ABSTRACT

Background: Unwanted pregnancies in women and complications of induced abortions are an important health problem in the world. There is recent trend of self-induced medical abortion pill use over the counter without knowing the recommendations. Objectives of this study were to study socio demographic profile, presentations, management of hospitalized women for medical abortion related complications. To explore out contraceptive practices in those women. To way out the reasons for choosing medical abortion by women.Methods: Women of reproductive age group (15-45 years) who admitted in department of Obstetrics and Gynaecology Medical College with the complications following self-induced medical abortion were enrolled. Patients’ demographic profile, socio-economic status, obstetric profile, KAP of contraception, logical reasons for self-induced abortion were studied. Management of each case was done according to hospital protocols.Results: The total number of patients that were enrolled for the study period was 100. 37% of hospitalized women were between 18-25 years age group and 45 % of them had marriage at early age (15-20 years) and 46% were belonged to lower middle-class group. Majority of women with single living child with their last child <1-year age had taken abortion pills for unplanned pregnancy (49%). 74% women had taken abortion pills over the counter for the first time and 88% women did have idea about contraception but never used. 70% of the women had to undergo surgical evacuation and six of them had undergone emergency laparotomy for ruptured tubal ectopic pregnancy. Post abortion counselling successfully convinced to accept combined oral contraceptive (31%), followed by IUCD and permanent sterilization and 16% of women by medical abortion pill refused to accept contraception.Conclusions: Pregnancy termination should be done by qualified medical personal with accurate information about safe medical abortion. Counselling of each couple is must to enhance continuous and consistent use of family planning methods.

ABSTRACT

Objective@#To explore the effect of post-abortion care service based on Wechat follow-up on contraceptive compliance of multiple induced abortion patients, and to evaluate the satisfaction with PAC service.@*Methods@#A quasi-experimental method was adopted. A total of 300 cases of induced abortion were selected in Beijing Obstetrics and Gynecology Hospital affiliated to Capital Medical University from March 2018 to March 2019, with 150 cases from eastern branch in the experimental group and 150 cases from western branch in the control group. The control group received routine nursing care and post-abortion care service based on telephone follow-up, while the experimental group received routine nursing care and post-abortion care service based on Wechat follow-up. The missing rate of follow-up, the contraceptive method and the occurrence of unexpected pregnancy within one year after operation were collected in the two groups. Patients′ follow-up compliance and nursing satisfaction were evaluated.@*Results@#The awareness of the risk of induced abortion in the experimental group was 90.4% (120/135) higher than that in the control group 77.5% (88/111), and the difference was statistically significant (χ2 value was 9.128, P=0.010). The contraceptive consciousness of the two groups was 99.3% (134/135) and 94.6% (105/135), respectively, and the difference was not statistically significant (χ2 value was 3.256, P=0.071). There was a statistically significant difference in the missing rate of follow-up between the two groups in January, March, June, and December (χ2 value was 13.664, P=0.003). Compared with 19.8% (22/111) in the control group, 73.3% (99/135) patients in the study group chose high-efficiency contraception, the difference was statistically significant (χ2 value was 69.795, P<0.01).There were no recurring pregnancies in the experimental group within one year, compared with 6.3% (7/111) in the control group, the difference was statistically significant (χ2 value was 6.630, P=0.003). In the experimental group, the participation rate of the follow-up were (95.37±12.29)%, which were higher than that in the control group (90.77±16.49)%. The difference was statistically significant (t value was 2.438, P<0.05). The overall satisfaction score of the experimental group was 26.55 ± 2.80, which was higher than that of the control group (23.66 ± 3.32). The difference was statistically significant (t value was 7.285, P <0.01).@*Conclusion@#Post-abortion care service based on Wechat follow-up could enhance patients′ compliance and satisfaction, guide patients to choose the right contraceptive methods, reduce the rate of repeated abortion and promote female reproductive health.

ABSTRACT

Objective To explore the implementation situation and influencing factors of long-acting reversible contraception (LARC) after receiving post-abortion care (PAC) in artificial abortion women.Methods According to the demographic characteristics and abortion risks,4 068 artificial abortion women in the family planning clinic of our hospital were classified.The LARC implementation situation after receiving PAC was compared among different characteristic populations.The Chi square test was utilized to analyze the statistical significance of the data by SAS software.Results The LARC rate in young women was 7.9%),which was lower than 27.7% in 19-40 years old women and 11.8% in perimenopausal women;the LARC rate in nulliparous women was 10.6%,which was lower than 27.7% in parous women;the LARC rate in high-risk abortion women was 33.1%,which was higher than 16.6% in low-risk abortion women,these data analysis had statistically significant significance (P 80%).Conclusion Young women,nulliparous women and postpartum women in high-risk artificial abortion should be ranked the focus group for PAC consulting.

ABSTRACT

Objective To investigate the preventive effect of post abortion care(PAC) on unsafe sexual behaviors and deficiency in the knowledge of unmarried females,especially the youth in order to prevent repeated.unintended pregnancies as well as to promote the unmarried female's reproductive health.Methods During the period from Jan.2012 to Jun.2013,171 unmarried young women (< 24 years old) were enrolled in the study who were underwent free willed induced abortion due to unwanted pregnancy in Maternal and Child Health-Care Hospital of Huangpu District of Shanghai.Their reproductive health related knowledge,attitude and behavior were investigated by questionnaires.Based on the information above,they were provided with a series of post abortion services and improvement of reproductive health cognition and prevention effect of repeat pregnancy were evaluated after the intervention.Results After the nosocomial PAC intervention,the research objects are improved in the contraceptive knowledge and contraceptive behavior largely than before.The awareness rate on the female fertile time was increased from 53.8% (92/171) to 80.6% (137/170).The common contraceptive awareness rate increased from 49.1% (84/171) to 91.8% (156/170).The difference was statistically significant (x2 =27.30,x2 =75.69,P < 0.05).After 1 year follow-up,the PAC intervention make in contraceptive behavior change.The contraception rate increased from 8.2% (14/171) to 65.3% (111/170).The contraceptive failure rate was reduced from 35.7% (61/171) to 4.7% (8/170) (x2 =165.24,36.12,P < 0.05).There was not complications occurrence.Conclusion PAC can improve the contraception rate and reduce the unmarried female youth repeated abortion rate effectively.
