ABSTRACT

@#A rare finding of a prostatic carcinoma metastasized in a ligamentum flavum at thoracic spine causing posterior column spinal cord compression. We reported a man with prostatic carcinoma presented with ataxic gait. Magnetic resonance imaging revealed an extradural cystic mass adjacent to the T4 intralaminar region indenting on the spinal cord. T4 posterior decompression via en bloc excision of the ligamentum flavum cyst and laminectomy was performed. Histopathology confirmed glandular tissue within the cyst wall which corresponds to prostatic adenocarcinoma cells. Two months post-operative, patient showed improvement in his muscle strength on left L2 myotome. The preoperative diagnosis is a challenge because of the ligamentum flavum cysts rarity in the thoracic spine and non-specific clinical signs and symptoms. To our best knowledge, this is the first report of metastatic adenocarcinoma of the prostate found as ligamentum flavum cysts.

ABSTRACT

Prostatic carcinoma (PCa) is one of the most common male malignancies, accounting for 10% of all male cancers, and has become a global health problem. At present, it is mainly tackled with radical prostatectomy and endocrine therapy. However, most patients will develop drug resistance, allowing the progression of PCa into castration-resistant prostate cancer (CRPC). Guided by the principles of holism and treatment based on syndrome differentiation, traditional Chinese medicine (TCM) alleviates cancer pain, regulates immune balance, and improves the quality of life of patients via multiple targets, multiple pathways, and multiple mechanisms without inducing obvious side effects, thus better exerting the preventive and therapeutic effects against PCa. This paper retrieved relevant articles concerning PCa intervention with TCM published in recent five years from PubMed and China National Knowledge Infrastructure (CNKI), and summarized the molecular mechanism of PCa, its etiology and pathogenesis in TCM, and TCM interventions. The findings showed that the active ingredients of Chinese medicinals, single Chinese medicinals and Chinese medicinal compounds inhibited PCa by interfering with not only the classical pathways of PCa such as androgen receptor(AR), Wnt/β-catenin, PI3K/Akt/mTOR, and NF-κB but also other pathways like ERS/UPR, RIPK, CIP2A/PP2A/ERK, EGFR, etc. The intervention of active ingredients from Chinese medicinals in PCa has been explored extensively, but there are fewer studies on single Chinese medicinals and Chinese medicinal compounds that can better reflect the unique advantages of TCM. Further research is needed to provide an important theoretical and experimental basis for the development of novel anti-PCa Chinese medicinal products and their clinical application.

Background: Prostatic carcinoma shows an unusually wide range of biological potential with well-known disparity between incidence and mortality for disease. Clinico-pathological studies suggests that angiogenesis and tumor neovascularity contributes to pathogenesis of prostate cancer. The aim of this study to present study was done to assess the validity of angiogenesis as a suitable prognostic marker in various prostatic disease specially the neoplasm’s including the malignant ones. Settings and design are Retrospective study.Methods: The present study of evaluation of angiogenesis as a prognostic marker in prostatic neoplasm especially carcinoma of prostate was done with 40 biopsy sample. The biopsy sample were obtained by suprapubic prostatectomy specimen and trans rectal needle biopsy specimen. Tissue sections were subjected to routine H and E staining. For demonstration of angiogenesis staining for reticulin fibers was applied.Results: The microvessels density  increases as the severity of lesion increases from benign to pre-neoplastic to frankly malignant. The micro vessel density in malignant lesions is approximately thrice that in benign lesions. Conclusions: Very few studies have been done in prostatic lesions Hence an attempt is made to demonstrate and correlate angiogenesis as a tumour marker.

