ABSTRACT

Objective: Chemotherapy-induced nausea and vomiting (CINV) can affect a patient’s quality of life and make them resistant to the treatment. We created an electronic patient reported outcome ePRO-linked pharmaceutical management system (PMS) for CINV (CINVePRO) for storing information, such as nausea and vomiting status, food intake, etc., and suggesting the type of anti-nausea medication and dosage changes to the physicians for controlling CINV.Design: At the Gifu University Hospital, the collaborative research institute, inpatients and pharmacists in charge used CINVePRO-PMS, and a questionnaire survey was done to assess the system’s reliability. Methods: The daily entry of data into CINVePRO shows the number and duration of vomiting, degree of nausea, and amount of food consumed and displays a list and graph of these data over time. The PMS enables pharmacists to list the presence or absence of nausea and the number of vomiting for all patients in their charge and record the intervention and display its list. Results: The questionnaire was distributed to 17 inpatients. All patients and pharmacists answered the questionnaire. According to the results of the questionnaire survey of patients, each screen of CINVePRO received a good evaluation that mentioned it was “easy to understand,” “easy to use,” and “especially useful for communicating one’s symptoms.” In addition, the results of a questionnaire survey of the pharmacists revealed that the system was rated as easy to check the patients’ symptoms and practical to use. Conclusion: CINVePRO-PMS was evaluated as a convenient and applicative system. However, linking CINVePRO to the electronic medical record of each hospital is necessary for sharing it among multiple professions.

ABSTRACT

Objective:To analyze the effects of prescription reviews of key monitored drugs using the drug regulatory platform in a district of Shanghai, and the influencing factors for the rate of unqualified prescriptions. Methods:A drug regulatory platform in a district in Shanghai began to implement prescription reviews since June 2018, extracting the reviewed prescriptions of all key monitored drugs and related information from June to December. Taking the unqualified rate of prescription in June as the baseline data, the χ2 test and the logistics regression model were used for data analysis. Results:From June to December 2018, clinical pharmacists reviewed 19 084 outpatient and emergency prescriptions and 10 607 inpatient prescriptions. The unqualified rates of outpatient and emergency prescriptions and inpatient prescriptions dropped from 16.52% and 23.28% in June to 4.45% and 13.80% in December, respectively. Logistics regression analysis showed that the rate of unqualified reviews of outpatient and emergency prescriptions was lower than that of inpatient prescriptions, and the rate of unqualified prescriptions in hospital was significantly lower than that in community health service centers; the rate of unqualified prescriptions for auxiliary drugs and nutritional drugs was significantly lower than that of antibacterial drugs. Conclusions:The prescription reviews of key monitored drugs can effectively promote the rational use of drugs. In the future, it is recommended to strengthen technical support for community health service centers and professional training for clinicians and pharmacists.

ABSTRACT

Objective: In order to increase the efficiency and effectiveness of pharmacotherapy and maximize the benefit/risk ratio, patient follow-up after medication is imperative. Such follow-up includes medical adherence, drug efficacy, early detection and treatment of adverse events. All pharmacy pharmacists should, at their service counter, select and correctly identify the most noticeable symptoms of adverse drug reactions for the patient, especially with their other roles and responsibilities. Thus, we evaluated the usefulness of M*Adhere SPEHEC® in pharmaceutical management. Methods: During the month of April 2019, 21 pharmacists at six pharmacies responded to the survey. Results: Of 21 pharmacists, 18 (85.7%) stated that the efficiency and/or quality of pharmacotherapy management had improved. The system enabled evidence-based identification of drug adverse event symptoms, and the time for pharmaceutical care was reduced by an average of 2 min. Conclusion: Based on these results, M*Adhere SPEHEC® was confirmed to be highly useful in improving the efficiency and quality of pharmaceutical management for patient follow-up.

