ABSTRACT

Background@#One of the early problems that children born with cleft lip and palate encounter is difficulty in feeding. This affects the child’s nutritional needs and the timing of the surgical intervention. Information on the appropriate feeding methods for children with cleft lip and palate will enable mothers to feed their babies properly and facilitate the implementation of appropriate interventions.@*Objectives@#The study described the feeding problems experienced by children with cleft lip and palate (CLP), and cleft palate (CP) ages 0-24 months, the feeding methods most preferred by Filipino mothers, the methods they found most useful, and the mothers’ reactions to the feeding issues their children face. Methods. The research is a pilot study which used a quantitative, cross-sectional, descriptive mixed method design. Thirty-two (32) mothers of children with cleft lip and palate, and cleft palate answered an 11-item online survey and participated in focused interviews from January to June 2022. Inferential statistics was used specifically frequency distribution to describe the data, and Fishers’ Exact Test and Pearson’s Chi-Square Test were used to analyze the data quantitatively to determine the significant association between the variables identified.@*Results@#Results showed that the feeding problems encountered by the children included nasal regurgitation, sucking, aspiration of liquids, latching on nipples, and swallowing. Mothers preferred to use regular feeding bottles (24.3%), specialized feeding bottles for children with cleft (21.6%), breast feeding and dropper (17.6%), syringe (9.5%), cup (6.8%), and spoon feeding (2.7%) methods. They also mentioned that they found the following feeding methods to be the most useful, regular feeding bottles (32.7%), specialized feeding bottles for cleft (23.1%), breast feeding (11.5%), spoon and dropper feeding (7.7%), and syringe feeding (1.9%).@*Conclusion@#The feeding problems experienced by Filipino children with CLP and CP mirror those that have been reported in other studies. The study revealed that mothers still prefer to use the traditional regular feeding bottles in feeding their babies and found this to be the most useful. Maternal reactions of the participants to the cleft condition and its feeding issues are similar to reported studies in other countries. The internet has been the primary source of information on cleft and feeding of the participants in the study.

ABSTRACT

Objetivo: verificar a associação entre o uso da bolsa canguru, desde a maternidade até o domicílio, e aleitamento materno exclusivo, entre o 1o e 2o mês de idade, em bebês a termo saudáveis; identificar o nível de autoeficácia para o aleitamento materno, das mães que utilizaram a bolsa canguru. Métodos: estudo transversal com mães que receberam uma bolsa canguru após o parto, na maternidade, e foram orientadas sobre manterem seus bebês nesta postura, pelo menos uma hora por dia. Entre um e dois meses após o nascimento, foram investigadas a frequência do uso da bolsa canguru, autoeficácia materna da amamentação e tipo de alimentação dos bebês. Para análise foi empregado o teste ꭓ2 ou Exato de Fisher e o coeficiente V de Cramer (p < 0,05). Resultados: participaram 98 mães. Todas utilizaram a bolsa canguru no alojamento conjunto, 39,8% utilizaram três ou mais vezes por semana, no domicílio entre o primeiro e segundo mês após o parto; 70,4% dos bebês permaneceram em Aleitamento Materno Exclusivo (AME), o nível da autoeficácia materna para amamentação foi alto. AME esteve associado ao uso da bolsa canguru no domicílio (p = 0,014). Conclusão: bebês a termo saudáveis podem se beneficiar do uso da bolsa canguru desde a maternidade, estendido até o domicílio.


Objectives: to verify the association between kangaroo slings used from maternity hospital to home, and exclusive breastfeeding among healthy full-term babies between the 1st and 2nd months of age; to identify breastfeeding self-efficacy level of those mothers using a kangaroo sling. Method: this cross-sectional study addressed mothers who received a kangaroo sling after giving birth, at maternity hospital, and were instructed to keep the babies in this posture at least one hour daily. The frequency with which the mothers used the kangaroo sling, breastfeeding self-efficacy, and the type of feeding were investigated between one and two months after delivery. ꭓ2 or Fisher's exact test and Cramer's V coefficient (p < 0.05) were adopted in the analysis. Results: ninety-eight mothers participated in the study. All of them used the kangaroo sling in rooming-in; 39.8% used it three or more times a week at home between the 1st and 2nd months after giving birth; 70.4% of the babies remained on Exclusive Breastfeeding (EBF), the level of breastfeeding self-efficacy was high, and EBF was associated with the use of a kangaroo sling at home (p = 0.014). Conclusion: healthy full-term babies can benefit from using a kangaroo sling, starting at the maternity hospital and extending its use to home.


