ABSTRACT

[Objective] Professor ZHANG Xiaosu treating childbearing anovulatory uterine bleeding academic thought and experience is summarized.[Methods] The clinical records of Professor ZHANG Xiaosu in the treatment of anovulatory uterine bleeding were studied. The experience and characteristics of ZHANG's treatment of the disease were summarized from the aspects of etiology, pathogenesis, treatment and prescription.[Results] For anovulatory uterine bleeding, Professor ZHANG Xiaosu believes that the disease of kidney deficiency, liver Qi stagnation is an important link in the pathogenesis, with "kidney, liver" for the treatment of law, urgent then treating the symptoms, treating the original for slow disease, at the same time stage of treatment, combination of traditional Chinese and western medicine, psychological counseling.[Conclusion] Professor ZHANG Xiaosu treats anovulatory uterine bleeding from the liver and kidney dialectical treatment, for ovulation disorders of uterine bleeding patients with the theory of guidance, in favor of young Chinese medicine students to study, promote the treatment of the disease and the development of Chinese medicine cause.

ABSTRACT

Objective To compare the effects of three different doses of letrozole(LE) on the promoting ovulation and pregnancy situation.Methods A total of 92 infertility patients with type Ⅱ ovulation disorder in this hospital from January 2013 to December 2014 were divided into three groups.The first group and second group were respectively given LE 2.5 mg/d and 5.0 mg/d from 3 d of menses for 5 d.The third group took LE 20 mg at draught on 3 d of menses.On the HCG injection day,follicle maturity time,number of mature follicles,ovulation rate,luteinized unrupture follicle syndrome (LUFS) and pregnancy rate were observed.Results The follicle maturity time,number of mature follicles,ovulation rate and clinical pregnancy rate on HCG injection day had no statistically significant difference among the three groups (P>0.05).The incidence of LUFS had statistically significant difference(P<0.05).Conclusion The promoting ovulation effect and clinical pregnancy rate have no difference among 3 different LE doses,but the occurrence rate of luteinized unruptured follicle syndrome(LUFS) in the 2.5 mg/d group is higher,therefore in the practical work,the medication administration mode may be selected according to the actual situation.

ABSTRACT

Objective To investigate the clinical efficacy of letrozole and clomiphene in the treatment of polycystic ovary syndrome in patients with ovulatory disorder. Methods Retrospective analysis of 480 cases diagnosed as polycystic ovary syndrome caused by ovarian infertility in patients with clinical diagnosis and treatment of information from January 2015 to January 2017, in which the treatment of cimetidine urinary hormone therapy as the control group (n=240), using letrozole urinary hormone therapy for the treatment group (n=240). The serum levels of sex hormones were measured before and after treatment in both groups. The endometrial thickness of patients was monitored by B-ultrasonography. Adverse reactions were recorded during treatment. Results The total effective rate was significantly higher in the treatment group than in the control group (P<0.05). The ovulation rate and pregnancy rate of the treatment group were 75.0% and 67.5% respectively, which was significantly higher than that of the control group (52.5%), 42.5% , χ2=(26.28, 30.30), P=(0.00,0.00). The levels of LH and T in the treatment group were (6.69±1.32) IU/L, (0.97±0.55) ng/mL, which was significantly lower than that in the control group (9.88±1.78) IU/L, (1.36±0.81) ng/mL. E2 in the treatment group was (44.25±10.71) pg/mL, significantly higher than that of the control group (39.35±7.69) pg/mL(P<0.05). The thickness of endometrium in the treatment group was significantly higher than that in the control group (P<0.05). No significant adverse reaction was observed during the treatment period. Conclusion Letrozole combined with urinary hormone treatment of polycystic ovary syndrome caused by ovarian disorder infertility patients with significant effect.

ABSTRACT

Luteinized unruptured follicle syndrome (LUFS), a specific type of ovulation disorder, is for a cause of female infertility. Acupuncture therapy has a good effect for LUFS. In order to understand the clinical effects and research situation of acupuncture therapy for LUFS, the author analyzed and reviewed the relevant literature. It was indicated by the findings that acupuncture therapy and integrative therapy of acupuncture and Chinese herbs have good therapeutic effect for this disease. Acupuncture therapy has the effect to promote ovulation. By the therapeutic principle to tonify the kidney and circulate blood, acupuncture therapy mostly was adopted in the period of ovulation. The acupoints are mostly selected from the lower abdomen and lower limbs, possibly based upon syndrome differentiation at the same time. The needling techniques are of certain characteristics. But, the issues of low quality, un-standardized inclusive, exclusive criteria and effective criteria still exist in the current clinical study.

ABSTRACT

Polycystic ovary syndrome (PCOS) occurs in 5 to 8% of females of reproductive age and is one of the leading causes of menstrual abnormalities and infertility. The treatment of PCOS varies, depending on the patient's age and background, particularly on the presence/absence of a desire to have children. In those desiring to have children, clomiphene therapy is the first-line treatment for PCOS. However, the anti-estrogen action of clomiphene may lead to a thinning of the endometrium and decreased cervical mucus production, while some patients show a poor response to the agent. In this study, saireito was administered to clomiphene-resistant PCOS patients, and the ovulation cycle improved in 6. Effective cases of saireito tended to have higher <i>oketsu </i>(blood stagnation) and <i>suitai </i>(water stagnation) statescores before treatment, also serum LH levels and LH/FSH ratios were lower due to the concurrent saireito medication. Furthermore, these effective cases had lower levels of serum testosterone than the non-effective and clomiphene-effective cases. This clinical report suggests the appropriateness of performing this therapy in accordance with <i>sho </i>(oriental patterns or symptoms), before second-line treatments such as gonadotropin therapy or laparoscopic surgery.

ABSTRACT

We report a case wherein a woman with HMG/HCG resistant ovarian disorder was treated with L-carnitine (LC) and dehydroepiandrosterone (DHEA). In this case, successful ovulation and pregnancy were accomplished after initial LC and DHEA administeration, followed by concurrent HMG/HCG administration.<br>
