ABSTRACT

ObjectiveTo explore the policy framework, theoretical system and principles of educational placement for children with special needs based on the International Classification of Functioning, Disability and Health (ICF) for the multi-faceted educational placement services and methods for these children. MethodsBased on ICF theory and methods, public policy research techniques, and educational policy analysis, this study systematically investigated the policy architecture and theoretical underpinnings for the educational placement of children with special needs, focusing on an inclusive education-oriented system of multiple placements. ResultsThe study analyzed educational policies, emphasizing the rights to education under the United Nations Convention on the Rights of Persons with Disabilities (CRPD) and UNESCO's guidelines on ensuring inclusivity and equity in education which encourage the provision of individualized educational support services and reasonable accommodations to enable the effective participation of students with disablities in education. China, the European Union (EU) and the United States (US) have enacted laws and policies promoting inclusive education, integrating children with disablities into the general education system, and providing them with the same educational opportunities as other children. The development of special education focuses on tailored educational services for those children who need additional support and resources. Policies underscore the need to evaluate the specific needs of children with disablities and provide individualized educational plan based on these needs. ConclusionBased on core content from the CRPD, UNESCO's guidelines, and relevant policies from China, the EU, and the US regarding the education and educational placement services for children with special needs, the theoretical framework and principles of educational placement for children with special needs based on ICF are discussed, proposing contents and methods for constructing a multi-faceted educational placement service system for children with special needs.

ABSTRACT

Situs inversus totalis (SIT) is a rare congenital condition, with an extremely low incidence. There is no difference between SIT individuals without onset of diseases and healthy counterparts. However, when SIT individuals suffer from diseases, the diagnosis and treatment are highly challenging due to insufficient understanding of SIT populations, especially for those complicated with end-stage liver disease and requiring liver transplantation. It is a huge challenge for surgeons whether SIT individuals serve as donors or recipients of liver transplantation. In this article, recent case reports related to liver transplantation in SIT patients were summarized, and the development, key procedures, clinical prognosis and postoperative complications of liver transplantation in SIT patients were reviewed.

ABSTRACT

Objective@#To investigate the effect of the socket-shield technique (SST) concurrent with immediate implant placement and provisionalization (IIPP) in the aesthetic restoration of anterior teeth.@*Methods@#A case of maxillary anterior tooth stumps with a thin labial bone wall was treated with SST for preservation of labial soft and hard tissue fullness, combined with an immediate implant placement and immediate provisional crown for restoring the shape of the tooth and gingival molding@*Results@#Immediate implant placement and provisionalization restored the morphology and function of the affected tooth in the shortest possible time. The patient's labial soft and hard tissue contours in the affected tooth area were well preserved in the 18-month follow-up after the application of the SST, which presented a better aesthetic result. The literature review indicates that the indications for SST are unrestorable maxillary anterior teeth, whose dental, periodontal and periapical tissues are healthy and intact. In the esthetic zone, root shielding is effective in maintaining the soft and hard tissue contour on the labial side of the implant. However, there is no consensus on the technical details of SST, such as the ideal coronal height and thickness of the shield, and the management of the gap between the shield and the implant. Thus, more clinical studies and histologic evidence are needed to provide a reference for clinical decision-making. In addition, digital technology can improve the accuracy of implant placement and shield preparation.@*Conclusion@#The correct application of SST combined with IIPP in the esthetic zone can ensure esthetic results. However, more high-quality evidence-based medical evidence is needed for its long-term efficacy, and indications should be strictly controlled during clinical application.

ABSTRACT

Introdução: O acompanhamento odontológico no pré-natal representa estratégia singular com atuação interdisciplinar na prevenção de agravos à gestante e complicações no parto. Objetivo: Identificar a interdisciplinaridade no acompanhamento odontológico no pré-natal na perspectiva do enfermeiro. Metodologia: Estudo com abordagem qualitativa fundamentado na Hermenêutica-dialética de Gadamer e Habermas em seis Unidades de Atenção Primária à Saúde de uma metrópole do Nordeste do Brasil, com seis enfermeiros por meio de entrevistas em profundidade no ano de 2018. A organização dos dados ocorreu através da análise temática de Minayo. Resultados: Evidenciaram-se após análise temática as categorias, ''O cuidado interdisciplinar do médico, odontólogo e enfermeiro''e ''Encaminhamento do enfermeiro durante o acompanhamento odontológico no pré-natal". Há implicações para o desenvolvimento de práticas interdisciplinares no acompanhamento odontológico no pré-natal, como fragilidades na continuidade do cuidado, na comunicação e interação na equipe. Conclusão: Há necessidade de ampliar a integralidade para qualidade no desenvolvimento de cuidados em saúde à gestante e diminuição de complicações maternas e neonatais em populações de mulheres brasileiras.


