ABSTRACT

@#AIM:To observe the effect of different time of wearing overnight orthokeratology lens(OOK lens)on the meibomian gland and detection rate of demodex.<p>METHODS:Case-control study. Totally 47(91 eyes)patients who wearing keratoscopy were enrolled from June 2019 to December 2019 in Hanyang Aier Eye Hospital. According to the wearing time, they were divided into short-term group(wearing time <2a)and long-term group(wearing time≥2a). Meanwhile, patients wearing frame glasses with gender, age and diopter matching with the two groups above were selected as the control group. All patients had non-invasive tear film rupture time(NI-BUT)examination, meibomial gland infrared photography, bulbar redness score(BRS), and eyelash sampling.<p>RESULTS: The three groups of NI-BUT were 12.68±4.59s, 12.37±4.97s, 10.45±6.04s,(<i>P</i>>0.05). BRS respectively were 0.99±0.41, 0.90±0.37, 0.84±0.35,(<i>P</i>>0.05). Meibomian gland score for all three groups: 1(0, 1)(<i>P</i>>0.05). In the three groups, the detection rate and quantity of demodex were 42%, 1(1, 2); 39%, 2(1, 2); 33%, 1(1, 2),(<i>P</i>>0.05). Among the patients with corneal staining, 5 were mite positive and 11 were mite negative(<i>P</i>>0.05). Meibomian gland score: 4 eyes were 0 grade, 12 eyes were 1 grade above(<i>P</i>>0.05). Short-term group 6 eyes, long-term group 10 eyes(<i>P</i>>0.05). <p>CONCLUSION:There was no significant effect on meibomian gland, the detection rate and quantity of demodex in patients wearing OOK lens. Whereas no significant relationship between meibomian gland score/ demodex detection rate and corneal staining.

ABSTRACT

Objective To observe the changes in tear quantity and tear break-up time(BUT) before and after wearing overnight orthokeratology (OK),and investigate the influence of OK on tear film in young adolescents.Methods Prospective clinic study was conducted in 60 adolescents,aged 8-16 years,who met the inclusion criteria during November 2016 and May 2017 in the Second Hospital of Dalian Medical University were chosen and fitted with OK (OK group).At the same time,60 adolescents who met the same requirement were fitted with spectacle (spectacle group).The two groups were both divided into three subgroups according to the age (subgroup of 8-10 years old,11-13 years old,and 14-16 years old).The BUT,Schirmer' s I test score,and corneal fluorescent (FL) were measured and recorded before wearing and 1 week,1 month,3 months,6 months and 12 months after wearing.Results Totally 57 patients in the OK group (114 eyes) and 59 patients in the spectacle group (118 eyes) completed the follow-up study.No significant difference in Schirmer' s Ⅰ test scores was observed between the OK group and spectacle group (all P>0.05).The BUT in OK group was significantly lower than that in the spectacle group at all the time points (all P < 0.05).Of all subgroups,the subgroup aged from 8-10 years exhibited the sharpest and most rapid reduction in BUT.Conclusion Based on our findings,long-term OK wearing has no effect on tear volume but reduces the stability of tear film.The results suggest that OK should be considered to be a good alternative for children and adolescents with spectacle;however,it may influence the quality of tear film.In addition,special attention should be paid to younger adolescents,and regular follow-up is needed.