ABSTRACT

OBJECTIVE:To provide reference for establi shing evidence-based pharmaceutical management and assessment system in China. METHODS :Through literature study and new medical policy interpretation ,the development course of hospital-based health technology assessment (HB-HTA)and its application in pharmaceutical management at home and abroad were traced;the development prospect of HB-HTA in China ’s hospital drug management was discussed. RESULTS & CONCLUSIONS: The HB-HTA makes scientific evaluation on the process of access and use of medical technology ,so as to optimize the allocation of hospital resources and ensure the quality of medical treatment. Foreign countries have used HB-HTA to assist the decision-making of hospitals. China ’s HB-HTA is still in its initial stage and lags behind the international leading level. As effective means for cost control and quality improvement ,HB-HTA should be applied in drug management and assessment system as soon as possible;it is necessary to explore suitable drug assessment mode (i.g. ambassaclor mode ,Mini-HTA mode ,internal committee mode,HB-HTA unit mode ),strengthen HTA personnel training ,establish HTA methodology (such as Mini-HTA mode assessment tool included outline ,general information ,general methods ,multi-structure evaluation results ,discussion, conclusions and recommendations),pay more attention to it and promote the transformation of achievements.

ABSTRACT

Case report:A 69 year-old man with an end-stage lung carcinoma had difficulty to come to our out-patient clinic due to severe pain and familial reasons. The reports from his sister were limited and his condition seemed no improvement. A certified palliative care pharmacist visited his home to assess his current status and situation in detail in order to provide better care for him. Result:Direct and accurate assessment of his pain status and compliance to his medication allowed our palliative team to provide tailored instructions and alleviate his suffering. Home visit helped to create better communication between the patient and the team and relieve his anxiety. The information from the visit provided his doctor with objective assessment of his current situation, which resulted in tailored prescriptions. This whole process was also effective to his smooth admission when he finally deteriorated. Summary:Home visit assessment and management by hospital pharmacists was very useful to the patients who have difficulty to come to the clinic by improving his quality of home life and sharing his social and clinical information between the out-patient clinic and the hospital. The home visit can be a good option for such patients.

ABSTRACT

PREAVOID is pharmaceutical intervention that is utilized to illustrate pharmacists’ contributions to medical care. Currently, there is a great need for pharmacists to provide good medical services to inpatients; as a result, many pharmacists conduct pharmaceutical management in hospital wards. However, pharmacists who have limited experience in working in the ward do not know exactly what they should check with respect to pharmaceutical management. To resolve this problem, we determined 16 pharmaceutical-management items based on PREAVOID that was conducted at Nagara Medical Center. Moreover, we conducted a pre-questionnaire survey assessing whether pharmacists who had worked in the ward for fewer than 4 years attended to these 16 check items in their daily work prior to our introducing the list to them. The results indicated that pharmacists who had fewer than 2 years of experience working in wards attended to the 16 check items less than those who had more than 2 years of experience, and approximately half of the pharmacists had not received adequate guidance before beginning work at the ward. In addition, most pharmacists indicated that clear check points were useful for conducting pharmaceutical management and the 16 check items were useful for their daily work. These results indicate that the 16 check items are a useful educational tool for enabling pharmacists to conduct high quality pharmaceutical management from the initial stage and that using the 16 check items is superior to pharmacists only gaining this ability via prolonged experience working in the ward.

ABSTRACT

Objective To understand the types and prices of drugs in outpatient pharmacies and retail pharmacies, study their differences and provide corresponding suggestions on policies. Methods The types and prices of drugs in 7 pharmacies and 8 hospital outpatient pharmacies in 4 provinces were studied and compared. Results Significant differences were found in the types and prices of drugs in these outpatient pharmacies and retail pharmacies. Retail pharmacies might fail to fill 60% of the physician prescriptions. The quality and price control of drugs in outpatient pharmacies was different from that in retail pharmacies. The types and prices of drugs in outpatient pharmacies were different from those in retail pharmacies. Many drugs independently-priced were extremely high. Pharmaceutical departments of hospitals had abundant senior and qualified pharmaceutical personnel whose pharmaceutical expertise was far superior to that of retail pharmacies. Conclusion There was a significant difference between outpatient pharmacies and retail pharmacies. Under the circumstances, it is not yet feasible to release policies and regulations that would separate pharmacies from hospitals or encourage outpatients to fill their prescriptions in retail pharmacies. Medical institutions should further standardize their purchases of drugs as well as prescriptions by physicians.
