ABSTRACT

Objective To design and implement a planning backup and restore system and evaluate its clinical effectiveness.Methods The default TPS menu of data save (File-Save) and system exit (File -Exit) was replaced by user designed script (Script_Save and Script_Exit) which backup the current planning status in a special format besides performing the normal save and exit operation.The treatment plan can be rolled back to any previously saved backup status in case the data were damaged or,the plan quality was deteriorated in the try and error planning process.Results The save and exit operation was simplified by using the user designed script.An extra disk space of 59.4MB per patient was required to backup the planning process and the backup was performed with no impact to the normal planning operation.1.9% of the plans were restored during the planning process.For all of the restoring operations,65% were performed within 1 day,80% were performed within 1 week,and 87.6% were performed within 1 month.Some restoring operations were even performed 4 months after the planning.Conclusions The designed backup and restore system is clinical applicable and valuable.Clinical using of the system can improve the planning efficiency and patient data safety.

ABSTRACT

Objective To explore whether the peritoneal cooling was better than other cooling methods on protection neuron damage of the hippocampus CA1 after cardiopulmonary resuscitation (CPR) in New Zealand rabbits.Methods Forty eight adult New Zealand rabbits were induced ventricular fibrillation by AC current and were resuscitated after cardiac arrest for 5 minutes.The rabbits were randomly divided into four groups according to the way of cooling methods,nomothermia group ( NT),peritoneal cooling group (PC),surface cooling group (SC) and local cooling group (LC).The changes of tympanic membrane temperature were recorded in each animal and blood plasma concentrations of electrolyte were tested in each group at different time points after restore of spontaneous circulation (ROSC).Brain tissue were removed,the numbers of vigorous and apoptotic neurons in the hippocampus CA1 area were counted after ROSC at 72h.One-way ANOVA or Mann-Whitney rank was used to determine the statistical significance between two groups.LSD-t test for multiple comparisons,R × C test for ROSC comparisons,a two-tailed value of P <0.05 was considered statistically significant. Results Hypothermia was rapidly induced in PC after ROSC,and the time of arriving at target temperature was (26 ±7) min in PC,(60 ±9) min in SC,(69 ± 12) min in LC respectively; in the maintain hypothermia period,the tympanic membrane temperature was maintained at 33~ 35 ℃ in each group exception nomothermia group (NT).There were no differences with main electrolyte,acid-abase liquid balance and renal function between each group at each time point after ROSC.The numbers of vigorous neurons in hippocampus CA1 area were ( 37.07 ± 6.43 ) /40 × in NT group,(35.13 ± 6.97) /40 × in LC group,(55.76 ± 10.13 ) /40 × in PC group,and (50.70 ± 7.38 ) /40 × in SC group (PC:NT,P<0.01,SC:NT,P<0.01,PC:SC,P=0.043,PC:LC,P<0.01,LC:NT,P=0.52).The numbers of apoptotic neurons were (44.07 ±6.09) /40 × in NT group,(29.88 ±4.81 ) /40× in PC group,( 33.55 ± 5.67 ) /40 × in SC group and ( 42.27 ± 5.20 ) /40 × in LC group respectively (PC:NT,P <0.01,SC:NT,P <0.01,PC:LC,P <0.01,SC:LC,P <0.01,PC:SC,P=0.026,LC:NT,P =0.364 ).Conclusions The new peritoneal cooling method could rapidly induce and maintain hypothermia,and it had better protections on neurons in hippocampus CA1 than surface cooling and local cooling method after ROSC in New Zealand rabbits.

ABSTRACT

Objective To evaluate low-weight mesh for tension-free repair of inguinal hernia in the elders. Methods 120 old patients ( age at 60 -97 years) underwent tension-free repair of inguinal hernia by using low-weight mesh. The early complications, time of up and about, hospital stay, postoperative chronic pain,foreign body feelings and hernia recurrence were analyzed. Results The operations were successful in all cases and the average time of operation was (43 ± 12 ) min. 9 cases suffered from edema of the scrotum, 5 cases suffered urine retention, 4 cases suffered from incision hematoma. The average time of outof-bed was ( 17.4 ± 1.8 ) h, the average hospital days was ( 7.4 ± 1.9) d. However, there were 1.7%(2/120) patients suffering from slight chronic pain and 9. 1% (11/120) patients complaining foreign body feelings. There was no recurrence after follow-up for 6 to 48 months. Conclusions The clinical application of low-weight mesh for tension-free repair of inguinal hernia in old age is safe and effective, with an additional advantage of low occurrence of chronic pain and foreign body feelings.

ABSTRACT

Objective To investigate the therapeutic effect of acupuncture for restoring consciousness and inducing resuscitation in promoting revival of coma for the patient of diffuse axonal injury(DAI). Methods 60 cases of DAI (GCS≤ 8) were randomly recurited into a control group and a treatment group. The control group was treated by routine treatment, while the treatment group was treated with acupuncture on the basis of routine treatment .The improving rate of consciousness at the two months of treatment was compared between the two group, and the prognosis was evaluated according to GOS scores. Results The revival rate in the treatment group was 86.7%, higher than that of 60.0% in the control group (P<0.05). The revival time in the treatment group was shorter than the control group (X2=5.45、 t= 8.051, P<0.05 ) .Conclusion The integrated therapy of acupuncture for restoring consciousness and inducing resuscitation and western medicine can promote the effect of coma revival and improve life condition for the patient of DAI.

ABSTRACT

The rapid increase of the elderly population has generated unprecedented social and cultural conflicts. One of the fundamental factors underlying these turbulences would be the traditional concept on aging and the elderly, which has been prevailed by the negative view as an irreversible and inevitable process. However, the recent researches have revealed that aging is a biological process not for death but for survival, suggesting a responsive and adaptive aspect of being aging. This revolutionary change in the concept on aging may provoke aconfident aspect of aging, leading to a more productive and active participation of the elderly in the community. Therefore, the traditional view on aging, as a functionally deteriorated and morphologically altered state of the organism, should be corrected. Based on this new concept, the policy and strategy for the future society should be replanned. Moreover, the elderly individuals should try to maintain the best condition toward the functional longevity without despair or discourage. In this article, I briefly introduce the novel hypothesis on aging, that is, "The Gate Theory of Aging", and discuss the replace vs restore principles for aging control.

ABSTRACT

Objective To explore the conditions for the restoration of competitive dysbacteriosis with antibiotics. Methods The mathematical model of the two competitive floras was analyzed by the Qualitative Theory of Ordinary Differential Equations. Results Three different types of dysbacteriosis and their restoring conditions were obtained. Conclusion Different restoring schemes should be applied for the regulation of different types of dysbacteriosis. Misuse of antibiotics can not result in satisfactory therapeutic effect.
