ABSTRACT

The X-ray tube is one of the most important components in any X-ray system. In the beginning, physicists and physicians used gas ion tubes. The so-called Coolidge tube applied a high vacuum and is still used today. Medical examinations have required continuously improved designs of X-ray tubes (smaller focal spots at a higher output). The principle of the Goetze line focus is still applied in any diagnostic X-ray tube. Different anode materials and the rotating anode contributed to an increased output and reduced exposure time. Bearings needed special attention. Spiral groove bearings are the most advanced design today. The heat storage capacity of the anode and the tube housing assembly influences examination time and patient throughput. Cardiac imaging required less motion blurring in cine film images and increasing radiation exposure in interventional procedures called for measures to reduce dose. Protection against radiation and electric shock has always been a concern of design engineers. Focal spot sizes dedicated to specific applications and heat management within the total tube housing assembly will be future issues. Even with the event of ultrasound and MR technology, X-ray procedures will still be applied for diagnostic and interventional purposes.

ABSTRACT

Digital radiography (DR) is an advanced form of x-ray inspection which produces a digital radiographic image instantly on a computer. This technique uses x-ray sensitive plates to capture data during object examination, which is immediately transferred to a computer without the use of an intermediate cassette. The incident x-ray radiation is converted into an equivalent electric charge and then to a digital image through a detector sensor. Compared to other imaging devices, flat panel detectors, also known as digital detector arrays (DDAs) provide high quality digital images. They can have better signal-to-noise ratio and improved dynamic range, which, in turn, provides high sensitivity for radiographic applications. Flat panel detectors work on two different approaches, namely, indirect conversion and direct conversion. Indirect conversion flat panel detectors have a scintillator layer which converts x-ray photons to photons of visible light and utilise a photo diode matrix of amorphous silicon to subsequently convert the light photons into an electrical charge. This charge is proportional to the number and energy of x-ray photons interacting with the detector pixel and therefore the amount and density of material that has absorbed the x-rays. Direct conversion flat panel detectors use a photo conductor like amorphous selenium (a-Se) or Cadmium telluride (Cd-Te) on a multi-micro electrode plate, providing the greatest sharpness and resolution. The information on both types of detectors is read by thin film transistors. In the direct conversion process, when x-ray photons impact over the photo conductor, like amorphous Selenium, they are directly converted to electronic signals which are amplified and digitised. As there is no scintillator, lateral spread of light photons is absent here, ensuring a sharper image. This differentiates it from indirect construction.

ABSTRACT

Radioisotopes are unstable nuclei of elements (eg Molybdenum 99), which are transformed into stable nuclei while emitting radiation (particles, photons). This phenomenon is characterized as radioactivity. Radioactivity can be natural or artificial. Artificial radioactivity is that observed in isotopes produced artificially in a laboratory. The study of radioactive isotopes in combination with the development of systems for the detection of emitted radiation, was the trigger for the investigation of possible applications of radioisotopes in medicine. This research resulted in the creation of a new science, Nuclear Medicine, whose main purpose is to apply the properties of radioisotopes in the diagnosis and treatment of human diseases.

ABSTRACT

This study introduced the current testing content and standards of ECG medical electronic instruments, combined with actual clinical needs, and discussed the comprehensive verification and evaluation protocol for ECG medical electronic instruments. The protocol mainly includes hardware performance testing, automatic diagnostic function testing and clinical application evaluation. The protocol emphasizes the clinical practicality and importance of the comprehensive verification and evaluation program, and provides a reference for the institutions involved in the program.

