ABSTRACT

Introduction: achalasia is an esophageal motility disorder, characterized by irregular peristalsis and inadequate relaxation of the lower esophageal sphincter, typically manifests with dysphagia as its primary symptom. The advent of high-resolution manometry (HRM) has transformed our comprehension of achalasia. It has delineated three distinct subtypes based on variations in pressurization and contractions, contributing significantly to the understanding of this condition. This study aimed to describe and compare clinically and manometrically the patients suffering from the different subtypes of achalasia. Methods: it is a retrospective single-center study including all patients with achalasia diagnosed by high-resolution manometry (HRM) between April 2018 and January 2023. The diagnosis was retained based on the latest Chicago 3.0 classification. Results: out of 103 patients, 25.2% (n=26) had type I achalasia, 66.9% (n=69) type II, and 7.8% (n=8) type III. Patients with type I and II achalasia were younger than patients with type III achalasia (mean age 43.2 years vs. 55.6 years, P=0.0053). A female predominance was found in type I achalasia and a male predominance in type III. The most frequent symptoms were: dysphagia (found in all our patients). At HRM, all our patients had a pathological IRP with a mean of 26.95 mmHg for the 3 types of achalasia. Resting IBS pressures were higher in patients with type III achalasia compared to types I and II (35.28 mmHg vs 32.7 mmHg and 31.04 mmHg, P=0.40). Conclusion: achalasia is one of the most studied esophageal motility disorders. Tools such as HRM have permitted us not only to identify achalasia early but also to define different subtypes, which can have therapeutic implications.

ABSTRACT

ObjectiveTo identify the prototypical components and metabolites absorbed into blood and cerebrospinal fluid of Schisandrae Chinensis Fructus(SCF) based on sequential metabolism combined with liquid chromatography-mass spectrometry. MethodBlood and cerebrospinal fluid samples of integrated metabolism, intestinal metabolism and hepatic metabolism were collected from male SD rats after gavage and in situ intestinal perfusion administration, and ultra-performance liquid chromatography-quadrupole/electrostatic field orbitrap high-resolution mass spectrometry(UPLC Q-Exactive Orbitrap MS) was used to analyze and compare the differences in the spectra of SCF extract, blank plasma, administered plasma, blank cerebrospinal fluid and administered cerebrospinal fluid with ACQUITY UPLC BEH Shield RP18 column(2.1 mm×100 mm, 1.7 µm), the mobile phase was acetonitrile(A)-0.1% formic acid aqueous solution(B) for gradient elution(0-7 min, 95%B; 7-12 min, 95%-35%B; 12-17 min, 35%-15%B; 17-20 min, 15%-12%B; 20-22 min, 12%-5%B; 22-23 min, 5%B; 23-25 min, 5%-95%B; 25-28 min, 95%B). And heated electrospray ionization(HESI) was used with positive and negative ion modes, the scanning range was m/z 100-1 500. The prototypical constituents and their metabolites absorbed into blood and cerebrospinal fluid of SCF were identified according to the retention time, characteristic fragments, molecular formulae and the information of reference substances. ResultA total of 42 chemical components were identified in the extract of SCF, including lignans, flavonoids, amino acids, tannins, and others, of which lignans were the main ones. A total of 27 prototypical components and 14 metabolites were identified in plasma samples from different sites. A total of 15 prototypical components and 9 metabolites were identified in cerebrospinal fluid. The main metabolic reactions involved in the formation of metabolites were mainly demethylation, methylation, demethoxylation and hydroxylation. ConclusionThrough the systematic identification of the prototypical components and metabolites of SCF in rats, it provides data support for further better exploring the material basis of SCF in the treatment of central nervous system diseases.

ABSTRACT

ObjectiveThrough the correlation analysis between intestinal absorption profile and inhibition of macrophage foaming, the pharmacodynamic components of Zhuriheng dripping pills(ZRH) were explored to provide a basis for establishing its quality standard. MethodIntestinal absorption fluids with 0, 5, 10, 15, 20 times clinical equivalent doses were prepared by a rat everted gut sac(EGS), and the oxidized low density lipoprotein(ox-LDL)-induced RAW264.7 macrophage foaming model was used to investigate the effect of intestinal absorption fluid with different doses on the accumulation of lipids in RAW264.7 cells by oil red O staining and cholesterol content determination, and to screen for the optimal dose. Ultra performance liquid chromatography-quadrupole-electrostatic field orbitrap high-resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS) was used to analyze and identify intestinal absorption fractions of ZRH intestinal absorption fluids, and partial least squares-discriminant analysis(PLS-DA) and orthogonal partial least squares-discriminant analysis(OPLS-DA) were performed on different doses of ZRH intestinal absorption fluids using SIMCA 13.0 with peak area as the independent variable and the pharmacodynamic indicators as the dependent variables to screen the compounds with variable importance in the projection(VIP) value>1.0 as contributing components, and Pearson correlation analysis was used to determine the spectral effect relationship, determined the compounds and positive correlation with pharmacodynamic were as active ingredients. Molecular docking was used to verify the binding energy of peroxisome proliferator-activated receptor α(PPARα), PPARγ, PPARβ, human retinoid X receptor α(RXRA) and nuclear transcription factor-κB(NF-κB) with the active ingredients in ZRH intestinal absorption fluids. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) was performed to detect the mRNA levels of PPARγ, scavenger receptor A1(SRA1) and adenosine triphosphate-binding cassette transporter A1(ABCA1) in RAW264.7 cells, Westen blot was used to detect the expression level of PPARγ protein in RAW264.7 cells, and enzyme-linked immunosorbent assay(ELISA) was used to detect the levels of interleukin(IL)-1β and NF-κB in RAW264.7 cells. ResultAccording to the results of oil red O staining and cholesterol content determination, the ZRH intestinal absorption fluids could significantly reduce macrophage foaming, and intestinal absorption fluids with 15, 20 times clinical equivalent doses had the best effect, the 15-fold ZRH intestinal absorption fluid was finally determined as the study subject. Spectral effect relationship showed that 52 corresponding peaks in the ZRH-containing intestinal fluid were positively correlated with the efficacy, including organic acids, phenylpropanoids, iridoids, flavonoids, bile acids, coumarins and chromones. Target validation results showed that 86.9%-96.2% of the total components processed good binding activities with the key targets of PPARα, PPARγ, PPARβ, RXRA and NF-κB, and the docking energy values were all less than -6.0 kcal·mol-1(1 cal≈4.19 J). The results of validation showed that, compared with the normal group, the model group showed a significant increase in the levels of SRA1 and PPARγ mRNA expression, a significant decrease in ABCA1 mRNA expression, a significant increase in the level of PPARγ protein expression, and a significant increase in the levels of IL-1β and NF-κB(P<0.01), compared with the model group, the 15-fold intestinal absorption fluid group showed a significant decrease in the levels of SRA1 and PPARγ mRNA expression(P<0.05, P<0.01), ABCA1 mRNA expression level was significantly up-regulated, the levels of IL-1β and NF-κB were significantly reduced(P<0.01), and PPARγ protein expression level was significantly reduced(P<0.05). ConclusionThis study identifies 52 components and their metabolites in ZRH intestinal absorption fluid that are positively correlated with the inhibition of macrophage foaming, which may be related to the regulation of the PPARs pathway in cells and the reduction of the levels of inflammatory factors, and can provide a reference for the quality control and clinical application of ZRH.

