ABSTRACT

Sabe-se que para uma restauração ser considerada um sucesso clínico é preciso que haja uma concomitância saudável entre a estrutura dentária, o material restaurador e as estruturas biológicas periodontais. As restaurações transcirúrgicas caracterizam-se como procedimentos alternativos para possibilitar a restauração de dentes com cárie dentária extensa, restaurações subgengivais desadaptadas e fraturas. O presente estudo teve como objetivo realizar um relato de caso de um procedimento cirúrgico-restaurador, através de uma técnica de restauração transcirúrgica. Este estudo tem como justificativa relatar um caso clínico, colaborando com maiores informações sobre a escolha da técnica a ser empregada, mostrando em quais situações devemos escolher uma restauração transcirúrgica e quais seus benefícios. Também, salientar se devemos fazer a recuperação do espaço biológico do periodonto ou não(AU)


It is known that for a restoration to be considered a clinical success, there must be a healthy concomitance between the tooth structure, the restorative material and the periodontal biological structures. Transsurgical restorations are characterized as alternative procedures to enable the restoration of teeth with extensive dental caries, maladapted subgingival restorations and fractures. The present study aims to carry out a case report of a surgical-restorative procedure, using a transsurgical restoration technique. This study is justified by reporting a clinical case, providing more information on the choice of technique to be used, showing in which situations we should choose a transsurgical restoration and what its benefits are. Also, highlight whether we should recover the biological space of the periodontium or not(AU)

ABSTRACT

Restaurações diretas em resina composta são amplamente utilizadas em odontologia para restaurar dentes posteriores. Todavia, quando há grande destruição coronária, onde a distância do istmo excede dois terços da distância intercuspídea, as restaurações indiretas em resina composta são indicadas. O presente estudo teve como objetivo relatar a análise de um prontuário de um paciente que recebeu uma restauração indireta em resina composta em dente posterior amplamente destruído. Através da análise de prontuários de pacientes atendidos nas disciplinas de Estágios Supervisionados do Curso de Odontologia da FSG Centro Universitário no ano de 2023, foi selecionado um prontuário de um paciente que compareceu a clínica odontológica da FSG com uma restauração em amálgama fraturada com reparo em resina composta que apresentou sintomatologia dolorosa. O procedimento diagnóstico ocorreu através de exame clínico e radiográfico, que constatou a indicação de substituição da restauração insatisfatória e realização de uma restauração indireta em resina composta. Os resultados estéticos e funcionais apresentados demostraram a viabilidade da técnica restauradora indireta em resina composta para reabilitar dentes posteriores com ampla destruição coronária(AU)


Direct composite resin restoration are widely used in dentistry to restore posterior teeth. However, when there is large coronary destruction, that the distance from the isthmus exceeds two- thirds of the intercuspal distance, indirect composite resin restorations are indicated. This study aimed to report the analysis of a dental record of a patient who received an indirect restoration in composite resin in a badly destroyed posterior tooth. Through the analysis of dental records of patients seen in the disciplines of Supervised Internship of the Dentistry Course at FSG Centro Universitário in the year 2023, the dental record of a patient who attended the FSG dental clinic with fractured amalgam restoration with composite resin repair was selected who had painful symptoms. The diagnostic procedure took place through clinical and radiographic examination, which revealed the indication of replacing the unsatisfactory restoration and carrying out an indirect restoration in composite resin.The aesthetic and functional results presented demonstrated the viability of the indirect composite resin restoration technique for rehabilitating posterior teeth with extensive coronal destruction(AU)

ABSTRACT

Objective: In response to the demand for dental implants, extensive research has been conducted on methods for transferring load to the surrounding bone. This study aimed to evaluate the stresses on the peripheral bone, implants, and prostheses under scenarios involving of the following variables: prosthesis designs, vertical bone heights, load angles, and restorative materials. Material and Methods: Three implants were inserted in the premolar and molar regions (5-6-7) of the two mandibular models. Model 1 represented 0 mm marginal bone loss and Model 2 simulated 3 mm bone loss. CAD/CAM-supported materials, hybrid ceramic (HC), resin-nano ceramic (RNC), lithium disilicate (LiSi), zirconia (Zr), and two prosthesis designs (splinted and non-splinted) were used for the implant-supported crowns. Forces were applied vertically (90°) to the central fossa and buccal cusps and obliquely (30°) to the buccal cusps only. The stresses were evaluated using a three-dimensional Finite Element Analysis. Results: Oblique loading resulted in the highest stress values. Of the four materials, RNC showed the low stress in the restoration, particularly in the marginal area. The use of different restorative materials did not affect stress distribution in the surrounding bone. The splinted prostheses generated lower stress magnitude on the bone, and while more stress on the implants were observed. Conclusion: In terms of the stress distribution on the peri-implant bone and implants, the use of different restorative materials is not important. Oblique loading resulted in higher stress values, and the splinted prosthesis design resulted in lower stress (AU)


