ABSTRACT

Abstract: Introduction: plantar fasciitis or fasciosis is a cause of foot pain with cases resistant to conservative treatment. Surgery is reserved for patients who have not responded to conservative treatment, shock waves, or corticosteroid injections. The aim of this publications is to carry out a systematic review of the available literature and to describe a specific technique for the treatment of plantar fasciosis consisting of the longitudinal tearing of the plantar aponeurosis assisted with ultrasound. Material and methods: a systematic search was carried out for previous publications on longitudinal tenotomy in the treatment of plantar fasciitis. The Medical Subject Headings (MeSH) terms «Curettage¼, «Tenotomy¼ and «Plantar Fasciitis¼ were included. The electronic search included PubMed, Embase, Cochrane central register of controlled trials, Trip database, and National Institute for Health and Care Excellence (NICE) databases. A detailed description of the technique was included with the intention that it can be reproduced. Conclusion: longitudinal tenotomy represents an alternative for the treatment of plantar fasciitis. It is based on the extrapolation of knowledge in the Achilles tendon territory with a supporting pathophysiological basis. It is a non-invasive technique that can be performed on an outpatient basis and that would allow the rapid incorporation of the patient to their activities. Longitudinal tenotomy would prevent the patient from undergoing major surgeries.

ABSTRACT

Objective: To simulate and evaluate the scraping and grinding work of workers with different spinal anteversion angles, and to explore the effects of different anteversion angles on the erector spinae muscles of scrapers. Methods: In November 2019, 16 male college student volunteers were recruited to simulate workers' scraping and grinding work. The parameters were 25°, 15 times/min, 15°, 30 times/min, 5°, 60 times/min respectively. The surface electromyography (sEMG) was used to collect the electromyographic signals of the erector spinae muscles, and the surface electromyographic characteristics of the erector spinae muscles were evaluated with Borg Scale. Results: There were significant differences between the maximum voluntary contraction percentage (MVE%) of the left and right erector spinae muscles groups in the three groups with different spinal anteversion angles (F(left)=13.41, P(left)<0.001; F(right)=4.74, P(right)=0.005) , and the EMG amplitude was higher at 25°, 15 times/min. At 15°, 30 times/min, MVE% of the left side was significantly higher than that of the right side (t=2.58, P=0.021) . There was significant difference in the mean power frequency (MPF) of the right erector spinae muscle in the three groups (F=9.42, P<0.001) , but there was no significant difference in the MPF of the left erector spinae muscle (F=0.30, P=0.823) . The fitting line showed that the left erector spinae muscle showed a downward trend at 5°, 60 times/min (t=-5.39, P=0.012) . Conclusion: Scrapers are less likely to be fatigued when the posture is 15°, 30 times/min, but they are more likely to be fatigued when working at 5°, 60 times/min.

ABSTRACT

Objective: To observe the effect of scraping therapy (ST) on the immune balance of serum helper T (Th) 1/Th2 cells in autologous nucleus pulposus transplantation non-compressive lumbar disc herniation (LDH) model rats, and explore the immune mechanism of ST in reducing LDH pain. Methods: Seventy-two male Sprague-Dawley (SD) rats were randomly divided into a model group, a sham operation group and a ST group, with 24 rats in each group. Autologous nucleus pulposus transplantation non-compressive LDH models were established in the model group and ST group, while the rats in the sham operation group underwent sham operation without model establishment. On the fifth day after the model was successfully prepared, rats in the ST group received ST, once every other day, 3 times as a course for a total of 3 courses. Six rats in each group were randomly selected to observe their pain thresholds, and peripheral blood of the rats was collected before the first scraping treatment and at the end of the first, second, and third courses of treatment. The serum was isolated and enzyme-linked immunosorbent assay (ELISA) was applied for the detection of rat serum interferon (IFN)-γ, interleukin (IL)-4 and IL-10. Results: Compared with the model group, the pain threshold in LDH rats in the ST group increased (P<0.05), the serum IFN-γ level was significantly reduced (P<0.05), but the levels of IL-4 and IL-10 did not change significantly (both P>0.05). At the end of the second and third courses of treatment, the IFN-γ/IL-4 ratio was negatively correlated with the pain threshold in the rats, and the IFN-γ/IL-4 ratio was significantly reduced in the ST group (P<0.01). Conclusion: ST can help suppress the Th1 immunity in LDH rats triggered by the autologous nucleus pulposus, restore the immune balance of Th1/Th2, and reduce the pain of LDH.