ABSTRACT

Objective:To investigate the cleaning status of silicone oil attached ophthalmic surgical instruments in China, so as to provide reference and suggestions for the standard cleaning process of silicone oil attached ophthalmic surgical instruments.Methods:61 hospitals in 22 provinces (regions, municipality) in China were selected as the objects of investigation. The on-site treatment, pretreatment and routine cleaning of silicone oil attached ophthalmic surgical instruments were investigated by questionnaire from November 2019 to January 2020.Results:18.033% (11/61) hospitals had mixed silicone oil attached ophthalmic surgical instruments and other instruments intraoperatively, 11.475% (7/61) hospitals didn′t wipe silicone oil and other visible contaminants postoperatively. 18.033% (11/61) hospitals didn′t carry out pretreatment and 4.000% (2/50) hospitals used saline as pretreatment solution; 54.098% (33/61) hospitals were involved in ultrasonic cleaning. 32.787% (20/61) hospitals had water temperature 31-37 ℃ and 4.918% (3/61) hospitals had water temperature>37 ℃during the cleaning process; During pretreatment and routine cleaning, 38.000% (19/50) hospitals and 54.902% (28/51) hospitals selected enzymatic detergent respectively, the contact time between instruments and detergent in specialized hospital was more standardized than that in general hospital ( P<0.001); In flushing, rinsing and final rinsing, there were 29.412% (15/51) hospitals, 11.765% (6/51) hospitals and 3.922% (2/51) hospitals used water that didn′t meet the requirements. 17.647% (9/51) hospitals, 13.725% (7/51) hospitals and 13.725% (7/51) hospitals didn′t record the time of flushing, rinsing and final rinsing. Conclusions:There are still some problems in the cleaning of silicone oil attached ophthalmic surgical instruments in specialized and general hospitals, such as no on-site treatment and pretreatment, improper control of cleaning parameters, and difficulty in ensuring water quality and water time. It is necessary to formulate the cleaning process specification of silicone oil attached ophthalmic surgical instruments as soon as possible to ensure the cleaning quality and avoid the occurrence of postoperative eye complications.

ABSTRACT

Objective To design a new type of rehabilitation device to execute electrode fixation and precision positioning during transcranial electrical stimulation.Methods The device was composed of a positioning cap,a soft rubber keel scaffold, an inlaid nut,a unionbolt,an electrical stimulation shim and etc.The electrode was put into the shim at the target site,and fixation and positioning were realized by regulating the depth of the bolt into the slot to change the attachment between the electrode fixing cushion and cranial skin,and then electrical stimulation therapy could be carried out reliably and accurately. Results The device gained advantages over the traditional means in fixation time in one time, dropping-off rate, patient satisfaction and easy operation.Conclusion This device behaves well in simple structure,easy manufacture,low cost,fixation and large applicability to kinds of population and electrical stimulators,and enhances the reliability of transcranial electrical stimulation therapy to some extent.

ABSTRACT

In the research of artificial joint biomechanics, it is a common method in the world to evaluate the biomechanical properties of the implanted fixtures through experiments . The domestic research started late, and the corresponding testing methods were lacking. There is still no unified standard. In this paper, a complete hardware test system was designed and built around the existing mechanical testing machine, including:binocular vision catcher, torque bearing clamp, film pressure sensor and so on. The system can accurately measure the relative motion angle value, external torque value, and inter pressure value of each joint. It has some reference significance for the forming and standardization of the artificial joints' evaluation system.

ABSTRACT

Objective To discuss the reason of the problem, which are caused by pretreating reused medical instruments after usage in each hospital clinical departments;ensure the washing quality for the reused medical instruments and prolonging the working life of them. Methods The execution rate, washing quality, damage of which pretreating reused medical instruments in clinical departments were counted The Clinic of Infection, Nursing Department and Central Sterile Supply Department observed and adopted questionnaire for information, and then formulated targeted guidance and improvement according to the information. Results The execution rate of pretreating reused medical instrument after usage reached from 64.6% to 99.4%, the qualified rate of instrument washing had improved from 93.0% to 99.7%,the damage rate of instruments had decreased from 1.84% to 0.74%,and there were significant differences(χ2=9 413.8,38 160.6,2 820.2,all P<0.05). Conclusions Pretreating reused medical instrument after usage correctly and timely,can not only ensure the washing quality for the medical instruments, but also prolong the working life of them. It ensures the application value of the medical instruments.

