ABSTRACT

Objetivo: Evaluar los resultados a largo plazo en el tratamiento quirúrgico de la enfermedad pilonidal sacrocoxígea (EPS), comparando las diferentes técnicas quirúrgicas utilizadas, complicaciones, tiempo de cicatrización y recidiva. Material y Método: Estudio trasversal retrospectivo. Se revisaron las fichas de los pacientes intervenidos por EPS entre enero 2017 hasta agosto 2021. Para el seguimiento se obtuvo la información desde los registros de los controles periódicos en nuestro centro y aquellos que no continuaron con estos controles se contactaron telefónicamente. Resultados: Se recopilaron 309 casos, con una media de seguimiento de 15,6 meses. Las técnicas más utilizadas para la fase crónica de EPS fueron marsupialización y Karydakis, con tiempo de cicatrización completa de 54,5 y 18,2 días, respectivamente. La presencia de complicaciones prolongó el tiempo de cicatrización en 46,5 días en Karydakis, con mínimo impacto en marsupialización. La tasa de recidiva fue menor en Karydakis y en etapa aguda cuando se realiza destechamiento y legrado. La profilaxis antibiótica no afectó el riesgo de complicaciones postoperatorias. Discusión: El desarrollo de complicaciones postoperatorias tiene mayor impacto en la cicatrización de la técnica cerrada que en la abierta, siendo esta última más tolerante en cuanto a la cicatrización frente a complicaciones. Conclusión: La intervención con técnica de Karydakis logra la cicatrización de la herida operatorio en menor tiempo que la marsupialización, sin embargo, al desarrollar complicaciones, este tiempo aumenta 3,5 veces más. La recurrencia fue menor en Karydakis. En absceso se recomienda el desteche y legrado por sobre el drenaje exclusivo


Aim: To evaluate long-term results in the surgical treatment of sacrococcygeal pilonidal disease (SPD) comparing the different surgical techniques used, complications, healing time and recurrence. Material and Method: Retrospective cross-sectional study. The files of the patients operated on for EPS between January 2017 and August 2021 were reviewed. For follow-up, information was obtained from the records of the periodic controls in our center; those who did not continue with these controls were contacted by telephone. Results: 309 cases were collected, with a mean follow-up of 15.6 months. The most used techniques for the chronic phase of EPS were marsupialization and Karydakis, achieving complete wound healing in 54.5 and 18.2 days, respectively. The presence of complications prolonged the healing time by 46.5 days in Karydakis, with minimal impact on marsupialization. The recurrence rate was lower in Karydakis and in the acute stage when unroofing and curettage were performed. Antibiotic prophylaxis did not increase the risk of postoperative complications. Discussion: The development of postoperative complications has a greater impact on the healing of the closed technique than in the open technique, the last one seems to be more tolerant in terms of healing in the presence of complications. Conclusion: The intervention with the Karydakis technique achieves healing of the surgical wound in less time than marsupialization, however, when complications develop, this time increases 3.5 times more. Recurrence was lower in Karydakis. In abscess weaning and curettage is recommended over the exclusive drainage.

ABSTRACT

Introduction: The extravasation of saliva from the sublingual gland on the floor of the mouth results in a ranula. The most typical presenta?on is swelling under the tongue. The mylohyoid muscle may allow it to herniate, Causing a plunging ranula. Ranula can be treated in many ways, including ranula excision alone, excision of the sublingual gland with or without ranula, aspira?on of cys?c fluid, sclerotherapy, marsupializa?on, incision and drainage. Material and methods: This case study includes six pa?ents with ranula who underwent surgical treatment by marsupializa?on. The indica?ons, age and sex distribu?on, Marsupializa?on methods, pathology reports, recurrence rates, and complica?ons were seen. Results: Out of 6 pa?ents in our case series, no one pa?ent has shown to be any recurrence or complica?ons of marsupializa?on. Conclusions: Management of Ranula by Marsupializa?on is a good op?on, but if it recurs a?er marsupializa?on, Excision of the lesion and the affected gland is the best course of act?on.

