ABSTRACT

Entornos favorables a la práctica profesional de enfermería aseguran calidad de los cuidados proporcionados a los pacientes. La calidad hoy constituye una ventaja competitiva en las organizaciones de salud. Las personas son el activo más importante de las organizaciones y a través de sus competencias logran comportamientos que agregan valor. Las enfermeras de un centro de oncología ambulatorio privado que desarrollan el modelo de enfermera de cabecera, expresan sentimientos de cansancio, insatisfacción laboral, ausentismo. Para lograr un mejor entendimiento del significado de lo que están experimentando se decide realizar una intervención utilizando los principios de educación para diagnóstico y acompañamiento de soluciones consensuadas. Se logra la participación del 100%, a través de preguntas abiertas, las enfermeras expresan el significado que tiene para cada una de ellas ejercer el modelo de enfermera de cabecera. Surgen del propio equipo alternativas de intervención. Con sentimiento de esperanza, optimismo y escucha, evaluaron la sesión. Se complementa el diagnóstico con la aplicación del instrumento que sugiere el Consejo Internacional de Enfermeras para evaluar el ambiente laboral. Los temas de reconocimiento, recompensa, participación en decisiones, programas de mentores y políticas para problemas de carga de trabajo, obtuvieron los menores puntajes. Se recomienda mantener la metodología de adultos e incorporar modelos de cambio de conductas, para el diseño del plan, fijación de metas, medir avances y reconocer logros, contribuyendo así a entornos favorables en enfermería, prevención de burnout y cuidados de calidad para pacientes y familia.

ABSTRACT

Objective To investigate the clinical application of Morse Fall Scale by primary nurses.Methods Morse Fall Scale was used to assess the falling risk in 21,378 patients by the primary nurses.The application of the Morse Fall Scale, choices of right time for assessment and the occurrence of falls in patients with risks of falling were evaluated by a self-designed questionnaire.Results Among 21,378 patients, 222 patients were at risk of falling.In terms of the items like the history of falls,more than one diagnosis,intravenous injection,the accuracy of assessment was above 91.6%.In terms of the items like aided walking,gait,cognitive state,the assessment accuracy is below 86.0%.The accuracy of assessment timing selection in the situations like admission,the score by Morse fall scale over 24at admission and the accuracy in choosing right time for assessment was was above 90.2% after transferring to other deparments and falling.The accuracy in choosing the right time for fall assessment was below 67.9% under such conditions as apostasies,and oral administration of drugs probably leading to fall. Conclusions It is acceptable that the primary nurses use the Morse fall scale to assess patients with risky falls and for the right choice of assessment.Attention should be paid to strengthen the nurses training to improve the accuracy of the scale,to make sure to choose right time for the assessment so as to prevent the falls of patients to the greatest extent.

ABSTRACT

OBJECTIVE:To investigate the status quo of clinical hospital pharmacy in hospitals in Fujian province.METHODS:A total of 575 questionnaires were given out in 10 general hospitals to investigate the status quo of clinical pharmacy and the role of physicians and nurses(primary nurses)in the pharmaceutical care.RESULTS:The hospitals in Fujian attached great importance to clinical pharmacy,but physicians and nurses(primary nurses)had a poor knowledge on the clinical pharmaceutical care and lacked trust in pharmacists' capability.CONCLUSION:The status quo of clinical hospital pharmacy in hospitals in Fujian province is far from perfect,the key to improve clinical hospital pharmacy level is to improve pharmacists' professional quality.
