutes. The mean time from induction to incision was 17.0 ± 5.2 minutes, and the recovery time was 20.1 ± 10.3 minutes. None of the patients needed an advanced airway management. Further, none of them showed hemodynamic instability. CONCLUSIONS: VABB was successfully performed with MAC using dexmedetomidine, and there was no respiratory depression or hemodynamic instability.

ABSTRACT

Objective To compare the effect between Mammotome minimally invasive surgery and traditional surgery in resection of youth giant breast fibroadenoma (YGBF).Methods We randomly selected 73 patients who had YGBF and already accepted surgical treatment in our hospital from Jul.2009 to Jan.2016.The 75 patients recruited from screening programmes were alloted to either treatment according to their choice (37 cases in MS and 38 cases in traditional surgery).The median follow-up was 18 months (from 6 to 36 months).Comparative analysis was done between the two groups in operation time,incision healing time,incision length,intraoperative bleeding,wound infection,subcutaneous hematoma,residual tumor,breast deformation,scar size,and patient satisfaction degree.Results There was no statisticaly significant difference betwen these two groups in operation time,intraoperative blood loss,incision infection,subcutaneous hematoma,or residual tumor (P>0.05),however,MS group was superior to traditional surgery group in the incision healing time,incision length,breast deformation,scar size and postoperative satisfaction degree,and the difference was statistically significant(P< 0.05).Conclusion The advantage of Mammotome minimally invasive operation auxiliary for YGBF is obvious,such as fast wound healing,small incision,infection rate,small breast without deformation and scar etc.,also higher satisfaction degree to the treatment,which is regarded as the preferred surgical treatment for YGBF patients.

ABSTRACT

Objective To investigate the effect of single operation Mammotome system on breast benign tumor patients.Methods 86 cases of benign breast cancer patients admitted from Jan.2011 to Dec.2015 were divided into the control group (43 cases) and the observation group (43 cases) according to the random number table method.The control group were treated by traditional breast lesion resection and the observation group were treated by single operation of Mammotome system.The mean time of operation,the average blood loss and the size of surgical scar were recorded.The number of patients with deformity of breast and complications was recorded.The satisfaction of the two groups was evaluated by questionnaire.The recurrence rate of the two groups was statistically analyzed.Results The average operation time of the observation group was significantly shorter than that of the control group,and the scar size was significantly smaller than that of the control group (P<0.05).The deformity rate of the observation group was 0%,significantly lower than that of the control group (11.6%).The satisfaction rate was 97.7% in the observation group,significantly higher than that of the control group (83.7%,P<0.05).The postoperative complication rate was 4.7% in the observation group,significantly lower than that of the control group (18.6%,P<0.05).There was no recurrence in the two groups during 6 months of follow-up.Conclusion The application of single breast operation Mammotome treatment for benign breast tumor has the advantages of simple operation,shorter operation time,less intraoperative blood loss and surgical scars.It also can reduce the postoperative complications,keep breast shape,reduce the recurrence rate,so as to improve the patient satisfaction.

ABSTRACT

Objective To compare the effect of ultrasound-guided Mammotome minimally invasive operation and traditional surgery in treating benign breast tumor.Methods Eighty patients with benign breast tumor admitted to People's Hospital of Beijing Daxing District from October 2015 to April 2016 were randomly divided into group A and group B.Patients in group A accepted ultrasound-guided Mammotome minimally invasive operation (n =40),while patients in Group B accepted traditional surgery (n =40).Operation time,incision length,blood loss,patient satisfaction,postoperative pain,and the occurrence rates of complications were compared between the two groups.Results All the tumors in two groups were removed.Patients in group A had shorter operation time (10.4 ± 1.0) min,less blood loss (4.1 ± 0.5) ml,smaller length of incision (0.34 ± 0.04) cm (P < 0.05 or P < 0.01);also had less occurrence of complication (P < 0.05);and patients in group A had better patient's satisfaction (95%) (P < 0.01).Conclusions Ultrasound-guided Mammotome minimally invasive operation is helpful to shorten operation time and incision length,and reduce blood loss,also improve satisfaction.It is valuable for application and popularization in primary hospital.

