ABSTRACT

Objective:To explore the effect of electromyography biofeedback therapy combined with oxiracetam on peripheral blood heme oxidase-1 (HO-1), soluble apoptotic molecules and cognitive function in patients with senile vascular dementia (VaD).Methods:One hundred and fourteen elderly patients with VaD from May 2018 to May 2020 in Xingtai Third Hospital were selected and divided into two groups according to the random number table method, with 57 cases in each group. Both groups were given conventional treatment. On this basis, the control group was given oxiracetam, and the observation group was given electromyography biofeedback therapy combined with oxiracetam. The treatment effects after treated for 1 month was compared between the two groups. The levels of serum HO-1, soluble apoptotic molecules sFas, sFasL before and after treatment were compared between the two groups. Cognitive function evaluated by Wechsler Memory Scale (WMS), Mini Mental State Examination (MMSE). The scores of Chinese Stroke Scale (CSS), Ability of Daily Living (ADL) before and after treatment and adverse reactions were compared between the two groups.Results:After treated for 1 month, the total effective rate in the observation group was higher than that in the control group: 93.0%(53/57) vs. 77.2%(44/57), the difference was statistically significant ( P<0.05). After treated for 1 month, the level of serum HO-1 in the two groups was higher than that before treatment, and the level of serum HO-1 in the observation group was higher than that in the control group: (30.21 ± 4.05) μg/L vs. (24.19 ± 3.47) μg/L, the difference was statistically significant ( P<0.05). The levels of serum sFas and sFasL in two groups after treatment were lower than those before treatment, and the levels of serum sFas and sFasL in the observation group were lower than those the control group after treatment: (81.57 ± 16.23) ng/L vs. (118.49 ± 25.09) ng/L, (135.47 ± 24.41) ng/L vs. (200.71 ± 30.29) ng/L, the differences were statistically significant ( P<0.05). After treated for 1 month, the CSS scores in the observation group was lower than the control group: (13.48 ± 2.15) scores vs. (17.22 ± 3.02) scores; the WMS, MMSE, and ADL scores in the observation group were higher than those in the control group: (97.75 ± 10.27) scores vs. (88.43 ± 9.16) scores, (23.82 ± 2.50) scores vs. (21.38 ± 2.19) scores, (60.16 ± 6.24) scores vs. (51.29 ± 5.52) scores, the differences were statistically significant ( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Electromyography biofeedback therapy combined with oxiracetam has a significant effect in the treatment of elderly patients with VaD. It can significantly improve vascular endothelial function, regulate apoptosis factors, strengthen cognitive function, promote recovery of nerve function and daily living ability without increasing adverse reactions.

ABSTRACT

To compare the neuroprotective and anti-dementia pharmacological effects of chiral oxiracetam, glutamate and calcium ions were used to establish neuronal injury models in vitro, and the protective effects of chiral oxiracetam on primary neurons were assayed by MTT. Permanent bilateral common carotid artery occlusion (2-VO)-induced rats were randomly divided into sham group, model group, galantamine 3 mg‧kg-1 group, oxiracetam groups (30, 100 and 200 mg‧kg-1), S-oxiracetam groups (30, 100 and 200 mg‧kg-1) and R-oxiracetam 200 mg‧kg-1 group. The animal experiments in the present study were performed in accordance with the Ethical Guidelines of the Laboratory Animal Welfare Ethical Committee of Peking Union Medical College. Morris water maze and step-down test were applied to evaluate the cognitive dysfunction induced by cerebral hypoperfusion in rats. Oxiracetam, S-oxiracetam and R-oxiracetam exerted protective effects on primary neuronal damage caused by various stimuli, and oxiracetam and S-oxiracetam showed better neuro-protective effects. Morris water maze and step-down results showed that oxiracetam, S-oxiracetam and R-oxiracetam improved the cognition of 2-VO rats. In summary, S-oxiracetam exerted a better neuro-protective effect than oxiracetam and R-oxiracetam.