Objetivo: Em resposta à demanda por implantes dentários, extensa pesquisa foi realizada sobre métodos para transferir carga ao osso circundante. Este estudo buscou avaliar os estresses no osso periférico, implantes e próteses em cenários que envolvem as seguintes variáveis: designs de próteses, alturas ósseas verticais, ângulos de carga e materiais restauradores. Material e Métodos: Três implantes foram inseridos nas regiões dos pré-molares e molares (5-6-7) de dois modelos de mandíbula. O Modelo 1 representou perda óssea marginal de 0 mm e o Modelo 2 simulou perda óssea de 3 mm. Materiais suportados por CAD/CAM, cerâmica híbrida (HC), cerâmica nano-resina (RNC), dissilicato de lítio (LiSi), zircônia (Zr) e dois designs de próteses (sintetizadas e não-sintetizadas) foram utilizados para as coroas suportadas por implantes. Forças foram aplicadas verticalmente (90°) à fossa central e cúspides bucais e obliquamente (30°) apenas às cúspides bucais. Os estresses foram avaliados usando Análise de Elementos Finitos tridimensional. Resultados: Cargas oblíquas resultaram nos valores mais altos de estresse. Entre os quatro materiais, RNC mostrou baixo estresse na restauração, especialmente na área marginal. O uso de diferentes materiais restauradores não afetou a distribuição de estresse no osso circundante. Próteses sintetizadas geraram menor magnitude de estresse no osso, enquanto mais estresse nos implantes foi observado. Conclusão: Em termos de distribuição de estresse no osso peri-implantar e implantes, o uso de diferentes materiais restauradores não é crucial. Cargas oblíquas resultaram em valores mais altos de estresse, e o design de prótese sintetizada resultou em menor estresse. (AU)

@#Restorative materials such as conventional glass ionomer cement (CGIC), resin-modified glass ionomer cement (RMGIC), polyacid-modified composite resin (compomer), and giomer have the properties of fluoride release and refill, which may prevent or slow down the progression of caries. The caries resistance of these materials was evaluated according to the following criteria: fluorine-releasing ability, antibacterial activity, mechanical properties and anti-aging properties. The anti-caries effect of these materials mainly depends on the effect of fluoride ions on mineralization. However, materials with strong fluorine release ability, such as CGIC, are mainly used in pediatric dentistry and temporary restoration due to their poor mechanical properties. It is difficult to achieve both fluoride ion release potential and mechanical strength, and there are few materials that can provide ideal mechanical strength while maintaining a high standard of fluoride release. The fluoride releasing and recharging capacity of CGIC, RMGIC, compomer and giomer decrease successively. The trend of material modification, to a certain extent, tends to sacrifice the fluoride release capacity of materials to maintain the ideal mechanical strength. In recent years, scholars have tried to add a variety of fillers to further enhance the anti-cracking ability of materials and add antibacterial agents to compensate for the anti-caries effect to reduce the loss of fluoride release.

ABSTRACT Objective: To evaluate the association of oral health-related quality of life (OHRQoL) with the longevity of ART-restorations in children after 18 months. Material and Methods: A longitudinal clinical study with 62 six- to seven-year-old children, both genders, 31 of whom underwent four weeks of oral health education strategy (OHES) followed by ART (GOHES+ART) and the others only ART (GART). The Brazilian short-version of the Child Perceptions Questionnaire (16-CPQ8-10) was used to evaluate the OHRQoL and the perception of change in oral health was assessed by a single question. Data was analyzed using Mann-Whitney, Wilcoxon and Friedman's tests. Results: After 18 months, GOHES+ART reported a greater impairment on OHRQoL, mainly in oral symptoms domain (p<0.05). In this period, greater impairment of oral symptoms was reported in children with failed restorations and with treatment considered to have a minor failure (p<0.05). In inter-group analysis, the GOHES+ART, with 2 to 4 restorations, Class I, of small size, located in the upper arch and with a smaller failure, reported more oral symptoms than the GART. Conclusion: In the long-term, children submitted to OHES followed by ART reported more oral symptoms. Factors such as number, class, size and location of the restorations need to be considered for the implementation of an efficient and effective oral health program, as well as for greater longevity of ART-restorations.

Objective: whether gag reflex, a common problem encountered during dental procedures, is associated with the different types of the soft palate has not been addressed so far. This preliminary study sought to assess the potential association between the different types of soft palate and gag reflex. Material and Methods: one hundred dental patients were recruited. The type of soft palate was determined. Subjective (self-reported) gag reflex was recorded based on many questions and past experience and on a 0-6 VAS. Objective assessment of gag reflex was done using different maneuvers where the posterior part of the tongue and the soft palate were touched by dental mirror, and by taking impression for the upper arch. The association between the types of soft palate and the subjective and objective recorded gag reflex were statistically tested. Results: there were 53 (53%), 33 (33%) and 14 (14%) of the participants with class I, class II and class III soft palate, respectively. A significant association was found between the type of the soft palate and gag reflex in response to one of the subjective items (P= 0.039), more prominent among females (P= 0.009). Concerning the objective assessment, no significant associations were found among males. Meanwhile more females with class II and class III suffered gag reflex and/or actual gagging upon taking the impression (P = 0.001). Conclusion: this study illustrated an association between the type of soft palate and gag reflex, and its severity in females (more specifically soft palate types II and III) more than in males (AU)