Introducción: El seguimiento odontológico en el control prenatal representa una estrategia única con acción interdisciplinaria para la prevención de lesiones en la gestante y complicaciones durante el parto. Objetivo: Identificar la interdisciplinariedad en la atención odontológica prenatal desde la perspectiva de las personas profesionales de enfermería. Metodología: Estudio con un enfoque cualitativo basado en la hermenéutica-dialéctica de Gadamer y Habermas en seis unidades básicas de salud de una metrópolis del Nordeste de Brasil, con seis profesionales de enfermería, a través de entrevistas en profundidad en 2018. La organización de los datos se realizó mediante el análisis temático de Minayo. Resultados: A partir del análisis temático, surgieron las categorías ''El cuidado interdisciplinario de la persona profesional en medicina, odontología y enfermería''y ''Remisión de enfermería durante el prenatal odontológico". Existen implicaciones para el desarrollo de prácticas interdisciplinarias en el prenatal odontológico, como debilidades en la continuidad del cuidado, comunicación e interacción en el equipo. Conclusión: Existe la necesidad de ampliar la integralidad para la calidad en el desarrollo de la atención de la salud de la mujer embarazada y la reducción de las complicaciones maternas y neonatales en las poblaciones de mujeres brasileñas.


Introduction: Dental follow-up in prenatal care represents a unique strategy with interdisciplinary action for the prevention of injuries in pregnant women and complications during childbirth. Objective: To identify the interdisciplinarity in prenatal dental care from the nurses' perspective. Methodology: This will be a qualitative approach study with based on the Gadamer and Habermas's hermeneutics-dialectic in six primary health care units in a metropolis in Northeast Brazil, with six nurses through in-depth interviews in 2018. The organization of the data occurred through Minayo's thematic analysis. Results: After the thematic analysis, the categories of ''The interdisciplinary care of the doctor, dentist, and nurse''and ''Nurse referral during prenatal dental care''were evidenced. There are implications for the development of interdisciplinary practices in prenatal dental care such as weaknesses in the continuity of care, communication, and the team's interaction. Conclusion: There is a need to expand the comprehensiveness for the quality in the development of healthcare for pregnant women and the reduction of maternal and neonatal complications in populations of Brazilian women.

ABSTRACT

Objetivo: develar las tensiones percibidas por los actores comunitarios en el poblamiento y conformación del territorio durante la práctica de la Facultad de Medicina de la Universidad de Antioquia en la vereda Granizal entre 2009 y 2018. Metodología: estudio hermenéutico que usó técnicas de la teoría fundamentada para recolección y análisis. Se realizaron once entrevistas en profundidad a líderes y dos grupos focales a madres comunitarias, docentes y líderes y se analizaron con técnicas de codificación abierta, axial y selectiva. Resultados: existe una tensión permanente entre un liderazgo de diferentes matices y la unidad comunitaria, que recibe diversas amenazas por el desalojo persistente de sus espacios vitales por actores fuera de la ley ligados al narcotráfico, al vacío de Estado y a los intereses internos por el poder. Dicha tensión busca ser superada mediante el esfuerzo colectivo para aprender a subsistir y a través de los vínculos que nacen de la confianza generada con la Universidad y la Facultad de Medicina con su práctica, que han apoyado la comprensión progresiva de la determinación social del proceso salud-enfermedad, lo que ha permitido lograr mayor bienestar. Conclusiones: en Granizal existen unas luchas comunitarias constantes por vencer las privaciones de los mínimos vitales de subsistencia, en las cuales, la autonomía y el desarrollo humano se debaten entre la esperanza y la desesperanza por mantener la firmeza en la construcción del tejido social y en la búsqueda de salidas que posibiliten la vida y el bienestar.


Objective: to reveal the tensions perceived by community actors in the settlement and conformation of the territory during the internship of the School of Medicine of Universidad de Antioquia in the rural settlement Granizal between 2009 and 2018. Methods: Hermeneutic study that used Grounded Theory techniques to collect and analyze data. Eleven in-depth interviews with leaders were conducted and two focus groups with community mothers, teachers and leaders were carried out which were analyzed with open, axial and selective coding techniques. Results: there is a permanent tension between leadership with different shades and the community unity which receives various threats due to the permanent eviction of their vital spaces by outlaw actors linked to drug trafficking, to the absence of the State and to internal interests for power. This tension seeks to be overcome through the collective effort to learn to survive and through the links that are born from the trust generated with the University and the internship of the Faculty of Medicine that has supported the progressive understanding of the social determination of the health-disease process which has allowed for greater wellbeing. Conclusions: There are constant community struggles in Granizal to overcome the deprivations of subsistence vital minimums, in which autonomy and human development are debated between hope and despair to maintain firmness in the construction of the social fabric and in the search for solutions that make life and wellbeing possible.


