ABSTRACT

Objective:To investigate the clinical effect of botulinumtoxin type A (Botox-A) combined with electromyographic biofeedback therapy on the upper limb muscle spasm after stroke.Methods:86 cases of patients with upper limb muscle spasm after stroke in our hospital from January 2016 to January 2017 were selected and divided into the observation group and the control group, with 43 cases in each group.Patients in the control group were treated with electromyographic biofeedback therapy, and the observation group was treated with Botox-A based on the basis of control group. The improvement of upper limb muscle spasm,Upper limb movement function, the active range of wrist joint and life skills before and after treatment were compared between two groups. Results:After treatment, the total effective rate of improvement of upper limb muscle spasm of observation group were significantly higher than that of the control group (P<0.05);At 2 weeks and 4 weeks after treatment, the Fugl-Meyer scores,Wrist joint activities, modified Barthel index (MBI) of two groups were significantly higher than those before treatment (P<0.05),which were significantly higher in the observation group than those of the control group (P<0.05).Conclusion:Botox-Acombined with electromyographic biofeedback therapy had remarkable clinical effect on the upper limb muscle spasm after stroke, which could effectively reduce the upper limb spasticity, improve the arm and wrist movement ability and the ability of daily life.

ABSTRACT

Objective To observe the clinical efficacy of Tiao Ren Tong Du (regulating the Conception Vessel and unblocking the Governor Vessel) needling method plus bloodletting at the Jing-well points in treating post-stroke spastic state. Method Sixty-seven eligible patients were randomized into a group to receive Tiao Ren Tong Du needling plus bloodletting at the Jing-well points (treatment group) and a rehabilitation group (control group). For the two groups, the treatment was given once a day, 6 times as a treatment course, for 4 courses in total. The Modified Ashworth Scale was used to evaluate the spasticity intensity, and the Patient Reported Outcome (PRO) for post-stroke spastic hemiplegia was used to measure the patients from the following four aspects:subjective symptoms, objective symptoms, mental state, and social function. Result After intervention, the scores of spasticity, and physiological, mental, social, and environmental factors were significantly improved in both groups (P<0.01,P<0.05). Compared to rehabilitation, Tiao Ren Tong Du needling plus bloodletting at Jing-well points can more significantly release the muscular spasticity and reduce the PRO score (P<0.01). The recovery plus markedly-effective rate was 70.6% in the treatment group versus 42.4% in the control group. Conclusion Tiao Ren Tong Du needling plus bloodletting at Jing-well points can effectively improve post-stroke spastic state and the quality of life.

ABSTRACT

Objective To observe the curative effect of botulinum toxin A(BTX-A)repeated intramuscular injection for treatment of serious spastic cerebral palsy. Methods Thirty cases with serious spastic cerebral palsy received local repeated intramuscular injection. The interval time was 3 months. The dose of BTX-A was 4 U/kg.The muscle tone was assessed with the modified ashworth scale and range of motion with physical rating scale(PRS).The indexes were evaluated before injection and 3 months after the second injection. Paired-samples t-test was used in the statistic analysis.Result The muscle tone and PRS had remarkable improvement after second therapy with BTX-A(P