ABSTRACT

Objective To investigate the value of mammotome minimally invasive puncture drainage in the treatment of breast abscess.Methods 53 patients with the breast abscess were divided into minimally invasive group (27 cases)and open group(26 cases)according to the principle of completely random.The patients in the minimally invasive group were treated with mammotome minimally invasive puncture drainage under the ultrasound guidance.The patients in the open group underwent conventional breast abscess incision and drainage.All the patients were treated with systemic anti -infection and symptomatic treatment.Then,we collated and analyzed the statistical data from the operation time,postoperative pain time,pain degree,number of dressing,healing time,postoperative breast feeding and breast appearance aspects.Results There was no significant difference in operation time between the two groups.In the minimally invasive group,the time of pain,the degree of pain,the time of dressing change,the healing time,the proportion of nursing and the appearance of the breast were (2.6 ±0.8)d,(2.1 ±0.2)min,(7.3 ±0.6)times, (1 8.0 ±0.1 )d,81 .5%,(3.5 ±0.2)points respectively.Those in the open group were (5.2 ±0.2)d,(3.3 ± 0.3)min,(28.0 ±0.2)times,(30.0 ±0.8)d,26.9%,(2.1 ±0.2)points respectively.The differences between the two groups were statistically significant(t =-3.38,-5.1 2,-5.61 ,-5.39,χ2 =32.25,t =-6.1 2,all P <0.05). Conclusion Minimally invasive treatment is a good method in the treatment of breast abscess,and deserved to be promoted clinically.

ABSTRACT

Objective To explore the application of single-person operated mammotome minimally inva-sive surgery in treatment of benign breast lesions. Methods Clinical information of 403 patients were collected and retrospectively analyzed. 262 patients receiving ultrasound guided single-person operated mammotome mini-mally invasive surgery for benign breast masses were selected as the treatment group,141 patients undergoing open surgery for treatment of benign breast masses were selected as the control group.Operative time, introopera-tive blood loss,length of stay,postoperative healing time, as well as the occurrence of complications were com-pared between the two groups. Result Compared with the control group, the operative time was shorter, the in traoperative blood loss was less, the scar size and the postoperative healing time were shorter for the treatment group. The difference had statistical significance (P0.05). Conclusions Single-person operated Mammotome minimally invasive surgery has the advantages of good operative time,good cosmetic effects, small surgical trauma, less blood loss, fast postoper-ative healing and few complications, which is worthy of clinical application.

ABSTRACT

Objective To evaluate clinical efficacy and safety of single puncture and irrigation with Mammotome system in treatment of lactational breast abscess without drainage. Methods 32 patients with lacta-tional breast abscess were treated with ultrasound-guided puncture and irrigation by Mammotome system. Pus was cleared and necrotic material was removed. The residual cavity was irrigated with 3% hydrogen peroxide solution and Kangfuxin solution and no drainage was placed. Breast-feeding on time was encouraged. Results All pa-tients were successfully treated with single puncture and irrigation. The average length of hospital stay was 4 days. Except for 3 cases actively requesting to stop lactating, the other 29 patients continued to breast-feed. There was one case with postoperative hematoma, one with wound granuloma, and no other complications occurred. There was no relapse during the follow-up of 3 to 36 months. All patients were satisfied with the appearance of the breasts. Conclusions Single puncture and irrigation with Mammotome system without drain placement is a safe procedure in treatment of lactational breast abscess with the advantage of minimal invasion, low complication and does not affect breastfeeding.

ABSTRACT

Objective To investigate the curative effect of mammotome minimally invasive operation in the treatment of benign breast disease and analyse its characteristics.Methods Two hundred and five cases of benign breast disease patients admitted to the hospital in time sequence were divided into the observation group (105 cases) and control group(100 cases).Observation group used mammotome minimally invasive rotary cutting operation,and the control group used conventional breast tumor resection.The surgical results of two groups of patients,tumor complete resection or not,whether the patients for surgery satisfaction were observed.Patients of two groups were compared with intraoperative and postoperative relevant indicators,and the postoperative complications.Results In the observation group,105 patients with mammotome spiral cutting knife surgery of breast masses were completely resected,the satisfaction of patients for surgery was 96.2% (101/ 105),significantly higher than that of control group(81.0% (81/100),x2 =4.187,P<0.05).The operative time,blood loss,length of incision values of observation group were significantly lower than those in the control group((17.30±6.70) min vs (57.23 ± 8.96) min,(10.43 ± 5.14) ml vs (109.16 ± 13.45) ml,(10.27 ±0.06) cm vs (1.43±0.12) cm;t=18.741,26.167,11.421;P<0.05),the postoperative recovery time and the scar length were significantly lower than the control group((4.1±2.5) d vs (8.0±3.5) d,(0.15±0.03) cm vs (1.21±0.46) cm;t =5.176,2.647;P<0.05),breast deformation cases,tumor residual proporation were 1.9%(2/105) and 0(0/105),fewer than that in the control group(12.0%(12/100),9.0%(9/100);x2=6.721,11.470;P<0.05).In control group breast deformation subcutaneous hematoma and ecchymosis,wound infection,skin damage,pain,pigment calm incidence were 2.9% (3/105),5.7% (6/105),1.9% (2/105),2.9%(3/105),8.6%(9/105),2.9%(3/105),significantly lower than those in the observation group(9.0%(9/100),12.0(12/100),7.0% (7/100),16.0%(16/100),23.0%(23/100),8.0%(8/100);x2 =2.164,3.071,2.467,6.194,6.177,2.642;P< 0.05).Conclusion Mammotome minimally invasive rotary cut scalpel compare with conventional breast tumor excision undoubtedly has a more significant curative effect and patient satisfaction and have the characteristics of operation safety,efficiency,beautiful,fewer complications,humanized.