Objectivo: amostrar as tensões percebidas pelos atores comunitários no povoamento e conformação do território durante a prática da Faculdade de Medicina da Universidade de Antioquia no vilarejo Granizal entre 2009 e 2018. Metodologia: estudo hermenêutico que usou técnicas da teoria fundamentada para coleta e análise. Realizaram-se onze entrevistas a fundo a líderes e dois grupos focais a mães comunitárias, docentes e líderes e se analisaram com técnicas de codificação aberta, axial e seletiva. Resultados: existe uma tensão permanente entre uma liderança de diferentes matizes e a unidade comunitária, que recebe diversas ameaças pelo despejar persistente de seus espaços vitais por atores fora da lei e conectados com o narcotráfico, a o vazio do Estado e aos interesses internos pelo poder. Esta tensão procura ser superada mediante o esforço coletivo para aprender a subsistir e a través dos vínculos que nascem da confiança gerada com a Universidade e a Faculdade de Medicina com sua prática, que tem apoiado a compreensão progressiva da determinação social do processo saúde-doença, o que tem permitido lograr maior bem-estar. Conclusões: em Granizal existem umas lutas comunitárias constantes por vencer as privações dos mínimos vitais de subsistência, nas quais, a autonomia e o desenvolvimento humano, se debatem entre a esperança e a desesperança por manter a firmeza na construção do tecido social e na busca de saídas que possibilitem a vida e o bem-estar.

ABSTRACT

Os objetivos deste estudo foram relatar a experiência da utilização de oficinas culinárias como estratégia didático-pedagógica na disciplina de educação nutricional, e analisar as percepções dos estudantes quanto à interdisciplinaridade e aplicabilidade na atuação profissional, pela análise de conteúdo temática. Este é um relato de experiência da utilização de oficinas culinárias durante a disciplina de Educação Nutricional no curso de nutrição de uma universidade pública. Foram quantificados os tipos de receitas selecionadas e seus respectivos princípios dietéticos e/ou dietoterápicos. Identificamos que 80% das receitas selecionadas priorizavam o conteúdo de nutrientes dos ingredientes como princípio dietético/dietoterápico e somente 20% consideraram aspectos sensoriais e culturais. Na análise qualitativa, foram identificados três eixos temáticos: (1) O olhar do estudante sobre a oficina culinária; (2) O aprendizado da oficina culinária para sua prática profissional; (3) A racionalização da alimentação. A oficina foi considerada uma experiência inovadora e enriquecedora para a formação acadêmica, principalmente pela interdisciplinaridade. Conclui-se que o uso de oficinas culinárias como estratégia didático-pedagógica interdisciplinar nos cursos de Nutrição pode trazer benefícios na formação dos futuros nutricionistas (AU)


The objectives of this study were to report the experience of using culinary workshops as a didactic-pedagogical strategy in the nutrition education discipline, and analyze the students' perceptions regarding interdisciplinarity and applicability in their professional practices through thematic-content analysis. This is an experience report on the use of culinary workshops during the nutritional education discipline in the nutrition course of a public university. The types of recipes selected and their respective dietetic/diet therapy principles were quantified. We found that 80% of the selected recipes were based on the nutritional composition of foods and only 20% considered sensory and cultural aspects. In the qualitative analysis, 3 thematic axes were identified: (1) The student's perception of the culinary workshop; (2) The learning in the culinary workshop for their professional practice; (3) The rationalization of food. The workshop was considered an innovative and enriching experience for academic training, mainly due to its interdisciplinarity. It is concluded that the use of culinary workshops as an interdisciplinary didactic-pedagogical strategy in Nutrition courses can bring benefits to the training of future nutritionists (AU).