Objetivo: o reflexo de vômito, um problema comum encontrado durante procedimentos odontológicos, está ou não associado aos diferentes tipos de palato mole, não foi ainda abordado até o momento. Este estudo preliminar procurou avaliar a possível associação entre os diferentes tipos de palato mole e o reflexo de vômito. Material e Métodos:cem pacientes odontológicos foram recrutados. O tipo de palato mole foi determinado. O reflexo de vômito subjetivo (auto-relatado) foi registrado com base em muitas perguntas e experiências anteriores e em um VAS de 0-6. A avaliação objetiva do reflexo de vômito foi feita por meio de diferentes manobras onde a parte posterior da língua e o palato mole foram tocados por espelho dental e por meio de moldagem da arcada superior. A associação entre os tipos de palato mole e o reflexo de vômito subjetivo e objetivo registrado foi testada estatisticamente. Resultados: houve 53 (53%), 33 (33%) e 14 (14%) participantes com palato mole classe I, classe II e classe III, respectivamente. Foi encontrada associação significativa entre o tipo de palato mole e o reflexo de vômito em resposta a um dos itens subjetivos (P= 0,039), mais proeminente no sexo feminino (P= 0,009). Em relação à avaliação objetiva, não foram encontradas associações significativas entre os homens. Enquanto isso, mais mulheres com classe II e classe III sofreram reflexo de vômito e/ou engasgo real ao receber a impressão (P = 0,001). Conclusão: este estudo ilustrou uma associação entre o tipo de palato mole e reflexo de vômito e sua gravidade em mulheres (mais especificamente palato mole tipos II e III) mais do que em homens. ABSTRACT

Trata-se de trabalho teórico que busca aproximar a clínica do traumático de Ferenczi (psicanálise) das práticas de Justiça Restaurativa (JR). Através da exposição de algumas das proposições ferenczianas acerca da etiologia do trauma e de suas consequências para o psiquismo, pretendeu-se demonstrar que a condução dos círculos, conferências e encontros restaurativos podem levar a que os sujeitos (inclusive os facilitadores) tomem contato com experiências traumáticas que demandarão a construção de algum sentido e uma ética do cuidado que garanta ambiente hospitaleiro, evite retraumatismos e leve em conta a complexidade dos conflitos que se apresentam. Ao final, concluiu-se que o diálogo com a clínica do traumático de Ferenczi pode ser profícuo para aprimorar as metodologias restaurativas, sobretudo ao reclamar uma ética das intervenções de JR, apontar alguns limites e convocar outros saberes.


On this theoretical paper we seek to bring Ferenczi's clinic of trauma closer to Restorative Justice (JR) practices. Through the exposition of some Ferenczian propositions about the etiology of trauma and its consequences for the psychism, we intend to demonstrate that the conduction of so called circles, conferences and restorative meetings can lead the subjects (including facilitators) to get in touch with traumatic experiences. These experiences are in search for meaning and require an ethic of care. Such ethic aims at ensuring a hospitable environment, avoiding retraumatizations and taking into account the complexity of the conflicts that emerge. We concluded that the dialogue with Ferenczi's clinic of trauma can be fruitful to improve restorative methodologies (especially with regard to the ethics of JR interventions) point out some limits and summon other knowledges into the debate.


En este trabajo teórico, pretendemos acercar la clínica de lo traumático (psicoanálisis) de Ferenczi a la práctica de la Justicia Restaurativa (JR). A través de la exposición de algunas de las proposiciones ferenczianas sobre la etiología del trauma y sus consecuencias para el psiquismo, se pretendió demostrar que la conducción de círculos, conferencias y encuentros restaurativos puede llevar a los sujetos (incluyendo a los facilitadores) a entrar en contacto con experiencias traumáticas que exigirán la construcción de algún significado y una ética del cuidado que garantice un ambiente hospitalario, evite retraumatismos y tenga en cuenta la complejidad de los conflictos que se presentan. Al final, concluimos que el diálogo con la clínica del traumático de Ferenczi puede ser útil para el perfeccionamiento de las metodologías restaurativas, especialmente a la hora de reivindicar una ética de las intervenciones de JR, señalando algunos límites y convocando otros saberes.

Objetivo: A cárie radicular é um problema da Odontologia moderna, porém é notável a falta de diretrizes sobre o seu manejo. Objetivamos elaborar e adaptar um guia a partir da tradução das recomendações do consenso in-ternacional European Organization for Caries Research (ORCA) e European Federation of Conservative Dentistry (EFCD) para as tomadas de decisão clínica na intervenção do processo de cárie na pessoa idosa, com foco na cárie radicular. Materiais e métodos: O protocolo de tradução das recomendações do consenso internacional consistiu nas etapas: (1) tradução inicial, (2) síntese da tradução, (3) retradução, (4) revisão por comitê de especialistas, com adaptação cultural. A partir da tradução, foi desenvolvido um guia com diretrizes para tratamento de cárie radicular no Brasil. Resultados: Para prevenção de novas lesões é recomendada a escovação diária com dentifrício >1.500ppm/F. Dentifrícios com 5.000ppm/F ou vernizes (>20.000ppm/F) podem ser indicados para paralisar lesões radiculares ativas e para prevenção em pessoas idosas com alta suscetibilidade à cárie radicular, e o Diamino Fluoreto de Prata (>30%) para paralisar lesões ativas. Intervenções invasi-vas diretas são indicadas dependendo da situação clínica. Discussão: Nota-se uma falta de interesse em estudos primários sobre tratamentos para cárie radicular, criando assim uma lacuna em relação ao seu manejo, que reflete no nível de evidência detectado pelo consenso. Conclusão: Guias clíni-cos são importantes para reduzir a lacuna entre a pesquisa e a prática clínica. Essa tradução para o português facilitará o acesso dos dentistas bra-sileiros em relação a evidência consolidada até o momento para o manejo de cárie radicular.