ABSTRACT

@#AIM: To explore the effects of overnight orthokeratology lens and conventional frame glasses on the myopic diopter, uncorrected visual acuity and ocular parameters of myopia adolescent. <p>METHODS: Totally 102 cases of(204 eyes)of adolescent myopia patients were randomly divided into observation group and control group with 51 cases(102 eyes)in each group during April 2014 to April 2017. Control group was only given conventional frame glasses, and observation group was given overnight orthokeratology lens. The myopic diopter and uncorrected visual acuity(UCVA)before wearing glasses and at 1wk, 1, 3, 6mo and 1a of wearing glasses, and the ocular parameters before wearing glasses and at 1a after wearing glasses were observed in the two groups, and the occurrence of complications was compared between the two groups. <p>RESULTS: After 1wk to 1a of wearing glasses, the myopic diopter in observation group was gradually decreased(<i>P</i><0.05), and there was no significant difference in control group(<i>P</i>>0.05), but there was statistically significant difference between-groups at different time points(<i>P</i><0.05). After 1wk to 1a of wearing glasses, the UCVA in observation group showed a increasing tendency(<i>P</i><0.05), and the UCVA in observation group after 6mo to 1a of wearing glasses was significantly higher than that in control group(<i>P</i><0.05). At 1a after wearing glasses, the axial length, corneal endothelial cell density, central anterior chamber depth, and intraocular pressure in observation group were not significantly different from those before wearing glasses(<i>P</i>>0.05), and the axial length in control group was significantly longer than that before wearing glasses and that in observation group(<i>P</i><0.05). The total incidence rates of complications in observation group and control group were 10.8% and 6.9% respectively(<i>P</i>>0.05).<p>CONCLUSION: Overnight orthokeratology lens for adolescent myopia can effectively correct the myopic diopter, and improve the uncorrected visual acuity. It is less harmful to the eyes and less complications, and it is safe and reliable in clinical application.

ABSTRACT

PURPOSE: To evaluate the effects on the ocular surface and the compliance of overnight orthokeratology lenses wear in normal myopic adults experiencing contact lens discomfort. METHODS: In this study, 28 eyes of 14 subjects were analyzed using overnight orthokeratology (OOK). This prospective study investigated the compliance of OOK wear using questionnaires, spherical equivalents, keratometry, and central corneal thickness. Ocular surface disease index, tear osmolarity, Schirmer's test I, tear film break-up time, and ocular surface staining score were evaluated as ocular surface parameters. Meibomian gland function was evaluated by assessing lid margin abnormality and meiboscore using noncontact meibography. The tests were performed before and 1 and 6 weeks after OOK wear. RESULTS: The mean age of the subjects was 26.67 ± 3.26 years, and the average duration of previous contact lens use was 2.64 ± 2.41 years. The questionnaire results showed that OOK lenses were more comfortable to wear than conventional lenses. Significant changes in spherical equivalents, keratometry, and central corneal thickness were observed at 6 weeks compared with baseline values (all p 0.05). CONCLUSIONS: Based on the findings, OOK is a relatively safe modality with good compliance for normal myopic adults experiencing contact lens discomfort.

ABSTRACT

PURPOSE: The present study assessed the influence of overnight orthokeratology (ortho-k) on the myopic progression in Korean children and analyzed factors affecting myopic progression. METHODS: The ortho-k group was comprised of 31 patients satisfying the inclusion criteria for ortho-k. In the ortho-k group, spherical equivalent refractive error (SER) was measured at baseline, and after 2 weeks, 6 and 12 months. The control group was comprised of 31 patients who were matched according to age, gender, and baseline SER of the ortho-k subjects. RESULTS: In the ortho-k group, the mean +/- SD changes in SER from 2 weeks to 6 months, 6 to 12 months, and 2 weeks to 12 months were -0.17 +/- 0.50 D, -0.04 +/- 0.76 D, and -0.21 +/- 0.78 D, respectively. In the control group, the changes in SER from baseline to 6 months, 6 to 12 months, and baseline to 12 months were -0.38 +/- 0.42 D, -0.44 +/- 0.38 D, and -0.82 +/- 0.68 D, respectively. Significant differences were found between changes in SER from 6 to 12 months and from baseline to 12 months (p or = -0.5 D with the group of SER change < -0.5 D, numeric values of white-to-white diameters of the 2 groups were different, and a significant correlation was found between the range of SER change and the white-to-white diameter (Pearson's r = -0.471, p = 0.008). CONCLUSIONS: Ortho-k is effective for slowing myopic progression. The smaller the white-to-white diameter, the slower the myopic progression could be.