ABSTRACT

Objective To develop an unmanned aerial vehicle (UAV)-borne radiation monitoring system with high detection efficiency and nuclide identification ability for airborne monitoring in nuclear emergency. Methods The UAV-borne CeBr3 radiation monitoring system was composed of four cerium bromide (CeBr3) crystal detectors coupled with silicon photomultipliers (SiPMs) and other components including integrated modules, intelligent electronic devices, and new composite materials. Results According to various performance tests on the system, the crystal energy resolution was better than 5% (@0.662 MeV), the peak drift of the energy spectrum was within ±1 channel, the linear fit of energy was 0.99997, the change in the count rate of each energy window during 12 h long-term measurement was less than 5%, and the detection efficiency was higher compared with that of NaI (Tl) detectors of the same volume. Conclusion Through ground point source testing and theoretical calculation, the system has reliable ability to identify radionuclides, which can be used in nuclide identification and the preparedness and response for nuclear and radiation emergencies.

ABSTRACT

ObjectiveTo investigate the effects of Aconiti Coreani Radix and Typhonii Rhizoma on the urinary metabolites of gerbils with stroke by non-targeted metabolomics technique, and then to clarify the mechanism of the two, as well as their similarities and differences. MethodTwenty-four gerbils were randomly divided into control group(CG), model group(MG), Aconiti Coreani Radix group(RA) and Typhonii Rhizoma group(RT). Except for the CG, ischemic stroke model was constructed using right unilateral ligation of gerbil carotid artery in the remaining groups. Except for the CG and MG, rats in the other groups received whole powder suspension(0.586 mg·g-1) was administered for 14 days. The neurological deficit in each group was scored by Longa scoring on days 0, 3, 7 and 14. After the end of administration, the serum, brain tissue and urine of gerbils in each group were collected, and the rate of cerebral infarction was detected by 2,3,5-triphenyltetrazolium chloride(TTC), and the levels of interleukin(IL)-6, tumor necrosis factor(TNF)-α, malondialdehyde(MDA), superoxide dismutase(SOD), glutathione(GSH), and nitric oxide(NO) in serum and brain tissue were determined by enzyme-linked immunosorbent assay(ELISA). The urine metabolomics of gerbils in each group was studied by ultra performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UPLC-Q-Orbitrap-MS), and the data were processed by multivariate statistical analysis, and differential metabolites were screened based on value of variable importance in the projection(VIP) of the first principal component>1 and t-test P<0.05. Metabolic pathway analysis of the screened differential metabolites was performed using Kyoto Encyclopedia of Genes and Genomes(KEGG) database and Metaboanalyst 5.0. ResultCompared with the CG, the neurological deficit score was significantly increased in the MG(P<0.05), compared with the MG, the neurological deficit scores in the RA and RT were significantly reduced after 7 d and 14 d(P<0.05). Compared with the CG, the rate of cerebral infarction was significantly increased in the MG(P<0.05), compared with the MG, the rates of cerebral infarction in the RA and RT were significantly reduced(P<0.05). Compared with the CG, the levels of IL-6, TNF-α, and MDA in the serum and brain tissue of gerbils from the MG were significantly increased(P<0.05), and the levels of SOD, GSH and NO were significantly reduced(P<0.05). Compared with the MG, Aconiti Coreani Radix and Typhonii Rhizoma could down-regulate the levels of IL-6, TNF-α and MDA, and up-regulated the levels of SOD, GSH and NO. A total of 112 endogenous differential metabolites were screened by urine metabolomics, of which 16 and 26 metabolites were called back by Aconiti Coreani Radix and Typhonii Rhizoma, and could be used as potential biomarkers for both treatments in stroke gerbils, respectively. The results of the pathway analysis showed that both Aconiti Coreani Radix and Typhonii Rhizoma had regulatory effects on arginine and proline metabolism, pyrimidine metabolism, and aminoacyl-tRNA biosynthesis. In addition, Aconiti Coreani Radix could also regulate riboflavin metabolism, Typhonii Rhizoma could also regulate purine metabolism, glycine, serine and threonine metabolism, arachidonic acid metabolism, biosynthesis of pantothenate and coenzyme A, and β-alanine metabolism. ConclusionBoth Aconiti Coreani Radix and Typhonii Rhizoma have better therapeutic effects on stroke, with Aconiti Coreani Radix having stronger effects. From the metabolomics results, the main metabolic pathways regulated by Aconiti Coreani Radix involve amino acid metabolism, oxidative stress and so on, while Typhonii Rhizoma mainly involve amino acid metabolism, lipid metabolism, energy metabolism, etc.