Fundamento: El estudio teórico, el diagnóstico realizado y la experiencia de los investigadores, posibilitan formular como problema de la presente investigación: limitaciones en el desarrollo de la habilidad diagnosticar enfermedades dermatológicas en los residentes de la especialidad de Dermatología del Hospital General Provincial Docente "Dr. Antonio Luaces Iraola" de Ciego de Ávila. Objetivo: Elaborar una concepción didáctica del proceso de formación interdisciplinar de la habilidad diagnosticar enfermedades dermatológicas en los residentes de la especialidad de Dermatología, a partir de la caracterización del estado actual de esta habilidad. Metodología: Se realizó una investigación educativa con un componente descriptivo en el Hospital General Provincial Docente "Dr. Antonio Luaces Iraola" de Ciego de Ávila, en los cursos escolares desde 2016 al 2020. La población de estudio fueron los 16 residentes de 1.er año que matricularon la especialidad de Dermatología en el período de estudio. Se emplearon métodos del nivel teórico y empírico. Resultados: La caracterización realizada reveló limitaciones en el desarrollo de la habilidad diagnosticar enfermedades dermatológicas en los laboratorios de Anatomía Patológica, Microbiología y Parasitología Médica, por los residentes (100 %). La concepción didáctica del proceso de formación interdisciplinar de la habilidad diagnosticar orienta el proceso desde las actividades docentes-atencionales y prácticas de laboratorio en una consecutividad lógica y sistematización desde las diferentes formas de enseñanza y tipologías de clase. Integra la interdisciplinariedad y la utilización del método investigativo establecido en las ideas rectoras. Conclusiones: La concepción didáctica como aporte de la investigación resuelve la contradicción dialéctica entre la aplicación del método clínico y los procedimientos en la práctica de laboratorio que se da en ese proceso formativo y constituye un soporte didáctico que respalda las actividades prácticas en los laboratorios para cumplir con los objetivos del Plan de estudio de la especialidad.


Background: The theoretical study, the diagnosis conducted and the experience of the researchers make possible to formulate the problem of the present research: limitations in the development of the ability to diagnose dermatological diseases in residents of the Specialty of Dermatology of the General Provincial Teaching Hospital "Dr. Antonio Luaces Iraola" of Ciego de Avila. Objective: To elaborate a didactic conception of the interdisciplinary training process of the ability to diagnose dermatological diseases in residents of the specialty of Dermatology, based on the characterization of the current state of that ability. Methodology: An educational research with a descriptive component was conducted at the Provincial General Teaching Hospital "Dr. Antonio Luaces Iraola" of Ciego de Avila, in the 2016-2020 school years. The study population consisted of the 16 first-year dermatology residents who enrolled in the specialty during the study period. Results: The characterization conducted showed limitations in the development of the ability to diagnose dermatological diseases in anatomic pathology laboratories, Microbiology and Medical Parasitology, by residents (100%). The didactic conception of the interdisciplinary training process of diagnostic ability focuses on teaching and learning activities and laboratory practices in a logical consecutiveness and systematization from the different forms of teaching and class typologies. It integrates the interdisciplinary and the use of the research method that is established in the guiding ideas. Conclusions: The didactic conception, as a research contribution, resolves the dialectic contradiction between the application of the clinical method and the procedures in laboratory practice that occurs in this formative process which is a didactic support that backs up the practical activities in the laboratories in order to achieve the objectives of the study plan of the specialty.

ABSTRACT

ABSTRACT The Brazilian fauna of Meloidae is poorly studied, even though it includes more than 160 species. In this paper, we aimed at widening the knowledge on four species of blister beetles from this country. Specifically, we defined the uncertain range of Tetraonyx angulicollis, as extended in south-eastern Brazil rather than in Mexico, and implemented the description of the species with figures. We studied the taxonomy and distribution of three almost unknown species of Nemognatha from Brazil, São Paulo State, providing descriptions and figures of sexual characters and colour variability of N. beauregardi, to which is probably referable as a junior synonym of N. plaumanni, of N. rufoscutellaris and of N. cfr. gounellei. Moreover, we assigned these three species to the subgenus Pauronemognatha, recently recorded from South America.

Resumen Se trata de un artículo crítico y reflexivo que pretende discutir algunas de las incoherencias e incompatibilidades estructurales del modelo propuesto para la formación y atención en salud denominado Educación Interprofesional y Práctica Colaborativa (EIPC), en el contexto de racionalidad neoliberal de los países latinoamericanos. Se destaca cómo este modelo puede contestar dicha racionalidad, siendo relevante para la instauración de subjetividades profesionales críticas y éticas con su contexto. De esta manera se pretende contribuir a la creciente literatura sobre perspectivas críticas de la formación y el trabajo en salud.(AU)


Abstract This article addresses the new approach to training and healthcare work referred to as Interprofessional Education and Collaborative Practice (IECP). It focuses on some of the inconsistencies and structural incompatibilities of the model with the broader context of neoliberal rationality, with an emphasis on Latin-American countries. We suggest that IECP can contest such rationality and, in fact, develop a critical professional ethic. With that we intend to contribute to the growing literature on critical perspectives about training and working in health.(AU)