Objetivo: verificar la asociación entre el uso de la bolsa canguro, desde la maternidad hasta el domicilio, y amamantamiento materno exclusivo, entre el 1o e 2o mes de edad, en bebés a término saludables; identificar el nivel de autoeficacia para el amamantamiento materno, de las madres que utilizaron la bolsa canguro. Métodos: estudio transversal con madres que recibieron una bolsa canguro después del parto, en la maternidad, y fueron orientadas sobre mantener sus bebés en esta postura, por lo menos una hora por día. Entre uno y dos meses después del nacimiento, fueron investigadas la frecuencia del uso de la bolsa canguro, autoeficacia materna del amamantamiento y tipo de alimentación de los bebés. Para el análisis fue empleado el test ꭓ2 o Exacto de Fisher y el coeficiente V de Cramer (p < 0,05). Resultados: participaron 98 madres. Todas utilizaron la bolsa canguro en alojamiento conjunto, 39,8% utilizaron tres o más veces por semana, en el domicilio entre el primero y segundo mes después el parto; 70,4%, de los bebés, permaneció en Lactancia Materno Exclusivo (LME); el nivel de la autoeficacia materna para amamantamiento fue alto. El LME estuvo asociado al uso de la bolsa canguro en el domicilio (p = 0,014). Conclusión: bebés a término saludables pueden beneficiarse del uso de la bolsa canguro desde la maternidad, extendido hasta el domicilio.

ABSTRACT

Objetivo: analisar a percepção da equipe de enfermagem sobre o método canguru em uma maternidade de alto risco. Método: estudo exploratório, descritivo, de abordagem qualitativa, realizado por meio de entrevista semiestruturada com enfermeiros de uma maternidade de alto risco de Recife (PE), desenvolvida entre janeiro e fevereiro de 2020. As entrevistas foram transcritas e submetida a análise através da técnica de conteúdo de Bardin. Resultados: ocorreu a formulação de duas categorias, assistência de enfermagem no método canguru e benefícios e desafios encontrados no Método Canguru. As participantes relataram que os cuidados de enfermagem se baseiam principalmente em orientações as mães, a escassez de profissionais e baixa adesão foram evidenciados como principais desafios. Conclusão: constatou-se que a atuação de enfermagem no método canguru é um complexo processo, necessitando de uma equipe de enfermagem motivada e especializada


Objective: analyze the nursing team's perception of the kangaroo method in a high-risk maternity hospital. Method: exploratory, descriptive study, with a qualitative approach, carried out through semi-structured interviews with nurses from a high-risk maternity hospital in Recife (PE), carried out between January and February 2020. The interviews were transcribed and subjected to analysis using the content technique by Bardin. Results: two categories were formulated, nursing care in the kangaroo method and benefits and challenges found in the Kangaroo Method. Participants reported that nursing care is mainly based on guidance from mothers, the shortage of professionals and low adherence were highlighted as main challenges. Conclusion: it was found that nursing practice in the kangaroo method is a complex process, requiring a motivated and specialized nursing team


Objetivos: analizar la percepción del equipo de enfermería sobre el método canguro en una maternidad de alto riesgo. Método: Estudio exploratorio, descriptivo, con enfoque cualitativo, realizado a través de entrevistas semiestructuradas a enfermeros de una maternidad de alto riesgo de Recife (PE), realizado entre enero y febrero de 2020. Las entrevistas fueron transcritas y sometidas a análisis mediante el Técnica de contenidos de Bardin. Resultados: Se formularon dos categorías, los cuidados de enfermería en el método canguro y los beneficios y desafíos encontrados en el Método Canguro. Los participantes informaron que la atención de enfermería se basa principalmente en la orientación de las madres, la escasez de profesionales y la baja adherencia fueron destacados como principales desafíos. Conclusión: se encontró que la práctica de enfermería en el método canguro es un proceso complejo, que requiere de un equipo de enfermería motivado y especializado

Resumo Objetivo Analisar as evidências disponíveis sobre a transição alimentar de sonda orogástrica para aleitamento materno diretamente na mama com prematuros internados em unidades hospitalares. Métodos Revisão sistemática da literatura com busca nas bases de dados PubMed/MEDLINE, Web of Science, EMBASE, Scopus, Cochrane CENTRAL, CINAHL, com os critérios de inclusão: estudos experimentais, sem restrição temporal e nos idiomas português, espanhol e inglês. A avaliação metodológica foi realizada por meio das ferramentas Grading of Recommendations Assessment, Development and Evaluation (GRADE) e Revised Cochrane risk-of-bias tool for randomized trials (RoB 2.0) e consistiu em duas etapas: qualidade metodológica e o risco de viés dos estudos. Resultados Foram identificados 10 artigos, todos ensaios clínicos randomizados. As técnicas utilizadas na transição da dieta dos prematuros encontradas foram: sonda-dedo e seringa, copo e sonda-dedo, copo e mamadeira, colher e sucção não-nutritiva, sucção não-nutritiva, sucção não-nutritiva e estimulação oral, comportamento dos prematuros, cheiro do leite materno. Conclusão As técnicas evidenciadas permitiram a transição da dieta, em um período mais curto, reduzindo o tempo de internação, aumentando o ganho de peso e se mostraram seguras, desde que o prematuro tenha maturidade para ser realizada. Contudo, a mamadeira foi desaconselhada, pela ocorrência de episódios de dessaturação, aumento da frequência cardíaca e confusão de bico.

