ABSTRACT

ObjectiveTo explore the effects and mechanisms of Kaiyu Zhongyutang on insulin resistance, glucose and lipid metabolism, psychological state, and embryo outcome in the infertile patients with polycystic ovary syndrome (PCOS) due to liver depression and kidney deficiency. MethodThe 126 infertile patients with PCOS due to liver depression and kidney deficiency who underwent in vitro fertilization-embryo transfer (IVF-ET) in the Department of Reproduction of the affiliated hospital of Nanjing university of Chinese medicine were randomly assigned into the observation (Kaiyu Zhongyutang + metformin) and control (metformin) groups. The two groups were compared in terms of traditional Chinese medicine (TCM) syndrome scores, body mass index (BMI), glucose and lipid metabolism, homeostasis model assessment-insulin resistance (HOMA-IR), interleukin (IL)-6, IL-8, C-reactive protein (CRP), self-rating depression scale (SDS) score, self-rating anxiety scale (SAS) score, and clinical and laboratory scores of IVF after treatment. Result① After treatment, the observation group showed decreased scores of primary and secondary TCM syndromes and total TCM syndrome score (P < 0.05), and the control group presented decreased scores of irritability and depression and total TCM syndrome score (P<0.05). Compared with the control group, the observation group demonstrated reduced primary and secondary syndrome scores and total score after treatment (P<0.05). ② After treatment, both groups showed decreased BMI, lowered levels of fasting insulin (FINS), fasting plasma glucose (FPG), cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL), and HOMA-IR (P<0.05), and elevated high-density lipoprotein (HDL) level (P<0.05). Moreover, the observation group had lower BMI, FINS, FPG, TC, TG, LDL, and HOMA-IR and higher HDL than the control group (P<0.05). ③ The treatment in both groups lowered the levels of IL-6, IL-8, and CRP and decreased SDS and SAS scores (P<0.05). Moreover, the declines in the observation group were more obvious than those in the control group (P<0.05). ④ Correlation analysis before treatment, IL-6, IL-8, and CRP had positive correlations with BMI, HOMA-IR, TC, TG, LDL, SDS, and SAS (P<0.05) and a negative correlation with HDL (P<0.05). ⑤ The observation group showed reduced gonadotropin (Gn) using days and total Gn dose and higher two-pronuclear (2PN) fertilized oocytes, 2PN cleavage rate, normal fertilization rate, D3 transferable embryo number, D3 high-quality embryo number, high-quality embryo rate, and blastocyst formation rate than the control group (P<0.05). ConclusionKaiyu Zhongyutang can treat PCOS patients by improving the emotional and reproductive functions and alleviating insulin resistance and glucose and lipid metabolism disorders. Moreover, it can reduce the Gn dose and Gn using days in the IVF process, improve the quality and maturity of eggs, increase the egg fertilization rate, enhance the potential of embryo development, and increase the rate of blastocyst formation by inhibiting inflammation.

ABSTRACT

Background@#The majority of women described hysterosalpingography (HSG) as a painful procedure. There is little information on the features of HSG-associated pain and factors that predispose to increased pain experience. @*Objectives@#This study investigated preprocedure psychological state and related demographic factors as predictors of pain perception in women undergoing HSG. @*Methods@#The sample included 99 women selected through consecutive sampling at the Radiology Department, University of Benin Teaching Hospital, Benin City, Nigeria. The study utilized a cross-sectional survey design to collect data using Spielberger's State-Trait Anxiety Inventory, Zung Self-rating Depression Scale, and Visual Analog Scale for the perception of pain and state anxiety. @*Results@#The women with lower state anxiety reported significantly lower pain perception (X̅ = 6.69) than the women with high anxiety (X̅ = 7.93). Trait anxiety, state anxiety, and depression jointly predicted pain perception among the women undergoing HSG, with R2 = 0.117, F (3,95) = 6.797; P < 0.001. They collectively accounted for about 17.7% variance in pain perception. @*Conclusion@#Patients being prepared for the HSG procedure can be educated on concerns related to anxiety and coping strategies and be provided with anxiolytics or other medication as clinically indicated.

