ABSTRACT

Abstract Introduction In patients with chronic rhinosinusitis, conservative interventions with extended medical trials are often attempted prior to procedural treatment. Balloon sinuplasty (BSP) is an established procedure for symptomatic relief from chronic rhinosinusitis. However, data suggesting the suboptimal efficacy of prolonged medication management trials, prior to BSP, is lacking. Objectives The purpose of this study was to evaluate the efficacy of prolonged medication management trials, prior to BSP, for patients with chronic rhinosinusitis. Methods A retrospective review was performed for all adults with chronic rhinosinusitis who received extended medical management prior to their BSP at two outpatient clinics, from November 1, 2013, to June 31, 2018. The patients' Sino-Nasal Outcome Test (SNOT) scores were compared between baseline, post-medication trials, and post-BSP. Results The SNOT scores of a total of 64 patients were collected. Overall, patients showed a significant worsening of symptoms during the medication management trials from baseline (p = 0.002126) but significant improvement of symptoms after undergoing BSP (p < 0.0001). Conclusions The patient symptom burden worsened and prolonged during medication management trials. The BSP procedure alone showed significant improvement in the quality of life for chronic rhinosinusitis patients, when considering their SNOT scores. The worsening of patients' symptoms during medication management may invalidate the necessity of prolonged medication management trials.

Las características anatómicas de pacientes con nariz mestiza, generalmente, incluyen una nariz aparentemente grande, un dorso convexo con radix bajo y una base nasal ancha. La longitud de la columela y punta nasal se ve disminuida debido a que los cartílagos alares son cortos, débiles y delgados, proporcionando un soporte estructural insuficiente, mala definición y proyección de la punta nasal. La principal dificultad al manejar este tipo de narices es un marco osteocartilaginoso mal estructurado y débil. En los últimos años se han desarrollado técnicas quirúrgicas para mejorar los resultados estéticos y funcionales de la rinoplastía en estos pacientes. Se realizó una revisión exhaustiva de la literatura describiendo las técnicas quirúrgicas utilizadas en este tipo de nariz.


The anatomical characteristics of patients with mestizo nose usually include an apparently large nose, a convex dorsum with a deep radix, and a wide nasal base. The length of the nasal columella and tip is decreased because the alar cartilages are short, weak and thin, providing insufficient structural support, poor definition and nasal tip projection. The main difficulty in managing this type of noses is a poorly structured and weak osteocartilaginous framework. In recent years, surgical techniques have been developed to improve the aesthetic and functional results of rhinoplasty in these patients. A comprehensive literature review was conducted describing the surgical techniques used in this type of nose.

RESUMEN Introduction La epistaxis es un motivo frecuente de consulta en otorrinolaringología pediátrica. Diversos factores se han asociado, existiendo tanto causas locales como sistémicas. Ciertos estudios en adultos, han sugerido la asociación de colonización nasal por Staphylococcus aureus con la recurrencia de la epistaxis. Este hallazgo en población pediátrica tendría un potencial rol dentro del tratamiento y sería un posible blanco para profilaxis. Objetivo Comparar la prevalencia de colonización nasal por S. aureus en pacientes pediátricos con epistaxis recurrente, en el Servicio de Otorrinolaringología del Hospital Barros Luco Trudeau, respecto a pacientes sin este antecedente. Material y método Estudio caso-control, definiendo caso como pacientes entre 2 y 15 años que consultaron por cuadros de epistaxis anterior recurrente. Se tomaron cultivos nasales a ambos grupos. Resultados Se incluyeron 46 pacientes con epistaxis y 45 controles. La prevalencia de S. aureus fue de 39% en los casos y 37% en los controles, sin diferencias estadísticamente significativas. Al examen físico de la mucosa nasal, ninguna de las características estudiadas mostró una relación significativa con la presencia de S. aureus. Conclusión La colonización nasal de S. aureus de niños con y sin epistaxis recurrente no varía considerablemente. En este estudio, la presencia de S. aureus no se asoció estadísticamente a epistaxis recurrente.


ABSTRACT Introduction: Epistaxis is a frequent reason for consultation in pediatric otolaryngology. Several factors have been associated, both local and systemic. Some studies in adult population, have suggested the association of nasal colonization by Staphylococcus aureus with the recurrence of epistaxis. Finding this association in pediatric population could have a potential role in treatment and also as a possible target for prophylaxis. Aim: To compare the prevalence of nasal colonization by S. aureus between pediatric patients with recurrent epistaxis and controls with no previous recurrent epistaxis, in the Service of Otorhinolaryngology in Barros Luco Trudeau Hospital. Material and method: Case-control study, defining case as patients between 2 and 15 years who consulted for recurrent anterior epistaxis. Nasal cultures were taken from both groups. Results: 46 patients with epistaxis and 45 controls were included. The prevalence of S. aureus was 39% in the case group and 37% in the controls, without statistically significant differences. At the physical examination of the nasal mucosa, none of the characteristics studied showed a significant relationship with the presence of S. aureus. Conclusion: Nasal colonization of S. aureus in children with and without recurrent epistaxis does not vary considerably. In this study, the presence of S. aureus was not statistically associated with recurrent epistaxis.
