ABSTRACT

The 's three-needle technique is famous for mind regulation and treatment. From the perspective of and blood, the special principle and treatment pathway of 's three-needle technique for mind regulation and treatment were summarized and analyzed. Moreover, themethods of invigorating brain and regulating mind, calming heart and tranquilizing mind, strengthening spleen and stomach, dredging the meridian and channel were applied to guide the into the brain to regulate the mind. The mind is regulated by adjusting heart and blood, nourishing the with , adjusting the source of blood and and dredging the channel of and blood. With "adjusting to regulate mind, adjusting blood to regulate mind", a comprehensive treatment is achieved to increase the efficacy of mind regulation and treatment.

ABSTRACT

BACKGROUND AND OBJECTIVES: Surgery is considered the primary treatment for intracordal cyst. However, patients who had undergone surgery are still subject to recurrence and continued voice changes. Intracordal cysts naturally disappear in some patient population. Cyst does not always recur in patients who had received partial surgical removal, too. Contradicting results raises a question whether complete surgical removal of intracordal cyst is necessary and demonstrate need for better treatment. Herein, the author proposes novel surgical method technique intralaryngeal needle technique (INT), a technique using surgical needle for not only injection but also for aspiration and excision of cyst. This study aims to examine the potential of intralaryngeal needle technique in treating intracordal cysts. MATERIALS AND METHODS: Surgical procedures were done on in-patients diagnosed with intracordal cyst. 23 patients received follow-up screening after the surgery for one year. Patients' subjective satisfaction levels, acoustic measures, aerodynamic measures, laryngeal stroboscopic results were compared before and after the treatment. RESULTS: Overall patients were satisfied with novel surgical excision method. In terms of aerodynamic measures, maximum phonation time, mean air flow rate improved after the surgery. In terms of acoustic measures, Jitter, Shimmer, NHR, and voice pitch changes after the treatment showed statistically significant differences. Laryngeal stroboscopy results showed significant decreases in cyst sizes. Post-surgery patients had improved mucosal waves and amplitudes values. CONCLUSION: The results show the validity of intralaryngeal needle technique in reducing intracordal cyst size by excision, aspiration, and injection. The author believes this novel technique can be used as an alternative surgical method for intracordal cysts.

ABSTRACT

BACKGROUND: We prospectively evaluated the incidence and possible factors causing intramuscular injection during lumbar sympathetic ganglion block and compared the multiple needle technique to the single technique to obtain a profound and complete block effect. METHODS: Among 83 patients, 58 patients (group A, n = 27, multiple needle technique and group B, n = 31, single needle technique) were reevaluated for the changes of skin temperature (Ts) and mean segment of longitudinal contrast spread. After injecting the contrast agent, the incidence of psoas muscle injection and the change of Ts was compared between two groups. RESULTS: The incidence of psoas muscle injection was 21.3% (46/216) and it was associated with the level of injection (L2) significantly (chi-square = 14.773, P = 0.001). DT(post) (postblock temperature difference between ipsilateral and contralateral great toe, 4.6 +/- 2.8degrees C, 1.8 +/- 1.6degrees C, P < 0.001 for group A and B) and DT(net) (DT(post) - DT(pre), 3.9 +/- 2.7degrees C, 1.5 +/- 1.5degrees C, P < 0.001 for group A and B) was significantly higher in group A. The mean segment of longitudinal contrast spread was 8.1 +/- 0.9 for group A and 3.2 +/- 1.6 for group B (P < 0.001). CONCLUSIONS: The LSGB at the L2 level showed the lowest incidence of psoas muscle injection of contrast. Multiple needle approach showed more significant increase of DT(net) and DT(post).

ABSTRACT

Objective:It was a part of an overall study of the clinical research literature about ischemic cerebrovascular disease treated by various acupuncture techniques,namely "restoring consciousness and inducing resuscitation technique","needling Baihui(DU20) through Qubin(GB7)","Jiao’s three-needle technique" and "temporal three-needle technique".The papers collection for analysis were designed on the random,control and blind methods.Methods: A data bank was set up with Access 2003.A statistical analysis was made for the method of employing ratio and constituent ratio about the research literature mamaged with the ramdom,control and blind methods.Results: The ratio of those that were designed randomly is as follows: the "restoring consciousness and inducing resuscitation technique"(50%),"needling Baihui(DU20) through Qubin(GB7)"(100%)(Only two articles meet the request and futher exploration is needed),and the "temporal three-needle technique"(50%).And the "Jiao’s three-needle technique"(40%).The ratio of those that were designed with the control method is as follows: the "restoring consciousness and inducing resuscitation technique"(60%),"needling Baihui(DU20) through Qubin(GB7)"(100%)(Only two articles meet the request and further exploration is needed,the "temporal three-needle technique"(75%),and "Jiao’s three-needle technique"(70%).The blind method was not used.Conclusion: It is suggested that the three methods should be stressed in the study of acupuncture in the treatment of ischemic cerebrocvascular disease.Only in this way,can there be a maximum comparability and the sampling error is decreased.The bias due to subjective factors of the researchers and subjects can be overcome.

ABSTRACT

BACKGROUND: Paresthesia is unpleasant for patients and more importantly, is related to neurological injury in some cases. There have been few studies comparing combined spinal-epidural anesthesia (CSE) techniques such as needle-through-needle technique (NTN) and double segment technique (DST) regarding the incidence of paresthesia. METHODS: Eighty-four parturients undergoing CSE for an elective cesarean section were divided into NTN and DST groups. A CSE was performed using 27 G Sprotte needle, 18 G Tuohy needle and 20 G multiport catheter in both groups. In the NTN group, at L3-4 or L4-5, a spinal anesthesia was performed and then an epidural catheter was inserted through the same Tuohy needle. In the DST group, an epidural catheter was inserted at L1-2 or L2-3 and then a spinal anesthesia was done two interspaces caudally. Incidences of paresthesia with each procedure were recorded with block characteristics and adverse effects. RESULTS: Overall frequency of paresthesia was higher in the NTN group (54.8% vs. 29.3%, P = 0.033). Incidences of intrathecal paresthesia were three times higher (21.4% vs. 7.3%, not significant) and epidural paresthesia was significantly higher (45.2% vs. 22%, P = 0.044) in the NTN group. Sensory and motor block characteristics and side effects were mostly comparable between the groups. CONCLUSIONS: Double-segment technique shows less incidence of overall paresthesia compared to needle-through-needle.

ABSTRACT

OBJECTIVE: To determine whether there is a difference in nerve conduction studies depend on the body mass index (BMI) of subjects METHOD: Twenty normal healthy volunteers were enrolled for the study. A routine usual sensory and motor nerve conduction study and a sensory nerve conduction study using the near nerve needle technique were performed. BMI was calculated as weight (kg) divided by height (m) squared. In order to evaluate the effect of BMI on the various measure ments of the nerve conduction study, one-way analysis of variance (ANOVA) was used. RESULTS: The sensory nerve amplitudes of median, ulnar and sural nerves correlated significantly (p<0.05) with BMI. However, no correlation was noted between BMI and sensory nerve amplitude by near nerve needle technique. There was no statistical differences noted in the measurements of latency of examined motor and sensory nerves neither the velocity of examined motor nerves. CONCLUSION: In clinical practice, the effect of BMI should be taken into account when the interpretation of abnormal sensory nerve study has to be soli.