ABSTRACT

Objective:To analyze the adverse psychological state and its influencing factors in patients with dysphagia after cerebral stroke. Methods:In this cross-sectional survey, 120 patients with dysphagia after cerebral stroke in the Department of Neurology, Stroke Center in Henan Provincial People′s Hospital from January 2021 to March 2022 were selected with convenience sampling method for questionnaire survey. The depression anxiety stress scale-21 (DASS-21), family adaption partnership growth affection resolve index (APGAR) and social support rating scale (SSRS) were used to evaluate the adverse psychological state, family care degree and social support in those patients. And general data of the patients were collected too. A total of 120 questionnaires were sent out and reclaimed, among them 116 were valid. One patient withdrew from the study on his own. Finally, 115 patients were included as research objects. Analysis of variance and LSD-t test were used to compare the adverse psychological state of patients with different characteristics, Pearson correlation analysis was used to analyze the correlation between adverse psychological state and family care and social support. The multivariate linear regression analysis was used to analyze the influencing factors of adverse psychological state in patients with dysphagia after cerebral stroke. Results:The total DASS-21 score in the 115 patients with dysphagia after stroke was (43.32±6.58) points, of which, it was (16.02±3.45) points in the depression dimension, (14.55±3.27) points in anxiety dimension, (12.75±4.01) points in stress dimension; the total score of APGAR was (5.15±0.87) points, the total score of SSRS was (28.75±5.16) points. Family care and social support were both negatively correlated with adverse psychological state in those patients ( r=-0.514, -0.433, both P<0.05). Marital status, per capita monthly income, number of complications, family care and social support were the influencing factors of adverse psychological state in patients with dysphagia after cerebral stroke (β′=0.501, -0.365, 0.234, -0.269, -0.372, all P<0.05). Conclusions:Patients with dysphagia after cerebral stroke have a higher degree of adverse psychological state. Marital status, per capita monthly income, complications, family care and social support can affect the mental health in those patients.

ABSTRACT

Objective@# To observe the clinical effects of auricular point therapy on burning mouth syndrome (BMS) and its effect on the psychological state of patients and plasma β-endorphin. @*Methods @# A total of 105 patients diagnosed with BMS were randomly divided into an auricular acupoint application group (50 cases) and a drug treatment group (55 cases). The treatment course lasted one month. The patients in the auricular acupoint application group selected 3 points on their tongue, heart and Shenmen through traditional Chinese medical dialectics used for patients with BMS. Wangbuliu seeds were applied, two ears were pressed alternately and one ear was applied each time. The patient was instructed to press the treatment site three times a day, 1-2 min each time, until the auricle skin became reddish and hot. The patients in the drug treatment group took vitamin E 100 mg+oryzanol 10 mg+vitamin B2 10 mg orally three times a day. Before and after treatment, the pain intensity and mental and psychological state of the patients were evaluated. The patient's plasma was detected before and after β-endorphin treatment. @* Results@#The pain sensation intensity of the two groups decreased after treatment (P<0.001). After treatment, the scores of somatization (t = 2.118, P = 0.037), fear (t = 2.084, P = 0.039) and diet and sleep (t = 2.047, P = 0.043) in the auricular acupoint application group were significantly improved compared with the level before treatment. The level of β-endorphin in plasma was increased, and the difference was statistically significant (t = 2.247, P = 0.027) in the auricular acupoint application group after treatment. @*Conclusion@#Auricular point therapy is an effective method for patients with BMS, improving psychological state and promoting the synthesis of plasma β-endorphin may be one of its mechanisms.

ABSTRACT

【Objective】 To investigate the penile appearance, sexual function, psychological status and related influencing factors of adult patients who underwent hypospadias repair surgery in their minors, so as to provide reference for the diagnosis, treatment and prognosis of hypospadias. 【Methods】 This study included 50 adult hypospadias patients who underwent urethroplasty in our hospital during May 2005 and Aug. 2018. The present appearance, sexual function and psychological status were evaluated. The correlation and consistency between hypospadias objective scoring evaluation (HOSE) and pediatric penile perception score (PPPS) were analyzed. Factors affecting the results were determined with univariate and multivariate regression analysis. 【Results】 The satisfaction rate of HOSE was significantly correlated with the urethral length and complications (P=0.024, P=0.033). The satisfaction rate of PPPS was significantly correlated with the number of urethral operations and postoperative complications (P=0.041, P=0.023). There was a weak correlation between HOSE and PPPS (r=0.291, P=0.040), but almost no consistency (Kappa=0.2, P=0.107). Sixty percent of the patients paid attention to the ventral appearance of penis, whose dissatisfaction rate of PPPS was higher than those who did not pay attention to the ventral appearance of the penis (19/30 vs. 6/20, P=0.021). Patients with multiple operations (>1), postoperative complications or unsatisfactory penile appearance were more likely to have inferiority complex (52.6% vs. 22.6%, P=0.029; 59.1% vs.14.3%, P=0.001;61.5% vs. 24.3%, P=0.015). Multivariate regression analysis showed that dissatisfaction with the appearance of the penis was an independent risk factor for patients’ perception of their own physical defects. Among the patients who believed that they had physical defects, the percentage of patients with grade IV penile erectile hardness was significantly lower than that of those who denied they had physical defects (9/17 vs. 27/33, P=0.047). 【Conclusion】 The undesirable postoperative penis appearance is likely to have a negative impact on patients’ long-term psychological state, which might further damage the sexual function in adulthood. Surgeons should pay attention to the penile appearance during the conduction of hypospadias repair.