Aim: Root caries are a problem in modern dentistry, but the lack of guidelines regarding their management is notable. We aim to develop and adapt a guide based on the translation of the recommendations of the international consensus as outlined by the European Organization for Caries Research (ORCA) and the European Federation of Conservative Dentistry (EFCD) for clinical decision-making in the intervention of the caries process in the elderly, with a focus on root caries. Materials and methods: The protocol for translating the recommendations of the international consensus consisted of the following steps: (1) initial translation, (2) synthesis of the translation, (3) back-translation, (4) review by an expert committee with cultural adaptation. Based on the translation, a guide was developed with guidelines for the treatment of root caries in Brazil. Results: To prevent new lesions, daily brushing with toothpaste >1,500ppm/F is recommended. Toothpaste with 5,000ppm/F or varnishes (>20,000ppm/F) may be recommended to paralyze active root lesions and for prevention in elderly people with high susceptibility to root caries, and Silver Diamine Fluoride (>30%) to paralyze active lesions. Direct invasive interventions are indicated depending on the clinical situation. Discussion: There is a lack of interest in primary studies on treatments for root caries, thus creating a gap in relation to its management, which is reflected by the level of evidence detected in the consensus. Conclusion: Clinical guidelines are important to reduce the gap between research and clinical practice. This translation into Portuguese will facilitate access by Brazilian dentists to the consolidated evidence gathered to date for the management of root caries.

Background: In this COVID era, it's critical to promote nonaerosol procedures. Atraumatic restorative treatment (ART) is one of them, and it's particularly effective in children for lowering anxiety, enhancing dental health, and giving restorative care. Aim: The aim of this study was to assess the survival rate of ART compared with conventional treatment procedures in primary dentition. Materials and Methods: The review was done in accordance with the Preferred Reporting Items for Systematic reviews and Meta-analysis statement and is been registered in PROSPERO (CRD42021213729). The studies included comprised clinical investigations with randomized controlled trials (RCTs) which compared the survival rate of ART and conventional restorative treatments using the same or different restorative materials to treat carious lesion. RCTs in which ART was compared with conventional treatment on patients in the age group of 6–10 years with minimum follow-up of 6 months. Studies available as open access and free full text in PubMed, DOAJ, and Google Scholar databases, and published in English Language only were included in the study. Cochrane's collaboration tool for RCTs was used for the assessment of risk of bias. Results: The survival rate of single surface and multiple surface in primary dentition treated according to the ART compared with conventional treatment was found to be similar. Conclusion: The ART approach is equally helpful in managing dental caries in children and this method may be considered a useful intervention in clinical practice to enhance the dental health of children.

Objective:To evaluate the medium and long term clinical effect of three bone graft materials on periodontitis after periodontal regeneration.Methods:Initially, 16 patients with periodontitis after periodontal regeneration in Tianjin Stomatological Hospital from July 2017 to November 2018 were divided into three groups: Bio-Oss ( n=6), Bio-Gene ( n=5) and Bone 3(n=5) according to the differences in bone materials. Clinical and radiological results were evaluated at pre-surgery, post-6 months, and post-4 years. Results:The 6-month and 4-year clinical follow-up showed that, the depth of exploration (PD) and clinical attachment loss (CAL) of patients in the three groups were lower than those before operation (all P<0.05), and the changes of PD (ΔPD) and CAL (ΔCAL) were not statistically significant (all P>0.05). There was no statistically significant difference in gingival recession (GR) between the three groups (all P>0.05), and there was no statistically significant difference in the change of GR (ΔGR) between the groups (all P>0.05). From 6 months to 4 years after operation, there was no significant difference among ΔGR, ΔPD and ΔCAL groups (all P>0.05). At 6 months and 4 years after operation, the residual alveolar bone height (RBH%) of the three groups was significantly higher than that before operation (all P<0.05), and there was no significant difference between the two groups (all P>0.05); From 6 months to 4 years after operation, there was no significant difference in RBH% between the groups (all P>0.05). Conclusions:Three kinds of bone graft materials have significant clinical effects on periodontitis after periodontal regeneration, which can provide a reference for the selection of clinical regeneration materials.