La evaluación metodológica se realizó por medio de las herramientas Grading of Recommendations Assessment, Development and Evaluation (GRADE) y Revised Cochrane risk-of-bias tool for randomized trials (RoB 2.0) y consistió en dos etapas: calidad metodológica y riesgo de sesgo de los estudios. Resultados Se identificaron 10 artículos, todos ensayos clínicos aleatorizados. Las técnicas utilizadas para la transición de la dieta de prematuros fueron: dedo-jeringa y jeringa, vaso y dedo-jeringa, vaso y mamadera, cuchara y succión no nutritiva, succión no nutritiva, succión no nutritiva y estimulación oral, comportamiento de los prematuros, olor de la leche materna. Conclusión Las técnicas observadas permitieron realizar la transición de la dieta en un período más corto, con reducción del tiempo de internación y mejor aumento de peso y demostraron ser seguras, siempre que el prematuro tenga madurez para realizarlas. No obstante, se desaconseja la mamadera por la presencia de episodios de desaturación, aumento de la frecuencia cardíaca y confusión tetina-pezón. Abstract Objective To analyze the available evidence on the transition from orogastric tube feeding to breastfeeding directly from the breast with premature infants admitted to hospital units. Methods Systematic literature review with search in the following databases: PubMed/MEDLINE, Web of Science, EMBASE, Scopus, Cochrane CENTRAL, CINAHL, with the inclusion criteria: experimental studies, without temporal restrictions and in Portuguese, Spanish and English. The methodological assessment was carried out using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) and Revised Cochrane risk-of-bias tool for randomized trials (RoB 2.0) tools and consisted of two stages: methodological quality and the risk of bias of the studies. Results 10 papers were identified, all randomized clinical trials. The techniques used in transitioning the premature babies' diet were: finger tube and syringe, cup and finger tube, cup and bottle, spoon and non-nutritive sucking, non-nutritive sucking, non-nutritive sucking and oral stimulation, behavior of premature babies, and smell of breast milk. Conclusion The demonstrated techniques allowed the transition of the diet in a shorter period, reducing the length of hospital stay, increasing weight gain 1and proved to be safe, as long as the premature baby is mature enough to undergo the procedure. However, bottle feeding was not recommended due to the occurrence of episodes of desaturation, increased heart rate and nipple confusion. Una versión preliminar de este texto fue publicada en 2001 en ocasión del estreno del film. Dos décadas después y ante la explosión de la Inteligencia Artificial Generativa se presenta una versión ampliada que mantiene sin embargo la tesis original. Se trata de la importancia y vigencia de los mitos para comprender la distancia entre la lógica computacional y lo propio de la condición humana. En esta línea se analiza el film de Spielberg en interlocución con el célebre relato de Carlo Collodi y con los aportes del psicoanálisis para pensar el presente y el futuro de la Inteligencia Artificial


A preliminary version of this text was published in 2001 on the occasion of the film's premiere. Two decades later and given the explosion of Generative Artificial Intelligence, an expanded version is presented that nevertheless maintains the original thesis. It is about the importance and validity of myths to understand the distance between computational logic and the human condition. Along these lines, Spielberg's film is analyzed in dialogue with the famous story by Carlo Collodi and with the contributions of psychoanalysis to think about the present and future of Artificial Intelligence

En todos los ámbitos laborales, sin excepción, es imprescindible la vigencia ineludible del derecho irrestricto de toda madre a ejercer, practicar, su lactancia con su bebé. Y las razones médicas debieran formar parte de campañas incesantes para que este derecho de las mamás esté absolutamente naturalizado en todos los niveles y en cada rincón del país. La lactancia materna protege al bebé de enfermedades infecciosas y le da la nutrición ideal para una vida más sana. Es fundamental desde el primer día de vida extrauterina hasta los 6 meses con exclusividad y luego continúa con suplementos vitamínicos y nutrientes apropiados. En todo lugar de trabajo deben existir lactarios como una estrategia de Salud Pública y en Medicina del Trabajo. (AU)


In all areas of work, without exception, the unavoidable validity of the unrestricted right of every mother to exercise, practice, breastfeeding with her baby is essential. And the medical reasons should be part of incessant campaigns so that this right of mothers is absolutely naturalized at all levels and in every corner of the country. Breastfeeding protects the baby from infectious diseases and gives it the ideal nutrition for a healthier life. It is essential from the first day of extrauterine life until 6 months exclusively and then continues with appropriate vitamin and nutrient supplements. ABSTRACT

A somatização é uma manifestação corporal carente de simbolização. Relaciona-se com aspectos psicológicos tendendo a surgir no bebê, que se vê como uma extensão da mãe. O objetivo foi analisar aspectos psicológicos da relação materno-filial e suas implicações nas manifestações psicossomáticas. Trata-se de uma pesquisa documental, considerando os prontuários de crianças entre fevereiro/2015 e dezembro/2019, de uma clínica-escola do Grande ABC-SP. A busca realizada a partir de um software específico para arquivamento dos prontuários, utilizou como filtros os termos: infantil, queixas somatoformes, distúrbios alimentares e distúrbios do sono. Os dados foram categorizados em: a) identificação; b) evolução clínica; e c) aspectos da personalidade e o relacionamento materno. A análise qualitativa, de base psicanalítica, apontou que a dificuldade das mães em lidar com seus próprios conflitos influenciou no surgimento dos sintomas nos filhos. Observou-se relação entre os conflitos maternos e o tipo de queixa apresentada. Os resultados sustentam a importância da relação entre mães e filhos na queixa psicossomática. Fatores orgânicos no desenvolvimento infantil precisam ser considerados para além da natureza biológica, mas da dinâmica psíquica, ampliando a compreensão diagnóstica.