ABSTRACT

ObjectiveTo qualitatively analyze the chemical constituents and their tissue distribution in Lujiao formula based on ultra performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UPLC-Q-Orbitrap-MS). MethodThe separation was performed on an ACQUITY UPLC® BEH C18 column(2.1 mm×100 mm, 1.7 μm) with the mobile phase of 0.1% formic acid aqueous solution(A)-methanol(B) in a gradient elution(0-2 min, 4%B; 2-6 min 4%-12%B; 6-38 min, 12%-70%B; 38-38.5 min, 70%B; 38.5-39 min, 70%-95%B; 39-43 min, 95%B; 43-43.1 min, 95%-4%B; 43.1-45 min, 4%B), the flow rate was 0.3 mL·min-1 with the column temperature of 40 ℃ and the injection volume of 5 µL. The data were acquired by an electrospray ionization(ESI) in the full scanning mode of positive and negative ions, the scanning rang was set at m/z 100-1 500, the collision energies were 10, 20, 40 eV. Retention time, precise relative molecular mass and secondary mass spectrometry fragment ions were used to identify the compounds in Lujiao formula and analyze their tissue distribution, combing with self-established database and comparing with standard substances and published literature data. ResultA total of 260 compounds, including 156 flavonoids, 43 terpenoids, 18 coumarins, 13 organic acids, 7 phenylethanoids, 7 alkaloids and 16 others, were identified or hypothesized from Lujiao formula, 68 of which were identified by comparison with standard substances. And the results of tissue distribution showed that 100, 143, 129 and 126 prototype components were detected in blood, heart, liver and kidney, respectively. ConclusionThe chemical composition of Lujiao formula and their tissue distribution were systematic analyzed, which can provide reference for the quality control, clinical application, pharmacokinetics and pharmacodynamic material basis of Lujiao formula and its medicinal materials.

ABSTRACT

¿Porque grupos como las FARC firmaron un acuerdo de paz, mientras otros grupos como el ELN se niegan a hacerlo? La respuesta a este interrogante está ligada al concepto de punto de inflexión, que es el momento crucial en el que los acontecimientos giran hacia su resultado final. National Security Research Division (RAND) ha determinado indicadores comunes a los puntos de inflexión de ochenta y nueve conflictos contrainsurgentes, con el fin de promover e identificar la llegada de esta fase final del conflicto. Este trabajo presenta una metodología de análisis cualitativo basada en estos indicadores de debilitamiento de la insurgencia, que sugieren los posibles resultados de los conflictos contrainsurgentes. Estos indicadores se relacionan con la dinámica del conflicto colombiano, lo que permite validar determinadas condiciones estratégicas que posibilitaron la salida negociada al conflicto con las FARC (Fuerzas Armadas Revolucionarias de Colombia). Las conclusiones incluyen ocho indicadores presentes en el curso del conflicto sin los cuales no hubiese sido posible el acuerdo de paz en el resultado tipo II: Gobierno Gana. El estudio propone criterios de evaluación y gestión del conflicto.


Why did groups like FARC sign a peace agreement while other groups like ELN refuse to do so? The answer to this question is linked to the concept of a tipping point, which is the point at which events take a crucial turn toward their outcome. The National Security Research Division (RAND) determined strategic indicators common to tipping points in 89 counterinsurgency conflicts to promote and identify the arrival of this final phase of the conflict. This work presents a qualitative analysis methodology based on these indicators of insurgency weakening that suggest possible outcomes of counterinsurgency conflicts. These indicators are related to information on the dynamics of the Colombian conflict, which allows validation of certain strategic conditions that enabled the negotiated exit from the conflict with FARC. The conclusions include eight indicators present in the course of the conflict without which the peace agreement would not have been possible in the Type II outcome: Government Wins. The study proposes criteria for conflict evaluation and management.


Por que grupos como as FARC assinaram um acordo de paz, enquanto outros grupos, como o ELN, se recusaram a fazê-lo? A resposta a essa pergunta está ligada ao conceito de ponto de inflexão, que é o momento crucial em que os eventos se voltam para seu resultado final. A National Security Research Division (RAND) identificou indicadores comuns aos pontos de inflexão de 89 conflitos de contrainsurgência para promover e identificar a chegada dessa fase final do conflito. Este documento apresenta uma metodologia de análise qualitativa com base nesses indicadores de enfraquecimento da insurgência, que sugerem os possíveis resultados dos conflitos de contrainsurgência. Esses indicadores estão relacionados à dinâmica do conflito colombiano, o que permite a validação de determinadas condições estratégicas que possibilitaram a solução negociada do conflito com as FARC (Forças Armadas Revolucionárias da Colômbia). As conclusões incluem oito indicadores presentes no curso do conflito, sem os quais o acordo de paz não teria sido possível no resultado do tipo II: vitórias do governo. O estudo propõe critérios para avaliação e gerenciamento de conflitos.