ABSTRACT

O objetivo do estudo foi analisar os desafios da formação desenvolvida pelos docentes atuantes em cursos de Enfermagem da região norte do Brasil, na perspectiva da Educação Interprofissional. Trata-se de uma pesquisa descritiva, exploratória e qualitativa realizada em universidades públicas. Doze docentes participaram de entrevista semiestruturada. Utilizou-se a análise de conteúdo e cinco categorias foram selecionadas: 1) Compromisso do ensino e do trabalho com a integralidade do cuidado; 2) Aprimoramento do ensino, pesquisa e extensão na perspectiva da educação interprofissional; 3) Atuação integrada entre os profissionais da Saúde; 4) Necessidade de desenvolvimento docente alinhado aos princípios da educação interprofissional; e 5) Educação Permanente em Saúde como estratégia indutora do ensino e prática interprofissional. A Educação Interprofissional é um campo robusto e complexo. Logo, é fundamental uma prática docente capaz de empreender processos formativos inovadores, induzindo mudanças que impactem positivamente a formação e o trabalho em saúde.(AU)


Resumen Análisis de los desafíos de la formación desarrollada por los docentes que actúan en cursos de enfermería en la región norte de Brasil, desde la perspectiva de la educación interprofesional. Se trata de una investigación descriptiva, exploratoria y cualitativa realizada en universidades públicas. Doce docentes participaron en la entrevista semiestructurada. Se utilizó el análisis de contenido y se seleccionaron cinco categorías: 1) Compromiso de la enseñanza y del trabajo con la integralidad del cuidado; 2) Perfeccionamiento de la enseñanza, investigación y extensión desde la perspectiva de la educación interprofesional; 3) Actuación integrada entre los profesionales de la salud; 4) Necesidad de desarrollo docente alineado a los principios de la educación interprofesional; y 5) Educación permanente en salud como estrategia inductora de la enseñanza y de la práctica interprofesional. La educación interprofesional es un campo robusto y complejo. Por lo tanto, es fundamental una práctica docente capaz de emprender procesos formativos innovadores, induciendo cambios que impacten positivamente la formación y el trabajo en salud.(AU)


Abstract Study analyzing the challenges of training developed by teachers working in nursing courses in the Northern Region of Brazil, from the interprofessional education perspective. A descriptive, exploratory, qualitative research conducted in public universities in which twelve professors participated in a semi-structured interview. Content analysis was used, and five categories were selected: 1) Commitment to teaching and working with comprehensive care; 2) Improvement of teaching, research, and community extension from the perspective of interprofessional education; 3) Integrated action among health professionals; 4) Need for teaching development aligned to interprofessional education principles; 5) Continuing education in health as a strategy to induce interprofessional teaching and practice. Interprofessional education is a robust and complex field. Therefore, a teaching practice capable of undertaking innovative formative processes is key, inducing changes that positively impact training and work in health.(AU)

ABSTRACT

Este artigo tem por objetivo compreender a percepção dos estudantes de Odontologia sobre motivações e significados do aprendizado interprofissional no período pandêmico de ensino remoto, em universidade pública do Sul do Brasil. Trata-se de pesquisa qualitativa que utilizou a perspectiva teórico-metodológica da fenomenologia da percepção. Foram realizadas entrevistas e análise documental dos portfólios dos estudantes que vivenciaram a atividade de ensino com foco na educação interprofissional (EIP) remota, no período pandêmico. A experiência foi marcada por interações mediadas pelo uso de plataformas digitais entre estudantes, professores e profissionais, oportunizando o desenvolvimento de competências colaborativas. Desigualdades no acesso à internet/equipamentos adequados ao ensino remoto, situações pessoais familiares relacionadas ao viver uma pandemia e dificuldade de criação de vínculos foram barreiras relatadas. EIP vivenciada em cenários de prática do Sistema Único de Saúde (SUS) foi percebida pelos estudantes como essencial à formação dos profissionais da saúde.(AU)


El objetivo de este artículo es comprender la percepción de los estudiantes de Odontología sobre motivaciones y significados del aprendizaje interprofesional en el período pandémico de la enseñanza remota en una universidad pública del Sur de Brasil. Se trata de una investigación cualitativa que utilizó la perspectiva teórico-metodológica de la fenomenología de la percepción. Se realizaron entrevistas y análisis documental de los portafolios de los estudiantes que experimentaron la actividad de enseñanza con enfoque en la educación interprofesional (EIP) remota, en el período pandémico. Las barreras relatadas fueron las desigualdades en el acceso a internet/equipos adecuados a la enseñanza remota, situaciones personales-familiares relacionadas a vivir una pandemia y la dificultad de creación de vínculos. La EIP vivida en escenarios de práctica del Sistema Único de Salud fue percibida por los estudiantes como esencial para la formación de los profesionales de la salud.(AU)