Somatization is a bodily manifestation lacking symbolization. It relates to psychological aspects tending to appear in the baby, who sees himself as an extension of the mother. The objective was to analyze the psychological aspects of the mother-child relationship and its implications for psychosomatic manifestations. This is documentary research, considering the clinical reports of children between February/2015 and December/2019, from a clinical school in Grande ABC-SP. The search performed using clinical reports record file software, used terms as filters: infant, somatoform complaints, eating disorders and sleep disorders. Data were categorized into a) identification; b) clinical evolution; and c) aspects of personality and maternal relationship. The qualitative analysis, based on psychoanalysis, showed that mothers' difficulty dealing with their own conflicts influenced the appearance of symptoms in their children. There was a relationship between maternal conflicts and the type of complaint presented. The results support the importance of the relationship between mothers and children in psychosomatic complaints. Organic factors in child development need to be considered beyond the biological nature, but the psychic dynamics, expanding the diagnostic understanding.


La somatización es una manifestación en el cuerpo del no simbolizado. Se relaciona con aspectos psicológicos que tienden a aparecer en el bebé, quien se ven a sí mismos como una extensión de la madre. El objetivo fue analizar aspectos psicológicos de la relación madre-hijo y sus implicaciones en las manifestaciones psicosomáticas. Es una investigación documental, que toma en cuenta las historias clínicas de niños en los períodos de febrero/2015 y diciembre/2019, de una clínica escuela del Grande ABC-SP. La búsqueda fue hecha con auxilio de un software específico de archivo de las historias clínicas, se utilizó como filtros los términos: infantil, quejas somatomorfas, desórdenes alimentarios y desórdenes del sueño. Los datos se clasificaron en: a) identificación; b) evolución clínica; y c) aspectos de personalidad y relación materna. El análisis cualitativo de base psicoanalítica señalo que la dificultad de las madres para lidiar con sus propios conflictos influyó en la aparición de los síntomas en sus hijos. Hubo relación entre los conflictos maternos y el tipo de queja presentada. Los resultados respaldan la importancia de la relación entre madres y hijos en quejas psicosomáticas. Factores orgánicos del desarrollo infantil necesitan ser considerados más allá de la naturaleza biológica, más de la dinámica psíquica, para ampliar la comprensión del diagnóstico.

Considerando as transformações na maternidade ao longo da história e a atual crise identitária na relação da mulher com a maternidade, ou seja, o conflito entre os ideais de maternidade relacionados à figura da mulher/mãe e as demandas contemporâneas de realização individual no trabalho, realizou-se este estudo transversal, exploratório-descritivo de abordagem qualitativa, cujo objetivo consistia em descrever e analisar, a partir da psicanálise de casal e família, a experiência de mães que abandonaram da carreira profissional para a dedicação exclusiva à maternidade. Foram entrevistadas oito mulheres de classe média, entre 30 e 50 anos, que eram mães e abandonaram suas carreiras profissionais para se dedicarem à maternidade. As entrevistas que compuseram os resultados foram transcritas e as análises de conteúdo temática apontaram as múltiplas exigências, tanto sociais, quanto relacionadas à herança psíquica, que trazem implicações para a construção identitária dessas mulheres. Concluímos que um processo de elaboração dessa herança exige uma rede vincular de apoio e pressupõe a desidealização da maternidade, entendendo-a como plural, tanto no sentido de ser múltipla e diversa, quanto no sentido de ser uma construção vincular.


Noticing the history transformations in motherhood, and the current identity crisis in the woman's relationship between femininity and maternity, that is, the conflict between the ideas of motherhood related to the figure of the woman/mother and the contemporary demands of individual realization at work, this exploratory-descriptive, cross-sectional, study of a qualitative approach was carried out, whose objective was to describe and analyze, from the perspective of psychoanalysis of the linking configurations, the experience of mothers who abandoned the professional career for the exclusive dedication to motherhood. Eight middle-class women, aged between 30 and 50, who were mothers and abandoned their professional careers to dedicate themselves to motherhood-retirar, were interviewed. The interviews that composed the results were transcribed, and thematic content analyses pointed out the multiple demands, both social and related to psychic heritage, which have implications for the identity construction of these women. We conclude that an elaboration of this heritage requires a link network of support and presupposes the non-idealization of motherhood, understanding it as plural, both in the sense of being multiple and diverse, and in the sense of being a bonding construction.