@#【Objective】 The two databases,GEO(gene expression omnibus,GEO)and TCGA(the cancer genome alas ,TCGA),were analyzed using bioinformatics methods to screen differentially expressed genes associated and their related regulatory networks in prostate carcinoma. 【Methods】 The prostate carcinoma gene expression chip data (GSE46602 ,GSE55945) downloaded from the GEO database were integrated into the RNA- seq data of the TCGA database. And the differentially expressed genes analysis was performed using GEO2R and the edgeR package of R software to extract common significant differentially expressed genes. The clusterProfiler package of R software was used to enrich the GO(gene ontology ,GO)function enrichment analysis and KEGG(kyoto encyclopedia of genes and genomes, KEGG)pathway analysis. Differentially expressed genes were further constructed into a protein-protein interaction(PPI) network to screen out key genes for regulatory protein expression in prostate carcinoma. Gene analysis results were combined with TCGA clinical follow-up data to analyze the clinical prognostic value of key node genes. 【Results】A total of 278 significant differentially expressed genes were extracted,of which 178 genes were down- regulated and 100 genes were up-regulated. These genes were closely associated with the function and pathway enrichment such as the regulation of proliferation of epithelial cells,metabolism of benzene- containing compounds,the glutathione metabolism,and focal adhesion. The protein-protein interaction network analysis revealed three key protein expression modules and 12 key node genes. Among these key genes,EDN3(endothelin-3),EDNRB(endothelin receptor B)and AMACR(alpha-methylacyl- coa racemase)were closely related to the survival rate of prostate cancer patients. 【Conclusion】Through bioinformatics analysis of gene chip and RNA-seq data in prostate carcinoma,we found that EDN3,EDNRB and AMACR may play an important role in the occurrence and development of prostate carcinoma.

ABSTRACT

PTEN is the most commonly inactivated tumor suppressor gene in primary prostate cancer (PCa) and its loss is associated with poor clinical outcomes. ERG rearrangement is a genomic alteration frequently found in PCa and its prognostic significance has yielded mixed results. Although the association of PTEN and ERG biomarkers has potential impact on clinical outcomes, studies examining the two genes simultaneously are scarce in Brazilian populations. In this study, we retrospectively examined the relationship between ERG expression and PTEN loss in 119 surgically treated prostate cancer patients from Northeastern Brazil through immunohistochemical analysis. ERG expression was found in 41.0% (48/117) of cases and the loss of PTEN detected in 38.1% (40/105) of samples. ERG-positive cases were significantly associated with lower prostate weight; ERG negatively correlated with Gleason score above 6. The lack of associations for PTEN loss alone in this cohort is counter to the literature, which shows that PTEN loss is usually associated with more aggressive disease. The overlapping of the two biomarkers revealed that samples with positive ERG expression without PTEN loss were associated with lower Gleason and lower Grade group. This study contributes with the discussion about the development of the molecular profiling of prostate cancer. The further development of similar studies could help in stratifying specific risk groups, leading to a more personalized therapeutic decision for prostate cancer treatment.

ABSTRACT

Epidermal growth factor receptor (EGFR) is a 170-kDa tyrosine kinase transmembrane glycoprotein expressed in normal tissues in many organs and different types of tumours. In prostate, EGFR is expressed mainly in epithelial cells, phosphorylation of EGFR (pEGFR) which is assessed by immunohistochemical methods could be useful prognostic marker for prostate cancer cases. Tumours may affect the surrounding non-malignant tissue and pEGFR immunoreactivity in the morphologically normal prostate tissue can be used to retrieve prognostic information. In this study the membranous and cytoplasmic expression of EGFR is checked in both the basal and luminal cells. Intensity of the staining and the pattern of the staining were noted in benign, in-situ and malignant lesions and it was found that the staining intensity of the luminal cells increase with a subsequent decreased staining in the basal layer as the lesion progress towards malignancy. Subsequently the staining intensity and patterns were correlated with the Gleason grade for triaging of the cases into different prognostic groups.