ABSTRACT

High-resolution manometry (HRM) is a motility diagnostic system that measures intraluminal pressure of the gastrointestinal tract using a series of closely spaced pressure sensors. The topographic plot generated by HRM software makes it possible to visualize phonation pressures at the pharynx, UES, and body of the esophagus in real time, indicating pressure intensity by color, which permits easy data interpretation. It has been largely used for swallowing study and dysphagia diagnosis. Due to the proximity of the pharyngoesophageal and laryngeal structures, this technology instigated voice researchers. Despite the few studies published so far, high-resolution manometry has yet proven to be an extremely useful tool in obtaining entire pharyngoesophageal segment pressure measurements during phonation. It also allows natural voice production not interfering with the mouth area. HRM data already brought light to subglottic pressure, vertical laryngeal excursion, cricopharyngeal muscle activation, air flow, muscle tension associated with vocalization and pressure variations associated with different phonatory stimuli.


La manometría de alta resolución (HRM, por sus siglas en inglés) es un sistema de diagnóstico de motilidad que mide la presión intraluminal del tracto gastrointestinal mediante una serie de sensores de presión dispuestos de manera cercana. El gráfico topográfico generado por el software de HRM permite visualizar las presiones de fonación en la faringe, el EEI y el cuerpo del esófago en tiempo real, indicando la intensidad de la presión mediante colores que facilitan la interpretación de los datos. Ha sido ampliamente utilizado para el estudio de la deglución y el diagnóstico de la disfagia. Debido a la proximidad de las estructuras faringoesofágicas y laríngeas, esta tecnología ha despertado el interés de los investigadores en voz. A pesar de los pocos estudios publicados hasta ahora, la manometría de alta resolución ha demostrado ser una herramienta extremadamente útil para obtener mediciones de presión de todo el segmento faringoesofágico durante la fonación. Además, permite la producción natural de la voz sin interferir en el área de la boca. Los datos de HRM ya han arrojado luz sobre la presión subglótica, la excursión laríngea vertical, la activación del músculo cricofaríngeo, el flujo de aire, la tensión muscular asociada con la vocalización y las variaciones de presión asociadas con diferentes estímulos fonatorios.

ABSTRACT

Introduction: Anal squamous cell carcinoma is rare, but its incidence and mortality have been increasing globally; 90% of cases are related to human papillomavirus (HPV) infection(1,2). Patients with inflammatory bowel disease (IBD) have a higher risk of infection with this virus; an incidence of 5.5 per 100,000 patients has been identified in the IBD group compared to 1.8 in the non-IBD group(3). Materials and methods: A descriptive case series study was conducted with 21 patients with IBD and no perianal symptoms between January and July 2022 at the Institute of Coloproctology in Medellín. They underwent anal cytology, HPV genotyping, and high-resolution anoscopy after explanation and acceptance of the procedure. If lesions were found, ablative treatment was performed. Results: 23% of this cohort had low-grade squamous lesions, while 14.2% had high-grade lesions with dysplasia changes during anoscopy. Besides, 90.4% had positive HPV genotyping, of which 76.1% were high-grade. Conclusions: Our study suggests that this series of patients with IBD behaves as a high-risk group for developing premalignant lesions in association with HPV. High-resolution anoscopy is a cost-effective, painless, and affordable method that, in expert hands, may impact the diagnosis and management of premalignant lesions and decrease the incidence of anal cancer in patients with IBD.


Introducción: El carcinoma anal de células escamosas es raro, pero su incidencia y mortalidad han ido en aumento globalmente. El 90% de los casos están relacionados con la infección por el virus del papiloma humano (VPH)1,2 y los pacientes con enfermedad inflamatoria intestinal (EII) tienen un mayor riesgo de infección por este virus; se ha identificado una incidencia de 5,5 por 100.000 pacientes en el grupo de EII frente a 1,8 en el grupo sin EII3. Metodología: Se realizó un estudio descriptivo de serie de casos en el que se incluyó a 21 pacientes con EII, sin síntomas perianales, entre enero y julio de 2022 en el Instituto de Coloproctología de Medellín, a quienes se les realizó citología anal, genotipificación del VPH y anoscopia de alta resolución, previa explicación y aceptación del procedimiento. Si se encontraban lesiones, se realizaba tratamiento ablativo. Resultados: El 23% de esta cohorte tenía lesiones escamosas de bajo grado, mientras que el 14,2% presentaba lesiones de alto grado con cambios de displasia al momento de la anoscopia. El 90,4% tenían genotipificación positiva para VPH, de los cuales el 76,1% eran de alto grado. Conclusiones: Nuestro estudio sugiere que esta serie de pacientes con EII se comporta como un grupo de alto riesgo para desarrollar lesiones premalignas en asociación con el VPH. La anoscopia de alta resolución es un método rentable, indoloro y asequible que, en manos expertas, puede impactar el diagnóstico y manejo de lesiones premalignas y disminuir la incidencia de cáncer anal en pacientes con EII.

ABSTRACT

Introducción: La mediación adquiere elevada significación social, a partir de su vínculo con la prevención y solución de conflictos. Sin embargo, la revisión bibliográfica y la experiencia profesional de los autores indican que el tema es insuficientemente tratado en relación con el desempeño asistencial del médico de familia. El objetivo del artículo consistió en valorar aspectos asociados a este profesional como mediador en conflictos vinculados a la salud. Métodos: Investigación cualitativa comprendida entre febrero de 2023 y junio de 2023 en el área de salud del Policlínico Docente "René Vallejo Ortiz". Se emplearon métodos teóricos como el análisis y síntesis, el histórico-lógico y la sistematización. En el orden empírico se empleó la revisión de documentos y la encuesta a médicos de familia. Resultados: Fueron develados criterios valorativos sobre aspectos legales, curriculares, metodológicos y asistenciales vinculados al tema. También se obtiene, a partir de la autopercepción del médico de familia, valoraciones sobre la preparación para un desempeño mediador. Discusión: Se valoran integralmente aspectos asociados al médico de familia como mediador en conflictos vinculados a la salud y se revela una correspondencia con estudios precedentes respecto a la necesidad de optimizar en la educación médica la mediación como contenido de enseñanza. Conclusiones: Se enfatiza que el desempeño mediador del médico de familia en los conflictos vinculados a la salud, no se reducen a las posibilidades de intervención reconocidas en el ordenamiento jurídico. Se corrobora la pertinencia del tema y la necesidad de introducir trasformaciones en este sentido.