ABSTRACT

Objective To investigate the clinical application value of ultrasound-guided catheter drainage and vacuum-assisted Mammotome system in the treatment of lactational breast abscess. Methods The clinical data of 46 patients with lactational breast abscesses were analyzed retrospectively. All patients were divided into control group (28 patients) and treatment group (18 patients). Homemade irrigation and drainage tube was used in control group,and vacuum-assisted Mammotome system was used in treatment group. The curative effect of 2 groups was compared. Results All patients were cured. The mean healing time in treatment group was (9.5 ± 1.3) d,in control group was (12.5 ± 2.2) d, and there was significant difference (P 0.05). All patients recovered well without recurrence. Conclusions Ultrasound-guided catheter drainage should be employed as first line treatment of breast abscess with diameter greater than the 3 cm. If patient′s economic conditions are good and have high requirement on the beauty outcome, vacuum-assisted Mammotome system can be applied as treatment option.

Objective:Ultrasound guided by Mammotome spiral cut (MMT) system and the traditional bard needle aspiration biopsy in the diagnosis of nonpalpable breast lesions.Methods:Chooses January 2014 to 2016 January in our hospital were 113 cases of breast ultrasound,mammography found and nonpalpable breast lesions were retrospectively analyzed,a total of 167 breast lesions,of which 80 cases (120 lesions in ultrasound guided by MMT rotary cutting biopsy (MMT group),33 cases (47 lesions) take bard biopsy (Bard group),with postoperative pathological results as the gold standard,comparison of the two methods diagnostic value.Results:MMT group biopsy success rate of 100% was significantly higher than that of Bard group 87.23%,the difference has statistical significance (P<0.05);MMT method biopsy for differential diagnosis sensitivity of benign and malignant nonpalpable breast lesions is 96.67%,and the specificity was 97.78%,misdiagnosis rate was 3.33%,and the misdiagnosis rate was 2.22%;bard biopsy to identify diagnostic sensitivity of benign and malignant nonpalpable breast lesions was 83.33%,the specificity was 92.22%,misdiagnosis rate was 16.67%,misdiagnosis rate was 7.78%.MMT biopsy complication rate was 10% significantly lower than the Bard group 30.30%,the difference was statistically significant (P < 0.05).Conclusion:Ultrasound guided MMT biopsy needle puncture biopsy with Bard has a higher diagnostic value for breast nonpalpable lesions.

Objective To compare the advantages and disadvantages between the Mammotome ( MMT ) and conventional resection operation in the diagnosis and treatment of breast tumors . Methods A total of 288 patients during the period from February to July of 2014 were divided into MMT group ( 137 cases ) and control group ( 151 cases ) according to patient ’ s willingness .The surgical and post-operative results , postoperative complications and burden of disease were compared between the two groups . Results As compared to the control group, the MMT group had shorter operation time [(28.3 ±9.1) min vs.(37.5 ±14.2) min, t =-6.472, P=0.000], larger amount of tissue cut [(29.2 ±8.1) g vs.(16.5 ±7.9) g, t=13.462, P=0.000], lower visual analogue scales at one day after surgery [(4.2 ±3.6) points vs.(4.9 ±1.5) points, t=-2.189, P=0.029], lower incidence of postoperative breast shape change [10.2% (14/137) vs.55.0% (83/151), χ2 =64.392, P =0.000], and higher patient satisfaction scores [(7.2 ±1.8) points vs.(5.3 ±2.9) points, t=6.601, P=0.000], but more intraoperative bleeding [(22.3 ± 4.3) ml vs.(12.6 ±5.9) ml, t=15.807, P=0.000], higher incidence of postoperative breast hematoma [9.5% (13/137) vs. 2.6%(4/151),χ2 =6.050, P=0.014], and higher total costs [(5.12 ±1.97) ×103 yuan vs.(3.18 ±2.01) ×103 yuan, t=8.258, P=0.000]. Conclusion The MMT operation has advantages of short time , more cut tissue volume, mild postoperative pain, good postoperative breast shape , and high patient satisfaction , but disadvantages of easy development of hematoma and high total costs.