Cyclin D is the most closely relationship with tumor ,and it is currently known one of the sub-types of cyclins.CyclinD1,as a positive regulatory factor ,has an important role in the process of cell cycle ,espe-cially in cell proliferation and division .CyclinD1 is unusually up-regulated in lots of human cancers .CyclinD1 is a recently discovered protein which is highly expressed in the urinary tumors ,and its expression in tumors is asso-ciated with a more aggressive phenotype ,prognosis and recurrence .Therefore,the research of the target point to CyclinD1 in the urinary tumors may provide a new method for the therapy to urinary tumors .

Background: Prostatic carcinoma is one of the most important causes of mortality in elderly men mainly because of the late detection despite of the fact that it is a potentially curable disease. Fine needle aspiration cytology (FNAC) is an easy to perform outpatient procedure requiring no expensive equipment or anesthesia. Objectives: The present study was carried out in an attempt to evaluate the fine needle aspiration cytology in the diagnosis of carcinoma prostate. Materials and methods: The present study was performed on 27 patients admitted in the surgical wards, with complaints suggestive of prostatic disease, in whom there was found to be a suspicion of malignancy of the prostate gland. Results: Among 27 patients, 14 patients were diagnosed as prostatic malignancy on per rectal FNAC whereas, 19 patients were confirmed with prostatic cancer histologically. Out of 19 histologically confirmed cancer cases, 16 were also positive on FNAC i.e. 84.21% accuracy of FNAC in detecting prostatic malignancy. Conclusion: Fine needle aspiration cytology is easily available and inexpensive procedure. It is a reliable method in the diagnosis of prostatic cancer. Its positive results are relatively more reliable than the negative ones. It is an effective method in follow up of the cancer cases.

ABSTRACT

Objective To investigate the effects of β adrenergic receptor blocker carvedilol and cholinergic receptor blocker scopolamine on the biological behaviors of prostate cancer in C57BL/6 mice.Methods From June 2014 to November 2014, sixty male C57BL/6 mice, 10 weeks old, were injected with RM-1 cells (0.5 × 106) into bilateral dorsal prostate capsules of each mice by microsyringe and were randomly divided into three groups, including carvedilol group (K), scopolamine group (D) and saline group (N) after modeling.In K group, the carvedilol (10 mg/kg) was given to mice through gastric tube.The normal saline was given to them by subcutaneous injection.In D group, the normal saline was given to mice through gastric tube.Meanwhile, the scopolamine (1 mg/kg) was injected subcutaneously.In N group, daily normal saline was given by through gastric tube and subcutaneous injection.At the seventh day of postoperation, five mouse were sacrificed in each group every three days to observe the local growth,invasion and metastasis of prostate cancer to pelvic nodes and liver.Immunohistochemical determination of prostate tumor was made by TH staining and VAChT staining in each group.Results The volume of the prostate cancer was gradually increased in three groups.There was no statistical difference of prostate volume among three groups on the day 7 and 10 post-operatively (all P > 0.05).The prostate volume of K group, D group and N group on the day 13 were 0.05 ± 0.04, 0.18 ± 0.08, 0.14 ± 0.05 cm3, respectively (P <0.05).However, the statistical significance existed between K and N group (P < 0.05).There was no statistical difference of AOD between TH and VAChT among three groups on the day 7 (all P > 0.05).The AOD of TH and VAChT in prostate cancer of K group, D group and N group on the day 10 were [(0.0114±0.016), (0.114±0.002), (0.059±0.008)] and [(0.025 ±0.011), (0.0226±0.003), (0.009±0.003)], respectively (P <0.05).Those values on the day 13 were [(0.147 ±0.036),(0.129 ±0.025), (0.071 ±0.022)] and [(0.020 ±0.005), (0.020 ±0.002), (0.010 ±0.002)],respectively (P <0.05).TH and VAChT expression of K and D in 10th and 13th is higher than in N (all P < 0.05).At the 7th day, no metastasis of pelvic nodes was detected in all groups.At 10th day, 2 cases of K, no case of D and 3 cases of N were detected the lymph node metastasis.At the 13th day, 3 cases of K, 1 case of D and 4 cases of N were recorded the lymph node metastasis.At the 7th day, no metastasis of liver was detected.At 10th day, 1 case of K, no case of D and 2 cases of N were found the liver metastasis.At the 13th day, 3 cases of K, 1 case of D and 4 cases of N were recorded the liver metastasis.The lifesapn of tumor-bearing mice in K group, D group and N group was 16.8 ±0.8, 17.6 ±0.5, 15.8 ±0.1, respectively (P > 0.05).Conclusions β adrenergic receptor blocker carvedilol can suppress the growth of prostate cancer and cholinergic receptor blocker scopolamine may can inhibit metastasis of prostate cancer.