Considerando los cambios en la maternidad a lo largo de la historia y la actual crisis de identidad en la relación de la mujer con la maternidad, es decir, el conflicto entre los ideales de maternidad relacionados con la figura de la mujer/madre y las demandas contemporáneas de realización individual en el trabajo, se realizó este estudio transversal, exploratorio-descriptivo de abordaje cualitativo, cuyo objetivo consistía en describir y analizar, a partir del psicoanálisis de las configuraciones vincular, la experiencia de madres que abandonaron de la carrera profesional para la dedicación exclusiva a la maternidad. Fueron entrevistadas ocho mujeres de clase media, entre 30 y 50 años. que eran madres y abandonaron sus carreras profesionales para dedicarse a la maternidad-retirar. Las entrevistas que compusieron los resultados fueron transcritas y los análisis de contenido temático señalaron las múltiples exigencias, tanto sociales, como relacionadas a la herencia psíquica, que traen implicaciones para la construcción identitaria de esas mujeres. Concluimos que un proceso de elaboración de esa herencia exige una red vincular de apoyo y presupone la desidealización de la maternidad, entendiéndola como plural, tanto en el sentido de ser múltiple y diversa, como en el sentido de ser una construcción vincular.

ABSTRACT

Objetivo: analisar as percepções de mães encarceradas sobre a vivência da maternidade em privação de liberdade. Método: pesquisa exploratória, transversal, qualitativa, desenvolvida com 19 mulheres de uma penitenciária feminina da região Nordeste do Brasil. Utilizou-se como referencial teórico a Teoria do Apego de John Bowlby. A obtenção dos dados ocorreu por meio de questionário sociodemográfico e entrevista semiestruturada; posteriormente, os dados foram submetidos a análise de conteúdo de Bardin. Resultados: as entrevistadas são, na maioria, mulheres entre 23 a 28 anos, solteiras, com média de 2 filhos e com histórico de uso de álcool e/ou outras drogas, presas por tráfico de drogas. Duas categorias foram estabelecidas: O Significado da Maternidade e Vivência da Maternidade em Privação de Liberdade. Conclusão:observou-se o sofrimento do binômio com a separação parental, ocasionando prejuízos para o crescimento e desenvolvimento das crianças. As mães encarceradas estão expostas ao ambiente insalubre e desumanizado, o que acarreta sofrimento psíquico.


Objective: to analyze the condition of motherhood in deprivation of liberty as perceived by mothers in prison. Method: an exploratory, transversal, qualitative research was conducted using John Bowlby's Attachment Theory as a theoretical framework with 19 mothers from a women's penitentiary in northeastern Brazil. Data were collected by means of a sociodemographic questionnaire and semi-structured interviews, and later examined by Bardin's content analysis. Results: most of the participants are single, between 23 and 28 years old, with an average of two children and a history of alcohol consumption and/or other drugs, arrested for drug trafficking. Content analysis formed two categories: The Meaning of Motherhood and Experiencing Motherhood in Deprivation of Liberty. Conclusion: the binomial mother-infant suffered with parental separation, resulting in child growth and development issues. Incarcerated mothers are exposedto an unhealthy and dehumanized environment, leading to psychological distress


Objetivo: analizar las percepciones de madres privadas de libertad sobre la experiencia de la maternidad en privación de libertad. Método: investigación exploratoria, transversal, cualitativa, desarrollada con 19 mujeres de un centro penitenciario de mujeres del Nordeste de Brazil. Se utilizó como marco teórico la teoría del apego de John Bowlby. La recolección de datos se realizó desde un cuestionario sociodemográfico y una entrevista semiestructurada; posteriormente,se aplicó el análisis de contenido de Bardin. Resultados: los datos mostraron mujeres de entre 23 y 28 años de edad, solteras, con promedio de dos hijos y con antecedentes de consumo de alcohol y/u otras drogas, detenidas por tráfico de estupefacientes. Se establecieron dos categorías: El Sentido de la Maternidad y la Experiencia de la Maternidad Privada de Libertad. Conclusión: se observó el sufrimiento del binomio con la separación parental, provocando perjuicios en el crecimiento y desarrollo de los niños. Las madres encarceladas están expuestas a un entorno insalubre y deshumanizado, lo que genera angustia psicológica.

ABSTRACT

OBJETIVO: Determinar el consumo de alimentos en mujeres embarazadas, mujeres que dan de lactar y niños de 0 a 5 años, atendidos en Centros de Salud de Primer Nivel en la ciudad de La Paz en el mes de septiembre de 2018. MATERIAL Y MÉTODOS: Estudió transversal descriptivo correlacional en 126 mujeres embarazadas, 120 mujeres que dan de lactar, 105 niños de 0 a 5 meses y 117 niños de 6 a 60 meses, que asistieron a consulta en Centros de Salud. Se aplicó el método de Recordatorio de 24 Horas (R24H) para determinar el consumo de alimentos, método antropométrico para la evaluación nutricional. RESULTADOS: La ingesta promedio en mujeres embarazadas 1999 kcal y mujeres que dan de lactar 1943 Kcal, es menor a la recomendada; inadecuada en grasas, calcio y zinc; suficiente en proteínas, hidratos de carbono, vitaminas A y C; el hierro es deficiente en embarazadas. Los niveles de adecuación de la dieta en niños de 6 a 23 meses en calcio 88,3%, hierro 75%. De 24 a 60 meses en: calorías 86,3%, grasas 46,9% y zinc 50,6%. CONCLUSIONES: El consumo promedio observado en mujeres y niños es inferior a las recomendaciones nutricionales. El estado nutricional no corresponde al exceso o déficit en la ingesta. La ingesta deficiente de micronutrientes, es debida al consumo insuficiente de verduras y frutas.