@#To investigate the effects and possible molecular mechanism of S-oxiracetam(S-ORC) on learning and memory impairment in mice, mice were divided into 5 groups, control group, model group, high-dose of S-ORC (0.96 g/kg), medium-dose of S-ORC (0.48 g/kg) and low-dose of S-ORC (0.24 g/kg) treatment groups.Step-down test and Y-maze test were used to investigate the effects of S-ORC on the brain.The results of step-down test revealed that the mice in high and medium-dose groups could significantly decrease the reaction time, fault times and prolong the incubation periods of memory compared with the model group.Compared with the model group, the fault times of mice in high and medium-dose groups decreased significantly and the right times to find the safety increased significantly in Y-maze test.Furthermore, through treatment with S-ORC (high and medium-dose groups), the content of Ach in mice brain was significantly higher than that in model group, and the level of AChE decreased significantly.The above results suggest that the underlying mechanism of S-ORC on learning and memory impairment in mice may include the amelioration of the central cholinergic nervous system.

ABSTRACT

OBJECTIVE:To identify the main unknow impurity of Oxiracetam capsule and determine its content ,so as to improve the standard of quality control. METHODS :Two-dimensional UPLC -IT-TOF-MS was adopted to qualitatively analyze the unknown impurity. One-dimensional liquid chromatogram analysis was performed on ST PAK C 18 ES column with mobile phase consisted of 0.02 mol/L sodium dihydrogen phosphate solution at the flow rate of 0.5 mL/min. The column temperature was set at 30 ℃,sample size was 20 μL. The detection wavelength was set at 210 nm. Two-dimensional liquid chromatogram analysis was performed on Techmate C 18-STⅡ column with mobile phase consisted of 0.02 mol/L ammonium acetate solution at the flow rate of 0.5 mL/min. The column temperature was 30 ℃. Mass spectrometry was adopted (electropray ionization source ,MS+ and MS - mode data acquisition ). After the target impurity was located by one-dimensional liquid chromatography ,it was transferred to two-dimensional liquid chromatography-mass spectrometry system for qualitative analysis. The unknown impurity structure was inferred by means of molecular formula prediction module “Accurate Mass Calculator ”in LCMS Solution ,and the refined impurity products by preparation and purification were standardized and confirmed . The impurity content was determined by HPLC (with the same condition of one-dimensional liquid chromatography for qualitative analysis ). RESULTS :The main unknown impurity in Oxiracetam capsules is oxiracetam acid. The content of the refined product was 99.5% after preparation and purification. The contents of oxiracetam acid in 9 batches of Oxiracetam capsules were 0.05% -0.14% . CONCLUSIONS :The established two-dementional UPLC-IT-TOF-MS method can accurately locate the peak position of the impurity oxiracetam acid ,and analyze its structure,while the corresponding content determination method can better separate the impurity from the main drug and other components,with good sensitivity ,precision,repeatability,stability and accuracy. The quality of the finished product of Oxiracetam capsules can be well controlled by using above method .

ABSTRACT

Oxiracetam (ORC) is a commonly used nootropic drug for improving cognition and memory impairments. The therapeutic effect and underlying mechanism of ORC in vascular dementia (VaD) treatment remain unknown. In this study, 3-month-old male Sprague-Dawley rats with permanent bilateral common carotid artery occlusion-induced VaD were treated orally with low (100 mg/kg) or high (200 mg/kg) dose ORC once a day for 4 weeks. The results of the Morris water maze test and Nissl staining showed that ORC treatment significantly alleviated learning and memory deficits and neuronal damage in rats with VaD. Mechanistically, the protein levels of a panel of genes associated with neuronal apoptosis (Bcl-2, Bax) and autophagy (microtubule-associated protein 1 chain 3, Beclin1, p62) were significantly altered by ORC treatment compared with VaD, suggesting a protective role of ORC against VaD-induced neuronal apoptosis and autophagy. Moreover, the Akt/mTOR pathway, which is known to be the upstream signaling governing apoptosis and autophagy, was found to be activated in ORC-treated rats, suggesting an involvement of Akt/mTOR activation in ORC-rendered protection in VaD rats. Taken together, this study demonstrated that ORC may alleviate learning and memory impairments and neuronal damage in VaD rats by altering the expression of apoptosis/autophagy-related genes and activation of the Akt/mTOR signaling pathway in neurons.