Objective To discuss the diagnostic values of transrectal ultrasound-guided puncture biopsy in prostate carcinoma(PCa).Methods A total of 238 patients with suspected PCa received tran-srectal ultrasound-guided puncture biopsy.Results Based on the results of biopsy pathology,88 cases (37.0%)were diagnosed as PCa,105 cases(44.1%)were benign prostate hyperplasia,15 cases (6.3%) were low-grade prostatic intraepithelial neoplasm,17 cases(7.1%)were atypical hyperplasia,8 cases(3. 4%)were adenomatoid tumor,and 5 cases(2.1%)were granulomatous prostatitis.Conclusion Tran-srectal ultrasound-guided puncture biopsy is accurate and safe.It is an effective method in diagnosing pros-tatic carcinoma.

In this study the expression of metalloproteinases 2 (MMP-2) and 9 (MMP-9) in canine normal prostates and with proliferative disorders was evaluated to verify the role of these enzymes in extracellular matrix remodeling (ECM) and in the tissue invasion process. A total of 355 prostatic samples were obtained, from which 36 (10.1%) were normal prostates, 46 (13.0%) with benign prostatic hyperplasia (BPH), 128 (36.1%) with proliferative inflammatory atrophy (PIA), 74 (20.8%) with prostatic intraepithelial neoplasia (PIN), and 71 (20.0%) with prostatic carcinoma (PC). Difference in cytoplasmic immunohistochemical staining of MMP-2 and MMP-9 between acinar epithelium and periacinar stroma was found regarding the different diagnosis. The correlation between MMP-2 and MMP-9 expression in relation to the number of labeled cells in acinar epithelium and periacinar stroma, as well as to the staining intensity in the periacinar stromal cells was evidenced in canine prostates with PIA. In conclusion, MMP-2 and MMP-9 expression has a variation in canine prostate according to the lesion, with lower expression in normal tissue and with BPH, and higher expression in those with PIA, PIN and PC. Moreover, the inflammatory microenvironment of the PIA has influence in the activity of both enzymes.


Este estudo teve como objetivo avaliar a expressão das metaloproteinases 2 (MMP-2) e 9 (MMP-9) em próstatas caninas normais e com desordens proliferativas, verificando o papel dessas enzimas na remodelação da matriz extracelular (MEC) e no processo de invasão tecidual. Um total de 355 amostras prostáticas foram obtidas, sendo 36 (10,1%) normais, 46 (13,0%) com hiperplasia prostática benigna (HPB), 128 (36,1%) com atrofia inflamatória proliferativa (PIA), 74 (20,8%) com neoplasia intraepitelial prostática (PIN) e 71 (20,0%) com carcinoma prostático (CP). Houve diferença de imunomarcação citoplasmática para MMP-2 e MMP-9 entre o epitélio acinar e o estroma periacinar, quanto aos diferentes diagnósticos. Observou-se correlação entre a expressão de MMP-2 e MMP-9 em relação ao número de células marcadas no epitélio acinar e estroma periacinar, bem como para a intensidade de marcação das células estromais periacinares em próstatas caninas com PIA. Conclui-se que há variação na expressão de MMP-2 e MMP-9 em próstatas caninas de acordo com a lesão, com menor expressão em próstatas caninas normais e com HPB, e maior naquelas com PIA, PIN e CP. Ainda, o microambiente inflamatório na PIA influencia a atividade de ambas as enzimas.

