ABSTRACT

Se describe el caso clínico de una paciente de 67 años de edad con varias comorbilidades, entre ellas la policitemia vera, quien acudió al Cuerpo de Guardia del Hospital Clinicoquirúrgico Universitario Dr. Ambrosio Grillo Portuondo de Santiago de Cuba por presentar síntomas y signos de un síndrome de abdomen agudo oclusivo. Se indicó intervención quirúrgica de urgencia, que permitió confirmar el diagnóstico presuntivo de afección vascular mesentérica de tipo trombótica. La inmediatez del tratamiento quirúrgico, la reversibilidad del daño vascular sin necesidad de procedimiento de resección intestinal y la administración efectiva de anticoagulantes permitieron una evolución favorable y sin complicaciones.


The case report of a 67 years patient with several comorbidities is described, among them polycythemia vera, who went to the emergency room of Ambrosio Grillo Portuondo University Clinical Surgical Hospital in Santiago de Cuba due to symptoms and signs of a syndrome of occlusive acute abdomen. An emergency surgical intervention was indicated, that confirmed the presumptive diagnosis of mesenteric vascular thrombosis. The immediacy of the surgical treatment, the reversibility of the vascular damage without necessity of intestinal resection procedure and the effective use of anticoagulants allowed a favorable clinical course without complications.

ABSTRACT

An 81-year-old woman, long-term non-progressor HIV infected, asymptomatic, not using ART, with a seven-year clinical follow-up in a reference unit, TCD4+ cell count values ranged from 719 to 1151 cells/µl, TCD8+ from 579 to 897 cells/µl and a viral load with higher value of 51 viral copies/ml but with undetectable results in most of the tests performed. The report of the long-term non-progressor HIV carrier aged over 80 years is somewhat unusual, considering the physiological/immunological changes that occur with the aging process concomitantly with HIV infection.


Mulher de 81 anos, infectada pelo HIV há muito tempo, não progressor, assintomática, sem uso de TARV, com acompanhamento clínico de sete anos em unidade de referência, os valores de contagem de células TCD4+ variaram de 719 a 1151 células/ µl, TCD8+ de 579 a 897 células/µl e uma carga viral com maior valor de 51 cópias virais/ml, mas com resultados indetectáveis na maioria dos testes realizados. O relato do portador de HIV de longa data não progressor com idade superior a 80 anos é um tanto incomum, considerando as alterações fisiológicas/imunológicas que ocorrem com o processo de envelhecimento concomitante à infecção pelo HIV.


Mujer de 81 años, infectada por VIH no progresor de larga evolución, asintomática, no usuaria de TAR, con seguimiento clínico de siete años en una unidad de referencia, los valores de recuento de células TCD4+ oscilaron entre 719 y 1151 células/ µl, TCD8+ de 579 a 897 células/µl y una carga viral con mayor valor de 51 copias virales/ml pero con resultados indetectables en la mayoría de las pruebas realizadas. El reporte de portadores de VIH no progresores a largo plazo mayores de 80 años es algo inusual, considerando los cambios fisiológicos/inmunitarios que ocurren con el proceso de envejecimiento concomitante con la infección por VIH.

ABSTRACT

Objective:To explore the correlation between sarcopenia and balance in old women. Methods:From September to November, 2017, 224 women aged 60 to 70 were recruited. They were divided into normal group (n = 198) and sarcopenia group (n = 26) according to the results of muscle volume, grip strength and 6-meter walking speed (6MWS), and tested with single foot standing with eyes close (SSEC) and the five times of sit-to-stand (5STS). The correlation of the time of SSEC and 5STS to muscle volume, grip strength and 6MWS were analyzed with Pearson's Correlation Analysis in both groups, respectively. Results:The incidence of simple decrease in muscle volume, grip strength and 6MWS was 20.5%, 14.3% and 2.2%, respectively, and the incidence of sarcopenia was 11.6%. The time of SSEC was shorter in the sarcopenia group than in the normal group (t = 4.072, P < 0.001), and the time of 5STS was longer (t = -5.461, P < 0.001). The time of SSEC correlated with 6MWS in normal group (r = 0.675, P < 0.01), and it correlated with 6MWS (r = 0.492) and grip strength (r = 0.286) in the sarcopenia group (P < 0.001). The time of 5STS correlated with muscle volume (r = 0.258, r = 0.321), grip strength (r = 0.351, r = 0.462) and 6MWS (r = 0.337, r = 0.396) in both groups (P < 0.001). Conclusion:Compared with muscle mass and strength, the muscle activities remain well in the old women in the process of aging. Both static and dynamic balance reduce in the old women with sarcopenia. The static balance mainly relates with muscle activities, while the dynamic balance relates with all the muscle mass, strength and activities.

ABSTRACT

O vestuário pode ser entendido como uma forma de comunicação e constituição da identidade de gênero ao longo do envelhecimento. A pesquisa buscou caracterizar a relação que as mulheres estabelecem com o vestuário na velhice. Utilização do método etnográfico na familiarização com campo e posterior aplicação de questionário misto. As participantes foram idosas frequentadoras da oficina de teatro da UATI EACH/USP, Brasil. O vestuário foi visto como importante para a composição da aparência na velhice. Constataram-se mudanças nas escolhas de vestuário, decorrentes, especialmente, da percepção de transformações corporais ocorridas ao longo do envelhecimento. A criação de novas estratégias no modo de se vestir nesse período da vida esteve ancorada na tensão entre negação da velhice e adequação etária. Os dados apontaram forte desejo de consumo, principalmente por peças que atendessem a demandas ergonômicas e estéticas, trazendo conforto. Salienta-se a relevância da aparência, sobretudo o vestuário, para a compreensão de aspectos socioculturais da velhice feminina.