ABSTRACT

Objective:To establish a method for the determination of the potential genotoxic impurities [ methyl chloroacetate and ( R, S) 4-ethyl chloro-3-hydroxybutyrate ] in oxiracetam raw material .Methods:GC-MS was conducted , and the potential genotoxic impurities were extracted by ethyl acetate .The column was a VF-1701 ms capillary column (30 m ×0.25 mm, 0.25 μm) with pro-grammed temperature and the inlet temperature was 220 ℃.The column flow was 1.0 ml· min-1 and the purge flow was 5.0 ml· min-1 .The split injection was used and the split ratio was 5:1 .The carrier gas was high purity helium and the detector was a mass spectrometer detector .The ion source temperature was 230℃and the interface temperature was 230 ℃.The delay time of solvent was 4 min and the ionization mode was electron impact .The scanning ( detection ) method was selective ion monitoring , the electron energy was 70 eV, and the injection volume was 1.0 μl.Results:The separation between the impurities met the requirements .The concen-tration linear range was 50-400 ng· ml-1 (r≥0.9995).The recoveries were 89.7%-96.3% (RSD=2.3%, n=9) and 91.0%-105.3%(RSD=4.4%, n=9), respectively.Conclusion:The method is simple, accurate, sensitive and rapid, and can be used for the determination of two potential genotoxic impurities in oxiracetam raw material .

ABSTRACT

Objective@#To explore the therapeutic effects of edaravone, oxiracetam combined with Shuxuetong on cerebral hematoma and improvement of neurological function in patients with cerebral hemorrhage.@*Methods@#A total of 96 patients with cerebral hemorrhage were randomly divided into observation group and control group by the random number table method, with 48 cases in each group.The observation group received intravenous drip of edaravone (4.0g added into 0.9% sodium chloride injection to 250mL, intravenous drip, 1 time/d), oxiracetam(30mg added into 0.9% sodium chloride injection to 100mL solution, 30min intravenous drip, 2 times/d), and Shuxuetong injection(4mL added into 0.9% sodium chloride injection to 250mL, intravenous drip, 1 time/d) on the basis of routine treatment, and the control group was treated with routine treatment for cerebral hemorrhage.The efficacy and safety in the two groups after treatment of 21 days were observed.@*Results@#After treatment, the neurological deficit score and cerebral hematoma volume of the two groups were improved(all P<0.05), but the improvements of the observation group was significantly better than those of the control group[(4.44±3.20)points vs.(10.53±2.86)points, (21.83±3.85)points vs.(26.71±3.49)points, (7.83±5.31)mL vs.(13.74±6.16)mL, t=9.818, 6.506, 5.035, all P<0.01]. The total effective rate of the observation group was higher than that of the control group(87.5% vs.52.1%, χ2=14.740, P<0.05). No drug-related adverse reactions occurred in two groups.@*Conclusion@#The application of oxiracetam and edaravone combined with Shuxuetong injection in patients with cerebral hemorrhage can effectively improve the brain edema and nerve defect, it has a positive significance to promote the patients' recovery after cerebral hemorrhage, and it is worthy of clinical application.

Objective@#oevaluateclinical curative effect of oxiracetam injection in the treatment of delayed encephalopathy after acute carbon monoxide poisoning (DEACMP) .@*Methods@#Methods 52 patients with DEACMP were randomly divided into the observation group and the control group, 27 cases in the observation group and 25 cases in the control group. The 2 groups were adopted the treatment to improve the cerebral microcirculation and other symptomatic, the observation group on the basis of treatment for the treatment of oxiracetam Injection. Quantitative electroencephalogram (QEEG) and event-related potential P300 were used to evaluate the therapeutic effects of the 2 groups before and after treatment.@*Results@#After treatment, QEEG value and event related potential P300 in observation group were decreased with statistically significant, respectively (P<0.05) , compared with the control group after treatment, the observation group excepted the occipital lobe, left parietal lobe, left around central and other indicators, QEEG and P300 oflatent period was shortened while the bank widens with statistical significance (P<0.05) .@*Conclusion@#Olathe injection of DEACMP patients recovery have certain curative effect.

