ABSTRACT

Objective:To explore the value of 5G robotic remote ultrasound in the diagnosis of plateau pulmonary edema(HAPE).Methods:A total of 27 patients who quickly entered Nagqu, Tibet at an altitude of 4 600 m-5 600 m from March to December 2021 and developed one of the clinical symptoms of HAPE were collected. All patients were examined by 5G remote robotic ultrasound and lung CT respectively. Kappa test was used to analyze the consistency of the two diagnostic results, and McNemar test was used to compare the difference in diagnostic results. The ROC curve was used to analyze the sensitivity and specificity of remote lung ultrasound scores in the diagnosis of HAPE.Results:Among the 27 patients, 16 showed thickening of pleural line, increasing of B line, lung consolidation, pleural effusion, etc. Meanwhile, 11 showed no abnormality. Additionally, 8 cases had diffuse pulmonary fluid in both lungs, and 8 cases had localized pulmonary fluid. ROC curve showed that the area under the curve of lung ultrasound score for the diagnosis of HAPE was 0.947 (95% CI=0.78-0.99, P<0.001). The sensitivity and specificity were 0.933 and 0.917, respectively. Lung CT diagnosis was positive in 15 cases. Lung CT showed thickening of lung texture, ground glass, small nodular shadow, fine reticulate shadow, etc. The diagnostic results of the two techniques were in good agreement (Kappa=0.924, P<0.001), and there was no significant difference between the two methods ( P>0.05). Conclusions:5G remote robotic ultrasound has high consistency with CT in the diagnosis of HAPE and is an alternative early diagnosis method for HAPE. It may have clinical application value in scattered medical resources and remote plateau areas.

ABSTRACT

Acute plateau disease refers to a variety of physiological and pathological reactions produced by the body in a short period of time after rapidly advancing into the high-pressure and low-oxygen plateau area with an altitude above 2 500 meters, mainly including acute mountain sickness (AMS), high altitude pulmonary edema (HAPE) and high altitude cerebral edema (H A C E), which seriously affects the health of people in the acute plateau and even threats their lives. The establishment of an animal model of acute plateau disease with good reproducibility and a sound evaluation system are the basis of the research on acute plateau disease. Acute plateau disease is mainly caused by the low oxygen conditions on the plateau, so the animal model of acute plateau disease can be established in plateau environment simulation cabin or plateau field, simple breeding or animal treadmill assisted sports training. The indicators that indicate the success of the model establishment are commonly used blood gas, inflammation factors, organizational water content and pathological section. In this article, the animal models of acute plateau disease established in recent years are reviewed from the aspects of modeling environment, modeling methods and evaluation indicators.

ABSTRACT

Traditional Chinese medicine inheritance support system (TCMISS, V2.5) was used in this study to analyze the common medicines, frequency of medicine combination, core herbal combinations and new prescriptions for the treatment of plateau disease from the Four-Volume Medical Code, Mannose Herbal Mirror and other related books. TCMISS V2.5 software was used to construct the database of drug treatment for plateau disease. The frequency analysis and association rules apriori algorithm, improved mutual information method and other data mining methods were used to explore the law of drug compatibility. After the analysis of 531 prescriptions for plateau disease, 20 common symptoms of plateau disease were summarized such as insomnia and tinnitus. There were 539 kinds of herbs in the plateau disease prescriptions, including 33 kinds of herbs whose medicinal frequency>=50, such as Terminalia chebula and Myristica fragrans, and T. chebula had the highest medicinal frequency; 14 commonly used herbal combinations, and "Aucklandia lappa-T. chebula" had the highest frequency; 13 core herbal combinations were obtained after entropy clustering analysis, including 8 combinations of three herbs such as "Allium sativum-Carum carvi-Ferula sinkiangensis" and 5 combinations of four herbs such as "A. lappa-Aquilaria sinensis-Ewgewia caryophyllata-Myristica fragrans", and 5 new prescriptions such as "A. sativum-C. carvi-F. sinkiangensis-A. lappa-Choerospondia axillaris-A. sinensis-M. fragrans". The main symptom of "insomnia" was chosen to analyze the treatment of insomnia, and the core herbal combinations mainly including A. sinensis, T. chebula, and C. axillaris as well as the new prescriptions mainly containing Carthamus tinctorius, Meconopsis horridnla, and Punica granatum were obtained. The prescriptions for the treatment of plateau disease were clarified by TCM inheritance system, and the clinical prescription regularity and characteristics of the combination of common medicines were summarized, to provide reference and new ideas for its clinical application and development of new drug research.