Background It has been proved by many studies that forest environment has good restorative effects. Some researchers have attempted to identify natural factors affecting restorativeness. However, current studies have not focused on the effect of seasonal variation of forest environments on its restorative strength. Objective To evaluate the restorative effect changes of two types of forest environments across different seasons. Methods The experiments were conducted in April (spring), June (summer), and October (autumn) of 2017. Forty subjects were recruited in spring,18 in summer, and 20 in autumn. In spring, the subjects were randomly assigned to a forest park in suburban area and one in semi-pristine area in Beijing where each subject only experienced one kind of forest environment. In summer and autumn, each subject experienced two kinds of forest environments. They spent the first day in the suburban area forest park, and the second day in the semi-pristine area forest park. First the subjects completed the Brief Profile of Mood States (BPOMS) and had their heart rate been monitored for 10 min before test followed by a 30 min forest experience with sedentary sitting. At the end of each experience, the subjects completed the BPOMS again, and also the Restoration Environment Scale-Chinese version. Objective physical environment indicators were monitored simultaneously during forest experience. The changes of each indicator were analyzed before and after forest experience in selected three seasons. Results In the objective physical environment indicators, the concentrations of anion in selected three seasons in the semi-pristine area forest were higher than the values in the suburban development area forest; the concentration of anion in autumn was much higher than those in the other two seasons. In the assessment of the Restorative Environment Scale, the total scores of restorativeness of the semi-pristine area forest were generally higher than the scores in the suburban development area forest, but significant differences were found only in spring and summer (P < 0.001 in spring, P=0.022 in summer, and P=0.083 in autumn). In terms of physiological indicators, the average heart rate was decreased, while short-term R-R interval standard deviation (SD1) and root mean square of difference of successive intervals (RMSSD) were increased after the designed forest experience sessions in the three seasons, and there was statistical difference between forest types. In terms of psychological indicators, a decrease in the score of total mood disorder was presented after the experience session. In spring, there was a trend that the forest park in suburban development area had a better regulation effect on total mood disorder (P=0.07). In summer and autumn, there was a trend that the semi-pristine area forest park had a better regulation effect on total mood disorder (P=0.062 in summer and P=0.001 in autumn). Conclusion Generally, forest environments can improve the psychological and physiological indicators of human, and the improvement varies with frosts types and seasons. The restorative effects of forests in semi-pristine areas is better than those in suburban development area.

OBJECTIVE@#To investigate the effect of polishing on surface roughness, gloss and optimum polishing time of various computer aided design/computer aided manufacturing (CAD/CAM) restorative materials and to provide a proper polishing procedure for dental clinicians.@*METHODS@#Five CAD/CAM restorative materials including vita mark Ⅱ (VM), vita enamic (VE), lava ultimate (LU), shofu block HC (SB) and brilliant crios (BC) were selected. Six specimens were prepared for each material. The specimen was fixed on a custom-made polishing apparatus and sequentially polished with Sof-Lex poli-shing disk system including medium disk (with abrasive particle sizes of 10-40 μm), fine disk (with abrasive particle sizes of 3-9 μm) and superfine disk (with abrasive particle sizes of 1-7 μm). Surface roughness (Ra value) and gloss value were measured every 10 seconds until the numerical values were no longer changed. Then the surface roughness, gloss value and polishing time were recorded and the specimen was moved to the next sequence of polishing. Finally, statistical analysis was performed using SPSS 24.0.@*RESULTS@#For all the restorative materials, the Ra values were significantly reduced (P < 0.05) and the gloss values were significantly increased (P < 0.05) after sequentially polishing with Sof-Lex disks. No significant difference was detected among Ra values of all the tested materials (P>0.05) after sequential polishing. The gloss values of LU [(68.1±4.5) GU] and BC [(68.2±5.8) GU] were significantly higher than those of VE [(48.1±8.1) GU] and BC [(53.2±5.8) GU], P < 0.05. To obtain optimal surface smoothness, VM cost the shortest polishing time [40 (30, 55) s] among all the restorative materials (P < 0.05). No significant differences in the total polishing time were observed among VE [140 (135, 145) s], LU [130 (120, 140) s], SB [140 (130, 150) s] and BC [130 (120, 140) s], P>0.05.@*CONCLUSION@#The surface roughness of all CAD/CAM restorative materials were decreased after sequentially polishing with Sof-Lex disk system. To obtain the smoothest surface, different types of restorative materials might need different polishing times using Sof-Lex polishing disk system. For ceramic restorative material VM, we recommend polishing only with medium disk for 40 s. For hybrid restorative material VE and composite restorative material LU, SB and BC, we recommend polishing with medium disk, fine disk and superfine disk in sequence for 130-140 s in total.

Abstract There are many glass ionomer cements available on the Brazilian market for Atraumatic Restorative Treatment (ART), however, there is still a gap in the literature regarding their cost-effectiveness. Objectives To evaluate the influence of restorative materials (Ketac Molar, 3M ESPE; and Vitro Molar, Nova DFL) in the two-year survival rate and cost-effectiveness of occluso-proximal ART restorations in primary molars. Methodology A total of 117 children (aged four to eight years) with at least one occluso-proximal carious lesion in primary molars were selected and randomly divided in treatment groups (KM or VM) in this parallel randomized controlled trial. Treatments followed ART premises and were conducted in public schools by trained operators in Barueri, Brazil. A trained, calibrated, and blinded examiner performed the evaluations after two, six, 12, and 24 months (k=0.92). Kaplan-Meier survival analysis was used to estimate restoration survival and Cox regression was used to test the association with clinical factors (α=5%). For cost analysis, material and professional costs were considered. Monte Carlo analysis was used to generate a cost-effectiveness plane and bootstrapping was used to compare material costs over the years. Results The overall survival rate was 36.9% after two years (48.6% for KM and 25.4% for VM). Restorations with VM failed more than those with KM (HR=1.70; 95% CI=1.06-2.73; p=0.027). VM presented lower initial cost, but no difference was observed between groups considering the two-year incremental cost. Conclusion After a two-year evaluation, KM proved to be a better option than VM for occluso-proximal ART restorations in primary molars. ClinicalTrials.gov: NCT02267720