ABSTRACT

Introduction: Dentigerous cysts are the second most common developmental odontogenic cysts of the jaw, arising from im- pacted, embedded or unerupted permanent teeth. They apparently develop by accumulation of fluid between the reduced enamel epithelium and the tooth crown of an unerupted tooth. There is usually no pain or discomfort associated with the cyst unless there is an acute inflammatory exacerbation. Case Presentation: The purpose of this case report is to present a case of a dentigerous cyst with an unusual presentation in a 6-year-old child came with the swelling over right back region of the jaw which was asymptomatic appearing bony hard on palpation. Radiography showing unilocular radiolucency with an unerupted mandibular tooth crown is seen. Histology re- vealed H & E-stained section showing non keratinized epithelium 2-3 cell layer thick underlying connective tissue wall is loose fibrocellular with mild to moderate inflammatory cells infiltrate comprising of plasma cells, lymphocytes & numerous blood vessels and the definitive diagnosis of Dentigerous cyst was made. Management is done using enucleation of the cystic space with curettage & extraction of associated teeth. Conclusion: Management of odontogenic lesions, such as dentigerous cysts in children, presents an unparalleled challenge to general and pediatric dentists alike. A thorough understanding of the nature of the lesion backed by good clinical history and state-of-art radiography can go a long way in helping the clinician to arrive at the correct therapeutic choice of approach, ameliorate the condition in the best long-term interests of the young patient.

ABSTRACT

Objetivo: Caracterizar las preferencias y frecuencia de las técnicas quirúrgicas para la resolución de la enfermedad pilonidal (EPS) en los cirujanos/as colorrectales de Latinoamérica. Material y Método: Estudio transversal descriptivo analítico mediante encuesta electrónica validada por pares expertos. Distribuida entre los cirujanos/as colorrectales de Latinoamérica. Fueron excluidos los cirujanos/as no subespecialistas en cirugía colorrectal. Resultados: Se obtuvieron 372 respuestas de 15 países latinoamericanos, con media de 18,9 ± 12,5 años como subespecialistas. El 66,7% utiliza técnicas abiertas para EPS crónica, las técnicas más usadas son marsupialización (31,5%), destechamiento (27,7%) y resección con técnica de Karydakis (17,7%), colgajo de Limberg (6,1%), Bascom (5,4%), plastias en Z o V-Y (4%), McFee (3,8%) y Epsit (3,8%). La intervención más utilizada para la resolución de la EPS aguda es el drenaje bajo anestesia formal con curetaje y/o marsupialización (51,1%). El 45,3% de los cirujanos/as cambia de técnica durante su carrera. Discusión: Gracias a la amplia distribución y representatividad de los encuestados se logró plasmar la preferencia de manera realista acerca de las inclinaciones en el manejo de la EPS por parte de los subespecialistas del continente, aportando información de la que no se tiene precedente. Conclusión: Las técnicas abiertas son las preferidas para la resolución de la EPS crónica, las técnicas más utilizadas son marsupialización, destechamiento y Karydakis. Es frecuente el cambio de técnica quirúrgica preferente dentro de los subespecialistas, existiendo una relación entre escoger técnicas abiertas en la medida que los cirujanos/as tienen más años de experiencia.


Aim: To characterize the preferences and frequency of surgical techniques for the resolution of pilonidal disease (PSD) in colorectal surgeons of Latin America. Materials and Method: Cross-sectional descriptive and analytical study using an electronic survey validated by expert peers. Distributed among colorectal surgeons in Latin America. Surgeons who were not subspecialists in colorectal surgery were excluded. Results: 372 responses were obtained from subspecialist surgeons in colorectal surgery from 15 Latin American countries, with a mean of 18.9 ± 12.5 years as subspecialists. 66.7% use open techniques for chronic PSD, the most used techniques: marsupialization (31.5%), unroofing (27.7%) and resection with the Karydakis technique (17.7%), Limberg flap (6, 1%), Bascom (5.4%), Z or VY plasties (4%), McFee (3.8%) and Epsit (3.8%). The most used intervention for the resolution of acute PSD is drainage under formal anesthesia with curettage and/or marsupialization (51.1%). 45.3% of surgeons change techniques during their career. Discussion: Due to the wide distribution and representativeness of the respondents, it was possible to capture the preference in a realistic way about the management of the PSD by the subspecialists of the continent, contributing with information with which there is no precedent. Conclusion: Open techniques are preferred for the resolution of chronic PSD, marsupialization, unroofing and Karydakis were used the most. The change of surgical technique within subspecialists is frequent, there is a relationship between preferring open techniques to the extent that surgeons have more years of experience.