Introduction: Mediation acquires high social significance, based on its link with the prevention and resolution of conflicts. However, the bibliographic review and the professional experience of the authors indicate that the topic is insufficiently addressed in relation to the care performance of the family doctor. The objective of the article was to assess aspects associated with this professional as a mediator in conflicts related to health. Methods: Qualitative research between february 2023 and june 2023 in the health area of ​​the "René Vallejo Ortiz" Teaching Polyclinic. Theoretical methods such as analysis and synthesis, historical-logical and systematization were used. In the empirical order, the review of documents and the survey of family doctors were used. Results: Evaluative criteria on legal, curricular, methodological and healthcare aspects linked to the topic were revealed. Assessments of preparation for mediation performance are also obtained from the family doctor's self-perception. Discussion: Aspects associated with the family doctor as a mediator in conflicts related to health are comprehensively assessed and a correspondence with previous studies is revealed regarding the need to optimize mediation as teaching content in medical education. Conclusions: It is emphasized that the mediating performance of the family doctor in conflicts related to health is not reduced to the possibilities of intervention recognized in the legal system. The relevance of the topic and the need to introduce transformations in this sense are corroborated.

ABSTRACT

Considerando-se os desafios da composição de grupos de diálogos em temas socialmente polêmicos, este estudo teve como objetivo apresentar as estratégias utilizadas para compor um grupo de diálogo a partir da metodologia do Projeto de Conversações Públicas entre pessoas com distintas opiniões sobre a abordagem do tema da diversidade sexual e de gênero nas escolas brasileiras. As transcrições das audiogravações das conversas preparatórias e dos encontros grupais e as anotações de diário de campo compuseram o corpus de análise deste estudo. A análise temática de conteúdo serviu para o destaque das seguintes estratégias consideradas úteis para a composição grupal: 1) Reconhecendo as pessoas envolvidas no conflito; 2) Entrando em contato com pessoas-chave; 3) Recrutando participantes reservas; 4) Pensando formas alternativas de presença no grupo; 5) Reconhecendo quem precisaria estar na conversa; 6) Flexibilidade com relação a entrada de novas/os participantes; 7) Lidando com os estereótipos com relação as/aos facilitadoras/es; 8) Co-construindo estratégias para maior adesão grupal. Esses eixos analíticos foram discutidos a partir da literatura da área.


Considering the challenges of composing dialogue groups in polemic issues, this study aimed to present the strategies used to compose a dialogue group based on the methodology of the Public Conversations Project among people with different opinions on the approach to the topic of sexual and of gender in Brazilian schools. Transcripts of audio recordings of preparatory conversations and group meetings and field diary notes composed the corpus of analysis of this study. The thematic content analysis served to highlight the following strategies considered useful for group composition: 1) Recognizing the people involved in the conflict; 2) Contacting key people; 3) Recruiting reserve participants; 4) Thinking about alternative forms of presence in the group; 5) Recognizing who would need to be in the conversation; 6) Flexibility regarding the entry of new participants; 7) Dealing with stereotypes regarding facilitators; 8) Co-building strategies for greater group membership. The selected material was discussed based on the literature in the area.


Considerando los desafíos de la composición de grupos de diálogo, este estudio tuvo como objetivo presentar las estrategias utilizadas para la composición de un grupo de diálogo basado en la metodología del Proyecto Conversaciones Públicas entre personas con opiniones diferentes sobre el abordaje del tema sexual y de género en las escuelas brasileñas. Transcripciones de grabaciones de audio de conversaciones preparatorias y reuniones grupales y notas del diario de campo componen el corpus de análisis de este estudio. El análisis de contenido temático sirvió para resaltar las siguientes estrategias consideradas útiles para la composición del grupo: 1) Reconocimiento de las personas involucradas en el conflicto; 2) Ponerse en contacto con personas clave; 3) Reclutamiento de participantes de reserva; 4) Pensar en formas alternativas de presencia en el grupo; 5) Reconocer quién debería estar en la conversación; 6) Flexibilidad en la entrada de nuevos participantes; 7) Abordar los estereotipos sobre los facilitadores; 8) Estrategias de co-construcción para una mayor membresía del grupo. ABSTRACT

Este artículo revisó la visión de paz del Episcopado colombiano en los gobiernos de Álvaro Uribe (2002-2010). La constante participación de los prelados en los procesos de paz ha escapado de la atención de la investigación académica, y no ha tenido en su visión de la paz su foco de interés. Este artículo, desde el paradigma hermenéutico-crítico, y el método de Análisis Crítico del Discurso, concluye que dicha participación ha estado marcada por su postura a favor de la paz positiva, que no solo establece la ausencia de guerra y violencia directa, sino también la presencia de la justicia social.


This article reviewed the Colombian Episcopate's vision of peace under Alvaro Uribe's Governments (2002-2010). The constant participation of the prelates in peace processes has escaped the attention of academic research, and their vi sion of peace has not been their focus of interest. This article, from the hermeneutic-critical paradigm, and the method of Critical Discourse Analysis, concludes that such participation has been marked by their stance in favor of positive peace, which not only establishes the absence of war and direct violence, but also the presence of social justice.

