ABSTRACT

Prolapse or downward displacement of pelvic organs especially vagina, uterus and rectum is a common and disabling condition among women of menopausal age group. It affects their quality of life also. Displacement of vaginal anterior compartment results in cystocele. Quoting Acharya Susruta reference, the aim of this study is to find the efficacy of Tila taila yoni Abhya?ga in Cyuta avastha of Vasti with respect to first degree cystocele. 30 subjects were selected satisfying inclusion and exclusion criterias with the approval of Institutional ethical committee. Clinical evaluation is done with the help of relevant subjective and objective parameters. The subjective parameters were assessed before and after the treatment for a period of one month with UDI questionnaire and objective parameter with Baden-Walker system of grading Pelvic Organ Prolapse. Three months follow-up evaluation was also done with same parameters. The Wilcoxon Signed Rank Test is used to statically decompose the clinical data. Subjective parameters showed improvement of the condition viz frequency and urgency of urination, urine leakage with and without any activity, cough, sneeze, small amount of urine leakage, difficult and incomplete bladder emptying, bulging in vagina However, leakage not related to urgency, lower abdominal pressure, painful urination and lower abdomen or genital area, pelvic area heaviness or dullness, pelvic discomfort and burning micturition showed only a little reduction with the treatment. Objective parameter is also highly statistically significant at p value < 0.001.

ABSTRACT

Through the functional combination of relevant departments involved in hospital procurement, to simplify and unify the work process, we establish a standardized procurement system, to realize the pre-procurement budget and approval, power balance, strengthen the fairness and openness of procurement process. By introducing the closed-loop process of in-process supervision to ensure the impartiality of review and post-evaluation control, it comprehensively strengthens the internal control of procurement management, and finally realizes the purpose of strengthening procurement risk prevention and procurement quality management.

ABSTRACT

OBJECTIVE: This study presented the analysis period, the complexity of combined therapy and comparator choice as the key limitations in the economic evaluation of new drugs, and discussed programs for coping with these limitations. METHODS: This study evaluated the post-evaluation, risk-sharing agreement, extra funding program, and flexible incremental cost-effectiveness ratio (ICER) threshold as actions or programs that would increase accessibility to costly new drugs. The study also presented the cases of other countries. The application of the post-evaluation was considered to deal with high uncertainty regarding new drugs. RESULTS: The risk-sharing agreement was introduced in European countries as well as South Korea and has been responsible for the shift from using the financial schemes to outcome-based schemes. The drug funding program has had troubled in securing stable extra funds. The application of higher ICER in the economic evaluation of expensive and innovative oncology drugs was criticized because of the inequity between oncology patients and patients with other diseases. CONCLUSION: Therefore, introducing and applying actions that would increase the accessibility to costly new drugs in South Korea have been deemed necessary after careful reviews and discussions with various stakeholders (insurer, policy makers, pharmaceutical companies and patients).

ABSTRACT

In order to solve the drawbacks of egalitarianism,induced demand and ineffective incentive in the current distribution mode,the leverage of the salary mobilization and the hospital development is used to put forward the relative value of RBRVS as the theoretical guide to accounting the medical service project,and to establish doctor post value and comprehensive goal assessment system.Finally,a salary distribution model for doctors in county-level hospitals based on doctors' workload post value and comprehensive goals assessment will be formatted,which will reflect public welfare and incentive mechanism.