ABSTRACT The commercial bovine bone mineral most commonly used is available in two particle sizes and studies have demonstrated contradictory results regarding bone formation volume using small or large particles. The aim of this systematic review and meta-analysis was to compare the bone formation volume and residual bovine bone volume in sinus floor augmentation using small and large particles. The following outcome measures were assessed: bone formation volume (%) and residual bovine bone particles volume (%) assessed by histomorphometric analysis. The initial screening resulted in 236 records. After removal of duplicated articles and analysis of titles, abstracts and full texts, three articles were included in the meta-analysis. The bone formation volume and residual bovine bone volume did not differ between small and large particles, with low heterogeneity of studies. The particle size of bovine bone mineral did not influence bone formation percentage; small and large particles of bovine bone graft presented similar residual bone mineral; more randomized clinical trials should be performed to completely confirm that bovine bone mineral particle size does not affect the result of sinus floor augmentation.


RESUMO O enxerto ósseo bovino comercial mais comumente utilizado está disponível em dois tamanhos de partícula, que tem demonstrado resultados contraditórios em relação à formação óssea usando partículas pequenas ou grandes. O objetivo desta revisão sistemática e meta-análise é comparar o volume de formação óssea e o volume de enxerto residual em levantamento de seio usando partículas pequenas e grandes. Os seguintes dados foram comparados: volume de formação óssea (%) e volume residual de partículas de enxerto (%) dados por análise histomorfométrica. A seleção inicial resultou em 236 artigos. Após remoção de artigos duplicados e análise dos títulos, resumos e textos completos, três artigos foram incluídos na meta-análise. Os volumes de formação óssea e de enxerto residual não diferenciam entre os tamanhos de partícula pequeno e grande, com baixa heterogeneidade dos artigos. O tamanho de partícula de enxerto ósseo bovino não influencia o percentual de formação óssea; partículas de enxerto ósseo bovino pequenas e grandes apresentaram enxerto residual similar; mais ensaios clínicos randomizados deveriam ser realizados para confirmar que o tamanho de partícula de enxerto não afeta os resultados de levantamento de seio maxilar.

ABSTRACT The treatment of cavity carious lesions in anterior deciduous teeth is a frequent demand in the pediatric dentistry clinic, since the control of the disease in early childhood still presents itself as an inter-factor challenge in child health care. While the scientific evidence on the minimally invasive principles of approach in Dentistry is presented at an increasing level, the alternatives for restorative treatment in anterior deciduous teeth still face the low availability of reports that associate minimal intervention with the aesthetic needs inherent to the treatment. Thus, this article presents a series of cases in which direct restorations in composite resin, without removing carious tissue using polyvinyl matrices, were treatment alternatives for moderate and severe caries lesions in anterior deciduous teeth. It was observed, therefore, that the use of this artifact helped the rehabilitation of compromised dental elements with speed and satisfactory 50 result, suggesting that this is an appropriate approach for application in Pediatric Dentistry, 51 since it offers resistance, durability, besides providing aesthetics and functionality, it presents lower cost and agility as it eliminates the laboratory phase.


RESUMO O tratamento de lesões cavitadas de cárie em dentes decíduos anteriores é uma demanda frequente na clínica de odontopediatria, visto que o controle da doença na primeira infância ainda se apresenta como um desafio interfatorial nos cuidados em saúde infantil. Enquanto a evidência científica nos princípios minimamente invasivos de abordagem em Odontologia apresenta-se em nível crescente, as alternativas para tratamento restaurador em dentes decíduos anteriores ainda esbarram na baixa disponibilidade de relatos que associem a mínima intervenção às necessidades estéticas inerentes ao tratamento. Assim, este artigo apresenta uma série de casos em que as restaurações diretas em resina composta, sem a remoção de tecido cariado utilizando matrizes de polivinila foram alternativas de tratamento para lesões de cárie moderadas e severas em dentes decíduos anteriores. Observou-se que o uso desse artefato auxiliou a reabilitação dos elementos dentários comprometidos com rapidez e resultado satisfatório, sugerindo que essa é uma abordagem adequada para aplicação em Odontopediatria, já que oferece resistência, durabilidade, além de proporcionar estética e funcionalidade, apresenta menor custo e agilidade na medida que dispensa a fase laboratorial.