ABSTRACT

Objective To investigate the effects of oxiracetam combined with Shuxuetong injection on hemorheology and serum Hcy levels in patients with cerebral infarction. Methods 60 patients with cerebral infarction were randomly divided into 2 groups, 30 cases in the observation group and were treated with oxiracetam combined with Shuxuetong injection. 30 cases in the control group were treated with Shuxuetong injection. The hemorheology and serum Hcy levels in the two groups were compared. Results After treatment, the hemorheological indexes and serum Hcy levels in the observation group were lower than those in the control group (P<0.05). The treatment effect of the observation group was better than that of the control group (P<0.05). The scores of neurological deficits in the observation group were significantly better than those in the control group (P<0.05). The number of complications in the observation group was 2 cases, the incidence of complications was 6.7%, which were significantly lower than 9(30.0%) in the control group, the differences between the two groups was statistically significant (P<0.05). Conclusion The application of oxiracetam combined with Shuxuetong injection in patients with cerebral infarction can effectively improve the hemorheology and reduce the serum Hcy level.

ABSTRACT

Objective To compare the effects of piracetam and oxiracetam on elderly cognitive dysfunction after cerebral hemorrhage.Methods Elderly patients (82 cases) with cerebral hemorrhage treated in Tongchuan People's Hospital from January 2012 to December 2015 were selected and divided into two groups according to the different treatment drugs.Two groups were treated by oxiracetam and piracetam respectively.The Montreal cognitive assessment scale (MoCA),simple mental state examination (MMSE),and daily life ability scale (ADL) score of the two groups before and after treatment were compared.The neuron-specific enolase levels and cognitive function after treatment of 1,3,and 6 months were also compared.Results After treatment,MoCA,MMSE,and ADL scores of two groups were significantly improved (P < 0.05),and oxiracetam group were significantly better than piracetam group (P <0.05).After treatment 6 months,MoCA,MMSE,ADL effective and total effective rate of the two groups were significantly higher (P <0.05),and oxiracetam group were significantly better than those ofpiracetam group (P < 0.05).After treatment for 1,3,and 6 months,the neuron-specific enolase levels were significantly lower than those before treatment (P < 0.05),and in the treatment of 3 and 6 months,oxiracetam group were significantly lower than piracetam group (P < 0.05).The incidence of adverse reactions had no significant difference.Conclusion Oxiracetam can improve the elderly cognitive dysfunction after cerebral hemorrhage,the curative effect was increased with the prolonging of the time,and can reduce the neuron-specific enolase levels,which has high clinical value.

ABSTRACT

Objective To investigate the therapeutic effect of oxiracetam combined with dexamethasone for treatment of patients with severe acute carbon monoxide poisoning (ACOP).Methods Eighty-seven patients with severe ACOP admitted to Affiliated Hospital of Xuzhou Medical University from January 2013 to July 2016 were enrolled, and they were divided into observation group (47 cases) and control group (30 cases) according to random number table method. The two groups were given conventional nerve nutrition, hyperbaric oxygen and symptomatic treatment, while in the observationgroup, on the basis of conventional treatment the patients received intravenous drip of dexamethasone 5 mg (with addition of normal saline 100 mL), once a day for consecutive 3 days, and intravenous drip of oxiracetam 3.0 g plus 100 mL normal saline, once a day for consecutive 7 days. The changes of mini-mental state examination (MMSE), Barthel index and Glasgow coma scale (GCS) were observed after discharge for 30 days, and the differences of the incidence of delayed encephalopathy after acute carbon monoxide poisoning (DEACMP) and hospitalization time between the two groups were compared.Results After treatment the MMSE and GCS scores of the two groups were higher than those before treatment, and the Barthel score was lower than that before onset. After discharge for 30 days, MMSE score (26.93±2.92 vs. 24.20±6.82), GCS (14.41±1.32 vs. 13.98±2.13), Barthel (94.78±12.62 vs. 85.25±19.57) of the observation group were significantly higher than those of control group, and the incidence of DEACMP was obviously lower than that in the control group [8.5% (4/47) to 17.5% (7/40)], the differences between the two groups being statistically significant (allP 0.05).Conclusion Early combined application of oxiracetam and dexamethasone can significantly improve the cognition and self-care abilities and reduce the incidence of DEACMP in patients with severe ACOP after discharge for 30 days.