ABSTRACT

The diagnosis of synchronous prostatic and rectal carcinomas is uncommon. To make a correct diagnosis, biopsies of both sites are mandatory. Pathological slides should be compared and immunohistochemical staining should be taken into consideration. In this paper, an unexpected case of synchronous rectal and prostatic carcinomas arising in an 84-year-old male with hematemesis and pelvic pain is reported. These two tumoral components have a distinctive histological appearance. Immunohistochemical evaluation confirmed the diagnosis of these synchronous tumors. This case emphasizes that rectal and prostatic carcinomas can arise simultaneously. In this situation, providing clinicopathological correlation and deciding the necessity of intraoperative consultation in proper time are extremely important.

ABSTRACT

Vaccination with xenogeneic and syngeneic endothelial cells is effective for inhibiting tumor growth. Nontoxic diphtheria toxin (CRM197), as an immunogen or as a specific inhibitor of heparin-binding EGF-like growth factor, has shown promising antitumor activity. Therefore, immunization with or administration of viable human umbilical vein endothelial cells (HUVECs) combined with CRM197 could have an enhanced antitumor effect. Six-week-old C57BL/6J male mice were vaccinated with viable HUVECs, 1 x 10(6) viable HUVECs combined with 100 μg CRM197, or 100 μg CRM197 alone by ip injections once a week for 4 consecutive weeks. RM-1 cells (5 x 10(5)) were inoculated by sc injection as a preventive procedure. During the therapeutic procedure, 6-week-old male C57BL/6J mice were challenged with 1 x 10(5) RM-1 cells, then injected sc with 1 x 10(6) viable HUVECs, 1 x 10(6) viable HUVECs + 100 μg CRM197, and 100 μg CRM197 alone twice a week for 4 consecutive weeks. Tumor volume and life span were monitored. We also investigated the effects of immunization with HUVECs on the aortic arch wall and on wound healing. Vaccination with or administration of viable HUVECs+CRM197 enhanced the inhibition of RM-1 prostatic carcinoma by 24 and 29 percent, respectively, and prolonged the life span for 3 and 4 days, respectively, compared with those of only vaccination or administration with viable HUVECs of tumor-bearing C57BL/6J mice. Furthermore, HUVEC immunization caused some damage to the aortic arch wall but did not have remarkable effects on the rate of wound healing; the wounds healed in approximately 13 days. Treatment with CRM197 in combination with viable HUVECs resulted in a marked enhancement of the antitumor effect in the preventive or therapeutic treatment for prostatic carcinoma in vivo, suggesting a novel combination for anti-cancer therapy.

ABSTRACT

We present the case of an 81-year-old patient with testicular metastasis from prostate carcinoma. After the initial diagnosis of prostate cancer, he had an 8-year course of hormonal therapy and showed no clinical evidence of metastasis to other organs. Asymptomatic metastasis of prostate carcinoma to the testis is a rare clinical condition. We diagnosed his condition, based on histopathology following a subcapsular orchiectomy and transurethral resection of the prostate.

In recent years, there is some new recognition about the origin of prostatic carcinoma, prostatic carcinoma has been considered as a stem cell disease. Prostate cancer stem cells have close relationship with prostatic carcinoma formation, advancement, relapse, metastasis and drug resistance. As the development of science and technology continues, studies on the isolation and correlated phenotype identification of prostate cancer stem cells have lead us to further recognize prostate cancer stem cells, as well pave a new way for the diagnosis and therapy of clinical prostatic carcinoma.