ABSTRACT

Sirenomelia also called mermaid syndrome is a congenital, rare, lethal, multisystemic human malformation of unknown etiology. The characteristic feature of sirenomelia is the fusion of the lower limbs, resulting in the appearance of a mermaid’s tail, and thus the name “mermaid syndrome.” This condition is also characterized by various severe urogenital abnormalities and the presence of a singular umbilical cord blood vessel, and it is more common in infants of diabetic mothers and in monozygotic twins. The incidence is around 1 in 60,000–70,000 pregnancies. The majority of affected fetuses are stillborn, whereas the rest of the live-born die in the early neonatal period due to complications of the gastrointestinal and urogenital systems. We are reporting a case of sirenomelia in a neonate born to an unregistered multipara mother. The baby had perinatal asphyxia, sirenomelia, dextrocardia, low set ears, lymphatic malformation, bilateral renal agenesis, absent external genitalia and anus, single umbilical artery, and congenital corneal clouding. There was no antenatal ultrasonography done and the baby died at 6 h of life.

ABSTRACT

Abstract Objective: To investigate the association between emotional and behavioral difficulties in schoolchildren and maternal educational practices. Methods: Participants and Setting: 631 mother-child pairs were evaluated in a poor urban district in Recife, northeastern Brazil. Cross-sectional study carried out between 2013 and 2014. It integrates a prospective cohort study designed to investigate the consequences of intimate partner violence that occurred during pregnancy, postpartum, and seven years after birth for the physical and mental health of women and their children. Maternal educational practices were assessed using the Parent-Child Conflict Tactics Scale (CTSPC - mother-child version) and the child's behavioral and emotional disorders through the Strengths and Difficulties Questionnaire (SDQ) mother-version. The association was estimated through crude and adjusted prevalence ratios, using Poisson regression considering the 95% confidence interval. Results: The prevalence of violent maternal educational practices was 91.8% (572/631) being 89.7% (566/631) of psychological aggression and 75.6% of physical aggression, subdivided into corporal punishment (73.5%), physical maltreatment (35.8%) and severe physical maltreatment (1.7%). Corporal punishment (Adjusted PR 1.5; CI 95%: 1.1-2.1; p = 0.010) and severe physical maltreatment (Adjusted PR 1.9; CI 95%: 1.3-2.8; p = 0.002) were associated with emotional and behavioral difficulties in schoolchildren. Conclusion: The high prevalence of violent maternal educational practices, especially corporal punishment and severe physical maltreatment is associated with emotional and behavioral disorders in children. Therefore, it is necessary to interventions that promote parental support and effective use of non-violent discipline in conducting the educational process to establish healthier family relationships and to prevent/mitigate the impact of emotional and behavioral problems in children. © 2022 Sociedade Brasileira de Pediatria. RESUMO Objetivo: compreender a experiência do processo de desmame para as mães primíparas de um município do interior do Ceará, Brasil. Método: estudo descritivo e qualitativo, realizado com oito mulheres, mediante entrevistas semiestruturadas. Coleta de dados de agosto de 2022 a março de 2023, na Atenção Primária à Saúde, adotando-se a análise de conteúdo de Bardin. Resultados: emergiram três categorias temáticas: motivos para a mãe decidir sobre o desmame; estratégias empregadas pela mãe para realizar o desmame; e consequências do desmame para a mãe e a criança. As percepções maternas sobre a motivação do desmame relacionaram aspectos comportamentais da criança e necessidades maternas. Variadas estratégias foram classificadas em desmame gradual ou abrupto, que geraram mudanças no comportamento das crianças e sentimentos contraditórios nas mães. Considerações finais: o desmame é vivenciado de forma desagradável, gerando sofrimento. Desvela-se a desassistência por profissionais e a necessidade de ampliação de estudos.


ABSTRACT Objective: To understand the experience of the weaning process for primiparous mothers in a municipality in the interior of Ceará, Brazil. Method: Using semi-structured interviews, a descriptive, qualitative study was conducted with eight women. Data collection from August 2022 to March 2023 in Primary Health Care, using Bardin's content analysis. Results: three thematic categories emerged: reasons for the mother's decision to wean, strategies employed by the mother to wean, and the consequences of weaning for the mother and child. Maternal perceptions of the motivation for weaning were related to behavioral aspects of the child and maternal needs. Various strategies were classified as gradual or abrupt weaning, which generated changes in the children's behavior and contradictory feelings in the mothers. Final considerations: weaning is experienced unpleasantly, generating suffering. It reveals the lack of professional assistance and the need to expand studies.