This article aims to understand undergraduate dental students' perception of the motivations and meanings of interprofessional learning in the pandemic period of remote teaching at a public university in the South of Brazil. This qualitative research used the theoretical-methodological perspective of phenomenology of perception. The researchers conducted interviews and a documentary analysis of the portfolios of students who attended a teaching activity with a focus on remote interprofessional education (IPE) in the pandemic period. The experience was marked by interactions between students-teachers-professionals mediated by the use of digital platforms, and provided opportunities of development of collaborative skills. Inequalities in access to internet services and to devices suitable for remote teaching, personal-familial situations related to experiencing a pandemic, and difficulties in creating bonds were the reported barriers. IPE in settings of the Brazilian National Health System was perceived by students as essential for the training of health professionals.(AU)

ABSTRACT

[ Abstract ] Objective To explore the effect of 3D print-based navigation module assisted placement of thoracolumbar pedicle screws. Methods From January 2019 to May 2021, we received 70 thoracic and lumbar fracture patients, divided into 3D technical group and conventional method group according to the surgical method, with 35 patients in each group. In the 3D technology group, pedicle screws were placed under the sight of the navigation module, while in the conventional group, pedicle screws were placed under the conventional C-arm fluoroscopy. The amount of intraoperative bleeding and time of C arm were counted in each patient. According to the different number of pedicle screw implantation in each patient, the average amount of blood loss, time and C-arm fluoroscopy times of each screw implantation were compared between the two groups. Ideal screw angles were designed for patients in both groups before surgery. Compared with the preoperative design, the difference between preoperative and postoperative screw angle and head angle was calculated and set as the deviation value. Two sets of data were compared. Visual analogue score(VAS), Japanese Orthopaedic Association (JOA) score, Oswestry disability index(ODI), vertebral height recovery ratio and Cobb’ s angle were compared between the two groups. Results The amount of blood loss, required time and exposure times of C-arm in 3D screw implantation group were significantly lower than those in conventional screw implantation group(P0. 05); In terms of Cobb’s angle and vertebral height recovery ratio, the 3D technique group was better than the conventional method group (P<0. 05). Conclusion The 3D printed navigation module can assist the precise placement of thoracolumbar pedicle screws, shorten the operation time, reduce intraoperative bleeding and c-arm exposure times, facilitate the recovery of the injured vertebral height, improve the efficacy.

ABSTRACT

Objective  There is a growing need to develop human resources with diverse experience, expertise, and a global perspective who can contribute to global rule-making in health. One of the government’s goals is to increase the number of Japanese personnel working for international organizations, but it has not reached the desired level. This study identified the barriers that medical and non-medical professionals have when applying for or working at international organizations, as well as the support measures they expect from the government, to highlight issues and examine employment support measures.Methods  A qualitative and descriptive research was conducted based on semi-structured interviews with 20 physicians, nursing professionals, graduates of public health graduate schools, non-medical professionals, students, etc., who were (1) seeking employment with international organizations, (2) currently employed, and (3) previously employed but left.Results  The following seven themes were highlighted as barriers to applying for or working at an international organization: 〈issues specific to Japanese society and people〉, 〈concerns about a career in global health〉, 〈discrepancy between a career as a healthcare professional and a career in global health〉, 〈need to strengthen skills〉, 〈preparation for competitive examinations〉, 〈importance of WLB〉, 〈issues inherent in international organizations〉. Conclusion  Possible support measures include individual career counseling, seminars on how to survive in international organizations, and seminars for female applicants of child-rearing age, based on the knowledge and experience of current/former employees of international organizations. As for the issues that hinder the dispatch of medical personnel overseas, the first step would be to compare and verify the positive effects that strengthening the overseas dispatch would have on their skills, the workplace environment, and the acceptance of foreign patients. The development and introduction of systems, such as sabbatical and long-term leave, and reinstatement of employment is especially required of global companies and international hospitals.

@#Introduction: Anatomical femoral tunnel placement is critical for anterior cruciate ligament reconstruction (ACLR). Tunnel placement may vary with different surgical techniques. The aim of this study was to compare the accuracy of femoral tunnel placement between the Anteromedial (AM) and Anterolateral (AL) visualisation portals on post-operative CT scans among a cohort of ACLR patients. Materials and methods: This cross-sectional study was conducted from January 2018 to March 2020 after obtaining ethics clearance. Patients who went for arthroscopic ACLR in our institute were divided into an AM (group 1) and an AL (group 2) based on the visualisation portal for creating the femoral tunnel and a 3D CT scan was done. The femoral tunnel position was calculated in deep to shallow and high to low direction using the Bernard Hertel grid. Femoral tunnel angle was measured in the 2D coronal image. Statistical analysis was done with the data collected. Results: Fifty patients with an average age of 26.36 (18-55) years ±7.216 SD were enrolled in the study. In this study, the AM technique was significantly more accurate (p<0.01) than the AL technique in terms of femoral tunnel angle. Furthermore, the deep to the shallow position was significantly (p= 0.018) closer to normative values, as determined by the chi-square test. The chances of error in tunnel angle in femoral condyle are 2.6 times greater in the AL technique (minimal clinical difference). Conclusion: To conclude, in ACLR the anteromedial visualisation portal can facilitate accurate femoral tunnel placement compared to the anterolateral visualisation portal.