ABSTRACT

Objective To observe the efficacy and safety of the cocktail therapy to treat patients with Parkinson's disease with dementia ( PDD) . Methods Sixty patients with PDD were randomly divided into treatment group(30 cases)and control group(30 cases). The dose 10 mg of donepezil hydrochloride was taken 1 time a day in the control group;on the basis,the dose 0. 2 g of butylphthalide soft capsules was taken pre-dinner, 3 times a day,the dose 0. 8 g of oxiracetam was taken 3 times a day and 2 Ginkgo Biloba Leaves Extract tablets was taken 3 times a day in the treatment group,period of treatment was 6 months. The curative effect was measured by using Montreal cognitive assessment scale (MoCA),Blessed-Roth dementia table,severity of clinical dementia rating scale (WMS) rating, respectively before treatment and on the 3th, 6th month after treatment,and monitored the adverse reactions. Results There were no statistically significant differences on the scores of MoCA, WMS and Blessed-Roth dementia table between treatment group and control group before treatment ( all P>0. 05 ) . The scores of MoCA, WMS in the treatment group and control group on 6 months after treatment were significantly higher than those before treatment and 3 months after treatment, and the scores of Blessed-Roth dementia table in the treatment group and control group on 6 months after treatment were significantly lower than those before treatment and 3 months after treatment ( P0. 05). Conclusion The curative effect of cocktail therapy is better than traditional remedies for treating patients with PDD, and it is high safety.

Objective:To establish a bacterial endotoxins testing method for L-oxiracetam injection. Methods: According to the gel-clot technique for bacterial endotoxin inspection and the related regulations in Chinese Pharmacopoeia (2015 edition, volume Ⅳ, general rule 1143), tachypleus amebocyte lysate (TAL) from two different manufacturers were used for bacterial endotoxins test on 3 batches of L-oxiracetam injection with methodology studies. Results:Diluted to 8. 3 mg·ml-1 , the samples showed no interference a-gainst the bacterial endotoxins test. Conclusion:The bacterial endotoxins test method for L-oxiracetam injection is applicable with the endotoxin limit of 3 EU·ml-1 .

ABSTRACT

Objective To investigate the effect of oxiracetam on (Hcy), high sensitive C-reactive protein (hs-CRP), uric acid (UA) and cysteine protease inhibitor (Cys C) and blood lipid index level in patients with acute cerebral hemorrhage.Methods 98 patients with acute cerebral hemorrhage were randomly divided into observation group ( n =49 ) and control group ( n =49 ) .The control group was given routine treatment, the observation group was given oxiracetam treatment on the basis of control group.Two groups were treated for 2 weeks.Results The observation group after treatment NIHSS score lower than control group(P<0.05), GCS score and ADL score higher than control group (P<0.05);The serum levels of Hcy, hs-CRP, Cys, C levels lower than control group post-treatment, while the level of UA was higher than that of control group (P<0.05);The observation group TG, TC, LDL-C levels were lower than control group post-treatment ( P <0.05 ); Two groups had no severe adverse reactions. Conclusion Oxiracetam by reducing and Hcy in patients with acute cerebral hemorrhage hs-CRP, Cys C level, increased UA level, and improve blood lipid function, and thus improve symptoms.

ABSTRACT

OBJECTIVE:To observe the clinical efficacy and safety of cinepazide maleate combined with oxiracetam in the treat-ment of vascular dementia. METHODS:62 patients with vascular dementia were randomly divided into treatment group and control group,with 31 patients in each group. Patients in control group were treated with Oxiracetam injection 4g/d for 3weeks. Patients in treat-ment group were treated with cinepazide maleate 160 mg/d for 3 weeks on the basis of control group. The patients of 2 groups were exam-ined with Mini-mental State Examination (MMSE)and Activity of Daily Living Scale (ADL)before and after treatment. RESULTS:There was statistically significant difference in MMSE and ADL scores of 2 groups before and after treatment(P<0.05);MMSE score and ADL score of treatment group was better than that of control group,with statistical significance(P<0.05);Obvious ADR was not found in both groups. CONCLUSIONS:Cinepazide maleate combined with oxiracetam can significantly improve ADL and MMSE score in patients with vascular dementia,improve the quality of life,showing significant and good clinical efficacy.

