ABSTRACT

Objective The present study aims to investigate the effect of the early nasal jejunum nutrition (NJN) on the inflammatory markers of severe acute pancreatitis (SAP) and to research if the NJN improves the recovery of SAP. Method SAP inpatients were observed during 2016. One group who accepted the NJN was experimental group and another group who did not accept the NJN was control group.The difference of serum albumin (ALB), procalcitonin (PCT), C reactive was compared to observe the effect of the early nasal jejunum nutrition (NJN) on the inflammatory markers of severe Acute Pancreatitis protein (CRP), interleukin 6 (IL-6) and serum amylase (AMY) between the highest and lowest levels in two group respectively and the time of diet recovery between two groups. Results (1) The difference of interleukin 6 (IL-6) between the highest and lowest levels in experimental group was significantly smaller than that of the control group;the difference of ALB, CRP and PCT between the highest and lowest levels in experimental group was not different from the control group;2) The time of diet recovery of experimental group was shorter than that of the control group. Conclusion The early NJN can reduce the IL-6 level and the time of diet recovery,and then,improve the SAP recovery.

ABSTRACT

Objective To investigate the value of endoscopic assisted nasal jejunal tube placement and jejunum nutrition in treatment of critically illed patients. Methods This study was a prospective study. 56 patients with severe cerebrovascular diseases in the department of NICU from May 2014 to May 2017 were enrolled in this study. All of the patients were found to have an increased gastric remnant within 72 hours after admission (>100 ml). In the nasal jejunal group, 28 patients received nasal endoscopy and nasal endoscopic feeding, and 28 cases were treated with routine nasogastric tube and enteral nutrition. Comparing the two groups of patients with gastrointestinal complications (including vomiting, gastroesophageal reflux, abdominal distension, diarrhea, stress ulcer incidence) and nutritional status parameters (including serum albumin, prealbumin, hemoglobin), incidence of hospital acquired pneumonia and inflammation indexes (including WBC, PCT and CRP), stay at NICU time, NICU expenses, GCS score and mortality rate. Results The success rate of nasal intestinal tube placement under nasal endoscopy guided wire was 100.0%. The nasal jejunal nutrition status parameters were better, NICU check-in time was shorter, NICU expenses were lower, and the inflammation index was lower than the control group. There were no significant differences in the incidence of gastrointestinal complications, the incidence of hospital acquired pneumonia, the GCS score and the fatality rate between the two groups. Conclusion For the patients with severe gastric remnant increase in NICU, it is worth recommending that naso jejunal tube be inserted into the jejunum by endoscopy and using jejunum nutrition.

ABSTRACT

Objective:To investigate the methods and nutritional effects of endoscopic jejunal ac cess for enteral feeding.Methods:106 cases placed jejunum feeding tube were randomly divided into the observation group and the control group,54 cases in the observation group and 52 cases in the control group.43 patients underwent nasogastric enteral nutrition were as the nasogastric tube group.The traditional endoscopic jejunal access method was used in control group,and the modified endoscopic jejunal access method was used in observation group.The operation time,the success rate of indwelling,complications and nutritional indexes were compared among these three groups.Results:The operative time of the observation group and nasogastric tube group was significantly shorter than that of the control group (P < 0.05).The success rate of indwelling was significantly lower in the control group (P < 0.05).There was no significant difference in the incidence of postoperative complications among these three groups (P > 0.05).After intubation and nutritional treatment in all the groups,hemoglobin,plasma albumin and serum albumin levels were significantly improved (P < 0.05).There were no significant differences in nutritional indexes between the observation group and the control group (P > 0.05).The levels of hemoglobin,plasma albumin and plasma prealbumin in nasogastric tube group were significantly lower than those in the observation group and the control group (P < 0.05).Conclusion:Modified endoscopic jejunal access for enteral feeding can effectively shorten the intubation operation time and improve success rate and nutritional indexes.