Objetivo: Esta revisão de escopo teve como objetivo comparar a taxa de sucesso de restaurações confeccionadas com cinco marcas diferentes de Cimento de Ionômero de Vidro de Alta Viscosidade (CIVAV), referência no Tratamento Restaurador Atraumático (Atraumatic Restorative Treatment-ART).Métodos: Buscas no PubMed, Scopus e Web of Science foram feitas. Buscas no OpenGrey e no Google Scholar também foram feitas. Critérios de inclusão foram estudos (ensaios clínicos e estudos de acompanhamento) que avaliavam a taxa de sucesso de restaurações confeccionadas com CIVAV. Não houve restrição de idioma ou data. Título/resumo e texto completo das referências encontradas foram avaliados por dois autores. Referências que preencheram os critérios de inclusão foram selecionadas. Extração de dados foi feita. Modelos de regressão de Cox foram idealizados para comparação da probabilidade de sucesso das restaurações com diferentes tipos de CIVAV. Os resultados foram relatados em razão das chances (RC) e intervalos de confiança (IC). Análises de Kaplan Meyer foram feitas para o cálculo da probabilidade de sucesso de restaurações com CIVAV. Resultados: Nove artigos avaliando o ChemFil Rock, ChemFlex, Equia Fil, Ketac Molar ou Fuji IX foram incluídos. Restaurações com cimentos de ionômero de vidro Fuji IX (RC = 3,51; IC = 1,96 ­ 6,28), Ketac Molar (RC = 4,01; IC = 2,40-6,68) e ChemFlex (RC = 4,20; IC = 1,01-17,66) apresentaram uma taxa de sucesso significativamente maior do que aquelas com ChemFil Rock. EquiaFil alcançou a segunda menor eficácia, ligeiramente maior que o ChemFil Rock. Conclusão:Restaurações com Fuji IX, Ketac Molar e ChemFlex apresentaram altas taxas de sucesso ao longo do tempo de avaliação do ART.


Aim: This scoping review aimed to compare the success rate of restorations using five different brands of High Viscosity Glass Ionomer Cement (HVGIC), reference material in Atraumatic Restorative Treatment (ART). Methods: Searches were carried out in PubMed, Scopus, and Web of Science. OpenGrey and Google Scholar searches were also performed. Inclusion criteria were studies (clinical trials and follow-up studies) that evaluated the success of restorations using HVGIC. Title, abstract, and full text of the references were evaluated by two authors. Articles that met the inclusion criteria were included. Data extraction was performed. Cox regression models were created to compare the success rate of restorations produced with different types of HVGIC. The results were reported as odds ratio (OR) and confidence intervals (CI). Kaplan Meyer analyses were performed to calculate the HVGIC restorations' probability of success.Results: Nine articles evaluating ChemFil Rock, ChemFlex, Equia Fil, Ketac Molar, or Fuji IX were included. Restorations with glass ionomer cements Fuji IX (OR = 3.51; CI = 1.96 - 6.28), Ketac Molar (OR = 4.01; CI = 2.40-6.68), and ChemFlex (OR = 4.20; CI = 1.01-17.66) had a significantly higher success rate than those with ChemFil Rock. EquiaFil achieved the second lowest efficacy, slightly higher than ChemFil Rock.Conclusion: Restorations with Fuji IX, Ketac Molar, and ChemFlex showed high success rates over the ART evaluation time.

Abstract: Appropriate research reports are important to facilitate the evaluation of studies and the decision-making by dentists and policymakers. This meta-research study assessed the conformity of randomized clinical trials (RCTs) on atraumatic restorative treatment (ART) restorations with the CONSORT recommendations and their risk of bias (RoB). Cochrane Library, MEDLINE, BBO, LILACS, Scopus, and Web of Science databases were searched from April 2019 to June 2021 for RCTs that assessed the longevity of ART restorations in children. A specific tool was used to assess adherence to the CONSORT recommendations; RoB was evaluated with the Cochrane risk-of-bias tool. Descriptive analyses included the number of studies by journal, follow-up period, country, and quality assessments. A total of 2,181 papers were retrieved and 36 of them were analyzed qualitatively. The overall CONSORT mean score (CONms) was 22.52 ± 6.17 out of 32 points. The best described items were intervention and outcomes, whereas allocation concealment was described in only 22% of the papers. Significant differences in CONms were detected in the analysis by country and publication dates. High CONms were observed in recently published papers (26.7 ± 3.1) when compared to first ART studies (18.1 ± 4.6; p < 0.001). RoB was low in four studies, unclear in 11, and high in 21. Adherence of the papers to the CONSORT recommendations was not fully achieved and most of the papers had unclear and high RoB (PROSPERO registration #CRD42020201460).

ABSTRACT

A busca por naturalidade dos procedimentos restauradores em reabilitação oral tem contribuído para o desenvolvimento de vários tipos de materiais livres de metal ("metal free"). A confecção de próteses com estes sistemas totalmente puros, mostra-se como excelente alternativa restauradora, com potencial estético superior às próteses metalocerâmicas. É indiscutível que a tecnologia CAD/CAM (Computer-aided design/ Computer-aided manufacturing) vem se tornando realidade nos consultórios e laboratórios de Prótese Dentária de todo o mundo. Esta tecnologia possibilita o uso de cerâmicas e polímeros na fabricação de próteses, com qualidade e resistência mecânica satisfatórias, padronizando os processos, reduzindo custos e tempo clínico-laboratorial. A grande diversidade destes novos materiais restauradores livres de metal, com suas distintas propriedades estéticas e mecânicas, faz com que seja necessária uma seleção racional desses materiais, a fim de se alcançar a longevidade desejada do tratamento protético reabilitador. Dentre estes materiais, destacam-se as cerâmicas vítreas, as zircônias monolíticas, as cerâmicas infiltradas por polímeros e as resinas nanocerâmicas. O objetivo do trabalho foi avaliar os dados mais relevantes e atuais sobre as propriedades destes novos materiais, os sistemas disponíveis, suas indicações, limitações e seu uso associado com a tecnologia CAD/CAM. Esta revisão consistiu em uma pesquisa eletrônica da literatura publicada nos últimos 15 anos, nas bases de dados: Medline/Pubmed, Embase, Scielo, Bireme, Lilacs e BBO, utilizando-se os seguintes descritores do MESH: [CADCAM AND restorative materials AND monolitic] OR [Monolitic CAD-CAM restorative materials]. Concluiu-se que os materiais monolíticos obtidos pelo sistema CAD/CAM foram desenvolvidos para oferecer uma melhor adaptação e durabilidade das peças, com ótimas taxas de sobrevida e redução de tempo clínico e laboratorial, oferecendo ao paciente conforto, sessões clínicas mais curtas e restaurações de qualidade superior aos sistemas convencionais. Todos os materiais citados nesta revisão apresentaram vantagens e desvantagens, cabendo ao profissional selecionar o melhor material para cada situação clínica em específico, levando em conta os prós e contras de cada um(AU)