Abstract Introduction: The effect of the quantity of olfactory neuroepithelium in the middle turbinate on the postoperative olfactory function for middle turbinate concha bullosa patients has not yet been evaluated. Objective: The primary aim of this study was to investigate the olfactory structures in the middle turbinate by immunohistochemical analysis of the olfactory marker protein and to correlate the immunostaining results with the olfaction test results for patients with middle turbinate concha bullosa. Methods: Surgical materials of 18 middle turbinate concha bullosa patients who had undergone lateral marsupialization surgery were immunostained with olfactory marker protein antibodies. Smell diskettes olfaction test was applied to all of the study group patients both preoperatively and three months postoperatively. A visual analog scale was used to quantify the sense of nasal obstruction. Results: It was observed that the postoperative smell scores and the nasal obstruction visual analog scale values were significantly improved as compared to the preoperative values (p<0.05). In addition, there was a significant correlation between the smell score gain and the visual analog scale gain values (r = 0.682). Results also indicated no significant correlation between the olfactory marker protein staining scores and the smell scores (p > 0.05). Conclusion: This first paper demonstrated that the quantity of the olfactory mucosa in the middle turbinate was not a determining factor for the postoperative smell function degree for middle turbinate concha bullosa patients. The underlying cause of the olfactory deficit for middle turbinate concha bullosa patients seems to be obstruction related rather than the middle turbinate's olfactory mucosa containing status.

ABSTRACT

Introducción: el quiste dentígero, también conocido como quiste folicular, es la segunda forma más habitual de los quistes de la región maxilar después del quiste radicular. Tiene una incidencia del 20% con respecto a todos los quistes odontogénicos y se encuentra más en el género masculino entre los 20-40 años. El manejo conservador en lesiones quísticas de gran tamaño es indispensable para evitar defectos óseos o daño a estructuras vecinas. Sin embargo, en lesiones de menor tamaño, la enucleación continúa siendo el tratamiento de elección. Objetivo: presentar un caso clínico de un quiste dentígero y hacer una revisión de la literatura actualizada. Caso clínico: paciente masculino de 23 años que presentó aumento de volumen en zona geniana derecha y, además, en su ortopantomografía se observó una lesión en el sector de los incisivos superiores. Se realizó enucleación de la lesión, cuyo diagnóstico histológico previo fue quiste dentígero y posteriormente se obtiene una correcta regeneración ósea tras un año de seguimiento Conclusión: el manejo del quiste dentígero es variado: puede ir desde la enucleación quirúrgica hasta la descompresión y marsupialización del mismo. No obstante, todos los tratamientos actuales se basan en contrarrestar los factores de expansión quística que permiten a esta lesión alcanzar grandes tamaños intraóseos y poder causar desde rizolisis de dientes adyacentes hasta una asimetría facial como sucedió en este caso clínico.


Introduction: The dentigerous cyst, also known as a follicular cyst, is the second most common form of cysts of the maxillary region after the radicular cyst. It has an incidence of 20% with respect to all odontogenic cysts, it is found more in the male gender between 20-40 years of age. Conservative management of large cystic lesions is essential to avoid bone defects or damage to neighboring structures. However, in smaller lesions, enucleation continues to be the treatment of choice. Objective: To present a clinical case of a dentigerous cyst and to review the updated literature. Clinical case: A 23-year-old male patient who presented increased volume in the right genital area and his orthopantomography showed a lesion in the upper incisor sector. Enucleation of the lesion was performed, whose previous histological diagnosis was dentigerous cyst and later a correct bone regeneration was obtained after one year of follow-up. Conclusion: The management of the dentigerous cyst is varied, it can range from surgical enucleation to decompression and marsupialization. However, all current treatments are based on counteracting cystic expansion factors that allow this lesion to reach large intraosseous sizes and can cause from rhizolysis of adjacent teeth to facial asymmetry, as happened in this clinical case.