RESUMEN Objetivo: conocer la experiencia del proceso de destete de las madres primíparas en un municipio del interior de Ceará, Brasil. Método: estudio descriptivo y cualitativo realizado con ocho mujeres, mediante entrevistas semiestructuradas. Recogida de datos de agosto de 2022 a marzo de 2023, en Atención Primaria, utilizando el análisis de contenido de Bardin. Resultados: surgieron tres categorías temáticas: motivos de la decisión materna de destetar; estrategias empleadas por la madre para llevar a cabo el destete; y consecuencias del destete para la madre y el niño. Las percepciones maternas de la motivación para el destete estaban relacionadas con aspectos conductuales del niño y con las necesidades maternas. Se clasificaron diversas estrategias como destete gradual o brusco, que provocaron cambios en el comportamiento de los niños y sentimientos contradictorios en las madres. Consideraciones finales: el destete se vive de forma desagradable, generando sufrimiento. Esto revela la falta de ayuda de los profesionales y la necesidad de ampliar los estudios.

ABSTRACT

Objective:To investigate the birth weight (BW) of infants born to pregnant women living with human immunodeficiency virus (HIV) and its associated factors, and to provide more evidence for the prevention of mother-to-child transmission (PMTCT) in China.Methods:This study was a retrospective cohort study. Between January 2004 and December 2021, pregnant women living with HIV and their infants in Hubei Province were recruited and followed up, and clinical data were collected through hospital medical records and HIV/acquired immunodeficiency syndrome comprehensive response information management system. The multivariable linear regression was performed on the collected data to investigate associated influencing factors of BW.Results:In total, 531 pregnant women living with HIV (581 pregnancies) and 581 infants were enrolled. Of the 581 infants, 36 were HIV-positive, with a PMTCT rate of 6.2%. The mean BW of the infants was (3 075.0±470.2) gram. Protease inhibitor (PI) based-anti-retroviral therapy (ART) ( β=-0.1, 95% confidence interval ( CI)-188.2 to -37.1, P=0.004), ART in the first trimester( β=-0.1, 95% CI -201.9 to -65.5, P<0.001), infant HIV infection ( β=-0.1, 95% CI -310.4 to -68.2, P=0.002), hepatitis C virus infection ( β=0.1, 95% CI 71.2 to 410.4, P=0.005) and gestational age ( β=0.6, 95% CI 155.9 to 191.5, P<0.001) were associated with decreased BW. Conclusions:While improving the effectiveness of PMTCT for HIV, more attention should be paid to pregnant women who received ART in the first trimester and PI-based ART for preventing lower BW and improving maternal and infantile health.

Objective To study the expression of selenoprotein genes in human immunodeficiency virus(HIV)infection and its mother-to-child transmission,so as to provide a theoretical basis for the prevention,diagnosis,and treatment of acquired immunodeficiency syndrome.Methods The dataset GSE4124 was downloaded from the Gene Expression Omnibus(GEO).Two groups of HIV-positive mothers(n=25)and HIV-negative mothers(n=20)were designed.HIV-positive mothers included a subset of transmitter(TR)mothers(n=11)and non-transmitter(NTR)mothers(n=14).Then,t-test was carried out to compare the expression levels of selenoprotein genes between the four groups(HIV-positive vs. HIV-negative,NTR vs. HIV-negative,TR vs. HIV-negative,TR vs. NTR).Univariate and multivariate Logistic regression were adopted to analyze the effects of differentially expressed genes on HIV infection and mother-to-child transmission.R software was used to establish a nomogram prediction model and evaluate the model performance.Results Compared with the HIV-negative group,HIV-positive,NTR,and TR groups had 8,5 and 8 down-regulated selenoprotein genes,respectively.Compared with the NTR group,the TR group had 4 down-regulated selenoprotein genes.Univariate Logistic regression analysis showed that abnormally high expression of GPX1,GPX3,GPX4,TXNRD1,TXNRD3,and SEPHS2 affected HIV infection and had no effect on mother-to-child transmission.The multivariate Logistic regression analysis showed that the abnormally high expression of TXNRD3(OR=0.032,95%CI=0.002-0.607,P=0.022)was positively correlated with HIV infection.As for the nomogram prediction model,the area under the receiver-operating characteristic curve for 1-year survival of HIV-infected patients was 0.840(95%CI=0.690-1.000),and that for 3-year survival of HIV-infected patients was 0.870(95%CI=0.730-1.000).Conclusions Multiple selenoprotein genes with down-regulated expression levels were involved in the regulation of HIV infection and mother-to-child transmission.The abnormal high expression of TXNRD3 was positively correlated with HIV infection.The findings provide new ideas for the prevention,diagnosis,and treatment of acquired immunodeficiency syndrome.