ABSTRACT

Introduction: Limited information is available regarding the clinical and manometric characteristics of different subtypes of achalasia. This study aims to describe these characteristics in patients treated at a prominent hospital in Colombia. Methods: This descriptive observational study included patients diagnosed with achalasia using high-resolution esophageal manometry at Hospital Universitario San Ignacio in Bogotá, Colombia, between 2016 and 2020. We documented the clinical manifestations, manometric findings, treatment approaches, and response to treatment based on the subtype of achalasia. Results: A total of 87 patients were enrolled, with a median age of 51 years, and 56.4% of them were female. The majority had type II achalasia (78.1%), followed by type I (16%) and type III (5.7%). All patients presented with dysphagia, 40.2% experienced chest pain, and 27.6% had gastroesophageal reflux. The clinical parameters, including integrated relaxation pressure value (IRP; median: 24 mmHg, interquartile range [IQR]: 19-33), upper esophageal sphincter pressure (UES; median: 63 mmHg, IQR: 46-98), and lower esophageal sphincter pressure (LES; median: 34 mm Hg, IQR: 26-45), were similar across the different subtypes. Esophageal clearance was incomplete in all patients. Among the 35 patients who received intervention, Heller's myotomy was the most commonly employed procedure (68.5%), followed by esophageal dilation (28.6%). All patients experienced symptomatic improvement, with a median pre-treatment Eckardt score of 5 (IQR: 5-6) and a post-treatment score of 1 (IQR: 1-2). Conclusions: Type II achalasia is the most prevalent subtype. The clinical and manometric findings, as well as treatment response, exhibit similarities among the different subtypes of achalasia. In Colombia, the outcomes of this condition align with those reported in other parts of the world.


Introducción: existe información limitada sobre las características clínicas y manométricas de los diferentes subtipos de acalasia. Este estudio describe dichas características en pacientes manejados en un hospital de referencia en Colombia. Método: estudio descriptivo observacional que incluye a pacientes con diagnóstico de acalasia por manometría esofágica de alta resolución manejados en el Hospital Universitario San Ignacio de Bogotá, Colombia, entre 2016 y 2020. Se describen las manifestaciones clínicas, hallazgos manométricos, tratamiento utilizado y respuesta al mismo según el subtipo de acalasia. Resultados: se incluyeron a 87 pacientes (mediana de edad: 51 años, 56,4% mujeres). La mayoría de tipo II (78,1%), seguido por tipo I (16%) y tipo III (5,7%). Todos presentaron disfagia, 40,2% dolor torácico y 27,6% reflujo gastroesofágico. La clínica y los valores del integral de presión de relajación (IRP; mediana: 24 mm Hg, rango intercuartílico [RIC]: 19-33), presión del esfínter esofágico superior (EES; mediana: 63 mm Hg, RIC: 46-98) y presión del esfínter esofágico inferior (EEI; mediana: 34 mm Hg, RIC: 26-45) fueron similares en los diferentes subtipos. El aclaramiento esofágico fue incompleto en todos los pacientes. Entre 35 pacientes que recibieron manejo intervencionista, la miotomía de Heller fue la intervención más utilizada (68,5%), seguido por la dilatación esofágica (28,6%). La totalidad de estos pacientes presentó una mejoría sintomática con la mediana de Eckardt pretratamiento de 5 (RIC: 5-6) y postratamiento de 1 (RIC: 1-2). Conclusiones: la acalasia tipo II es la más común. La clínica y los hallazgos manométricos y respuesta a tratamiento son similares entre los subtipos de acalasia. En Colombia, esta entidad se comporta de forma similar a lo reportado en otras partes del mundo.

ABSTRACT

Aim: Surgical correction of congenital heart defects (CHD) often requires interruption of blood flow through cardiopulmonary bypass (CPB) and aortic cross-clamping (ACC), for which duration(s) are considered to be prognostic factors, along with intensive care unit (ICU) length of stay (ICULOS). The aim of this study was to evaluate these surgical prognostic factors in pediatric patients with different types of CHD regarding their type of lesion and associated genetic factors. Study Design: Cross-sectional cohort study with 307 pediatric patients. Place and Duration of Study: Pediatric Intensive Care Unit (ICU) of Hospital da Criança Santo Antônio, in Porto Alegre/RS, Brazil, from 2006-2009 (3 years) Methodology: After inclusion criteria, we studied 266 pediatric patients admitted for the first time in a reference cardiac pediatric ICU from Southern Brazil following cardiac surgery. Intraoperative prognostic factors such as duration of CPB, ACC and ICULOS, in addition to dysmorphological and cytogenetic examinations were compiled and analyzed. P-values of <0.05 were considered significant. Results: CPB time was associated to four outflow tract defects (Tetralogy of Fallot [ToF], transposition of the great arteries [TGA], double outlet right ventricle, and truncus arteriosus [TA]), atrioventricular septal defect, and hypoplastic left heart syndrome (HLHS) (P < 0.001). ACC duration was associated with three outflow tract defects (ToF, TGA, and TA) and HLHS (P < 0.001). Moreover, CPB and ACC times showed an association with cyanotic and complex heart defects, as well as prolonged ICULOS (P < 0.001). There was no relationship between these prognostic factors and syndromic aspects or cytogenetic findings. Conclusions: CHD type has an impact over CPB and ACC duration and ICULOS, whereas genetic factors are not associated with those prognostic factors.