ABSTRACT

The study of hemodynamics, which refers to dynamics inside the blood circulation, mainly includes the flow rate, flow resistance, pressure, shear stress, disturbed flow, as well as their associations in between. Therefore, with its important significance in the clinical treatments of vessel curvature, arterial stenosis or occlusion, pathological artery branches and aneurism, study about hemodynamics is essential to human health. Currently, extensive researches on hemodynamics have been conducted with respect to artery bypass, coronary arterial stenosis, abdominal aortic aneurysm, atherosclerosis, cerebral aneurysm and swirling flow. With the development of such research on hemodynamics, surgical planning and interventional therapy have improved rapidly. The influence mechanism of hemodynamic parameters, including pressure, flow resistance, flow rate, wall shear stress, blood viscosity, flow separation, turbulent flow, vortex on the post-operation complications could be deeply explored with the help of more and more clinical apparatus and have gained some achievements.

ABSTRACT

Objective To design a training box for laparoscopic skills. Methods Based on ergonomics, the size and the contour of the training box were designed with the abdominal surface data of 30 adults and extension degree of the abdominal wall under pneumoperitoneum. Port location was designed considering the optimal manipulation angel, azimuth angle and elevation angle. A flexible camera system and LED light belts were used to simulate laparoscope and light source. Training tasks were designed according to the McGill Laparoscopic Simulation study and basic laparoscopic skills. Finally, the 3D design and assembling of the training box were achieved by Autodesk Inventor, and rapid prototyping and computerized numerical control technology were used to manufacture the prototype. Results A multi-purpose training box with seven training tasks for laparoscopic surgery was successfully designed, with a size of 300 mmX 350 mmX150 mm. It was mainly composed of box, baseboard, camera system, drawer system, light belts, and seven training task modules. Conclusion The multi-purpose training box in this study is ergonomically designed and is easy to use. It can be used for training the skills of traditional laparoscopic procedures and single port surgery.

ABSTRACT

Objective To learn the policy outcomes of the management of category-B large medical instruments deployed in Beijing.Methods Call into play the data from the surveys made in 2005,2007 and 2010 on the five types of category-B large medical instruments deployed in the city.Summarize and analyze the outcomes of such instruments in terms of their total configuration volume,and the growth,distribution and use.Results From 2005 to 2010,CT scored the fastest growth of these five types of instruments,with 48 units deployed; SPECT was the lowest,with 8 units deployed.Conclusion The volume growth,instruments distribution,and use efficiency of such instruments in Beijing are rationally evolving,which proves that the deployment and management of category-B large medical instruments is compliant with conditions of the city.

ABSTRACT

OBJECTIVE To study the effect and development of centralizing management of the overall recycled medical instruments toward the hospital nursing management by the Center for Sterilization,Supply and Distribution.METHODS The centralizing method and decentralizing method toward the recycled handling of the medical instruments were adopted and compared their effect toward nursing management quality,vocational safety,hospital infection management,and of the two methods cost and benefit management,respectively.RESULTS The centralizing management group had distinct advantage than the decentralizing one on the qualified rate of cleaning,packaging,the qualified rate of the sampled incubation of the aseptic products and work efficiency.CONCLUSIONS Through the centralizing management of the Center for Sterilization,Supply and Distribution toward the overall recycled hospital instruments,it can improve the hospital′s nursing staff resource utilization and nursing quality,help to perfect the nursing staff′s vocational safety;decline the hospital infection risk;increase the output of the hospital nursing work and reduce its operational cost;and can make for improving and protecting the environment.

Objective To connect with provincial or even national report center for adverse events of medical instruments and set up a computer-aided system for supervision.Methods By designing software,setting up database and online connecting with report center,the computer-aided system for adverse events of medical instruments was utilized to automatic report,early-warning monitoring and report.Key technologies were analyzed.Results Based on computer network,adverse events of medical instruments were automatically reported,collected and managed.Information and consultation could be provided rapidly without error.Conclusion The system not only facilitates management department in medical instruments managing,producing and application,but set a theoretic basis for development and academic teaching of new medical instruments.