The naturalness of restorative procedures in oral rehabilitation has contributed to the development of various types of metal-free materials ("metal free"). The manufacture of prostheses with these totally pure systems is shown to be an excellent restorative alternative, with an aesthetic potential superior to metal-ceramic prostheses. It is indisputable that CAD/ CAM (Computer-aided design/Computer-aided manufacturing) technology is becoming a reality in dental practices and laboratories around the world. This possibility of manufacturing ceramic materials and polymers in strength manufacturing, with strength technology, manufacturing processes, standardizing quality clinical time, higher cost and clinical time. These new aesthetic properties of metal, these dif ferences with aesthetic and mechanical characteristics, these differences are necessary for a rational selection of materials with a choice of rehabilitative prosthetic treatment. Among these materials, they stand out as glass ceramics, as monolithic zirconia, as polymer-infiltrated ceramics and as nanoceramic resins. The purpose of working with your materials, and their associated use with CAD/CAM technology. All those mentioned in this review present advantages and advantages of each material, and it is up to the professional to select the best one for each specific clinic, in each situation of materials and against indications of materials(AU)

ABSTRACT In Dentistry, any user with one or more limitations, of temporary or permanent mental nature, of physical, sensory, emotional or medical growth, is considered to be a Patient with Special Needs, preventing him from being subjected to a conventional dental situation. These patients form a group which may be considered at high risk for the development of oral diseases, according to the kind of pathogen. Among them, is the Williams-Beuren Syndrome, a rare congenital disease with cardiovascular involvement, mental retardation, dysmorphic face, idiopathic hypercalcemia, musculoskeletal problems, dental and growth anomalies. Familial and sporadic cases are thought to result from deletion of genetic material from adjacent genes located on the long arm of chromosome 7. This article reports a clinical case involving a four-year-old boy with Williams-Beuren Syndrome, referred to the clinic of the Specialization Course for Patients with Special Needs, at the São Leopoldo Mandic Dental Research Center, Campinas (SP). As the patient was resistant to dental care, in order to reduce anxiety and increase collaboration in clinical sessions, the Hixizine® medication was administered. For atraumatic restorative treatment and restorations, the following materials were used: Cleanjoy®, Futurabond DC®, Ionofil Plus® Grandioso®, Grandio® and Profluorid®. As result, it was possible to reach a level of excellence using the indicated materials and specific protocols. Based on this work and the lived experience, it can be observed that these patients can receive welcoming dental care in specialized clinics.


RESUMO Na Odontologia é considerado Paciente com Necessidades Especiais todo usuário que apresenta uma ou mais limitações, temporárias ou permanentes, de ordem mental, física, sensorial, emocional, de crescimento ou médica, que o impeça de ser submetido a uma situação odontológica convencional. Esses pacientes formam um grupo que pode ser considerado de alto risco para o desenvolvimento de doenças bucais, de acordo com o tipo de patogenia. Dentre elas, tem-se a Síndrome de Williams-Beuren, uma desordem congênita rara com envolvimento cardiovascular, retardo mental, face dismórfica, hipercalcemia idiopática, problemas musculoesqueléticos, anomalias dentárias e de crescimento. Os casos são hereditários ou esporádicos e a causa está na concepção com a deleção no braço longo do cromossomo 7. Este artigo relata um caso clínico envolvendo um menino de quatro anos, portador da síndrome, encaminhado para atendimento na Clínica do Curso de Especialização em Pacientes com Necessidades Especiais do Centro de Pesquisas Odontológicas São Leopoldo Mandic, Campinas (SP). Como o paciente apresentava resistência ao atendimento odontológico, com o objetivo de diminuir a ansiedade e aumentar a colaboração, nas sessões de atendimento foi administrado o medicamento Hixizine®. Para o tratamento restaurador atraumático e restaurações estéticas utilizou-se os seguintes materiais: Cleanjoy®, Futurabond DC®, Ionofil Plus®, Grandioso®, Grandio® e Profluorid®. Como resultado, foi possível atingir grau de excelência utilizando os materiais indicados e protocolos específicos. Mediante o caso relatado e frente à experiência vivenciada, pode-se perceber que esses pacientes podem receber um atendimento odontológico acolhedor nas clínicas especializadas.