(analítico) Este estudio analiza la relación entre la empatía y la disposición a la reconciliación socioemocional en adolescentes infractores de la ley. Con un diseño no experimental, correlacional-predictivo, participaron 171 jóvenes entre 15 y 23 años, judicializados con medida privativa de la libertad en Colombia, 81.1% (n = 137) hombres y 19.9 % (n = 34) mujeres. Se utilizó el índice de reactividad interpersonal y la escala de reconciliación socioemocional. Se hallaron correlaciones positivas (p < 0.05) entre la empatía y la reconciliación socioemocional. El análisis de regresión mostró que la empatía tiene efecto predictor positivo (p < 0.001) sobre la reconciliación socioemocional total (R2=0.154), intrapersonal (R2 = 0.081) e interpersonal (R2 = 0.185). Se concluye que la empatía incide en los procesos de reconciliación socioemocional de los adolescentes judicializados.

RESUMO: Objetivo: construir e validar um cenário de simulação clínica voltado para o ensino do gerenciamento de conflitos para estudantes de graduação em enfermagem. Método: estudo metodológico no qual construiu-se um cenário de gerenciamento de conflitos, na cidade de São Paulo, SP, Brasil, em 2022, a partir da revisão de literatura e da experiência pedagógica dos pesquisadores. Realizou-se validação de conteúdo com juízes da área por meio da técnica Delphi. Os dados foram analisados com Índice de Validade de Conteúdo (IVC>80%). Resultados: o cenário descreve a cena de um estagiário de enfermagem que solicita ao técnico de enfermagem realizar um banho no leito de um paciente. Tal fato é permeado por conflitos interpessoais. A validação de conteúdo contou com oito experts em simulação clínica e dois estudantes de enfermagem. Houve três "rounds", chegando ao IVC 100%. Conclusão: o cenário construído foi validado e contribuirá para o ensino prático desta competência na graduação.


ABSTRACT Objective: Develop and validate a clinical simulation scenario for undergrad nursing students to learn conflict management. Method: Methodological study in which a conflict management scenario was constructed in the city of São Paulo, SP, Brazil, in 2022, based on the literature review and the pedagogical experience of the researchers. Content validation was carried out with judges in the field using the Delphi technique. The data was analyzed with a Content Validity Index (CVI>80%). Results: The scenario describes the scene of a nursing trainee asking the nursing technician to bathe a patient in bed. This is permeated by interpersonal conflicts. The content validation involved eight experts in clinical simulation and two nursing students. There were three "rounds," reaching IVC 100%. Conclusion: The scenario was validated and will contribute to the practical teaching of this skill in undergraduate courses.


RESUMEN Objetivo: Construir y validar un escenario de simulación clínica destinado a enseñar la gestión de conflictos a estudiantes universitarios de enfermería. Método: Estudio metodológico en el que se construyó un escenario de gestión de conflictos en la ciudad de São Paulo, SP, Brasil, en 2022, a partir de la revisión bibliográfica y de la experiencia pedagógica de los investigadores. La validación del contenido se llevó a cabo con jueces sobre el terreno mediante la técnica Delphi. Los datos se analizaron con un Índice de Validez del Contenido (IVC>80%). Resultados: el escenario describe la escena de un practicante de enfermería que pide al técnico de enfermería que bañe a un paciente en la cama. Esto está impregnado de conflictos interpersonales. En la validación del contenido participaron ocho expertos en simulación clínica y dos estudiantes de enfermería. Hubo tres rondas, alcanzando el IVC al 100%. Conclusión: el escenario fue validado y contribuirá a la enseñanza práctica de esta habilidad en los cursos de licenciatura.

ABSTRACT

Human cysticercosis is caused by cysticercus cellulosae, larvae of a tapeworm, taenia solium. Cysticercois can involve any tissue in the body even it has maximum tendency towards neural tissue. The most common affected sites are central nervous system, subcutaneous tissue, eyes and muscles. An isolated case of intramuscular cysticercosis is a rare presentation. Here we present the case of a 35 year old female with a swelling on the posterolateral aspect of right arm and was diagnosed as myocysticercosis which was abuting to the extensor muscle that is triceps muscle on high resolution ultrasound and ct scan and it was managed by antihelminthic medications followed by surgical excision.

Compreender as estratégias de resolução de conflitos utilizadas por adolescentes na relação com seus pais é fundamental para entender como ocorre seu desenvolvimento saudável. Este artigo investigou a resolução de conflitos de adolescentes em situações de confronto entre o seu domínio pessoal e o controle parental. 36 adolescentes com idades entre 15 e 17 anos, divididos igualmente conforme o sexo, responderam a uma entrevista semiestruturada, que continha quatro situações de conflito hipotéticas. Os dados foram submetidos à análise de conteúdo semântica e a testes não paramétricos. Os resultados foram categorizados em sete estratégias: Assunção de culpa, Submissão, Mentira, Hostilidade, Diálogo/Explicação, Negociação e Outra. A forma predominante de resolução utilizada foi o Diálogo/Explicação, considerada como uma forma recorrente de defender o domínio pessoal. Foram encontradas diferenças em relação ao sexo dos participantes e à situação hipotética. Por fim, os resultados são discutidos em termos de grau de autonomia e tipo de defesa do domínio pessoal.(AU)


Understanding the conflict resolution strategies used by adolescents in their relationship with their parents is fundamental to understanding how their healthy development occurs. This article investigated the resolution of conflicts by adolescents in confrontation situations between their personal domain and parental control. A total of 36 adolescents, aged 15 to 17 years, divided equally according to sex, answered a semi-structured interview that contained four hypothetical conflict situations. Data were subjected to semantic content analysis and non-parametric tests. The results were categorized into seven strategies: Assumption of Guilt, Submission, Lie, Hostility, Dialogue/Explanation, Negotiation, and Other. The predominant form of resolution used was Dialogue/Explanation, considered a recurrent form of defense of the personal domain. Differences were found depending on the participants' gender and the hypothetical situation. Finally, the results are discussed regarding the degree of autonomy and type of defense of the personal domain.(AU)