Background: Fistula in ano is a very common perianal condition seen in outpatient departments. Fistulotomy and fistulectomy are two conventional options of surgery. The present study is designed to observe wound healing time and mean postoperative pain score in the comparison of outcome of the fistulectomy to fistulotomy with marsupialization. Methods: This prospective randomized trial was conducted in the surgical department of the Civil Hospital Karachi for a period of 12 months, in which 60 patients with low anal fistula were divided into 2 groups. Thirty patients in group A were treated with fistulectomy, and 30 in group B were treated with fistulotomy with marsupialization. The postoperative pain severity was assessed after 24 hrs through a visual analogue scale and on weekly and fortnightly follow-ups for 6 weeks. Wound healing was assessed by clinical examination on weekly and fortnightly follow-ups for 6 weeks to estimate the mean healing time. Results: The mean pain score was significantly lower in group B in comparison to group A (3.6±1.99 versus 2.40±1.52; p=0.01). The mean wound healing time was shorter in group B in comparison to group A (4.23±0.77 versus 5.80±0.41 weeks; p=0.0005). Conclusion: Fistulotomy with marsupialization is a simple, easy, and more effective method than fistulectomy for the treatment of simple perianal fistula. (AU)

Resumen Los quistes laríngeos, y en particular los quistes de vallécula, son una entidad infrecuente en la práctica clínica habitual. Sin embargo, su localización y aparición en neonatos y lactantes, pueden suponer una causa reconocida de estridor y obstrucción de vía aérea superior que, si no es diagnosticada y tratada de forma precoz, puede tener consecuencias fatales. Presentamos el caso de un lactante que presenta estridor inspiratorio. Se realiza una nasofibrolaringoscopía observándose una formación quística en la vallécula, y una ecografía cervical que muestra dicha formación quística sugerente de un quiste del conducto tirogloso como primera posibilidad etiológica. Ante estos hallazgos, se solicitan pruebas tiroideas y una gammagrafía que son normales, por lo que se decide intervenir al paciente bajo laringoscopia en suspensión, realizando una marsupialización del quiste, sin complicaciones posteriores. El análisis histopatológico posterior confirmó el diagnóstico de un quiste de vallécula. Se debe considerar esta patología en los casos de estridor inspiratorio en lactantes ya que, aunque es infrecuente, su diagnóstico precoz y tratamiento adecuado son determinantes.


Abstract Laryngeal cysts, and especially vallecular cysts, are a rare entity in everyday clinical practice. Nevertheless, their location and the fact that they appear in newborns and infants, must be recognized as a cause of stridor and upper airway obstruction, since their misdiagnosis and late treatment can have fatal consequences. We present the case of an infant with inspiratory stridor. We performed a nasofibrolaryngoscopy where a vallecular cystic lesion was observed, and a neck ultrasound showed a cyst, described as a thyroglossal duct cyst, as the main etiology. Thyroid function tests and a gammagraphy, were both normal, hence the patient underwent a suspension microlaryngoscopy and marsupialization of the cyst, without further complications. The histopathology confirmed the diagnosis of a vallecular cyst. This entity must be considered in infants with inspiratory stridor. Although it is a rare lesion, its early diagnosis and adequate management is crucial.

RESUMEN: Introducción: El queratoquiste o tumor odontogénico queratoquístico es una de las neoplasias odontogénicas "benignas" más frecuentes. Existen múltiples opciones de tratamiento, pero no existe consenso sobre ellas. Este resumen busca evaluar la efectividad de la enucleación secundaria a descompresión o marsupialización. Métodos: Realizamos una búsqueda en Epistemonikos, la mayor base de datos de revisiones sistemáticas en salud, la cual es mantenida mediante el cribado de múltiples fuentes de información, incluyendo MEDLINE, EMBASE, Cochrane, entre otras. Extrajimos los datos desde las revisiones identificadas, analizamos los datos de los estudios primarios, realizamos un metaanálisis y preparamos una tabla de resumen de los resultados utilizando el método GRADE. Resultados y conclusiones: Identificamos cuatro revisiones sistemáticas que en conjunto incluyeron cinco estudios primarios, los cuales corresponden a estudios observacionales. Concluimos que no es posible establecer con claridad si la enucleación secundaria a descompresión o marsupialización disminuye la recidiva de queratoquiste. Otros desenlaces como dolor postoperatorio, infección y fractura patológica no fueron reportados.