OBJECTIVES@#This study aimed to evaluate the long-term clinical efficacy of simple taper retentive implants in the posterior dental area after immediate implantation for 5-7 years.@*METHODS@#Selected from January 2015 to December 2017 in the Fourth Affiliated Hospital of Nanchang University dental clinic line tooth area immediately after the implant prosthesis, a total of 38 patients, 53 implants, were deep into (bone under 2 mm or higher) and the upper structure was repaired. In addition, after the completion of tracking observation of 60-90 months, the implant surrounding bone health was recorded and analyzed.@*RESULTS@#After 5-7 years of follow-up, 1 of the 53 implants failed to fall out, and the implant retention rate was 98.1%. The amount of bone resorption in the proximal and distal margins 5-7 years after implant restoration was (0.16±0.94) mm and (-0.01±1.29) mm, respectively, and the difference in bone height between the proximal and distal margins of the implant and the immediate post-restoration period was not statistically significant (P>0.05). No statistically significant differences were found in the effects of periodontitis, implant site inflammation, and smoking on peri-implant marginal bone resorption (P>0.05).@*CONCLUSIONS@#The single taper-retained implant broadens the indications for immediate implant placement in the posterior region, and its deep sub-osseous placement (≥2 mm below the bone) avoids to a certain extent the disturbance of the implant by external stimuli and the exposure of the cervical abutment of the implant, with the good long-term stability of the marginal bone around the implant.

ABSTRACT

Tracheo-Innominate artery fistula is a rare but devastating complication after tracheostomy. We report a 17-year old man who underwent the transection of the innominate artery and tracheal patch closure (under partial sternotomy) after the endovascular covered stent placement for the recurrent tracheo-innominate artery fistula. Fortunately, his postoperative course was uneventful without any new neurological, bleeding, or infective complication 34 months after the surgery.

ABSTRACT

OBJECTIVE@#To compare the effectiveness of O-arm navigation and ultrasound volume navigation (UVN) in guiding screw placement during minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) surgery.@*METHODS@#Sixty patients who underwent MIS-TLIF surgery for lumbar disc herniation between June 2022 and June 2023 and met the selection criteria were included in the study. They were randomly assigned to group A (screw placement guided by UVN during MIS-TLIF) or group B (screw placement guided by O-arm navigation during MIS-TLIF), with 30 cases in each group. There was no significant difference in baseline data, including gender, age, body mass index, and surgical segment, between the two groups ( P>0.05). Intraoperative data, including average single screw placement time, total radiation dose, and average single screw effective radiation dose, were recorded and calculated. Postoperatively, X-ray film and CT scans were performed at 10 days to evaluate screw placement accuracy and assess facet joint violation. Pearson correlation and Spearman correlation analyses were used to observe the relationship between the studied parameters (average single screw placement time and screw placement accuracy grading) and BMI.@*RESULTS@#The average single screw placement time in group B was significantly shorter than that in group A, and the total radiation dose of single segment and multi-segment and the average single screw effective radiation dose in group B were significantly higher than those in group A ( P<0.05). There was no significant difference in the total radiation dose between single segment and multiple segments in group B ( P>0.05), while the total radiation dose of multiple segments was significantly higher than that of single segment in group A ( P<0.05). No significant difference was found in the accuracy of screw implantation between the two groups ( P>0.05). In both groups, the grade 1 and grade 2 screws broke through the outer wall of the pedicle, and no screw broke through the inner wall of the pedicle. There was no significant difference in the rate of facet joint violation between the two groups ( P>0.05). In group A, both the average single screw placement time and screw placement accuracy grading were positively correlated with BMI ( r=0.677, P<0.001; r=0.222, P=0.012), while in group B, neither of them was correlated with BMI ( r=0.224, P=0.233; r=0.034, P=0.697).@*CONCLUSION@#UVN-guided screw placement in MIS-TLIF surgery demonstrates comparable efficiency, visualization, and accuracy to O-arm navigation, while significantly reducing radiation exposure. However, it may be influenced by factors such as obesity, which poses certain limitations.