Objective To investigate the changes of the immunological factors in subcutaneous exudate and blood components of the rats receiving cutaneous scraping method, and to compare the changes of skin histopathological features before and after cutaneous scraping under microscope. Methods SD rats were randomly divided into two groups,cutaneous-scraping group and non-cutaneous-scraping group. And then each group was divided into three subgroups. The observation indexes included the levels of interleukin (IL)-1β,IL-6 and interferon gamma (IFN-γ) in the blood and the skin, routine blood examination, and skin histopathological features. Results In cutaneous-scraping group, the number of white blood cells in the blood and the levels of IL-1 β and IFN-γ in skin tissues were increased (P < 0.05),the hemolysis rate was increased (P < 0.05).However,the levels of IL-1β,IL-6 and IFN-γin the blood showed no obvious changes.Under the microscope, severe skin edema, vascular congestion and dilatation, and infiltration of inflammatory cells were found in the skin after cutaneous scraping. Conclusion The cutaneous scraping method can activate the immune response rapidly, and the immunological components of the subcutaneous exudate after cutaneous scraping are helpful to the disease treatment.

ABSTRACT

Background: Standardized skin surface biopsy (SSSB) is considered to be the gold standard technique to evaluate the density of Demodex mites for the diagnosis of demodicidosis. Potassium hydroxide (KOH) preparation of skin scrapings is a much simpler procedure that can be used to detect pathogens in the superfi cial skin. Objective: To evaluate the reliability of potassium hydroxide preparation of skin scrapings as compared to the standard skin biopsy technique with regard to capacity to detect Demodex mites, time consumed and technician satisfaction. Methods: One hundred outpatients presenting with facial erythema of uncertain cause were enrolled. Standardized skin surface biopsy and potassium hydroxide preparation of skin scrapings were undertaken in adjacent areas on the patients’ right cheek. Limitation: Patients with normal facial skin were excluded from the study. Results: The accuracy of Demodex mite detection by potassium hydroxide preparation of skin-scrapings when compared to the standard procedure is 82%. The sensitivity, specifi city, positive and negative predictive values of this method are 75%, 84.2%, 60% and 91.43%, respectively. There was no statistically signifi cant difference between the standard and skin scraping techniques (P = 0.238) with regard to mite detection. Mean preparation time while using the skin scraping technique was 6 times less than that of the standard technique. For interpretation also, skin scraping technique (3.6 min) consumed much less time than the biopsy technique (9.8 min). Moreover, experienced technicians were more satisfi ed with skin scraping. Conclusion: Potassium hydroxide preparation of skin scrapings is an effective, time saving and practical technique to detect Demodex mites with accuracy comparable to the standard biopsy method.

ABSTRACT

Objective To study the clinical effects of the combination of the topping tongue method and the scraping tongue method in speech training after the operation of ankyloglossia with lingua apical articulation disor-der.Methods Based on the random control study design,50 cases of ankyloglossia with lingua apical articulation disorders were randomly allocated into experimental group (25 cases)and control group (25cases).Patients of ex-perimental group were given routine speech training with the topping tongue and scraping tongue methods.Patients of control group were given routine speech training, and data of voice intelligibility of two groups of patients were collected at the time before speech training and after 3 courses of speech training. Results The voice intelligibility of two groups of patients before speech training had no statistically significant differences(t =0.726,P >0.05 ).But the voice intelligibility of the patients after 3 courses of speech training in both groups was obviously improved. There were statistically significant differences(P <0.001).Patients in the experimental group had a higher voice in-telligibility score assessed after 3 courses of speech training than the control group. After treatment, the voice intel-ligibility of the patients in experimental group increased from 37.91% to 87.69%.The voice intelligibility of the pa-tients in the control group increased from 37.69% to 72.99%.There were statistically significant differences(P <0.001).Conclusion The combination of the topping tongue and scraping tongue methods can improve the speech intelligi-bility for patients after operation of ankyloglossia and promote rehabilitation training of the lingua- apical articulation.