ABSTRACT

Objective To explore the morbidity,the diagnosis and the method of therapy of accidental prostatic carcinoma. Methods From Jan of 1984 to May of 2004, 19 cases of prostatic accidental carcinoma were confirmed on pathological examination after prostatectomy for BPH. Bilateral orchiectomy and Estrogen treatment were performed in 6 cases and Bilateral orchiectomy in 7 cases alone but no treatment in 6 cases. Results 5 of the patients wereA1 stage and 14 A2 stage. 12 of them were followed up for 3 to 120 months. 14 of them survived and one untreated died of metastasis to pubis and vertebra after one year. Conclusions Most patients of prostatic accidental carcinoma are A1 and have a better prognosis. Bilateral orchiectomy and Estrogen treatment might improve the patients’s survival rate.

Lymph node involvement is the most important prognostic factor of rectal cancer. Cancer originating from sites other than the rectum rarely metastasizes to the mesorectal lymph node. We report a rectal cancer patient with a synchronous metastatic prostatic carcinoma to the mesorectal lymph node.

Objective To study microvascular density(MVD),DNA ploidy and immunophenotype in prostatic intraepithelial neoplasia(PIN) as well as their relationship with its biological properties.Methods We studied paraffin-embedded specimens from 24 cases of benign prostatic hyperplasia(BPH),23 cases of low-grade prostatic intraepithelial neoplasia(LGPIN),and 26 cases of prostatic carcinoma(PC) with high-grade prostatic intraepithelial neoplasia(HGPIN).Image analysis was used to measure the DNA average ploidy and MVD,and after that the expressions of 34?El2,P63,P504s and CD34 were detected by immunohistochemistry SP.Results DNA ploidy and MVD in PIN were higher than those in BPH,but lower than in PC,and correlation was detected among different grades of PIN.MVD had a positive correlation with DNA aneuploidy in different grades of PIN(P

ABSTRACT

Objective To evaluate the value of MRI in diagnosis of prostate diseases.Methods Clinical data and MRI findings of prostatic carcinoma(PC) in 43 cases and benign prostatic hyperplasia(BPH) in 33 cases confirmed pathologically were reviewed blindly by two radiologists and compared with that of pathology.The sensitivity,specificity,positive predictive value(PV+),negative predictive value(PV-) and accuracy of MR imaging were also calculated.Results The sensitivity,specificity,PV+,PV-and accuracy of MR imaging in diagnosing prostatic lesions were 90.7%,84.8%,88.6%,87.5% and 88.2% for PC,84.8%,90.7%,87.5%,88.6% and 88.2% for BPH,respectively.PC located in the peripheral zone were 67.4%(29/43 cases),in the central zone were 9.3%(4/43 cases),the prostates presented irregular enlargement with diffuse low signal intensity in 16.3%(7/43 cases),7.0%(3/43 cases) lesions did not found by MRI even after getting pathological results.The size of the mass ranged from 0.3~10.0 cm,mean 2.4 cm.All of 40 cases,the lesions showed low signal intensity without border and capsule.The affected structures around the lesions inculuded prostatic capsule(n=34),seminal vesicle(n=24),muscle(n=8),rectal(n=5),bladder(n=6) and ilium(n=1),lymph nodes metastasis(n=11),bone(n=13) and lung(n=2).The lesions of BPH located in the central zone were 87.9%(29/33 cases),showing enlargement of the central zone with inhomogeneous signal intensity,the other 4 cases in peripheral zone showed low signal intensity with border and 2 cases with capsule.Conclusion MRI has higher sensitivity,specificity,positive predictive value,negative predictive value,and accuracy in diagnosis of prostatic diseases.

ABSTRACT

Objective To investigate the association of inducible nitric oxide synthetase (iNOS) expression with the expression of mutation type p53 (mt p53) in prostatic carcinoma (PCa). Methods The expression levels of iNOS and mt p53 were in 28 PCa specimens and 10 cases of benign prostatic hyperplasia (BPH) were detected by immunohistochemical method. Results The expression levels of both iNOS and mt p53 in PCa were higher than those in BPH. The expression of iNOS was related to the expression of p53 protein. Conclusion iNOS and mt p53 may play a role in the pathogenesis of PCa.