ABSTRACT: Introduction: Keratocystic odontogenic tumor is one of the most common odontogenic neoplasms. Many treatment modalities have been recommended for the treatment of keratocystic , but there is no consensus regarding the optimal treatment. This summary seeks to evaluate the effectiveness of enucleation secondary to decompression or marsupialization. Methods: We searched in Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic reviews, reanalyzed data of primary studies, conducted a meta-analysis and generated a summary of findings table using the GRADE approach. Results and conclusions: We identified 4 systematic reviews including 5 studies overall, all corresponding to observational studies. There is uncertainty whether secondary enucleation to decompression/marsupialization reduces recurrence rate as the certainty of the evidence has been assessed as very low. No studies were found that looked at postoperative pain, infection and pathologic fracture.

ABSTRACT

O cisto odontogênico glandular (COG) é o cisto de desenvolvimento mais agressivo e raro dentre todos os odontogênicos, portando características epiteliais glandulares ou salivares. O presente artigo visa retratar uma abordagem mais conservadora para esta importante lesão, no sentido de minimizar a morbidade decorrente da abordagem mais radical. Paciente com 57 anos, leucoderma, assintomático e edêntulo, compareceu ao ambulatório de estomatologia com notável expansão no fundo de saco vestibular e rebordo inferior direito, com tempo indeterminado de evolução. Os exames imaginológicos revelaram uma lesão radiolúcida unilocular extensa e bem delimitada na região anterior da mandíbula. Nas condutas propedêuticas, foi realizada uma punção aspiratória positiva para líquido cístico seguido de biópsia incisional. Diante destes achados, a marsupialização foi realizada no sentido de se conseguir a redução das dimensões lesionais, seguido da enucleação com estectomia periférica após 9 meses. Nos primeiros 2 anos de proservação o paciente mantém- -se sem recidivas e em constante acompanhamento. Conclui-se que o dentista pode preferir o método conservador pela técnica de marsupialização seguida de enucleação na abordagem de extenso cisto odontogênico glandular, desde que haja colaboração do paciente nos cuidados pós-operatórios.


The glandular odontogenic cyst (GOC) is the most aggressive and rare cyst of development among all odontogenics, with glandular or salivary epithelial characteristics. This article aims to portray a more conservative approach to this important lesion, in order to minimize the morbidity resulting from the more radical approach. Patient 57 years-old, leucoderma, asymptomatic and edentulous, attended the stomatology outpatient clinic with a remarkable expansion in the vestibular bag fundus and lower right border, with indeterminate evolution time. Imaging examinations revealed an extensive and well delimited unilocular radiolucent lesion in the anterior region of the mandible. In the propaedeutic procedures, a positive aspiration puncture was performed for cystic fluid followed by incisional biopsy. In view of these findings, marsupialization was performed in order to reduce lesion dimensions, followed by enucleation with peripheral ostectomy after 9 months. In the first 2 years of proservation, the patient remains without relapses and in constant follow-up. It is concluded that the dentist may prefer the conservative method by the marsupialization technique followed by enucleation in the approach of extensive glandular odontogenic cyst, provided there is collaboration of the patient in the postoperative care.

Resumen Introducción: La enfermedad pilonidal sacrocoxígea (EPSC) es una patología crónica de resorte quirúrgico. Para su tratamiento se han descrito múltiples técnicas quirúrgicas, existiendo 2 grandes grupos: las técnicas abiertas y las cerradas. El objetivo del presente trabajo es comparar y analizar los resultados quirúrgicos de 2 técnicas, una abierta (marsupialización) y otra cerrada (Karydakis). Materiales y Método: Estudio de cohorte retrospectivo de pacientes operados electivamente con diagnóstico de quiste pilonidal por un único cirujano, entre julio de 2013 y julio de 2017 utilizando estas dos técnicas. Resultados: Se incluyeron 71 pacientes. 30 pacientes con marsupialización y 41 con Karydakis. Todos hospitalizados. Todos de alta al día siguiente de la cirugía. Ningún paciente requirió rehospitalización ni cirugías adicionales. En el análisis estadístico se identifican beneficios de la técnica de Karydakis en cuanto a complicaciones, dolor postoperatorio, dolor para sentarse, incapacidad laboral y tiempo de cicatrización. Conclusiones: En este artículo la cirugía con técnica de Karydakis tiene ventajas en relación a la marsupialización, considerándola como primera opción para la EPSC simple.


