ABSTRACT

The application of traditional Chinese medicines (TCMs) has a history of more than 2000 years, which have the characteristics of multi-component, multi-target, and high safety. Post-infectious cough (PIC) is a respiratory disease with high incidence. It belongs to subacute cough and accounts for as much as 40%–50%. Cough is the main clinical manifestation of PIC. PIC seriously affects people's life quality because of complex etiology, long-term course of disease, treatment difficulties and other characteristics. Western medicines are based on the principle of symptomatic treatment, so they are often difficult to control PIC fundamentally. These factors could due to that PIC is prolonged and unable to heal repeatedly. TCMs have obvious advantages in treating PIC, with accurate curative effects, less side effects and adverse reactions and are effective in improving PIC-related symptoms and indicators, enhancing patients' life quality and reducing pain. TCMs, guided by holistic concept and syndrome differentiation, advocate determine treatment on the basis of pattern types, and have remarkable clinical treatment effects. As for TCMs etiology, pathogenesis and syndrome types of PIC, TCM scholars have not yet reached a unified standard. However, most of them think that wind pathogen can cause PIC alone, or it can be combined with other evils, which might be the main mechanism of PIC. This paper discusses the advantages and limitations of TCMs in PIC treatment from etiology, pathogenesis, distribution of syndrome types and treatment of TCMs. This article focuses on the treatment methods and pharmacodynamic material basis of wind pathogen, providing ideas in treating PIC of TCMs clinically and innovative drug development.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the difference in the therapeutic effect on post-infectious cough differentiated as wind-cold retention in the lung between the combined therapy of scraping anddecoction and the simple application ofdecoction.</p><p><b>METHODS</b>Eighty patients were randomized into a combined therapy group and a Chinese herbal medicine group, 40 cases in each one. In the Chinese herbal medicine group, the oral administration ofdecoction was used. The main ingredients included,,,,,,,, etc., one dose a day, twice a day. In the combined therapy group, on the basis of the treatment as the Chinese herbal medicine group, scraping therapy was added and applied to the bladder meridian of foot-, the lung meridian of hand-, the conception vessel and the governor vessel, focusing on Tiantu (CV 22), Baihui (GV 20), Dazhui (GV 14), Feishu (BL 13), Fengmen (BL 12), Taiyuan (LU 9), Lieque (LU 7) and Fengchi (GB 20), once a week and one-week treatment as one session. Totally, the continuous two sessions were required in the two groups. The cough symptom score, cough remission time, relapse, TCM syndrome score, the score of Leicester cough questionnaire (LCQ), SP concentration in the supernatant of the induced sputum before and after treatment as well as clinical efficacy were observed in the two groups.</p><p><b>RESULTS</b>The cough symptom score, TCM symptom score and SP concentration in the supernatant of the induced sputum were all apparently reduced after treatment in the patients of the two groups (all<0.01). The scores in the combined therapy group were reduced in the higher amplitude as compared with those in the Chinese herbal medicine group (all<0.01). The total effective rate was 95.0% (38/40) in the combined therapy group, better than 87.5% (35/40) in the Chinese herbal medicine group (<0.05). Regarding the cough remission time and relapse rate, the results in the combined therapy group were better than those in the Chinese herbal medicine group[(5.3±1.2) d vs (7.4±1.5) d,<0.01; 0% (0/19) vs 62.5% (5/8),<0.01]. The scoreo of LCQ was all apparently improved in the patients of the two groups (both<0.01), and the score in the combined therapy group was higher than that in the Chinese herbal medicine group (<0.01).</p><p><b>CONCLUSIONS</b>Scraping therapy combined withdecoction and the simple application ofdecoction all relieve the symptoms of post-infectious cough and improves the living quality. The therapeutic effects of the combined therapy are superior to the oral administration ofdecoction.</p>

ABSTRACT

Objective To explore the efficacy and safety of different doses of budesonide mixed suspension atomization inhalation in children with post infectious cough,and provide reference for the rational use of budesonide.Methods One hundred and twenty children with post infectious cough aged 2-14 years old were selected and divided into three groups by random digits table method (40 cases in each group).Control group was treated with 4 mg once montelukast sodium chewable tablets for 2-5 years old children,or 5 mg once for 6-14 years old children.Low-dose group was treated 2 times/d with a dose of 0.5 mg budesonide mixed suspension diluted in normal saline to 4 ml based on control group and high-dose group was treated 2 times/d with a dose of 1.0 mg budesonide mixed suspension diluted in normal saline to 4 ml based on control group.The outcome of curative effects were recorded and analyzed after 7 days of treatment.Results The total efficacy rate was 70.0%(28/40),52.5%(21/40),35.0%(14/40) in high-dose group,low-dose group and control group after 3 days of treatment,and there was significant difference between high-dose group and control group (P< 0.01).The total efficacy rate was 92.5%(37/40),90.0%(36/40),72.5% (29/40) in high-dose group,low-dose group and control group after 7 days of treatment,and there was no significant difference(P > 0.025).Few adverse events associated with inhaled corticosteroids,such as thrush,sore throat,hoarseness were observed in three groups.Conclusions Budesonide mixed suspension atomization inhalation alleviates symptom of post infectious cough significantly with no obvious side effects,even in high-dose group which has a more outstanding efficacy.Therefore,this therapy is valuable in clinical application.

ABSTRACT

To investigate the establishment and evaluation of TCM diagnosis and treatment protocols of post infectious cough based on its status and problems. It will make the TCM syndrome standardization easy to popularize,and realize the optimization and simplification of TCM treatment protocols,establishment of western medicine diagnostic criteria beseem different hospital,and selection of scientific and accepted outcome measurements,to improve level of TCM diagnosis and treatment of post infectious cough and quality of its clinical research.
