ABSTRACT

Objective: To analyze the effect of Fuyuan Huoxuetang for the postoperative syndrome of partial splenic arterial embolization (PSE) in patients with hypersplenism of blood stasis type hepatic cirrhosis. Method: 86 patients with hypersplenism of blood stasis type hepatic cirrhosis treated with PSE were divided into observation group (44 cases) and control group (42 cases). Patients in both groups were treated with routine liver protection, antiviral, postoperative prophylactic anti-infection and symptomatic supportive care. Patients in treatment group started to take the Fuyuan Huoxuetang on the first day after the operation of PSE. The degree of pain, duration of fever, abdominal distention, and biochemical index changes of the two groups were observed 7 days after surgery. Result: After postoperative treatment, both groups of patients showed significant increases in the levels of -glutamyl transpeptidase (GGT) and white blood cell(WBC)(PP(P0.05) after the treatment with oral Fuyuan Huoxuetang. As compared with control group, the pain score was significantly lower in observation group (PPPConclusion: Fuyuan Huoxuetang could effectively interfere with the postoperative syndrome of PSE in patients with hepatic cirrhosis combined with hypersplenism by reducing pain, shortening the duration of fever and relieving the degree of abdominal distension and constipation.

ABSTRACT

Objective To discuss the pathogeny,treatment and prophylactic measures of postcholecystectomy syndrome (PCS).Methods The clinical data of 150 patients with laparoscopic PCS in our department from October 2000 to March 2009 were analyzed.Results Etiological factors were found in 131 patients:one hundred and twelve cases were due to the reasons of biliary system,including bile duct residual stones after cystic resection,the injury bile duct stenosis,a long residual cystic canal,nipple benign stricture,bile duct tumor etc;Nineteen examples were due to other reasons,including gallbladder stone merger reflux gastritis,gastroduodenal ulcer,diverticulum beside duodenal nipple,and so on,which resulted in the symptoms un-release after cystic resection.Nineteen cases were not found organic lesion.In ones whose etiological factors were definite,117 cases were treated with different surgeries according to different etiological factors;another 33 cases were treated with conservative treatment.Total 145 cases were followed up,and 139 cases in them were cured or relieved at different degrees.Conclusion Careful preoperative examination,normalized operation avoiding damaging bile duct and leaving behind bile duct stones can effectively prevent laparoscopic PCS.

ABSTRACT

Objective: To observe the therapeutic effect of treating postoperative gastrointestinal syndrome with combined needles and herbs. Method: Altogether 117 in-patients were randomly allocated into the treatment group of 86 cases and control group of 31 cases. Point injection was mainly adopted in the treatment group plus Chinese herbal decoction, while Western medicine was adopted in the control group. And then the therapeutic effect of the two groups was compared. Result: The total effective rate of the treatment group and control group were 96.5% and 80.6% respectively, and the treatment course of the treatment group was significantly shorter than the control group (P < 0.01). Conclusion: Combined needles and herbs have excellent effect in treating postoperative gastrointestinal syndrome and were superior to Western medicine.

ABSTRACT

PURPOSE: A retrospective study on the usefulness of selective spinal nerve root block among lumbar herniated intervertebral disc (HIVD), spinal stenosis and postoperative syndrome over 10 years. MATERIAL AND METHOD: From a total 1195 patients, whose symptoms were not improved by conservative treatment, 505 treated by selective nerve root block were divided into 3 groups; 150 (29.7%) with HIVD, 313 (62.0%) with spinal stenosis and 42 with postoperative syndrome, and were followed up from Oct. 1992 to Dec 2001. The degree of pain and activity were evaluated by a visual analogue scale method at the out-patient department or through telephone interviews. RESULTS: The end-results of selective spinal nerve root block, with more than 50% reduction in pain occurred in 380 (75.3%) of the 505 patients. The effectiveness was greater in young patients with HIVD than elderly patients with spinal stenosis. After discharge, 160 patients (31.7%) needed no other treatment: 14 (31.0%) with an extrusion type HIVD and 17 (20.0%) with spondylolisthesis. The only 98 patients (19.4%) needed a surgical procedure after selective spinal nerve root block. CONCLUSION: Selective spinal nerve root block in patients with lower back and radiating pain is a valuable conservative treatment to quickly improved symptoms and avoid surgical procedures and the continuous administration of drugs.