Introduction: Sacrocoxygeal pilonidal disease (EPSC) is a chronic pathology of surgical solution. For its treatment, multiple surgical techniques have been described, there being 2 large groups: open and closed techniques. The aim of the present study is to compare and analyze the surgical results of 2 techniques, one open (Marsupialization) and another closed (Karydakis). Materials and Method: Retrospective cohort study of electively operated patients with diagnosis of pilonidal cyst by a single surgeon, between July 2013 and July 2017 using these two techniques. Results: 71 patients were included. 30 patients with marsupialization and 41 with Karydakis. All hospitalized. All discharge the day after surgery. No patient required rehospitalization or additional surgeries. In the statistical analysis, benefits of the Karydakis technique are identified in terms of complications, postoperative pain, sitting pain, work incapacity and healing time. Conclusions: In this study, surgery with Karydakis technique has advantages in relation to Marsupialization, considering it as the first option for simple EPSC.

Introdução: O ameloblastoma é uma neoplasia benigna, localmente invasiva, originada do epitélio odontogênico que ainda não possui sua etiologia esclarecida. Este tumor pode ser identificado radiograficamente como unicístico ou multicístico e geralmente, está relacionado com um dente incluso. O tratamento desta patologia do complexo maxilofacial que acomete tanto a maxila quanto a mandíbula, é baseado no diagnóstico clínico, imagens (raios x (RX), tomografia computadorizada (TC) e histopatológico. O objetivo deste trabalho é descrever e avaliar através de tomografia computadorizada (TC), o tamanho do tumor durante o período em que foi submetido à descompressão, para que as dimensões do mesmo possam ser mensuradas e comparadas em intervalos, dessa forma, comprovar a efetividade dos métodos de descompressão e marsupialização quando bem indicados.Relato de caso:No presente relato de caso, o paciente apresenta ameloblastoma unicístico de variante histológica plexiforme, conforme o exame histopatológico e está sendo tratado por meio de descompressão com a finalidade de se obter a diminuição do volume da lesão cística e consequente neoformação óssea, uma vez que a descompressão possibilita este processo. Introduction: Ameloblastoma is a benign neoplasm, locally invasive, originating from the odontogenic epithelium that still does not have a clear etiology. This tumor can be radiographically identified as unicystic or multicystic and is usually related to an included tooth. The treatment of this pathology of the maxillofacial complex that affects both the maxilla and the mandible is based on clinical diagnosis, images (x-rays (X-rays), computed tomography (CT) and histopathology. The objective of this work is to describe and evaluate using computed tomography (CT), the size of the tumor during the period in which it was subjected to decompression, so that its dimensions can be measured and compared at intervals, thus proving the effectiveness of the methods of decompression and marsupialization when well indicated. Case report: In the present case report, the patient has unicystic ameloblastoma of a plexiform histological variant, according to the histopathological examination and is being treated by means of decompression in order to obtain a decrease in the volume of the cystic lesion and consequent bone neoformation, once that decompression makes this process possible. ABSTRACT

Los quistes nasolabiales son lesiones quísticas poco frecuentes, que se presentan como ocupación de la fosa canina, el ala nasal o el vestíbulo nasal. Usualmente son asintomáticas, pero pueden infectarse. El diagnóstico se realiza con el examen físico y exámenes imagenológicos, como la tomografía computarizada y/o resonancia magnética. El tratamiento consiste en la extirpación quirúrgica completa por abordaje sublabial, o por marsupialización endoscópica transnasal. Se describen dos casos, uno de ellos es una paciente de sexo femenino que presenta deformidad nasal producto del crecimiento progresivo de un quiste nasolabial unilateral, y otro de un paciente de sexo masculino que presenta una celulitis facial severa, con una tomografía computarizada que muestra quistes nasolabiales bilaterales. Los quistes nasolabiales deben ser considerados como parte del diagnóstico diferencial en otorrinolaringología en cuadros de deformidad nasal y aumento de volumen facial.