Enterarse de las estrategias de resolución de conflictos que los adolescentes utilizan en la relación con sus padres es fundamental para comprender cómo ocurre el desarrollo saludable de los adolescentes. A partir de una entrevista semiestructurada, presentamos cuatro situaciones hipotéticas de conflicto que fueron analizadas y respondidas por 36 adolescentes de entre 15 y 17 años, divididos según el género. Los datos se sometieron a un análisis de contenido semántico y a pruebas no paramétricas. Los resultados se categorizaron en siete estrategias de resolución de conflictos: Asunción de culpa, Sumisión, Mentira, Hostilidad, Diálogo/Explicación, Negociación y Otros. La forma de resolución más utilizada fue Diálogo/Explicación, y esta categoría fue una forma de defensa del dominio personal. Asimismo, se encontraron diferencias en función del género de los participantes y conforme la situación hipotética. Los resultados se discuten en términos de grado de autonomía y tipo de defensa del dominio personal.(AU)

ABSTRACT

A violência conjugal é um grave problema de saúde pública. Iniciativas que busquem compreender os elementos envolvidos na violência são relevantes para sua melhor compreensão e para o desenvolvimento de ferramentas interventivas. Este artigo apresenta um estudo qualitativo sobre as concepções de relacionamento amoroso presentes nos relatos de onze homens acusados de violência conjugal, conforme a Lei 11.340, Maria da Penha, em uma capital do Brasil. Foram utilizados questionários sociodemográficos e grupos focais para coleta das informações. Através da Análise Temática, destacam-se os resultados concernentes ao conceito de relacionamento amoroso, comunicação e estratégias de resolução de conflitos, bem como suas subcategorias. Verificou-se o quanto a figura tradicional masculina enfrenta um processo de ambiguidade em seu papel social, o que gera reverberações na relação conjugal. Ademais, o uso da violência como estratégia ineficaz de resolução de conflitos e a idealização amorosa ocupam um lugar de destaque. Salienta-se a importância de pesquisas e intervenções com esta população


Marital violence is a serious public health problem. Initiatives aimed at understanding the elements involved in domestic violence are relevant to better understand it and to develop intervention tools. This is a qualitative study and it aims to analyze and understand the conceptions of love relationships in the reports of men accused of conjugal violence and framed under Law 11.340, Maria da Penha, in a Brazilian capital. A Sociodemographic questionnaire and Focus Group were applied to data collection. Thematic Analysis was used to highlight the results concerning the concept of love relationships, couple's communication, and conflict resolution strategies and their subcategories. It was verified how much the traditional male figure faces a process of ambiguity in his social role, affecting the marital relationship. In addition, the use of violence as an ineffective strategy for conflict resolution and the love idealization occupy a prominent place. The importance of research and interventions in this population is emphasized


La violencia marital es un grave problema de salud pública. Las iniciativas destinadas a comprender los elementos involucrados en la violencia son relevantes para su mejor comprensión y para el desarrollo de herramientas de intervención. Este artículo presenta un estudio cualitativo sobre las concepciones sobre la relación amorosa presente en los informes de once hombres acusados de violencia conyugal (Ley 11.340, Maria da Penha), en una capital brasileña. El cuestionario sociodemográfico y el grupo focal se utilizaron para recopilar información. A través del análisis temático, destacamos los resultados sobre el concepto de relación de amor, comunicación y estrategias de resolución de conflictos. Se verificó cuánto se enfrenta la figura masculina tradicional a un proceso de ambigüedad en su papel social, produciendo reverberaciones en la relación matrimonial. Además, el uso de la violencia como una estrategia ineficaz para la resolución de conflictos y la idealización del amor ocupan un lugar destacado. Se enfatiza la importancia de la investigación y las intervenciones con esta población

MethodsRetrospective analysis was performed on patients (51 cases) suspected of ocular foreign bodies admitted to our hospital from January 2016 to December 2020 At the same time, DR corneal suture ring localization and CT three-dimensional reconstruction localization of orbital foreign bodies were performed, and the detection rate of foreign bodies by the two methods was calculated to analyze the location of ocular foreign bodies. ResultsThere were 38 cases of intraorbital foreign body identified by DR corneal suture ring localization method, and 46 cases of intraorbital foreign body identified by CT three-dimensional reconstruction localization method. The accuracy of CT three-dimensional reconstruction localization method was 90.20%, which was higher than that of DR corneal suture ring localization method 74.51% (P<0.05). Intraocular foreign bodies in 23 cases could be distinguished by DR corneal suture ring localization, and 25 cases by CT 3D reconstruction localization. The maximum diameter of intraocular foreign bodies that could be distinguished by CT 3D reconstruction localization was (2.65±0.14) mm. The diameter of intraspherical foreign body was (2.94±0.36) mm (P<0.05) lower than that which could be distinguished by DR corneal suture ring localization method. The results of DR corneal suture ring localization of orbital foreign body showed no difference compared with the results of CT 3D reconstruction localization (P>0.05). Forty cases of high density images inside and outside the eye could be clearly distinguished by CT 3D reconstruction. By using DR corneal suture ring localization method, 23 cases were confirmed to be intraocular high-density shadows, and 15 cases could not be confirmed to be intraocular high-density shadows (P < 0.05). ConclusionsFor the location of intraorbital foreign bodies, CT three-dimensional reconstruction can be used as a conventional method for locating ocular foreign bodies with high resolution and accuracy, and can detect fine metal particles inside the eyeball. Meanwhile, for a few small foreign bodies in the iris root, ciliary body and lateral suspension ligament, it is necessary to locate orbital foreign bodies with DR corneal suture ring.