ABSTRACT

ObjectiveTo explore the effect of therapeutic postural placement on postural control and balance in stroke patients with hemiplegia. MethodsFrom January, 2020 to June, 2022, 60 stroke patients in Cangzhou Hospital of Integrated TCM-WM•Hebei were randomly divided into control group (n = 30) and observation group (n = 30). Both groups accepted routine rehabilitation, while the observation group accepted therapeutic postural placement based on the concept of Bobath additionally. Fugl-Meyer Assessment-Lower Extremity (FMA-LE), Berg Balance Scale (BBS), Postural Assessment Scale for Stroke Patient (PASS) and Pro-kin balance instrument were used to evaluate the postural control and balance function before and four weeks after treatment, while the area of ellipse and length of motion with eyes open and closed were recorded, respectively. ResultsAfter treatment, the scores of FMA-LE, PASS and BBS significantly increased (|t| > 3.856, P < 0.001), and the area of ellipse and length of motion with eyes open and closed decreased in both groups (|t| > 4.083, P < 0.001); all the indexes were better in the observation group than in the control group (|t| > 2.261, P < 0.05). ConclusionThe therapeutic postural placement could effectively improve the postural control ability and balance function in stroke patients.

@#Introduction: Nursing is a theory-practice course that requires high-quality and effective clinical education experiences. Clinical placement is one of the most important parts of nursing education; that can be challenging, stressful, and causes unpredictable changes. However, studies describing the challenges during clinical placement faced by nursing students are scarce. This study aimed to explore the challenges of Malaysian nursing students during a clinical placement at hospitals. Methods: A phenomenology qualitative study was conducted among nursing students at a public university in Malaysia. A semi-structured interview was used to collect data from October 2020 until November 2020. All interviews were recorded, transcribed, and analysed using thematic analysis. Results: Five themes emerged from this study: 1) attitude of the nurse, 2) patient’s distrust, 3) student’s issues, 4) learning environment, and 5) theory-practice gaps. Conclusion: This study revealed that nursing students face many challenges during their clinical postings. Therefore, various methods could be applied to increase the quality of practical sessions. Additionally, further research is needed to assess the coping strategies used by nursing students in dealing with the challenges during their clinical placement and the effectiveness of the interventions taken by the authority in helping the students.

ABSTRACT

Objective:To investigate the characteristics, process, and prognosis of esophageal stricture after circumferential endoscopic submucosal dissection (ESD), and to preliminarily analyze the prevention and treatment effects of simple dilation, stent placement, mucosal transplantation, and glucocorticoid (hereinafter referred to as hormone) application in esophageal stricture.Methods:From August 2017 to March 2022, at the First Affiliated Hospital of Zhengzhou University, the clinical and follow-up data of 55 patients who underwent circumferential ESD for early esophageal cancer and precancerous lesions were retrospectively analyzed. According to the prevention and treatment methods for esophageal stricture, the patients were divided into two groups: simple dilation group (23 cases) and combined dilation group (32 cases). The combined dilation group was divided into mucosal transplantation subgroup (9 cases), stent placement subgroup (14 cases), hormone application subgroup (7 cases), and bleomycin subgroup (2 cases, excluded from comparative analysis due to limited cases). Overall prognosis of patients was observed. Treatment efficacy, prognosis, and adverse events were compared among the simple dilation group, mucosal transplantation subgroup, stent placement subgroup, and hormone application subgroup. Independent samples t-test, chi-square test, and Fisher′s exact test were used for statistical analysis. Results:Among the 55 patients, the follow-up time was (894.1±417.7) days. Refractory esophageal stricture (total dilation times ≥ 5) occurred in 33 patients (60.0%). Fifty-two patients (94.5%) achieved clinical remission of the stricture. The total number of dilations was 5.8±4.0, and the average dysphagia-free period was (52.3±37.1) days. The dysphagia-free period of mucosal transplantation subgroup was longer than that of the simple dilation group, stent placement subgroup, and hormone application subgroup ((114.5±50.0) days vs. (40.9±20.0), (39.7±10.0), and (40.9±25.5) days, respectively), and the differences were statistically significant ( t=4.82, 3.77 and 3.14, P<0.001, =0.011, =0.009). There were no statistically significant differences between the simple dilation group and the mucosal transplantation subgroup, stent placement subgroup, and hormone application subgroup in the total number of dilations (6.8±4.8 vs. 3.0±2.5, 5.8±2.2, and 5.7±5.0), stricture remission rate (95.7%, 22/23 vs. 8/9, 13/14, and 7/7), and incidence of adverse events (17.4%, 4/23 vs. 5/9, 5/14, and 2/7; all P>0.05). Conclusions:Esophageal stricture formed after circumferential ESD shows the characteristics of recurrence and intractability. The over all number of dilations is high, and the average dysphagia-free period is short. Most patients can achieve clinical remission of the stricture after multiple times of endoscopic dilation treatment. However mucosal transplantation, stent placement, and hormone application cannot well intervene the natural process of esophageal stricture.