ABSTRACT

Hepatitis B virus (HBV) infection is a global public health problem, and the prevention of mother-to-child transmission (MTCT) is an important intervention method. As a new nucleoside reverse transcriptase inhibitor, tenofovir alafenamide (TAF) has the characteristics of strong liver targeting and low peripheral blood exposure. Although it has been used in the treatment of chronic hepatitis B widely, its application in blocking mother-to-child transmission of HBV is still limited. This paper reviews the characteristics of TAF, the effects of TAF on infants and pregnant women, in order to provide new ideas for the prevention of mother-to-child transmission of HBV.

Mother-to-child transmission of hepatitis B virus (HBV) is one of the main ways of transmission and the main cause of chronic hepatitis B after infection. Therefore, preventing mother-to-child transmission of HBV is particularly important in reducing the incidence of chronic hepatitis B. Currently, nucleoside ( acid) analoids ( Nas ) used for mother-to-child blocking of HBV include lamivudine (LAM) , tibivudine (LdT) and tenofovir fumarate ( TDF). Propofol tenofovir fumarate (TAF) has also been used in pregnant chronic hepatitis B pa- tients. This paper summarizes the efficacy, safety and antiviral treatment indications and termination time of the above-mentioned drugs in mother-to-child preventing to provide suggestions for the selection and rational application of mother-to-child preventing Nas.

@#Abstract: Objective To analyze the influencing factors of mother-to-child transmission of hepatitis B virus after combined immunological blockade, and to evaluate the effect of mother-to-child blockade, and to provide a basis for health policies and health interventions for preventing mother-to-child blockade of hepatitis B virus. Methods A total of 11 363 pairs of HBsAg positive pregnant women and their infants aged 7-12 months in Hainan Province from 2015 to 2020 were included in the study. The general situation, the situation of health care and delivery in this pregnancy and perinatal period, the detection of hepatitis B markers, the situation of antiviral therapy, the general situation of mother and infant during delivery and the implementation of blockade measures for mother-to-child transmission of hepatitis B were collected and analyzed. Results Among the 11 363 pairs of HBsAg positive pregnant women and their infants delivered in hospitals in Hainan province from 2015 to 2020, the positive rate of HBsAg in children at 7-12 months after birth was 1.47 %, and the difference in HBsAg positive rate of infants born in different years was not statistically significant (P>0.05). There were no significant differences in the positive rate of HBsAg among children born to pregnant women with different nationalities, educational levels, occupations, delivery modes, delivery places, obstetric operations and perineal laceration, abnormal perinatal period, children with different genders and premature delivery and perinatal (all P<0.05). There was significant difference in HBsAg positive rate among infants born to pregnant women of different ages, the positive rate of HBsAg of infants born to young pregnant women was higher than that of older pregnant women (P<0.05). The rate of antiviral therapy was low in HBeAg positive pregnant women, and the positive rate of HBsAg in their infants was 2.54%, which was higher than 0.83% in HBeAg negative pregnant women (P<0.05). Conclusions Combined immunological blockade with hepatitis B vaccine and hepatitis B immunoglobulin can effectively prevent the mother-to-child transmission of HBV. HBsAg-positive women can give birth at the right age, and HBeAg-positive pregnant women can be treated with antiviral therapy to block mother-to-child transmission, providing the important basis for the formulation of hepatitis B prevention and control strategies and measures.

OBJECTIVES@#To study the moderating effect of mother-child relationship in the association between maternal parenting stress and emotional and behavioral problems in preschool children, and to provide reference for the prevention and control of emotional and behavioral problems in preschool children.@*METHODS@#Using a stratified cluster sampling method, 2 049 preschool children were surveyed from November to December 2021, who sampled from 12 kindergartens in Wuhu City, Anhui Province. The emotional and behavioral problems of preschool children were assessed with the Strength and Difficulties Questionnaire. Pearson correlation analysis was used to evaluate the relationship of maternal parenting stress and mother-child relationship with children's emotional and behavioral problems. The PROCESS Macro was used to analyze the moderating effect of conflicted and dependent mother-child relationships in the association between maternal parenting stress and emotional and behavioral problems in these preschool children.@*RESULTS@#Among these preschool children, maternal parenting stress was positively correlated with the scores of emotional symptoms, conduct problems, hyperactivity, and peer problems subscales and total difficulty scores (P<0.001); intimate mother-child relationships were negatively correlated with the scores of conduct problems, hyperactivity, and peer problems subscales and total difficulty scores (P<0.001); conflicted and dependent mother-child relationships were positively correlated with the scores of emotional symptoms, conduct problems, hyperactivity, and peer problems subscales and total difficulty scores (P<0.001). After controlling for relevant confounding factors, conflicted mother-child relationship (β=0.05, P=0.001) and dependent mother-child relationship (β=0.04, P=0.012) were found to have a moderating effect on the association between maternal parenting stress and total difficulty scores in these preschool children.@*CONCLUSIONS@#Negative mother-child relationships play a moderating role in the association between maternal parenting stress and emotional and behavioral problems in preschool children. Prevention of emotional and behavioral problems in preschool children should focus on reducing maternal parenting stress and improving negative mother-child relationships.
