ABSTRACT

@#In evidence-based family practice, we place great emphasis on shared decision-making with the goal of helping patients make informed decisions about their care and subsequently to improve patient health outcomes. Appropriate decision-making in family practice is affected by information delivery and patient education and interpersonal clinician-patient communication. Effective communication is essential for improved health outcomes, adherence to treatment and patient satisfaction. Informed patients are more likely to participate actively in their care, make wiser decisions, come to a common understanding with their physicians, and adhere more fully to treatment. Communicating evidence can transform a physician-dominated relationship into one that is patient-centered relationship. Techniques to improve patient communication have been associated with greater satisfaction especially when patients are involved in the decision-making process. Research about medical evidence in a particular clinical encounter has shown equally important elements: a) expertise of the provider; b) medical evidence; and c) the patient’s preferences, goals and concerns. These elements though separate, but when combined, results to a well-informed medical decision emphasizing the role of medical evidence in future communication and patient-engagement activities. Models of patient-centered communication are recommended.

La comunicación entre los profesionales de la salud y los pacientes es considerada como una herramienta primordial, dentro del modelo de atención centrado en el paciente debido a que afecta las intervenciones en salud. Los estudios relacionados con el tema enuncian el término denominado; "comunicación centrada en el paciente", con el objetivo de ayudar a los profesionales a proporcionar atención en salud, concordante con los valores, las necesidades y preferencias del paciente, sin embargo, se halla un vacío en el conocimiento al abordar la comunicación enfermera-paciente. Objetivo: proponer una conceptualización de la comunicación enfermera-paciente en el marco del Modelo de atención centrado en el paciente. Metodología: Revisión integrativa de literatura, guiada por la metodología de (Whittemore & Knafl, 2005), la búsqueda de literatura se realizó en cinco bases de datos, un repositorio institucional y búsqueda manual de referencias, los límites de tiempo fueron 2006 a 2017; idiomas, español, inglés, portugués, el análisis de la información se realizó mediante la comparación constante y los elementos de análisis de concepto evolutivo de Rodgers, como foco. Resultados: se obtuvieron 34 estudios, identificando categorías de análisis; elementos, atributos que caracterizan el concepto y consecuencias de la comunicación dentro del modelo de atención centrado en el paciente. Conclusiones: para la conceptualización de la comunicación enfermera-paciente no existe una única teoría o modelo conceptual dentro de la disciplina o fuera de ella que pueda definirlo, pues este concepto varía de acuerdo a la perspectiva teórica y el contexto del paciente.


Communication between health-care professionals and patients is regarded as a fundamental tool within the patient-centered care, due to the fact that affects health interventions. Studies related with the topic enunciate the term refereed as patient-centered communication, with the objective of helping professionals to provide attention in health care according to the patient's values, necessities, and preferences; nevertheless, there is a gap in the knowledge when addressing nurse-patient communication. Objective: To propose a conceptualization for nurse-patient communication, framed within the patient-centered care. Methodology: Integrative literature review, guided by Whittemore & Knafl's methodology (2005). The literature review was carried out in five databases, an institutional archive, and manual search of references; for such literature review the time span used to narrow the results was from 2006 to 2017. The articles reviewed were in Spanish, English, and Portuguese; the information analysis was done by means of constant comparison, and as a focal point the elements of Rodgers' evolutionary concept analysis. Results: 34 studies were obtained, identifying analyses categories: elements, features that characterize the concept, and communication consequences within the patient-centered care. Conclusions: For the conceptualization of the nurse-patient communication, does not exists an exclusive theory or conceptual model within or outside the field of study which can define it, as this concept changes according to the theoretical perspective and the patients context