ABSTRACT

Objective To study the effects of β-aescine sodium plus oxiracetam on plasma homocysteine (Hcy) and monocyte chemotactic factor(MCP-1) protein in patients with acute cerebral infarction.Methods 117 patients with acute cerebral infarction were randomly divided into the two groups,the observation group (n =60 cases) and the control group (n =57 cases).The patients in the control group were treated with the conventional treatment,while the patients in the observation group were treated with joint treatment ofβ-aescine sodium plus oxiracetam.They were treated for 14 days.Plasma Hcy and MCP-1 were detected.Results The total efficicies were 95.0% in the observation group and 84.5% in the control group.There was a significant difference between two groups (x2 =5.801,P <0.05).Plasma Hcy and MCP-1 were decreased after treatment (t =9.551,4.296,2.001,21.070,P < 0.05,P <0.01).Plasma Hcy and MCP-1 in the observation group were lower than these in the control group (t =4.997,25.276,all P < 0.05).Conclusion β-aescine sodium plus oxiracetam can decrease plasma Hcy and MCP-1 in patients with acute cerebral infarction.

Objective To observe the efficacy of combination therapy with nimodipine and oxiracetam for type 2 diabetes melli‐tus(T2DM)patients with mild vascular cognitive impairment(mVCI) .`Methods Seventy‐two T2MD‐mVCI patients were divided into control group and observation group and each group was 36 cases .Control group was given routine treatment and nimodipine 40 mg ,3 times every day ;observation group was given oxiracetam 800 mg ,2 times every day ;the treatment course was 6 months .fast‐ing blood glucose(FBG) ,2 h postprandial hyperglycemia(2 h PBG) ,glycosylated hemoglobin A1C(HbA1C) ,C peptide level were recorded and MoCA cognitive function scale was used to evaluated patients cognitive function of each group before and after the treatment .Results Before the treatment ,both two groups with C peptide level were declined and mild cognitive impairment ,there were no statistically significant differences(P>0 .05) .After the treatment ,C peptide level ,MoCA scores and cognitive function sub‐scores of memory ,executive ,attention were significantly higher than before(P<0 .05);the observation group compared with the control group improved significantly(P< 0 .01) .Conclusion Combination therapy with nimodipine and oxiracetam could signifi‐cantly improve cognitive function in patients with T2DM‐mVCI ,its efficacy is superior to nimodipine .

ABSTRACT

Objective To study the influence of oxiracetam plus mouse nerve growth factor(mNGF) on serum lysophosphatidic acid 2(Lp-PLA2) and S 100B protein in patients with acute cerebral infarction.Methods 124 patients with acute cerebral infarction were randomly divided into the observation group (n =62 cases) and the control group(n =62 cases).The patients in the control group were treated through the conventional treatment,while the patients in the observation group were treated by oxiracetam plus mNGF on the basis of conventional therapy.They were treated for 21 days.Serum Lp-PLA2 and S100B were detected.Results The total effective rate of the observation group was 98.4%,which was significantly higher than 90.3% of the control group (x2 =3.96,P < 0.05).Serum LpPLA2 and S100B were decreased in two groups after treatment.Serum Lp-PLA2 and S100B in the observation group were lower than those in the control group(t =20.9091,3.7273,all P < 0.05).Conclusion Oxiracetam plus mNGF can decrease serum Lp-PLA2 and mNGF in patients with acute cerebral infarction.

ABSTRACT

AIM: To compare the efficacy and safety of oxiracetam and donepezil in treating Alzheimer disease with mild and moderate MMSE scores.METHODS: 68 patients were randomly divided into oxiracetam group(p.o.,n=34) and donepezil group(p.o.,n=34).The MMSE and Blessed-Roth measuring scales were used to evaluate the curative effects before treatment and three months after treatment.RESULTS: There were significant differences between the indexes expect for activities of daily living before and after treatment in two groups(P0.05).CONCLUSION: The curative effect and safety of oxiracetam and donepezil in treating mild and moderate Alzheimer disease are similar.