Clothing can be understood as a way to communicate and constitute a gender identity throughout the aging. This research aimed to characterize the relationship between women and clothing in old age. It was used an ethnographic method in the familiarization with the field and, subsequently, a mixed questionnaire. The participants were elderly women attending the theater workshop of the University for Seniors, from the School of Arts, Sciences, and Humanities from the University of São Paulo, Brazil. Clothing was seen as important for the composition of personal appearance in female old age. It was observed changes in the choices of clothing due to aging, especially the body aging. New strategies for dressing in old age were based on the tension between the denial of old age and the age adequacy. Data indicated a strong desire for consumption, especially for garments that meet ergonomic and aesthetic demands that bring comfort. It is emphasized the relevance of appearance, especially clothing, in the understanding of sociocultural aspects of female old age.


El vestuario es una manera de comunicarse y de constituir la identidad de género a lo largo del envejecimiento. En este artículo se presenta un estudio acerca de la relación que mujeres mayores establecen con su ropa. Se utilizó el método etnográfico en la familiarización con el campo y en la aplicación de un cuestionario mixto. Las participantes eran mujeres mayores que participaban de un taller de teatro de la Universidad para Mayores (UATI), de la Escuela de Artes, Ciencias y Humanidades de la Universidad de São Paulo, Brasil. El vestuario fue considerado importante para la composición de sus apariencias en la vejez. Fueron observados cambios en las prendas elegidas a lo largo de la vida, especialmente en razón del envejecimiento de sus cuerpos. Sus estrategias para vestirse en la vejez estaban relacionadas a una tensión entre la negación de la ancianidad y la adecuación de edad. Los datos apuntaron que a las mujeres investigadas les gustaba comprar ropa y lo hacían principalmente cuando encontraban prendas cómodas y adecuadas no solo a sus cuerpos sino también a su gusto personal. Finalmente, se señala la relevancia de la apariencia para la comprensión de aspectos socioculturales de la vejez femenina.

ABSTRACT

La seudoobstrucción colónica aguda o síndrome de Ogilvie es una afección en la cual hay apariencia clínica e imagenológica de obstrucción intestinal sin bloqueo mecánico. Se describe el caso clínico de una anciana de 65 años de edad, quien sufría esta condición clínica, por lo cual fue atendida en el Servicio de Cirugía del Hospital N´Gola Kimbanda, provincia Namibe en Angola e intervenida quirúrgicamente. La paciente evolucionó favorablemente y egresó sin dificultad


The acute colonic pseudo-obstruction or Ogilvie syndrome is a disorder in which there is a clinical and imagenologic appearance of intestinal obstruction without mechanic blockade. The case report of a 65 years old woman who suffered from this clinical condition is described, reason why she was assisted and surgically intervened in the Surgery Service of N´Gola Kimbanda Hospital, Namibe province in Angola. The patient had a favorable clinical course and she was discharged without difficulty

ABSTRACT

ObjectiveTo observe the clinical effect of the treatment of clearing liver fire,removing dampness,strengthening spleen and tonifying kidney in treating middle-aged and old woman with chronic urinary tract infection.Methods82 cases were randomly divided into two groups.42 cases in treatment group were treated with herbal prescription of clearing liver fire,removing dampness,strengthening spleen and tonifying kidney,and 40 cases in control group were treated with sensitive antibiotics.The clinical effect,safety and changes of immune index were compared between two groups.ResultsIn the treatment group,the recent cure rate was 76.8%,the total effective rate was 90.5% and the complete cure rate was 61.9%; while in the control group,the recent cute rate was 50%,the total effective rate 65% and the complete cure rate 27.5%.It showed significant difference between two groups (Z=2.740,P<0.05).There was no significant difference between two groups in hepatic and renal function,serum and urine β2-MG level before and after treatment and it showed a better clinical safety in treatment group.The level of IgA,IgG and IgM increased after the treatment in treatment group.Comparing with the control group,the results showed significant differences(t=9.1,10.5 and 11.2 respectively,P<0.01).In treatment group the level of CD4+was increased,CD8+was decreased,and the ratio of CD4+/CD8+ was increased obviously after the treatment.The differences before and after the treatment and between groups were significant(t=13.3,8.6,P<0.01).In control group the level of CD4+、CD8+、CD4+/CD8 +did not change significantly.The differences before and after the treatment had no statistical significance (P>0.05).ConclusionThe treatment with herbs of clearing liver fire,removing dampness,strengthening slpeen and tonifying kidney had better effect,less side effect and good research prospect.

ABSTRACT

Recurrent hypersomnia is a disorder characterized by recurrent episodes of hypersomnia that typically occur weeks or months apart. We describe a 60-year-old woman with a four-year history of recurrent hypersomnia. Physical examinations, laboratory tests, and brain MRI showed no significant abnormality in the patient. Nocturnal polysomnographic study showed high sleep efficiency, absent sleep stage 3 and 4, and reduced sleep latency. The multiple sleep latency tests showed short sleep latency and five episodes of sleep-onset REM periods. This is the oldest known case of recurrent hypersomnia.