Objective To investigate application value of two vacuum assisted biopsy systems in benign breast lesions and compare the advantages and disadvantages of EnCor and Mammotome system.Methods Vacuum assisted excision was performed on 346 benign breast lesions in 227 patients (EnCor with 103 cases, Mammotome with 124 cases) from November 2012 to May 2013, including 162 cases using EnCor system and 184 cases using Mammotome system.Results Duration of operation in EnCor group was (14.18± 4.21) minutes,in Mammotome group was (18.77 ± 7.06) minutes, and the difference was significant (P < 0.001) .Weight of tissues in Mammotome group was (6.87± 3.43) g, in EnCor group was (8.32± 1.18) g, and the difference was significant (P < 0.001).No significant differences were found in postoperative hematoma (P =0.58), postoperative pain (P =0.35), incidence of residual tumor (P =0.15).Conclusion Compared Mammotome group, EnCor group had significantly shorter duration.But using Mammotome system, the specimens can be observed directly.Mammotome is more suitable for women of childbearing age, it can preserve lactation function.There is no significant difference in complication and residual tumor of two vacuum assisted biopsy system in benign breast lesions.

Concern by 90.87% to 24.90% levels of satisfaction from 84.23% to 99.17%.Conclusion: The patients with lump in breast have psychological problems and deep pressure obviously. To provide the best care and health education instruction for the patients in preoperative may reduce anxiety and worry, improve the understanding of disease, Cooperate with the doctor. Make good relationship with patient. Increase patients’ satisfaction, Reduced complications.

Objective To evaluate the application value of ultrasound-guided Mammotome minimally invasive surgery system in non-palpable breast lesions.Methods A total of 325 lesions of 245 patients with breast lumps by ultrasound examination were under ultrasound-guided Mammotome minimally invasive surgery system.Results Accurate and complete resection of all of the 325 breast lumps was achieved,5 lesions with hematoma,the incidence rate was 1.54% (5/325).The pathologic diagnoses included:fibro-adenoma (172 lesions),intraductal papilloma (8 lesions),fibroadenosis (87 lesions),fibro-cystic adenosis (56 lesions),invasive ductal carcinoma (2 lesions).Conclusion The application of ultrasound-guided Mammotome minimally invasive surgery system in non-palpable beast lesions has advantages of accurate positioning,minimally invasive,beautiful.

Objective To explore the indication,operative methods,operation skill and prevention of complications of mammotome(MMT) in breast diseases.Methods From Aug.2009 to Aug.2013,3676 breast lesions in 1356 patients received MMT micro-invasive operation by decussation process with different incision and needle tracks according to the location of lesions.Experience of the indication,operation technique and prevention of complications were summed up.Results 775 lesions were in left breast and 639 in right breast.652 patients had single lesion and 865 patients had multiple lesions.The pathology results:320 cysts (8.71%,320/3676) were adenosis of breast with mammary duct ectasia or with metaplasia apocrine.36 solid and cystic tumors (0.98%,36/3676)were intraductal papillomas.3310 solid tumors (90.04%)included 13 breast cancers (0.39%,13/3310),2246 fibroadenomas(67.85%,2246/3310) and 1051 adenosis(31.75%,1051/3310).10nodules of calcification were adenosis of breast.13 patients with breast cancer received modified radical mastectomy.The postoperative results:skin bruises in 184 cases(5%),hematoma in 36 cases(0.98%).No postoperative residual was found.Conclusion According to the strict application of MMT minimally invasive surgery and by decussation process based on the appropriate incision and needle track,MMT minimally invasive surgery can achieve safe,minimally invasive,good-looking and completely removal effect.