Nasolabial cysts are a rare developmental cyst, presenting as a fullness of canine fossa, nasal ala or vestibule of the nose. They are usually asymptomatic but may become infected. The diagnostic approach includes physical examination and imaging studies such as computed tomography and/or magnetic resonance imaging. Treatment is complete surgical excision by sublabial approach, or transnasal endoscopic marsupialization. Here we describe two cases, one female presenting as nasal deformity due to progressive growth of unilateral nasolabial cyst, and a healthy young male presenting severe facial cellulitis, with a computed tomography showing bilateral nasolabial cysts. Nasolabial cyst should be incorporated in the differential diagnosis of nose deformities and facial swelling in otorhinolaryngology.

@#Marsupialization is effective in the treatment of cystic lesions of the jaw. It is a simple operation that can result in minimal trauma, the reduction of postoperative recurrence, and maximum preservation of the surrounding tissue structure and function. However, there is a certain failure rate in clinical treatment due to the improper grasp of indications and nonstandard operation. The highest failure rate reported in the literature is 32.6%. To further standardize the clinical application of marsupialization and improve the success rate of treatment, we put forward an expert consensus of marsupialization in the treatment of jaw cystic lesions by reviewing the domestic and foreign literature and summarizing the experience in marsupialization from some famous domestic experts. In this consensus, we propose three elements of marsupialization: the establishment of the opening, the maintenance of cyst plugs and regular washing. The scope of application of marsupialization includes jaw cysts and cystic ameloblastomas. It is necessary to standardize the position of the opening, the size of the opening and the manufacture of the cyst plug, and a panoramic film or cone beam computed tomography(CBCT) should be used to observe the changes in the cystic cavity before and after operation. A second-stage operation should be performed when the lesion is significantly reduced by more than 50% or at least 5 mm from important structures; furthermore, the teeth of focus should be treated according to the relationship between the lesion and tooth and the type of tooth.

@#Objective: To report successful marsupialization of a large Tornwaldt cyst using combined transnasal and transoral endoscopic surgery in a 7-year-old girl who presented with nasal obstruction. Methods: Design: Case Report Setting: Tertiary Government Training Hospital Patient: One Result: A 7-year-old girl presented with an 11-month history of recurrent yellowish nasal discharge gradually associated with nasal obstruction. Examination revealed a large, well encapsulated, broad-based cystic mass in the nasopharynx immediately adjacent to the posterior choanae, continuing posterior to the soft palate (pushing the uvula anteriorly) and extending inferiorly to the epiglottic area. Computerized Tomography (CT) demonstrated a well-circumscribed, midline hypodense mass with fluid attenuation obstructing the nasopharyngeal area extending inferiorly to the oropharyngeal area. Endoscopic marsupialization via transnasal and transoral approach was successful, and a respiratory epithelium-lined cyst consistent with a Tornwaldt cyst was confirmed by histopathologic examination Conclusion: Combined transnasal and transoral endoscopic marsupialization is possible a for a large symptomatic Tornwaldt cyst in a pediatric patient with relatively smaller and complex nasal cavities.

Abstract Introduction Paranasal sinus mucocele is a benign, expansive lesion associated with paranasal sinus obstruction. It affectsmostly adults, and ismost common in the frontal and ethmoidal sinuses. Objective To evaluate outcomes in patients undergoing surgical treatment for paranasal sinus mucocele. Methods Retrospective review of medical records of patients treated for paranasal sinus mucocele at the ENT department of a tertiary care hospital between 2005 and 2016. Results Forty-six patients underwent surgical treatment of paranasal sinusmucocele. Themean age was 50.1 years, and 56.5% were male. The most prevalent symptom was pain, and the frontal sinus was most commonly affected. The vast majority of patients (89.1%) underwent endoscopic sinusmarsupialization; 10.9% required combined open and endoscopic access. Seven recurrences occurred. Conclusion Sinus mucocele is an expansive disease that primarily affects the frontal sinus of adult patients. In most cases, endoscopic surgery is an effective treatment modality. (AU)

En la actualidad los procedimientos quirúrgicos han evolucionado con la intención de ser lo más conservadores posible, dando pie a una regeneración fi siológica más rápida y completa. La extirpación de quistes de gran tamaño de los maxilares mediante descompresión y marsupialización ha demostrado obtener excelentes resultados, siendo éste un procedimiento que permite evitar un amplio abordaje quirúrgico, aunque generalmente requiere un segundo tiempo para reconstruir el defecto. La quistectomía conservadora mediante múltiples trepanaciones de acceso, permite la eliminación del cuerpo patológico por completo sin la necesidad de un segundo procedimiento quirúrgico. La excelente cicatrización ósea fi siológica y adecuado reposicionamiento y soporte de tejidos blandos sin necesidad de utilizar membranas, son logrados gracias a los puentes óseos que se mantienen entre las trepanaciones, los cuales brindan soporte además de mantener células osteoprogenitoras. Se presenta caso clínico de enucleación conservadora de quiste periapical de gran tamaño y extracción de canino retenido en paciente masculino de 12 años de edad mediante trepanaciones múltiples (AU)


Nowadays the surgical procedures have evolved aiming to be as conservative as possible, resulting in a faster physiological regeneration. The removal of large maxillary cysts using decompression and marsupialization has proved to have excellent results, is this a procedure that avoids the use of large surgical access, although a second procedure is generally needed to completely remove the lesion. Conservative cystectomy using multiple access trepanations allows the complete elimination of the cyst without the need for a second surgical intervention. Excellent physiological bone healing and adequate soft tissue reposition without the need of grafting material and membrane is achieved thanks to the osseous bridges between the multiple trepanations, which gives support for soft tissue and provides osteoprogenitor cells. A clinical case of conservative enucleation of a large radicular cyst is presented as well as the extraction of a retained canine in a 12-year-old male patient using multiple trepanations (AU)

@#Decompression is an effective and widely used treatment for jawbone cystic lesions that can, to a great extent, preserve the function and appearance of the jawbone. However, some problems exist with its clinical application, such as the inappropriate determination of indication and the lack of standardized operational guidelines, resulting in treatment ineffectiveness or even failure. This paper aimed to summarize the clinical value of decompression for jawbone cystic lesions in terms of mechanism, scientific evidence, advancement, indications and effective evaluation by reviewing relevant literature and our clinical experience.

ABSTRACT

Ameloblastoma is a benign neoplasm originating from odontogenic epithelium. It is the most common neoplasm in the jaws and is characterized by aggressive behavior and local invasion. Unicystic ameloblastoma (UA) has a unilocular feature in radiologic examination and a cystic feature histologically. Decompression and marsupialization are conservative method of treatment of large UA. The purpose of decompression and marsupialization are size reduction of the mass, which makes it easy to handle at total enucleation with protection of nerve damage and facial deformity. Here we report successful conservative treatment of extensive UA using decompression and marsupialization with a review of literatures.

@#Radicular cysts are the most common inflammatory cysts and arise from the epithelial residues in the periodontal ligament as a result of periapical periodontitis following death and necrosis of the pulp. They are usually encountered in association with permanent teeth. However, occurrence in relation to deciduous teeth seems to be very rare. In this case, we presents treatment of radicular cyst associated with mandibular primary second molar following pulp therapy and discusses the relationship between pulp therapy and the growth of the cyst. The treatment consisted of decompression of the cyst sac, extraction of the involved primary tooth and 6 months follow up of the patient. Our purpose of the study was to evaluate the bone formation after the decompression and describe the benefit of this conservative surgical treatment. Case Report: A 7 years old girl, radiographic examination showed a large, circular, well-defined unilocular radiolucent area on the left mandible and impacted #34 and #35 which had incomplete roots and open apices. Signs of root resorption were evident in the adjacent teeth. Under local anesthesia, mandibular primary left first (#74) and second molars (#75) were extracted and decompression of the cyst was performed. A tissue sample was also taken for the biopsy. Conclusion: The follow-up was performed periodically under radiographic examinations and after 6 months the impacted teeth were at the position and new bone formation was evident at the former cyst site. This surgical technique preferred in young patients to promote the bone formation and encourage the eruption of permanent teeth.
